The occurrence of certain potential events in nuclear fuel reprocessing plants could lead to significant consequences involving risk to operating personnel or to the general public. This document is a compilation of such potential initiating events in nuclear fuel reprocessing plants. Possible general incidents and incidents specific to key operations in fuel reprocessing are considered, including possible causes, consequences, and safety features designed to prevent, detect, or mitigate such incidents.
