Abstract

In nuclear endocrinology there are two major problems connected with the determination of renal excretion of iodine in the form of organic compounds labelled in vitro, namely: (a) their quantitative separation from the iodide also present in urine, even if the concentration of the iodide is twenty times greater; and (b) the determination of their low-level radioactivity in large-volume urine samples, their concentration per millilitre of urine being approximately 10{sup -5} of the dose administered which, in turn, must be as low as possible. Classically, separation is carried- out by paper chromatography, for which only small samples of urine are used. This technique is laborious and requires time and equipment for chromatography. Determination of radioactivity on paper involves contamination of the entire band. In spite of correction, only 89.9% of the radioactivity is recovered, and even less when an extract of butanol is used. With the technique described by the authors, a large volume of urine sample is passed through a microcolumn of H{sup +} cation-exchange resin. The organic compounds of the radioiodine remain fixed on the cation- exchange resin and their radioactivity is measured by means of a Nal(Tl) crystal well-type scintillator. On verification for the given conditions, we found the recovery of the urine diiodotyrosine complete, with a standard error averaging 0.42% for 10 experiments. This method has been used for determining the deiodation of diiodotyrosine in 11 patients with normal deiodation and in 10 cretins with goitre, of whom five showed a deficiency of deiodation. The dose administered was 1 {mu}Ci of diiodotyrosine{sup -131}L The method allows for the administration of a radioactive dose at least 50 times lower than in classical methods, and makes possible the separation of diiodotyrosine and its recovery from a sample of 40-50 ml of urine (with a standard error of 0.42% in a period of a few hours) using Nal(Tl) well-type crystal scintillation equipment, which is currently available in nuclear medical laboratories. (author) [French] Dans l'endocrinologie nucleaire deux problemes importants se posent en ce qui concerne la determination de l'excretion renale de l'iode sous forme de composes organiques marques in vitro: a) leur separation quantitative de l'iodure coexistant dans l'urine, meme si la concentration de ce dernier est vingt fois plus grande; b) la determination de leur faible radioactivite dans des echantillons d'urine de grand volume, leur concentration par millilitre d'urine etant approximativement 10{sup -5} fois celle de la dose administree, dont la radioactivite doit etre, a son tour, aussi faible que possible. La separation s'effectue habituellement par chromatographie sur papier, pour laquelle on n'utilise que de petits echantillons d'urine. Cette technique, qui exige des appareils pour chromatographie, est d'une application difficile et longue. Elle implique une contamination de toute la bande de papier. Malgre les corrections apportees, la recuperation de la radioactivite n'est que de 89,9%, et moins encore lorsqu'on utilise un extrait de butanol. La technique des auteurs consiste a faire passer un echantillon de grand volume d'urine a travers une microcolonne d'un cationique H{sup +}. Les composes organiques du radioiode restent tixes sur le cationique et leur radioactivite est mesuree a l'aide d'une sonde a scintillation a cristal de Nal(Tl) creux. La recuperation de la diiodotyrosine de l'urine a ete verifiee dans des conditions donnees; on 1'a trouvee complete, avec une erreur standard de 0,42% sur la moyenne de dix experiences. Cette methode a ete appliquee pour determiner la desiodation de la diiodotyrosine chez onze patients presentant une desiodation normale et chez dix cretins avec goftre, dont cinq ont montre un deficit de desiodation. (author) [Spanish] La determinacion de la excrecion renal de yodo en forma de compuestos organicos marcados in vitro plantea dos serios problemas en endocrinologia nuclear: a) su separacion cuantitativa del yoduro tambien presente en la orina, incluso cuando la concentracion de este ultimo es veinte veces mayor; b) la determinacion de su debil actividad enmuetras de orina de gran volumen, y que su concentracion por mililitro de orina representa aproximadamente 10{sup -5} de la radiactividad administrada que, a su vez, debe ser lo mas baja posible. La separacion suele efectuarse por cromatografia sobre papel, para lo cual solo se utilizan pequenas muestras de orina. Esta tecnica es laboriosa y exige mucho tiempo y aparatos para cromatografia. Al proceder a la determinacion de la radiactividad sobre el papel, se encuentra una contaminacion de toda la tira. A pesar de las correcciones aplicadas, se recupera solamente el 89,9% de la radiactividad, y atin menos si se emplea un extracto butanolico. Los autores recurren a una tecnica consistente en hacer pasar un gran volumen de orina por una micro- columna de resina cationica en su forma hidrogenada. Los compuestos organicos del radioyodo quedan retenidos en la resina y se determinan midiendo la radiactividad de la columna con una sonda de centelleo de cristal de Nal hueco. Los autores comprobaron que la recuperacion de la diyodotirosina de la orina es practicamente cuantitativa, siendo el error tipo de 0,42% para diez experimentos. El metodo se ha empleado para determinar la desyodacion de la diyodotirosina en 11 pacientes que presentan una desyodacion normal y en 10 cretinos con bocio, en cinco de los cuales la desyodacion era deficiente. Se administro 1 {mu}Ci de diyodotirosina-{sup 131}I. Posle proverki dlja zadannyh uslovij rekuperacii diiodotirozina mochi my nashli molnuju rekuperaciju so standartnymi pogreshnostjami v srednem 0,42% dlja 10 jeksperimentov . Jetot metod primenjalsja dlja opredelenija deziodizacii diiodotirozina u 11 pacientov s normal'noj deziodizaciej i u 10 kretinov s zobom, u 5 iz kotoryh byl obnaruzhen deficit deziodizacii. Vvedennaja aktivnost' sostavljala 1 mkkjuri diiodotirozina, mechennogo jodom-131. Opisyvaemyj metod pozvoljaet vvodit' po krajnej mere v 50 raz men'shuju dozu aktiv - nosti, chem pri obychnyh metodah, razdeljat' diiodotirozin i obespechivat' ego rekuperaciju iz obrazca s ob{sup e}mom 40-50 ml mochi so standartnoj pogreshnost'ju porjadka 0,42%v techenie neskol'kih chasov.
