Title: Issue Management Risk Ranking Systems

Abstract

Thousands of safety issues have been collected on-line at the Idaho National Engineering and Environmental Laboratory (INEEL) as part of the Issue Management Plan. However, there has been no established approach to prioritize collected and future issues. The authors developed a methodology, based on hazards assessment, to identify and risk rank over 5000 safety issues collected at INEEL. This approach required that it was easily applied and understandable for site adaptation and commensurate with the Integrated Safety Plan. High-risk issues were investigated and mitigative/preventive measures were suggested and ranked based on a cost-benefit scheme to provide risk-informed safety measures. This methodology was consistent with other integrated safety management goals and tasks providing a site-wide risk informed decision tool to reduce hazardous conditions and focus resources on high-risk safety issues. As part of the issue management plan, this methodology was incorporated at the issue collection level and training was provided to management to better familiarize decision-makers with concepts of safety and risk. This prioritization methodology and issue dissemination procedure will be discussed. Results of issue prioritization and training efforts will be summarized. Difficulties and advantages of the process will be reported.   • As nuclear utility organizations seek higher levels of performance while striving to contain and control costs, a focal point of continuing interest is that of issue management. The definition of issue in this context is the compilation of items that make up the organization's things-to-do list. Issues usually result in some disposition; it may be a report, analysis, work task, or a larger set of work tasks that define a project. Sometimes issues are discarded after a cursory analysis, but this is relatively rare. Issues that appear on a list usually have some action associated with their disposition. Almost allmore » individuals and certainly all organizations have some scheme to prioritize and manage those items relevant to their success. These schemes range from almost invisible and informal to the extremely rigorous and formal. This paper addresses an enhanced issue management approach that blends several desirable features together, and has as its foundation, qualitative and quantitative aspects of probabilistic risk assessment methodology.« less
  • Currently, proliferation risk assessment models are designed to evaluate only a portion of the overall risk, focusing exclusively on either technological or social factors to determine the extent of a threat. Many of these models are intended to act as a means of predicting proliferation potential rather than assessing the system as a whole, ignoring the ability to enhance mitigating factors and manage, rather just establish the presence of, the threat. While the information garnered through these forms of analysis is necessary, it remains incomplete. By incorporating political, social, economic and technical capabilities as well as human factors such asmore » intent into a single, multi-faceted risk management model, proliferation risk can be evaluated more effectively. Framing this information around how to improve and expand the Regime already in place and establishing where there are gaps in the system allows for a more complete approach to risk management, mitigation and resource allocation. The research conducted here seeks to combine all three elements (intent, capability and opportunity) in a comprehensive evaluation which incorporates an assessment of state-level variables, possible proliferation pathways and technical capability. Each portion of the analysis is carried out independently then combined to illustrate the full scope of a State's nuclear infrastructure while showing areas of weakness in the institutional framework.« less
  • Dounreay hot particles (Particles) are small fragments of irradiated nuclear fuel that are present in littoral and marine environments adjacent to the Dounreay nuclear establishment in northern Scotland. The first Particle was identified by UKAEA, the site operator, and recovered from the Dounreay foreshore, in 1983 and a single Particle was recovered from the adjacent, publicly accessible Sandside Beach the following year. It was not until 1996, however, that significant numbers of Particles were identified and recovered. Since that time an extensive research and development programme (described herein) has been undertaken to identify the source of Particles, their movement andmore » lifetimes in the marine environment and their effects on human and environmental health. Particles were released to the North Atlantic Ocean in the mid to late 1960's and early 1970's. There is no evidence of an on-going source of Particles from the Dounreay site. The source of Particles recovered from the Dounreay foreshore and from local beaches is the cache currently residing in marine sediments adjacent to Dounreay. Sediment modelling studies indicate that the Dounreay Particles are generally transported sub-parallel to the coast in a north easterly direction. Studies to define contact frequencies and risks to human health suggest that the health risks associated with Particles are insignificant. There is, however, a significant perception of risk. UKAEA hopes to define a long term Particle management programme via the development of a best practical environmental option (BPEO) facilitated through consultation with all stakeholders.   • Risk assessment methods vary in nature and depth. Their application to the evaluation of information security issues should be decided on the basis of their capability to provide answers to practical and fundamental questions concerning the design and implementation of security controls in specific information systems. Quantitative risk analysis provides an objectively based approach to the problem of assessing and managing risk. As a decision making and risk assessment tool, it is not only capable of identifying potential losses that could be unacceptable for a given system, but it can be used to determine which specific security controls and countermeasuresmore » can be effective and cost justifiable. The Livermore Risk Analysis Methodology (LRAM) was developed to cover these objectives in a balanced and comprehensive way. Its model and procedures, from the identification of valuable assets to the prioritization and budgeting of proposed controls, are examined and discussed both from the technical and from the decision making/risk management perspectives.« less
