Title: Passive exposures of children to volatile trihalomethanes during domestic cleaning activities of their parents

Abstract

Domestic cleaning has been proposed as a determinant of trihalomethanes (THMs) exposure in adult females. We hypothesized that parental housekeeping activities could influence children's passive exposures to THMs from their mere physical presence during domestic cleaning. In a recent cross-sectional study (n=382) in Cyprus [41 children (<18y) and 341 adults (≥18y)], we identified 29 children who met the study's inclusion criteria. Linear regression models were applied to understand the association between children sociodemographic variables, their individual practices influencing ingestion and noningestion exposures to ΣTHMs, and their urinary THMs levels. Among the children-specific variables, age alone showed a statistically significant inverse association with their creatinine-adjusted urinary ΣTHMs (r{sub S}=−0.59, p<0.001). A positive correlation was observed between urinary ΣTHMs (ng g{sup −1}) of children and matched-mothers (r{sub S}=0.52, p=0.014), but this was not the case for their matched-fathers (r{sub S}=0.39, p=0.112). Time spent daily by the matched-mothers for domestic mopping, toilet and other cleaning activities using chlorine-based cleaning products was associated with their children's urinary THMs levels (r{sub S}=0.56, p=0.007). This trend was not observed between children and their matched-fathers urinary ΣTHMs levels, because of minimum amount of time spent by the latter in performing domestic cleaning. The proportion of variance ofmore » creatinine-unadjusted and adjusted urinary ΣTHMs levels in children that was explained by the matched-mothers covariates was 76% and 74% (p<0.001), respectively. A physiologically-based pharmacokinetic model adequately predicted urinary chloroform excretion estimates, being consistent with the corresponding measured levels. Our findings highlighted the influence of mothers' domestic cleaning activities towards enhancing passive THMs exposures of their children. The duration of such activities could be further tested as a valid indicator of children's THMs body burden. - Highlights: • First report on THMs exposure assessment in matched parents and children. • Duration of domestic cleaning by mothers influenced passive exposure to THMs in children. • Matched-fathers did little cleaning and thereby no contribution to passive exposure to THMs in children. • Reverse dosimetry showed a good agreement between predicted and observed urinary chloroform. • Passive exposures to THMs require new attention in survey questionnaires and epidemiology.« less

  • Personal exposures and breath concentrations of approximately 20 volatile organics were measured for 200 smokes and 322 nonsmokers in New Jersey and California. Smokers displayed significantly elevated breath levels of benzene, styrene, ethylbenzene, m + p-xylene, o-xylene, and octane. Significant increases in breath concentration with number of cigarettes smoked were noted for the first four aromatic compounds. Based on direct measurements of benzene in mainstream cigarette smoke, it is calculated that a typical smoker inhales 2 mg benzene daily, compared to 0.2 ng/day for the nonsmoker. Thus, cigarette smoking may be the most important source of exposure to benzene formore » about 50 million citizens of the United States. Passive smokers exposed at work had significantly elevated levels of aromatics in their breath. Indoor air levels in homes with smokers were significantly greater than in nonsmoking home during fall and winter but not during spring and summer. The average annual increase in homes with smokers was 3.6 ..mu..g/m/sup 3/ for benzene and 0.5 ..mu..g/m/sup 3/ for styrene-an approximate 50% relative increase in each case. Thus, exposure to benzene and styrene may be increased for the approximately 60% of children and other nonsmokers living in homes with smokers.« less
  • A dynamic exposure chamber was constructed to evaluate the performance of the 3M 3520 organic vapor monitor when exposed during 24 h to combined test atmospheres of benzene, 1,3-butadiene, carbon tetrachloride, chloroform, 1,3-dichlorobenzene, methylene chloride, styrene, tetrachloroethylene, and toluene at target concentrations of 10, 20, and 200 {micro}g/m{sup 3} in combination with temperatures of 10, 25, and 40 C and relative humidities of 12, 50, and 90%. These conditions are generally representative of the range of community air environments, both indoor and outdoor. The system consists of five distinct units: (1) dilution air delivery, (2) humidification, (3) VOC generation andmore » delivery, (4) mixing chamber, and (5) exposure chamber. High-emission permeation tubes were utilized to generate the target VOCs. Both the target temperatures and humidities were achieved and maintained for multiple consecutive days. The variation of the temperature in the exposure chamber was controlled within {+-}1 C, while relative humidity was controlled within {+-}1.5% at 12% RH, {+-}2% at 50% RH, and {+-}3% at 90% RH. Under constant preset temperatures and stable nitrogen flow through the VOC generation unit, various temporal patterns of permeation rates were observed over time. The lifetimes and permeation rates of the tubes differed by compound, length of the tube, and manufacturer.« less
  • The performance of the 3M 3520 organic vapor monitor (OVM) as a tool for monitoring inhalation exposures to volatile organic compounds (VOCs) in nonoccupational community environments was evaluated by using combined controlled test atmospheres of benzene, 1,3-butadiene, carbon tetrachloride, chloroform, 1,4-dichlorobenzene, methylene chloride, styrene, tetrachloroethylene, and toluene. Eight OVMs were simultaneously exposed to concentrations of 10,20, and 200 {micro}g/m{sup 3} in combination with temperatures of 10, 25, and 40 C and relative humidities of 12, 50, and 90% for 24 H. The results of this study indicate that the performance of the 3520 OVM is compound-specific and depends on concentration,more » temperature, and humidity. With the exception of 1,3-butadiene under most conditions and styrene and methylene chloride at very high relative humidities, recoveries showed a negative bias as compared to calculated chamber concentrations but were generally within {+-}25% of theory, indicating that the 3520 OVM can be effectively used over the range of concentrations and environmental conditions tested with a 24-h sampling period. Increasing humidities resulted in increasing negative bias from full recovery. Reverse diffusion experiments conducted at 200 {micro}g/m{sup 3} and five temperature/humidity combinations indicated diffusion losses only for 1,3-butadiene, methylene chloride, and styrene under increased humidity conditions. The recovery rates reported in this study can be used for estimating measurement biases when using OVMs for indoor, outdoor, and personal air monitoring of VOCs in community environments.« less
  • The All-Inclusive Wraparound Scheme seeks to develop new ways of multidisciplinary, interagency working that will make a difference to the lives of children with disabilities (aged 0-18 years) and their families and carers across the Southern Health and Social Services Board in Northern Ireland. A concurrent HIA was undertaken with the aims of identifying the positive and negative health impacts, producing clear recommendations to improve the service and informing the development of an evaluation and monitoring framework. Eight community-based projects linked to 15 partners were involved in the HIA. Health and social services, education, the Library Board, voluntary sector communitymore » groups, parents and children all took part. It was concluded that there was a need for better information for parents to enable them to access services. Partnership working with parents and between agencies needed to be improved and parents and professionals on partnership working. The handover between services at transition from preschool to school-age and from school-age to adult services needed to be better planned. Conducting a HIA early in the implementation of Wraparound has enabled the project to take account of more perspectives and influence the delivery of services for children with a disability.« less
