Abstract

The characteristic parameters of the metabolism of the two thyroid hormones (biological half-life, renewal rate, dilution space, pool, quantity of hormones consumed daily) are studied after the simultaneous intravenous injection of 200 {mu}Ci of {sup 125}I-T4 and {sup 131}I-T3, The plasma samples are first subjected to chromatography so as to separate the hormonal fraction from the iodides. Studies are then made on 10 control subjects, three subjects with hyperthyroidism, six with hypothyroidism, 41 diabetics, two subjects with nephrotic syndromes and five with renal insufficiency. In the case of the subjects with thyroid diseases, analysis of the results,reveals that, relative to the control subjects, there is a significantly reduced daily consumption of hormones on the part of the subjects with hypothyroidism and a significantly enhanced consumption on the part of those with hyperthyroidism. A parallel study of the thyroxine binding capacity of TBG and TBPA suggests an interpretation of these differences, which appear to be inversely proportional to the amount of thyroxine bound to the carrier proteins of the plasma. In alimentary diabetes, insulin therapy increases the amount of T4 and (even more so) T3 consumed daily, whereas hypoglycaemic sulphamides do not appear to have any effect. In the subjects with nephrotic syndromes, the T4 renewal rate is enhanced while the renal clearance is very high, in contrast to that of T3, which is not as closely bound to the proteins. In renal insufficiency, the renewal rate of T4 is greatly reduced while its dilution space and pool are enhanced. These results indicate the value of this technique both in the diagnosis of thyroid diseases and in the physiopathological interpretation of anomalies of the thyroid function in various metabolic diseases. (author) [French] Les parametres caracteristiques du metabolisme des deux hormones thyroiediennes (periode biologique, vitesse de renouvellement, espace de dilution, pool, quantite quotidiennement consommee d'hormones), sont etudies apres injection intraveineuse simultanee de 200 {mu}Ci de {sup 125}I T{sub 4} et de {sup 131}I T3. Les echantillons de plasma sont soumis a une chromatographie prealable pour separer la fraction hormonale des iodures. Sont ainsi etudies 10 sujets temoins, 3 hyperthyroidiens, 6 hypothroidiens, 41 diabetiques, 2 syndromes nephrotiques, 5 insuffisants renaux. Dans les maladies thyroiediennes, l'analyse des resultats confirme une difference significative entre les sujets temoins, les hypo et les hyperthyroidiens chez lesquels la consommation quotidienne d'hormones est tres abaissee ou au contraire augmentee. L'etude parallele de la capacite de transport de la thyroxine par la TBG et la TBPA autorise l'interpretation de ces variations qui semblent inversement proportionnelles a la quantite de thyroxine liee aux proteines vectrices du plasma. L'interet pratique de ce test doit etre souligne chez les sujets soumis as une surcharge par produit de contraste organo-iode et dans les formes atypiques de la maladie ou les investigations biologiques classiques sont d'interpretation difficile. Dans le diabete sucre, l'insulinotherapie augmente la quantite quotidiennement consommee de T{sub 4} et surtout de T{sub 3} tandis que les sulfamides hypoglycemiants ne paraissent avoir aucune incidence. Dans les syndromes nephrotiques, la vitesse de renouvellement de la T{sub 4} est accrue tandis que la clearance renale de cette hormone est tres elevee, a l'inverse de celle de la T{sub 3} moins liee aux proteines. Au cours de l'insuffisance renale, la vitesse de renouvellement de la T{sub 4} est tres abaissee tandis que son espace de dilution et son pool sont accrus. Ces resultats confirment l'interet de cette technique tant pour le diagnostic des maladies thyroidiennes que; pour l'interpretation physiopathologique des anomalies de la fonction thyroiedienne dans diverses affections metaboliques.
