On February 12, 1975, an explosion and fire occurred in the denitrator room of the separations A-Line facility, resulting in minor injury to two employees and about $230,000 damage to the building and equipment. The facility, which converts uranyl nitrate solution to UO/sub 3/, had operated for 20 years without major accident. Accidental transfer of tributyl phosphate--uranyl nitrate adduct into a denitrator was followed by rapid decomposition of the organic material. Combustible gases were released into the denitrator room and ignited. No contamination was spread outside the facility. A-Line returned to operation August 11, 1975, after equipment and procedures were modified to lower the probability of similar incidents to a very low level. 18 figures, 9 tables.
