Abstract

A scientific and updated bibliographic review is realized for handling and care of a pediatric patient with psoriasis disease. Health personnel related with this pathology must to know the different perspectives and angles of psoriasis, as well as clinical criteria, therapeutic and emotional in the treatment of patients. The incidence of psoriasis is recognized globally. Ethnic groups have developed with most frequently this disorder. The different clinical faces of psoriasis are studied. The morphological and topographical manifestations have presented a variety very similar to that of adults, and have made for the doctor difficult to make the diagnostic. Clinical studies that were realized in the last years, have reported etiological and pathogenic evidence, both genetic and immunological of this illness. Children with psoriasis usually have presented a mild illness, where psoriasis type plaque has been the predominant variant. Psoriasis in the population has required a special attention in triggers or aggravating factors of this disease such as infections, exposure to snuff, obesity, stress and interactions with other drugs. Psoriasis in the population has required a special attention in triggers or aggravating factors of this disease such as infections, exposure to snuff, obesity, stress and interactions with other drugs. The discovery and use of new drugs have led to different etiological factors for the handling of psoriasis; so it is important to know the function, availability and adverse effects that can to cause new therapies. Treatments must to include the provision of a safe and effective therapy for the maintenance for free long periods of lesions, reducing the severity of the disease, and inhibiting structural damage of joints. The topical treatment has been the therapy of first choice in mild psoriasis and localized. An interrogatory is recommended to decide objectively a systemic treatment, because the infant population has been a sensitive group of possible adverse effects. Methotrexate has been the treatment of choice for psoriasis related to arthropathy both adults and children. Phototherapy, including UVB, PUVA light and excimer laser is considered the therapeutic option in pediatric patients with dermatological disease. Use in conjunction with other topical therapies has increased the efficacy of this option [Spanish] Una revision bibliografica cientifica y actualizada es realizada para el manejo y cuidado de un paciente pediatrico con la enfermedad de psoriasis. El personal de salud relacionado con esta patologia debe conocer las diferentes perspectivas y angulos de la psoriasis, asi como los criterios clinicos, terapeuticos y emocionales en el tratamiento de los pacientes. La incidencia de la psoriasis es reconocida a nivel mundial. Los grupos etnicos han desarrollado con mayor frecuencia este trastorno. Las diferentes caras clinicas de la psoriasis son estudiadas. Las manifestaciones morfologicas y topograficas han presentado una variedad muy similar al de los adultos, y han hecho que al medico se le dificulte hacer el diagnostico. Los estudios clinicos que fueron realizados en los ultimos anos, han reportado evidencia etiologica y patogenica, tanto genetica como inmunologica de este padecimiento. Los ninos con psoriasis usualmente han presentado una enfermedad leve, donde la psoriasis tipo placa ha sido la variante predominante. La psoriasis en la poblacion ha requerido una especial atencion en los factores desencadenantes o agravantes de esta enfermedad tales como infecciones, exposicion al tabaco, obesidad, estres e interacciones con otras drogas. El descubrimiento y el uso de nuevos medicamentos han dirigido a los diferentes factores etiologicos para el manejo de la psoriasis; por lo cual es importante conocer la funcion, disponibilidad y efectos adversos que puede ocasionar las nuevas terapias. Los tratamientos deben incluir la provision de una terapia segura y efectiva para el mantenimiento de periodos largos libres de lesiones, disminuyendo la severidad de la enfermedad, e inhibiendo el dano estructural de las articulaciones. El tratamiento topico ha sido la terapia de primera eleccion en la psoriasis leve y localizada. Un interrogatorio es recomendado para decidir objetivamente un tratamiento sistemico, ya que la poblacion infantil ha sido un grupo sensible a posibles efectos adversos. El metotrexate ha sido el tratamiento de eleccion para la psoriasis relacionada con artropatia tanto en adultos como en ninos. La fototerapia, incluyendo la luz UVB, PUVA y laser excimer es considerada la opcion terapeutica en pacientes pediatricos con enfermedad dermatologica. El uso en conjunto con otras terapias topicas ha incrementado la eficacia de esta opcion.}
