Abstract

After an accident in a chemical plant in Seveso, Italy, on July 10, 1976, 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) spread over a populated area. The event was exceptional because children were also affected and because the contamination took place not only through direct exposure but also through inhalation and the ingestion of contaminated foods, especially fruits and vegetables. This paper illustrates the early dermatologic lesions, the late acneic (chloracne) lesions, and their evolution during a 10-year period. Peculiar cutaneous findings, histologic data, and a comparison with previously reported similar accidents are also included.

Following a major environmental accident near Seveso, Italy, on July 10, 1976, we attempted to determine if the 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) released into the atmosphere had any effect on the liver function and lipid metabolism of exposed children. 2,3,7,8-Tetrachlorodibenzo-p-dioxin levels (TCDD) were measured in serum specimens from Seveso, Italy, residents, who were potentially highly exposed to the 1976 explosion, and in controls. The residents were chosen so as to represent those who did and did not develop chloracne. Levels of TCDD as high as 56,000 parts per trillion (ppt) were found in these serum specimens that were collected in 1976. These TCDD levels are the highest ever reported, and yet almost all clinical laboratory tests on these individuals were normal; any abnormal test result was only transitory in nature. On Saturday, July 10, 1976, an explosion occurred during the production of 2,4,5-trichlorophenol in a factory north of Milan, Italy. A cloud of toxic material was released and included 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD). The size of the contaminated area was estimated primarily by measuring TCDD in the soil; additional criteria included the presence of dead animals and detection of dermal lesions among persons in the area. Findings from selected dose-response experimental studies on 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) carcinogenic action in rodents were utilized for low-dose lifetime cancer risk estimation in man. The Weibull, Armitage-Doll, and linearized Armitage-Doll mathematical models were employed in this work. Conversion factors, based on body surface, of 1/5 and 1/11 were also employed for rat- and mouse-to-man extrapolation. Conservative estimates from the linearized Armitage-Doll upper 99% confidence limit associate a 10(-5) risk probability level to TCDD daily doses of 0.15-2.4 pg/kg body wt., while levels of 10(-6) and 10(-7) are associated with uptakes 10- and 100-fold lower, respectively. Toxicological significance of 2,3,7,8-tetrachlorodibenzo-p-dioxin and related compounds in human adipose tissue

Reports of polychlorinated dibenzo-p-dioxins (PCDDs) and polychlorinated dibenzo-furans (PCDFs) in human tissues were reviewed to assess their toxicological significance. The predominance of 2,3,7,8-chlorinated congeners in human tissues and in the food chain, but not in other environmental matrices, suggests that the food chain is the major source of human residues. Exposures to unique distributions of congeners can result in recognizable patterns of excess 2,3,7,8-chlorinated PCDDs/PCDFs in humans.
