Abstract

163 employees of an insulating firm processing mainly crocidolite agreed to an industrial medicine internal medical check-up. The minimum exposure time was one day, whith a median of 3 years and a maximum time of 41 years. The period since the first occupational exposure to asbestos was between 3 and 49 years. After having been given a detailed medical explanation, 112 of the 163 examined employees agreed to undergo a sputum examination. In total 411 specimens reached the cytopathological diagnostics. 44 of 93 specially addressed employees underwent a follow-up examination after about one year. 162 further sputum specimens could be diagnosed in this manner. The specimen fraction without cells of the low respiratory tract was approx. 24% during the first examination and 27% during the follow-up examination. Established tumour-cell-negative findings without squamous metaplasias or dysplasias were obtained in approx. 14% during the first examination and in approx. 16% during the follow-up examination. Minor dysplasias were proved during the first examination in approx. 8%, during the follow-up examination in approx. 5%. Optional precancerous stages were found during the first examination in approx. 8%, during the follow-up examination in approx. 2%. The present possibilities and limits of the methodology are shown with regard to the insured person groups which are known at the central registration office for asbestos dust workers of the General Association of the commercial trade cooperative associations; consequences are derived for the decision-making committees of the commercial trade cooperative associations. (orig./MG). [Deutsch] 163 Mitglieder einer vorwiegend Krokydolith verarbeitenden Isolierfirma unterzogen sich einer arbeitsmedizinisch-internistischen Vorsorgeuntersuchung. Die Mindest-Gefaehrdungsdauer betrug einen Tag bei einem Median von drei Jahren und einem Maximalwert von 41 Jahren. Der Zeitraum seit der erstmaligen Asbestgefaehrdung vom Arbeitsplatz, lag zwischen drei und 49 Jahren. Nach eingehender aerztlicher Erlaeuterung folgten 112 der 163 untersuchten Kohortenmitglieder dem Angebot zur Sputumuntersuchung. Insgesamt 411 Proben gelangten zur zytopathologischen Diagnostik. Nach etwa einem Jahr folgten 44 von 93 angeschriebenen Kohortenmitgliedern einer Wiederholungsuntersuchung. Hierdurch konnten 162 weitere Sputumproben diagnostiziert werden. Der Probenanteil ohne Zellen der tiefen Atemwege betrug bei der ersten Untersuchung ca. 24%, bei der Nachuntersuchung ca. 27%. Sicher Tumorzell-negative Befunde ohne Plattenepithel-Metaplasien oder Dysplasien ergaben sich bei der ersten Untersuchung in ca. 44%, bei der Nachuntersuchung in 50%. Plattenepithelmetaplasien fanden sich bei der ersten Untersuchung in ca. 14%, bei der Nachuntersuchung in ca. 16%. Geringe Dysplasien liessen sich bei der ersten Untersuchung in ca. 8%, bei der Nachuntersuchung in ca. 5% nachweisen. Fakultative Praekanzerosen fanden sich bei der ersten Untersuchung in ca. 8%, bei der Nachuntersuchung in ca. 2%. Ergaenzend durchgefuehrte sputumzytologische Untersuchungen bei 17 Patienten mit zurueckliegender Asbestfaserstaub-Gefaehrdung am Arbeitsplatz zeigten einmal fakultativ praekanzeroese und einmal sicher positive Turmorzellen. Es werden die derzeitigen Moeglichkeiten und Grenzen der Methodik im Hinblick auf die bei der Zentralen Erfassungsstelle asbeststaubgefaehrdeter Arbeitnehmer des Hauptverbandes der gewerblichen Berufsgenossenschaften bekannten Versichertengruppen aufgezeigt und Schlussfolgerungen fuer die Entscheidungsgremien der gewerblichen Berufsgenossenschaften gezogen.
