Instructions

Complete all data elements on the form and click on the "Submit" button to send to the U.S. Department of Energy, Office of Scientific and Technical Information. The information provided is used to report on-going DOE-sponsored research to the DOE, U.S. Government, and public sectors.

  1. CONTRACT NUMBER. The DOE Contract or Grant number under which the work is being performed. Check the correct block for new contract or revision of a prior contract.

    NOTE: Checks will be performed to verify that projects marked as "new" do not exist and projects marked as a "revision" already exist. Additionally, all revisions must include the original and revised data.

  2. FUNDING MECHANISM. Please check the correct block indicating whether this project is being identified as a "Contract" or "Grant".

  3. PROJECT TITLE. Be as specific as possible. Use the words that are descriptive of the work done.

  4. PROJECT DESCRIPTION. Include objectives, approach, and expected results. Quantify where possible.

  5. PRINCIPAL INVESTIGATOR.

  6. ASSOCIATED REFERENCE NUMBER. Other DOE-related or Internal numbers which may be used to identify this project or information associated with it.

Please be aware that information submitted, will be marked as "Incomplete" until it is reviewed by the appropriate personnel within the U.S. Department of Energy, Office of Scientific and Technical Information.
