This report details findings from a survey of well integrity experiences elicited from operators of geologic carbon storage (GCS) and carbon dioxide enhanced oil recovery (CO2-EOR) sites around the world. The survey consisted of 41 questions organized in four sections and its goal was to obtain information about site characteristics and operator experiences with well integrity, monitoring methods, and risk assessment of legacy wells. Maintaining well integrity is critical to the success of geologic carbon storage (GCS) and carbon dioxide enhanced oil recovery (CO₂-EOR) operations. Wells that experience leakage because of integrity issues can potentially become a risk to the environment or human health if they release previously captured CO₂ back into the atmosphere or into freshwater aquifers. There are many GCS and CO₂-EOR sites in operation around the world. However, well integrity experiences at these sites are not widely documented in the public domain. Among the various risks associated with CO₂ storage in deep geologic formations, wells are important potential pathways for fluid leaks and groundwater contamination. From approximately 1949 until 1970, the Commodity Credit Corporation of the U.S. Department of Agriculture (CCC/USDA) operated a grain storage facility on federally owned property approximately 0.25 mi northwest of Savannah, Missouri (Figure 1.1). During this time, commercial grain fumigants containing carbon tetrachloride were commonly used by the CCC/USDA and the private grain storage industry to preserve grain in their facilities. This report summarizes progress of LANL activities related to the tasks performed under the LANL FWP FE102-002-FY10, National Risk Assessment Partnership (NRAP): Industrial Carbon Capture Program. This FWP is funded through the American Recovery and Reinvestment Act (ARRA). Overall, the NRAP activities are focused on understanding and evaluating risks associated with large-scale injection and long-term storage of CO{sub 2} in deep geological formations.
