Abstract

In order to obtain a better picture of the structural details of the diseased parenchyma of the liver, we have been using a device which has been adapted for use with the Scanner- Tracerlab and which produces liver scans made up of seven colours. The colours are chosen arbitrarily, each one corresponding to a given number of counts and representing zones of isoradioactivity, i.e. zones of liver tissue of relatively equivalent volume. The white in these colour scintigrams represents the radiation background and the black corresponds to the peak activity in the centre of the liver, where the parenchyma is thickest. The colours in between correspond to the different zones of isoradioactivity. This method has been used to examine 150 patients suffering from malignant (primary and secondary) and benign tumours, 80 patients suffering from chronic hepatitis and cirrhosis and 50 normal subjects. In most of the cases studied, intravenous injections of colloidal gold-198 (Amersham, UK) were used. In numerous cases parallel studies were carried out with mechanical scans, black-and-white photoscintigrams and colour scintigrams. Diagnoses were checked by means of punctures, laparoscopy, laparophotography or cinematography, biopsic punctures, surgery and necropsy. (author) [French] Pour obtenir des images qui refletent mieux les details de structure du parenchyme hepatique modifie par la maladie, les auteurs ont adapte au Scanner-Tracerlab un dispositif a l'aide duquel on obtient des cartes hepatiques en sept couleurs. Chaque couleur, choisie arbitrairement, correspond a un nombre d'impulsions et represente des zones d'isoradioactivite, c'est-a-dire de tissu hepatique de volume relativement egal. Dans le scintigramme en couleurs, le blanc represente la radioactivite de fond, tandis que le noir represente l'activite maximum, situee au centre du foie ou le parenchyme est plus epais. Les couleurs intermediaires correspondent aux differentes zones d'isoradioactivite. Les auteurs ont etudie, a l'aide de cette methode, 150 cas de tumeurs hepatiques malignes (primitives et secondaires) et de tumeurs benignes, 80 cas d'hepatite chronique et de cirrhose et 50 cas normaux. Dans la majorite descas, ils ont utilise l'or-198 colloiedal (Amersham-Angleterre) injecte par voie intraveineuse. Ils ont effectue parallelement, dans un grand nombre de cas, des mecanogrammes, photoscintigrammes en blanc et noir et scintigrammes en couleurs. (author) [Spanish] Para obtener imagenes que reflejasen mejor los detalles de la estructura del parenquima hepatico modificado por la enfermedad, los autores adaptaron al aparato Scanner-Tracerlab un dispositivo gracias al cual se pueden obtener diagramas hepaticos en siete colores. Cada color, elegido arbitrariamente, corresponde a un numero dado de impulsos y representa zonas de isorradiactividad, es decir, de tejido hepatico de volumen relativamente igual. En el centelleograma policromo, el blanco representa la radiactividad de fondo, mientras que el negro representa la actividad maxima, situada en el centro del higado, donde el parenquima es mas espeso. Los colores intermedios corresponden a las diferentes zonas de isorradiactividad. Recurriendo a este metodo, los autores estudiaron 150 casos de tumores hepaticos malignos (primitivos y secundarios) y de tumores benignos, 80 casos de hepatitis cronica y de cirrosis, y 50 casos normales. En la mayoria de los casos utilizaron el oro-198 coloidal (Amersham -Reino Unido) inyectado por vía endovenosa. En un gran numero de casos, efectuaron paralelamente mecanogramas, fotocentelleogramas en blanco y negro y centelleogramas policromos. Verificaron el diagnostico mediante puncion, laparoscopia, laparofotograffa o cinematografia, puncion biopsica, intervencion quirurgica o necropsia. El metodo centelleografico policromo permite distinguir mejor las variaciones de intensidad de la radiactividad, es decir, da una idea mas cabal de la desorganizacion o sustitucion del parenquima hepatico provocadas por los procesos tumorales.
