During the report period, there were two abnormal occurrences at the nuclear power plants licensed to operate. One involved a misalignment of a high head safety injection isolation valve. The other involved a failure of the high pressure safety injection system. There were two abnormal occurrences at other licensee facilities. Both involved calculated radiation exposures in excess of 10 CFR 20 limits. There were two abnormal occurrences reported by the Agreement States. One involved excessive radiation doses to hospital patients. The second involved overexposures of a radiographer and two barge crew members. The report also contains information updating some previously reported abnormal occurrences.
