Abstract

In-111-leukocyte scanning has recently been introduced as a clinically effective method for detecting inflammatory disease and abscesses. The authors present six cases that demonstrate the usefulness of this new technique in suspected inflammatory renal disease. Two patients had renal abscesses, two had acute pyelonephritis, one had acute focal bacterial nephritis (acute lobar nephronia), and one had a transitional cell carcinoma with associated acute and chronic inflammation.

Indium-111 oxine, polymorphonuclear cells isolated and labeled with /sup 111/In were used for studying abscesses and inflammatory conditions. There were 64 total scans done in 59 patients, 32 male and 27 female, aged 3 to 81 years (average, 51). The original clinical diagnosis was abscess in 33 patients. The whole blood cell scan was abnormal in 12 (36%) of these, and a good clinical correlation was obtained in 11 of the 12. Distribution of leukocytes labeled with In-111 oxine in dogs with acute inflammatory lesions

The biodistributions of In-111 oxine (with and without leukocyte labeling) of Ga-67 citrate and of In-111 chloride were compared in 30 dogs with chemical and bacterial abscesses and acute joint inflammation. Serial blood samples were taken and tissues radioassayed at 24 hr. The concentration of In-111-oxine leukocytes in all three types of inflammatory lesion was invariably much higher than that of Ga-67 injected simultaneously. For bacterial abscesses, the mean abscess-to-muscle concentration ratio was 3000 for labeled leukocytes and 72 for Ga-67. Limitations of indium-111 leukocyte scanning in febrile renal transplant patients

Indium-111-labeled leukocyte scanning was evaluated as a technique for investigating possible abscess as the cause of fever in 10 renal allograft recipients under therapy for rejection, acute tubular necrosis, or urinary infection. The usefulness of the method in this setting was found to be limited by marked nonspecificity of renal, pulmonary, and other focal leukocyte accumulation. Although wound infections were correctly identified, false-positive scans resulted in multiple nonproductive consultations and radiologic procedures (some invasive) and contributed to the decision to perform one negative exploratory laparotomy. This study was designed to evaluate the role of /sup 111/In-labeled leukocyte imaging and fecal excretion in the assessment of inflammatory bowel disease. We compared these tests to various indices of disease activity in Crohn's disease, to Truelove's grading in ulcerative colitis, and to endoscopy, x-ray, and pathology in both diseases. In many institutions, the use of /sup 111/In white blood cell (WBC) scans have become routine for abscess detection. Its use for the detection of bone and joint inflammation, renal disease, and myocardial infarction has also been reported. Two patients are presented whose /sup 111/In studies were of major clinical value in the management of their inflammatory bowel disease. In one, the diagnosis was suggested, and in the second, the extent of disease was defined.
