Abstract

The positron scanning method with Cu{sup 64} and As{sup 74} for the diagnosis of brain tumours has the advantage over {gamma}-scanning methods that it needs a shorter test time and moreover involves less irradiation hazards. This is especially true with Cu{sup 64} (half-life 12.8 h), which may be used in studies on out-patients. From the degree of concentration of the radioactive substance in the tumour, its type may be determined. In combination with cerebral angiography, the diagnosis of the type of tumour is still better established, a fact which is very important in determining the indication for operation. In the present, study, the exactness of the diagnosis amounted to 75- 80% of all cases of brain tumours. The most easily recognized lesions, through their high ability to concentrate the isotopes, were the meningiomas, abscesses and glioblastomas, while the oligodendrorogliomas and astrocytomas showed less ability to concentrate the radioactive substance and hence were less easily recognizable. A group of astrocytomas and almost all epidermoid tumours did not show any ability to concentrate the radioactive substance and in this group the combination with cerebral angiography is important for determining the diagnosis. The combination of electro-encephalography with scanning showed similar accuracy in tumour diagnosis as well as tumour exclusion, so that arteriography and/or hospital admission could be dispensed with whenever combined negative findings and lack of clinical symptoms were found. The value of this method in examination, of out-patients is thus clearly demonstrated. (author) [French] Pour le diagnostic des tumeurs du cerveau, la scintigraphie au moyen des positons emis par {sup 64}Cu et {sup 74}As presente l'avantage sur la gammagraphie de reduire la duree de l'explorat ion et de diminuer les risques d'irradiation. Il en est surtout ainsi pour {sup 64}Cu (periode de 12,8 h), qui peut etre utilise sur des patients non hospitalises. Le degre de concentration de la substance radioactive dans la tumeur permet d'identifier le type de cette tumeur. En combinant cette methode avec l'angiographie cerebrale, on peut augmenter encore la surete du diagnostic du type de tumeur, ce qui est tres important pour decider de l'opportunite d'une intervention chirurgicale. Dans l'etude qui a fait l'objet du memoire, on a obtenu une precision de 75 a btrfo dans le diagnostic de tumeurs du cerveau de toutes sortes. Les lesions les plus faciles a reconnaitre du fait de leur aptitude a fixer les radioisotopes sont les me- ningiomes, les abces et les glioblastomes ; les oligodendrogliomes et les astrocytomes presentent une tendance moindre a fixer la substance radioactive et sont donc moins aisement decelables. Certains astrocytomes et presque toutes les tumeurs epidermiques n'ont aucune tendance a fixer la substance radioactive; pour ce groupe, il est indispensable de combiner la scintigraphie avec l'angiographie cerebrale pour poser le diagnostic. La scintigraphie au moyen des positons a fait ses preuves lorsqu'il s'agit d'eliminer l'hypothese d'une tumeur dans des cas d'epilepsie; la surete du diagnostic atteint 90%. On a obtenu la meme precision en combinant l'electroencephalographie et la scintigraphie, tant pour identifier une tumeur que pour ecarter l'hypothese de son existence. On peut ainsi eviter l'arteriographie et l'hospitalisation chaque fois que l'ensemble des conclusions est negatif et qu'il n'existe aucun symptome clinique. La valeur de cette methode dans le cas de patients non hospitalises est donc bien etablie. (author) [Spanish] La exploracion positronica con {sup 64}Cu y {sup 74}As para diagnosticar tumores cerebrales ofrece la ventaja de que requiere menos tiempo y entrana menos riesgos de irradiacion que los metodos gammagraficos, especialmente cuando se recurre al {sup 64}Cu (periodo: 12,8 h), que puede emplearse en enfermos ambulantes. El grado de concentracion de la sustancia radiactiva en el tumor permite determinar el tipo de este ultimo. Si se combina ese metodo con la angiografla del cerebro se puede diagnosticar aun mejor el tipo del tumor, lo que tiene suma importancia para determinar si una intervencion quirurgica esta indicada. En el estudio que se describe en la memoria los diagnosticos fueron correctos en el 75-80% de todos los casos de tumores cerebrales. Las lesiones que con mayor facilidad se reconocen debido a su gran tendencia a concentrar los isotopos son los meningiomas, los abscesos y los glioblastomas, mientras que los oligodendrogliomas y los astrocitomas, que son menos capaces de concentrar la sustancia radiactiva, resultan mas dificiles de reconocer. Un grupo de astrocitomas y casi ninguno de los tumores epidermoideos son capaces de concentrar la sustancia radiactiva asi que, en este grupo, es importante confirmar el diagnostico por angiografia del cerebro. El metodo de la exploracion positronica ha demostrado su gran utilidad para descartar la presencia de tumores en casos en que se sospecha epilepsia sintomatica; el diagnostico ha resultado correcto en el 90% de dichos casos. La combinacion de la electroencefalografia y la exploracion ha sido tan eficaz para diagnosticar como para descartar la presencia de tumores, gracias a lo cual se ha podido renunciar a la arteriografia y a la hospitalizacion del enfermo cuando los resultados han sido negativos y no se han observado sintomas clinicos. El valor de ese metodo para examinar pacientes ambulantes ha quedado, por lo tanto, claramente demostrado. Metod pozitronnogo skennirovanija okazalsja ochen' cennym pri iskljuchenii diagnoza opuholi v sluchajah predpolagaemoj simptomaticheskoj jepilepsii; tochnost' diagnoza sostavljaet zdes'jao 90%. Pri kombinacii jelektrojencefalografii so skenniro- vaniem obnaruzhena odinakovaja tochnost' kak pri ustanovlenii, tak i pri iskljuchenii diagnoza opuholi, chto pozvolit obojtis' bez arteriografii i/ili gospitalizacii vo vseh sluchajah, kogda poluchena kombinacija otricatel'nyh dannyh i otsutstvujut simptomy bolezni.
