Assessment of hand blood flow: A modified technique
Radionuclide blood flow studies are frequently used in conjunction with static bone imaging of the hands to obtain valuable information regarding relative vascularity. This study was undertaken to demonstrate; a significant blood flow artefact with the conventional bolus injection technique (Tc-99m MDP) used in dynamic hand imaging, and a modified technique which virtually eliminates this artefact. The artefact is presumably due to reactive hyperemia induced by the tourniquet used to occlude venous return prior to injection. A modified technique was developed using intermittent injections of saline via a butterfly needle to maintain venous patency. This allows time for the reflex hyperemia to return to the basal state before injecting the radiopharmaceutical. Only 1 of 23 additional subject (4%) demonstrated significant artefactual asymmetry with the modified technique. A highly significant different (rho < .001) was observed between the two techniques. Additionally, there was no significant difference (rho = .50) between the observed pattern with the modified technique and the expected normal symmetrical pattern. Therefore, use of this technique will eliminate a potentially seriously misleading artefact.
