    The U.S. Department of Energy (DOE) Office of Independent Oversight and Performance Assurance (OA) conducted an inspection of environment, safety, and health (ES&H) programs at the DOE Pantex Plant during January and February 2005. The inspection was performed by the OA Office of Environment, Safety and Health Evaluations. OA reports to the Director of the Office of Security and Safety Performance Assurance, who reports directly to the Secretary of Energy. This volume of the report provides four technical appendices (C through F) containing detailed results of the OA review. Appendix C provides the results of the review of the application of the core functions of ISM for Pantex Plant work activities. Appendix D presents the results of the review of NNSA, PXSO, and BWXT feedback and continuous improvement processes and management systems. Appendix E presents the results of the review of essential safety system functionality, and Appendix F presents the results of the review of safety management of the selected focus areas.

    Consolidated Nuclear Security (CNS) Pantex took the initiative to organize a Review Panel of subject matter experts to independently assess the adequacy of the Pantex Tripping Man Analysis methodology. The purpose of this report is to capture the details of the assessment including the scope, approach, results, and detailed Appendices. Along with the assessment of the analysis methodology, the panel evaluated the adequacy with which the methodology was applied as well as congruence with Department of Energy (DOE) standards 3009 and 3016. The approach included the review of relevant documentation, interactive discussion with Pantex staff, and the iterative process of evaluating critical lines of inquiry.

    This document contains the findings and associated root causes identified during the Tiger Team Assessment of the Department of Energy's (DOE) Pantex Plant in Amarillo, Texas. This assessment was conducted by the Department's Office of Environment, Safety and Health between October 2 and 31, 1989. The scope of the assessment of the Pantex Plant covered all areas of environment, safety and health (ES H) activities, including compliance with federal, state, and local regulations, requirements, permits, agreements, orders and consent decrees, and DOE ES H Orders. The assessment also included an evaluation of the adequacy of DOE and site contractor ES H management programs. The draft findings were submitted to the Office of Defense Programs, the Albuquerque Operations Office, the Amarillo Area Office, and regulatory agencies at the conclusion of the on-site assessment activities for review and comment on technical accuracy. Final modifications and any other appropriate changes have been incorporated in the final report. The Tiger Team Assessment of the Pantex Plant is part of the larger Tiger Team Assessment program which will encompass over 100 DOE operating facilities. The assessment program is part of a 10-point initiative announced by Secretary of Energy James D. Watkins on June 27, 1989, to strengthen environmental protection and waste management activities in the Department. The results of the program will provide the Secretary with information on the compliance status of DOE facilities with regard to ES H requirements, root causes for noncompliance, adequacy of DOE and site contractor ES H management programs, and DOE-wide ES H compliance trends.

    This report presents the preliminary findings from the first phase of the Environmental Survey of the United States Department of Energy (DOE) Pantex Facility, conducted November 3 through 14, 1986.The Survey is being conducted by an interdisciplinary team of environmental specialist, led and managed by the Office of Environment, Safety and Health's Office of Environmental Audit. Individual team components are outside experts being supplied by a private contractor. The objective of the Survey is to identify environmental problems and areas of environmental risk associated with the Pantex Facility. The Survey covers all environmental media and all areas of environmental regulation. It is being performed in accordance with the DOE Environmental Survey Manual. The on-site phase of the Survey involves the review of existing site environmental data, observations of the operations carried on at the Pantex Facility, and interviews with site personnel. The Survey team developed a Sampling and Analysis Plan to assist in further assessing certain of the environmental problems identified during its on-site activities. The Sampling and Analysis Plan will be executed by the Oak Ridge National Laboratory. When completed, the results will be incorporated into the Pantex Facility Environmental Survey Interim Report. The Interim Report will reflect the final determinations of the Survey for the Pantex Facility. 65 refs., 44 figs., 27 tabs.

In this document, DOE describes the proposed action and a reasonable range of alternatives to the proposed action, including the ''No-Action'' alternative. The proposed action cited in the ''U.S. Department of Energy Application for a Texas Pollutant Discharge Elimination System Permit Modifying Permit to Dispose of Waste, No. 02296,'' December 1998, included the construction of a new wastewater treatment facility, a new irrigation storage pond, and the conversion of the current wastewater treatment facility into an irrigation storage pond. Although a permit modification application has been filed, if a decision on this EA necessitates it, an amendment to the permit application would be made. The permit application would be required for any of the alternatives and the filing does not preclude or predetermine selection of an alternative considered by this EA. This permit change would allow Pantex to land-dispose treated wastewater by irrigating agricultural land. This construction for the proposed action would include designing two new lagoons for wastewater treatment. One of the lagoons could function as a facultative lagoon for treatment of wastewater. The second lagoon would serve as an irrigation storage impoundment (storage pond), with the alternative use as a facultative lagoon if the first lagoon is out of service for any reason. The new facultative lagoon and irrigation water storage pond would be sited outside of the 100-year flood plain. The existing WWTF lagoon would be used as a storage pond

    This report summarizes the environmental monitoring program at Pantex Plant for 1989. It has been prepared in accordance with the United States Department of Energy Order 5400.1. This report presents monitoring data for both radioactive and nonradioactive species in the local environment. Plant activities involve the handling of significant quantities of uranium, plutonium, and tritium in the form of completed parts received from other DOE facilities, resulting in a very low potential for release of these radionuclides to the atmosphere. In May 1989, there was an accidental release of 40,000 curies (Ci) of tritium (conservatively estimated). The normal releases during the year were 0.12 Ci of tritium and 21 {mu}Ci of uranium-238. The USEPA issued a draft Administrative Order of Consent under section 3008(h) in 1988. Negotiations between DOE and EPA are still continuing. Chromium at a level above drinking water standards was found in a perched groundwater well in zone 12. Also, trace amounts of solvents and high explosives were found in the perched groundwater in zone 12. 12 refs., 10 figs., 53 tabs.

    NUS received authorization from DOE on August 14, 1987 to provide technical support services to assist the Office of Environmental Audit (OEV) in conducting the DOE Environmental Survey and to provide technical assistance on environmental compliance issues. The overall contract is to accomplish a one-time, no-fault baseline Survey of all DOE operating facilities, and to provide technical assistance and support for the resolution of environmental compliance issues. NUS has completed the Preliminary Reports and continues to support DOE on the Prioritization and Tiger Team Assessment efforts. The project requires a broad range of environmental protection expertise, necessitating senior-level personnel as the primary project staff. Many of the tasks assigned by DOE require quick startup and performance, and several tasks may be active at any one time. The objective of the DOE Environmental Survey Program is to identify and prioritize areas of existing environmental risk at 36 DOE facilities. NUS`role is to technically assist the Office of Environmental Audit in the implementation of the Surveys.

    Following the events of 1989 and the subsequent cessation of production of new nuclear weapons by the US, the mission of the Department of Energy (DOE) Nuclear Weapons Complex has shifted from production to dismantlement of retired weapons. The sole site in the US for accomplishing the dismantlement mission is the DOE Pantex Plant near Amarillo, Texas. Pending a national decision on the ultimate storage and disposition of nuclear components form the dismantled weapons, the storage magazines within the Pantex Plant are serving as the interim storage site for pits--the weapon plutonium-bearing component. The DOE has stipulated that Pantex will provide storage for up to 12,000 pits pending a Record of Decision on a comprehensive site-wide Environmental Impact Statement in November 1996.

    The University of North Dakota Energy and Environmental Research Center (EERC) is providing technical assistance and test materials to the US Department of Energy (DOE) Advanced Research and Technology Development (AR and TD) Materials Program investigating ceramic and advanced alloy corrosion in fossil energy systems. The main activity, which is reported here, is to perform thermochemical equilibrium calculations to develop recommendations for test conditions under which to perform corrosion measurements of structural and particle filter materials. The modeling is primarily being performed to determine possible mechanisms of corrosion, especially by species that vaporize in the gasifier then condense on downstream surfaces. For this year, the focus was on the stability of nickel in structural and filter alloys. This work was done in an effort to explain the existence of nickel-containing condensates found downstream of particulate filters in an EERC pilot-scale coal gasifier which operates under conditions similar to the Pinon Pine system, and to determine possible operating conditions that could reduce the wastage of nickel from structural and filter alloys.

Under this contract, BNF has provided management support services for OUT R&D activities for the following Program offices: (1) Office of Energy Management; (2) Office of Solar Energy Conversion; (3) Office of Renewable Energy Conversion; and (4) Deputy Assistant Secretary. During the period between 4/17/91 and 9/17/93, BNF furnished the necessary personnel, equipment, materials, facilities and travel required to provide management support services for each of the above Program Offices. From 9/18/93 to 12/17/93, BNF has been involved in closeout activities, including final product deliverables. Research efforts that have been supported in these Program Offices are: (1) for Energy Management -- Advanced Utility Concepts Division; Utility Systems Division; Integrated Planning; (2) for Solar Energy Conversion -- Photovoltaics Division; Solar Thermal and Biomass Power Division; (3) for Renewable Energy Conversion -- Geothermal Division; Wind, Hydroelectric and Ocean Systems Division; (4) for the Deputy Assistant Secretary -- support as required by the Supporting Staff.     This report documents a study (Stage Right Operational Safety Analysis) that was performed to evaluate the effects of new Stage Right operations on the safety of Pantex personnel who perform the operations and maintain the equipment. The primary concern of the evaluation was for personnel safety during Stage Right operations, but operations equipment damage and degradation also were taken into account. This analysis evaluates safety of the work process in the staging of dismantled nuclear weapon pits within the modified Richmond magazines only. This Stage Right Process and Operational Safety Analysis includes the following processes: moving the pelletized drums from the pallet trailer to the pallet turner, staging of pallets and removal of pallets from the magazine, recovery from an incident in a magazine, setting up, opening, and closing a Zone 4 magazine, inventory of pelletized drums in the magazines, transporting pelletized drums from Zone 12 to Zone 4, and maintenance on the shielded lift truck that involves removal of the cab shielding. The analysis includes the following undesirable consequences: injury to personnel, breach of an AL-R8 container, drop of a loaded pallet, damage to equipment, and equipment unreliability.

  20. Audit of construction of protective force training facilities at the Pantex Plant

    A goal of the Department of Energy project management system is to ensure that projects are necessary to fulfill mission needs and are cost effective. This requires that the Department justify each project and explore competitive alternatives. The objective of this audit was to assess the need to construct protective force training facilities at the Department`s Pantex Plant. Our audit disclosed that (1) construction of a physical training facility was not necessary to fulfill mission needs, and (2) the Department did not consider all viable alternatives to constructing a weapons tactics and training facility. These conditions occurred, in part, because a Justification for New Start was never prepared and approved for the Security Enhancements Major System Acquisition, which included these two projects. We recommended that the Manager, Albuquerque Operations Office, cancel construction of the physical training facility, make needed repairs and upgrades to the existing facilities, and reduce the cost of the Security Enhancements Major System Acquisition accordingly. Implementation of this recommendation will save about $1.7 million. We also recommended that the Manager direct Mason & Hanger to perform economic analyses of all viable alternatives to constructing a weapons tactics and training facility before proceeding with construction. Such analyses could lead to cancellation or rescoping of the proposed facility and result in savings to the Department. Albuquerque management did not agree to cancel construction of the physical training facility, but did agree to perform economic analyses of all viable alternatives to the proposed weapons tactics and training facility before proceeding with construction.

    The US Department of Interior`s Minerals Management Service (MMS) has the responsibility of regulating exploration and development by the oil and gas industry on the US Outer Continental Shelf (OCS). There is an area of active leases approximately 45 miles east-northeast of Cape Hatteras, North Carolina, referred to as the Manteo Unit. Chevron U.S.A. may propose to drill a single exploratory well in either Block 467 or 510 of the Manteo Unit with a specially outfitted drilling rig. To review environmental and socioeconomic information known, and needed, on the Manteo Unit, a workshop was conducted of February 4--5, 1998, with the North Carolina Department of Environment and Natural Resources (DENR). The objectives of the workshop were to review the state of knowledge for drilling a single exploratory well in either Block 467 or 510; share scientific information obtained since 1990; distinguish between exploration and development activities; share information on drilling technology and industry experience operating in similar physical environments; address scientific concerns regarding the potential impacts of OCS drilling on biological resources; and address concerns regarding onshore (social and economic) impacts from OCS drilling.

    This report documents the findings of an on-site audit of the U.S. Customs Cargo Inspection Facility (CIF) in Detroit, Michigan. The federal landlord for this building is the General Services Administration (GSA). The focus of the audit was to identify various no-cost or low-cost energy-efficiency opportunities that, once implemented, would reduce electrical and gas consumption and increase the operational efficiency of the building. Task forces were formed involving prominent energy leaders and experts from business, government, academia and civil society worldwide. The goal of the Task Forces is to inform the implementation of the initiative by identifying challenges and opportunities for achieving its objectives. This report contains the findings of Task Force One which is dedicated to the objective of achieving universal access to modern energy services by 2030. The report shows that universal energy access can be realized by 2030 with strong, focused actions set within a coordinated framework.

    The U.S. Department of Energy (DOE) is planning a two-phased program for the remediation of Hanford tank waste. Phase 1 is a pilot program to demonstrate the procurement of treatment services. The volume of waste treated during the Phase 1 is a small percentage of the tank waste. During Phase 2, DOE intends to procure treatment services for the balance of the waste. The TWRS Privatization Process Technical Baseline (PPTB) provides a summary level flowsheet/mass balance of tank waste treatment operations which is consistent with the tank inventory information, waste feed staging studies, and privatization guidelines currently available. The PPTB will be revised periodically as privatized processing concepts are crystallized.

    This report documents the findings from an onsite audit of the John Seiberling Federal building located in Akron, Ohio. The Federal landlord for this building is the General Services Administration (GSA). The focus of the audit was to identify various no-cost or low-cost energy efficiency opportunities that, once implemented, would reduce in either electrical and gas consumption and increase the operational efficiency of the building. This audit also provided an opportunity to identify potential capital cost projects that should be considered in the to acquire additional energy (electric and gas) and water savings to further increase the operational efficiency of the building.

    In an evaluation of the Department of Energy (DOE) Occupational Safety and Health programs for government-owned contractor-operated (GOCO) activities, the Department of Labor`s Occupational Safety and Health Administration (OSHA) recommended a technical information exchange program. The intent was to share written safety and health programs, plans, training manuals, and materials within the entire DOE community. The OSH Technical Reference (OTR) helps support the secretary`s response to the OSHA finding by providing a one-stop resource and referral for technical information that relates to safe operations and practice. It also serves as a technical information exchange tool to reference DOE-wide materials pertinent to specific safety topics and, with some modification, as a training aid. The OTR bridges the gap between general safety documents and very specific requirements documents. It is tailored to the DOE community and incorporates DOE field experience.

    Many demand response resources are technically capable of providing ancillary services. In some cases, they can provide superior response to generators, as the curtailment of load is typically much faster than ramping thermal and hydropower plants. Analysis and quantification of demand response resources providing ancillary services is necessary to understand the resources economic value and impact on the power system. Methodologies used to study grid integration of variable generation can be adapted to the study of demand response. In the present work, we describe and illustrate a methodology to construct detailed temporal and spatial representations of the demand response resource and to examine how to incorporate those resources into power system models. In addition, the paper outlines ways to evaluate barriers to implementation. We demonstrate how the combination of these three analyses can be used to translate the technical potential for demand response providing ancillary services into a realizable potential.

This task includes testing of novel materials and participation in the further development and validation of single cell test protocols. This task also covers technical assistance to DOE Working Groups, the U.S. Council for Automotive Research (USCAR) and the USCAR/DOE Driving Research and Innovation for Vehicle efficiency and Energy sustainability (U.S. Drive) Fuel Cell Technology Team. Assistance includes technical validation of new fuel cell materials and methods, single cell fuel cell testing to support the development of targets and test protocols, and regular advisory participation in other working groups and reviews. This assistance is made available to PEM fuel cell developers by request and DOE Approval.     Southwest Technology Development Institute (SWTDI), an independent, university-based research institute, has been the operator of the Southwest Region Photovoltaic Experiment Station (SWRES) for almost 30 years. The overarching mission of SWTDI is to position PV systems and solar technologies to become cost-effective, major sources of energy for the United States. Embedded in SWTDI's general mission has been the more-focused mission of the SWRES: to provide value added technical support to the DOE Solar Energy Technologies Program (SETP) to effectively and efficiently meet the R&D needs and targets specified in the SETP Multi-Year Technical Plan. : The DOE/SETP goals of growing U.S. PV manufacturing into giga-watt capacities and seeing tera-watt-hours of solar energy production in the U.S. require an infrastructure that is under development. The staff of the SWRES has supported DOE/SETP through a coherent, integrated program to address infrastructural needs inhibiting wide-scale PV deployment in three major technical categories: specialized engineering services, workforce development, and deployment facilitation.     The following is a synopsis of the major achievements attributed to the operation of the Weatherization Assistance Program Technical Assistance Center (WAPTAC) by the National Association for State Community Services Programs (NASCSP). During the past five years, the WAPTAC has developed into the premier source for information related to operating the Weatherization Assistance Program (WAP) at the state and local levels. The services provide through WAPTAC include both virtual technical support as well as hands-on training and instruction in classroom and in the field. The WAPTAC achieved several important milestones during its operation including the establishment of a national Weatherization Day now celebrated in most states, the implementation of a comprehensive Public Information Campaign (PIC) to raise the awareness of the Program among policy makers and the public, the training of more than 150 new state managers and staff as they assume their duties in state offices around the country, and the creation and support of a major virtual information source on the Internet being accessed by thousands of staff each month. The Weatherization Assistance Program Technical Assistance Center serves the Department of Energy's (DOE) Office of Weatherization and Intergovernmental Program as a valuable training and technical assistance resource for the network of 54 direct state grantees (50 states, District of Columbia and three Native American tribes) and the network of 900 local subgrantees (comprised of community action agencies, units of local government, and other non-profit organizations).     This document defines the technical and functional requirements for a gas test loop (GTL) to be constructed for the purpose of providing a high intensity fast-flux irradiation environment for developers of advanced concept nuclear reactors. This capability is needed to meet fuels and materials testing requirements of the designers of Generation IV (GEN IV) reactors and other programs within the purview of the Advanced Fuel Cycle Initiative (AFCI). Space nuclear power development programs may also benefit by the services the GTL will offer. The overall GTL technical objective is to provide developers with the means for investigating and qualifying fuels and materials needed for advanced reactor concepts. The testing environment includes a fast-flux neutron spectrum of sufficient intensity to perform accelerated irradiation testing. Appropriate irradiation temperature, gaseous environment, test volume, diagnostics, and access and handling features are also needed. This document serves to identify those requirements as well as generic requirements applicable to any system of this kind.

    Purpose Provide an independent engineering review of the major projects being funded by the Department of Energy, Office of Environmental Restoration and Waste Management. The independent engineering review will address questions of whether the engineering practice is sufficiently developed to a point where a major project can be executed without significant technical problems. The independent review will focus on questions related to: (1) Adequacy of development of the technical base of understanding; (2) Status of development and availability of technology among the various alternatives; (3) Status and availability of the industrial infrastructure to support project design, equipment fabrication, facility construction, and process and program/project operation; (4) Adequacy of the design effort to provide a sound foundation to support execution of project; (5) Ability of the organization to fully integrate the system, and direct, manage, and control the execution of a complex major project.

    The STI product is the Final Technical Report from ReliOn, Inc. for contract award DE-EE0000487: Recovery Act PEM Fuel Cell Systems Providing Emergency Reserve and Backup Power. The program covered the turnkey deployment of 431 ReliOn fuel cell systems at 189 individual sites for AT&T and PG&E with ReliOn functioning as the primary equipment supplier and the project manager. The Final Technical Report provides an executive level summary, a comparison of the actual accomplishments vs. the goals and objectives of the project, as well as a summary of the project activity from the contract award date of August 1, 2009 through the contract expiration date of December 31, 2011. Two photos are included in the body of the report which show hydrogen storage and bulk hydrogen refueling technologies developed as a result of this program.

    Proceedings of an expert workshop convened by the U.S. Department of Energy and Industry Canada, and hosted by the National Renewable Energy Laboratory and the California Air Resources Board, May 14-15, 2014, in Sacramento, California, to address the topic of hydrogen energy storage (HES). HES systems provide multiple opportunities to increase the resilience and improve the economics of energy sup supply systems underlying the electric grid, gas pipeline systems, and transportation fuels. This is especially the case when considering particular social goals and market drivers, such as reducing carbon emissions, increasing reliability of supply, and reducing consumption of conventional petroleum fuels. This report compiles feedback collected during the workshop, which focused on policy and regulatory issues related to HES systems. Report sections include an introduction to HES pathways, market demand, and the "smart gas" concept; an overview of the workshop structure; and summary results from panel presentations and breakout groups.

    This Technical Manual contains descriptions of the calculation models and mathematical and numerical methods used in the RADTRAN 6 computer code for transportation risk and consequence assessment. The RADTRAN 6 code combines user-supplied input data with values from an internal library of physical and radiological data to calculate the expected radiological consequences and risks associated with the transportation of radioactive material. Radiological consequences and risks are estimated with numerical models of exposure pathways, receptor populations, package behavior in accidents, and accident severity and probability.

    This final technical report describes results and findings from a research project to examine the role of microbial phosphohydrolase enzymes in naturally occurring subsurface microorganisms for the purpose of promoting the immobilization of the radionuclide uranium through the production of insoluble uranium phosphate minerals. The research project investigated the microbial mechanisms and the physical and chemical processes promoting uranium biomineralization and sequestration in oxygenated subsurface soils. Uranium biomineralization under aerobic conditions can provide a secondary biobarrier strategy to immobilize radionuclides should the metal precipitates formed by microbial dissimilatory mechanisms remobilize due to a change in redox state.

    The 1992 Resource Program will propose actions to meet future loads placed on the Bonneville Power Administration (BPA). It will also discuss and attempt to resolve resource-related policy issues. The Resource Program assesses resource availability and costs, and analyzes resource requirements and alternative ways of meeting those requirements through both conservation and generation resources. These general resource conclusions are then translated to actions for both conservation and generation. The Resource Program recommends budgets for the Office of Energy Resources for Fiscal Years (FY) 1994 and 1995. BPA's Resource Program bears directly on an important BPA responsibility: the obligation under the Northwest Power Act{sup 3} to meet the power requirements of public and private utility and direct service industrial (DSI) customers according to their contractual agreements. BPA's Draft 1992 Resource Program is contained in four documents: (1) 1992 Resource Program Summary; (2) Technical Report; (3) Technical Assumptions Appendix; and, (4) Conservation Implementation Plan. This volume is the Draft 1992 Resource Program Technical Report, a comprehensive document that provides supporting data and analyses for Resource Program recommendations.
