Abstract

In order to estimate the radiation dose to the critical organ due to internal contamination with a radionuclide, it is necessary to measure the amount of radionuclide in the body. When the radionuclide emits gamma- or X-radiation, or beta-radiation giving rise to suitable Bremsstrahlung, direct measurements of the body content can be made in a whole-body activity measuring facility, but when there is no suitable emission of radiation it is necessary to resort to estimation of the body, content from measurements of the amount of radionuclide in excreta, blood, breath or swabs taken from the nose or mouth. There has been some study of the excretion of radionuclides in urine as an index of internal contamination, and urine analysis is the most extensively practised form of bioassay. The small amount of reference data available about the metabolism of radionuclides is still, however, a very serious weakness and a great deal more research is urgently necessary. In this review three broad categories have been used in classifying the different modes of metabolism of various radionuclides. The third group, really a specially important sub-group of the second, comprises the bone seekers. The pattern of urinary excretion and particularly its relationship to the radiation dose delivered to the critical tissues, is distinctly different for these different groups. It is emphasized that, following the recognition of an accidental intake, it is desirable to analyse a carefully planned series of urine samples, to provide a measure of the urinary excretion rate for a suitable period. The uncertainty associated with each individual result because of fluctuation in excretion rate can thus be minimized. A somewhat different approach is suggested in the case of urine sampling for routine surveillance of a group of personnel. The data available about metabolism of the radionuclide are used to evaluate an investigation level, namely, the urinary excretion rate corresponding to a chosen level of body content. It is important that measurements for routine surveillance should be made with maximum accuracy and carer fully recorded. These measurements may be used much later in making an assessment of .the chronically- acquired body content. A summary is presented of the human data available on metabolism and urinary excretion of tritium, caesium, uranium, strontium, radium and plutonium. (author) [French] Afin d'evaluer la dose de rayonnements a l'organe critique due a la contamination interne par un radionucleide, il faut mesurer la quantite du radionucleide dans le corps. Quand le radionucleide emet des rayons gamma ou X, ou des rayonnements beta donnant.naissance a un rayonnement de freinage convenable, on peut proceder a-desmesures directes de la charge corporelle de ce radionucleide dans un anthroporadiametre, mais lorsque le radionucleide n'emet pas de rayonnements convenables, on est oblige d'evaluer la charge corporelle d'apres les mesures de la quantite du radionucleide contenue dans les excreta, le sang, l'haleine ou des tampons provenant du nez ou de la bouche. Certaines etudes ont ete faites sur l'elimination urinaire des radionucleides, consideree comme un indice de la contamination interne, et l'analyse de l'urine est la forme la plus repandue de l'analyse d'echantillons biologiques. Cependant, le caractere fragmentaire des donnees de reference dont on dispose sur le metabolisme des radionucleides constitue toujours une faiblesse tres grave et il faut faire d'urgence beaucoup de travaux de recherche dans ce domaine. Dans cette etude, on a classe les differents modes de metabolisme des divers radionucleides en trois grandes categories. Le premier groupe comprend les radionucleides repartis dans l'ensemble du corps de facon assez uniforme. Le deuxieme groupe comprend ceux qui se concentrent particulierement dans un ou plusieurs organes. Le troisieme groupe, qui est en fait un sous-groupe particulierement important du deuxieme, comprend les osteophiles. Le regime de relimination urinaire, et notamment sa relation avec la dose de rayonnements recue par les tissus critiques, sont nettement differents pour ces divers groupes. Les auteurs insistent sur le fait qu'apres la constatation d'une absorption accidentelle il convient d'analyser une serie soigneusement constituee d'echantillons d'urines, afin d'obtenir une mesure du taux d'elimination urinaire pour une periode convenable. On peut ainsi reduire au minimum le degre d'approximation inherent a chaque resultat individuel, du aux fluctuations du taux d'elimination. Dans le cas de prelevements d'echantillons en vue de la surveillance reguliere d'un groupe d'employes, on prppose d'aborder la question de facon quelque peu differente. On utilise les donnees dont on dispose sur le metabolisme du radionucleide pour evaluer un niveau d'alarme, a savoir le taux d'elimination urinaire correspondant a un niveau choisi de la charge corporelle. Il importe d'effectuer avec le maximum de precision les' mesures destinees a la surveillance reguliere et de les noter soigneusement. Ces mesures peuvent etre utilisees beaucoup plus tard pour faire une evaluation de la charge corporelle resultant d'une exposition chronique. (author) [Spanish] Para determinar la dosis de irradiacion en el organo critico, debida a contaminacion interna por un radionuclido, es preciso medir la cantidad del mismo contenida en el organismo. Cuando dicho radionuclido emite rayos gamma, X o radiaciones beta que generan radiaciones de frenado, es posible medir directamente la carga corporal con un antropogamma- metro, pero cuando las radiaciones emitidas no se prestan para ello, hay que calcular la carga corporal basandose en la medicion de la cantidad de radionuclidos contenida en las excreciones, la sangre, el aliento o en frotis tomados de la.nariz o de la boca. Se ha estudiado con bastante detalle la excrecion de radionuclidos por via urinaria como indice de la contaminacion interna, siendo el analisis de orina el procedimiento de bioanalisis mas comunmente practicado. Con todo, la escasez de datos disponibles sobre el metabolismo de los radionuclidos constituye aun un grave inconveniente y se estima necesario realizar con caracter urgente nuevas y amplias investigaciones. En la memoria, las diferentes modalidades de metabolismo de los radionuclidos se clasifican en 3 grandes grupos. El primer grupo comprende los radionuclidos que se distribuyen de manera relativamente uniforme por todo el organismo. El segundo incluye los que se concentran particularmente en uno o mas organos. El tercer grupo, que constituye en realidad un subgrupo particularmente importante del segundo, comprende los radionuclidos osteofilos. El regimen de excrecion por via urinaria y, en particular, su relacion con la dosis de irradiacion recibida por los tejidos criticos es netamente diferente para cada uno de estos grupos. Se recalca que una vez detectada una absorcion accidental, conviene analizar una serie de muestras de orina tomadas con arreglo a un plan detallado, para poder medir la velocidad de excrecion por via urinaria durante un periodo adecuado. De este modo se consigue reducir al minimo la indeterminacion inherente a cada resultado aislado, debida a fluctuaciones de la velocidad de excrecion. En el caso del muestreo de la orina para la vigilancia regular de un grupo de personas; se sugiere un metodo algo diferente. Los datos disponibles acerca del metabolismo del radionuclido se utilizan para calcular un nivel de investigacion, a saber, la velocidad de excrecion por via urinaria correspondiente a un nivel determinado de la carga corporal. Es esencial que las mediciones para la vigilancia regular se efectuen con la maxima precision y se registren minuciosamente. Tales mediciones pueden utilizarse mucho tiempo despues para evaluar la carga corporal cronica.
