    A Human Reliability Program (HRP) is designed to protect national security as well as worker and public safety by continuously evaluating the reliability of those who have access to sensitive materials, facilities, and programs. Some elements of a site HRP include systematic (1) supervisory reviews, (2) medical and psychological assessments, (3) management evaluations, (4) personnel security reviews, and (4) training of HRP staff and critical positions. Over the years of implementing an HRP, the Department of Energy (DOE) has faced various challenges and overcome obstacles. During this 4-day activity, participants will examine programs that mitigate threats to nuclear security and the insider threat to include HRP, Nuclear Security Culture (NSC) Enhancement, and Employee Assistance Programs. The focus will be to develop an understanding of the need for a systematic HRP and to discuss challenges and best practices associated with mitigating the insider threat.

In accordance with the EERE Peer Review Guide, the review provides an independent, expert evaluation of the strategic goals and direction of the program and is a forum for feedback and recommendations on future program planning. The purpose of the review was to evaluate DOE-funded projects for their contribution to the mission and goals of the Program and to assess progress made against stated objectives.

    Over the last 20 years the Idaho National Laboratory (INL) has adopted a number of operations and safety-related programs which has each periodically taken its turn in the limelight. As new programs have come along there has been natural competition for resources, focus and commitment. In the last few years, the INL has made real progress in integrating all these programs and are starting to realize important synergies. Contributing to this integration are both collaborative individuals and an emerging shared vision and goal of the INL fully maturing in its high reliability operations. This goal is so powerful because the concept of high reliability operations (and the resulting organizations) is a masterful amalgam and orchestrator of the best of all the participating programs (i.e. conduct of operations, behavior based safety, human performance, voluntary protection, quality assurance, and integrated safety management). This paper is a brief recounting of the lessons learned, thus far, at the INL in bringing previously competing programs into harmony under the goal (umbrella) of seeking to perform regularly as a high reliability organization. In addition to a brief diagram-illustrated historical review, the authors will share the INLs primary successes (things already effectively stopped or started) and the gaps yet to be bridged.

    This paper describes the introduction of stochastic linear programming into Operations DER-CAM, a tool used to obtain optimal operating schedules for a given microgrid under local economic and environmental conditions. This application follows previous work on optimal scheduling of a lithium-iron-phosphate battery given the output uncertainty of a 1 MW molten carbonate fuel cell. Both are in the Santa Rita Jail microgrid, located in Dublin, California. This fuel cell has proven unreliable, partially justifying the consideration of storage options. Several stochastic DER-CAM runs are executed to compare different scenarios to values obtained by a deterministic approach. Results indicate that using a stochastic approach provides a conservative yet more lucrative battery schedule. Lower expected energy bills result, given fuel cell outages, in potential savings exceeding 6percent.

    The Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) is a software application developed for performing a complete probabilistic risk assessment (PRA) using a personal computer running the Microsoft Windows? operating system. SAPHIRE is primarily funded by the U.S. Nuclear Regulatory Commission (NRC) and developed by the Idaho National Laboratory (INL). The INL's primary role in this project is that of software developer and tester. Using the SAPHIRE analysis engine and relational database is a complementary program called GEM. GEM has been designed to simplify using existing PRA analysis for activities such as the NRC’s Accident Sequence Precursor program. In this report, the theoretical framework behind GEM-type calculations are discussed in addition to providing guidance and examples for performing evaluations when using the GEM software. As part of this analysis framework, the two types of GEM analysis are outlined, specifically initiating event (where an initiator occurs) and condition (where a component is failed for some length of time) assessments.

    This report summarizes the Accelerated Reliability testing of five lead-acid battery-equipped Chevrolet S-10 electric vehicles by the US Department of Energy's Field Operations Program and the Program's testing partners, Electric Transportation Applications (ETA) and Southern California Edison (SCE). ETA and SCE operated the S-10s with the goal of placing 25,000 miles on each vehicle within 1 year, providing an accelerated life-cycle analysis. The testing was performed according to established and published test procedures. The S-10s' average ranges were highest during summer months; changes in ambient temperature from night to day and from season-to-season impacted range by as much as 10 miles. Drivers also noted that excessive use of power during acceleration also had a dramatic effect on vehicle range. The spirited performance of the S-10s created a great temptation to inexperienced electric vehicle drivers to ''have a good time'' and to fully utilize the S-10's acceleration capability. The price of injudicious use of power is greatly reduced range and a long-term reduction in battery life. The range using full-power accelerations followed by rapid deceleration in city driving has been 20 miles or less.

SAPHIRE can be used to model a complex systems response to initiating events, quantify associated damage outcome frequencies, and identify important contributors to this damage (Level 1 PRA) and to analyze containment performance during a severe accident and quantify radioactive releases (Level 2 PRA). It can be used for a PRA evaluating a variety of operating conditions, for example, for a nuclear reactor at full power, low power, or at shutdown conditions. Furthermore, SAPHIRE can be used to analyze both internal and external initiating events and has special features for transforming models built for internal event analysis to models for external event analysis. It can also be used in a limited manner to quantify risk in terms of release consequences to both the public and the environment (Level 3 PRA). SAPHIRE includes a separate module called the Graphical Evaluation Module (GEM). GEM provides a highly specialized user interface with SAPHIRE that automates SAPHIRE process steps for evaluating operational events at commercial nuclear power plants. Using GEM, an analyst can estimate the risk associated with operational events in a very efficient and expeditious manner. This reference guide will introduce the SAPHIRE Version 7.0 software. A brief discussion of the purpose and history of the software is included along with general information such as installation instructions, starting and stopping the program, and some pointers on how to get around inside the program. Next, database concepts and structure are discussed.     The overall mission of the U.S. Department of Energy’s Office of Electricity Delivery and Energy Reliability (OE) is to lead national efforts to modernize the electric grid, enhance the security and reliability of the energy infrastructure, and facilitate recovery from disruptions to the energy supply. In support of this mission, OE conducts a portfolio of research and development (R&D) activities to advance technologies to enhance electric power delivery. Multiple benefits are anticipated to result from the deployment of these technologies, including higher quality and more reliable power, energy savings, and lower cost electricity. In addition, OE engages State and local government decision-makers and the private sector to address issues related to the reliability and security of the grid, including responding to national emergencies that affect energy delivery. The OE R&D activities are comprised of four R&D lines: High Temperature Superconductivity (HTS), Visualization and Controls (V&C), Energy Storage and Power Electronics (ES&PE), and Distributed Systems Integration (DSI).

    The Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) suite of programs can be used to organize and standardize in an electronic format information from probabilistic risk assessments or individual plant examinations. The Models and Results Database (MAR-D) program of the SAPHIRE suite serves as the repository for probabilistic risk assessment and individual plant examination data and information. This report demonstrates by examples the common electronic and manual methods used to load these types of data. It is not a stand alone document but references documents that contribute information relative to the data loading process. This document provides a more detailed discussion and instructions for using SAPHIRE 5.0 only when enough information on a specific topic is not provided by another available source.

Many countries continue to make efforts to strengthen nuclear security by enhancing systems of nuclear material protection, control, and accounting (MPC&A). Though MPC&A systems can significantly upgrade nuclear security, they do not eliminate the "human factor." Gen. Eugene Habiger, a former "Assistant Secretary for Safeguards and Security" at the U.S. Department of Energy’s (DOE) nuclear-weapons complex and a former commander of U.S. strategic nuclear forces, has observed that "good security is 20% equipment and 80% people." Although eliminating the "human factor" is not possible, accounting for and mitigating the risk of the insider threat is an essential element in establishing an effective nuclear security culture. This paper will consider the organizational role in mitigating the risk associated with the malicious insider through monitoring and enhancing human reliability and motivation as well as enhancing the nuclear security culture.

    The CONTAIN code was developed by Sandia National Laboratories under the sponsorship of the US Nuclear Regulatory Commission (NRC) to provide integrated analyses of containment phenomena. It is used to predict nuclear reactor containment loads, radiological source terms, and associated physical phenomena for a range of accident conditions encompassing both design-basis and severe accidents. The code`s targeted applications include support for containment-related experimental programs, light water and advanced light water reactor plant analysis, and analytical support for resolution of specific technical issues such as direct containment heating. The NRC decided that a broad technical review of the code should be performed by technical experts to determine its overall technical adequacy. For this purpose, a six-member CONTAIN Peer Review Committee was organized and a peer review as conducted. While the review was in progress, the NRC issued a draft ``Revised Severe Accident Code Strategy`` that incorporated revised design objectives and targeted applications for the CONTAIN code. The committee continued its effort to develop findings relative to the original NRC statement of design objectives and targeted applications. However, the revised CONTAIN design objectives and targeted applications. The Committee determined some improvements are warranted and provided recommendations in five code-related areas: (1) documentation, (2) user guidance, (3) modeling capability, (4) code assessment, and (5) technical assessment.

    In 2005, the Federal Energy Regulatory Commission (FERC) began discussing regulatory policy for reactive-power procurement and pricing in competitive electricity markets. This paper summarizes findings from a unique, interdisciplinary program of public-interest research that lays a formal foundation for evaluating aspects of FERC staff recommendations and offers early insights that should be useful in guiding policy implementation, specifically by: (1) clarifying the consumers and economic characteristics of reactive power as a basis for creating incentives to appropriately price it, (2) defining specific challenges in creating a competitive market for reactive power as well as new tools needed to help ensure such a market functions efficiently, and (3) demonstrating the importance of accounting for the physical characteristics of the transmission network in planning for reactive power and avoiding the exercise of market power by suppliers.

    Geothermal Technologies Office conducted its annual program peer review in April of 2013. The review provided an independent, expert evaluation of the technical progress and merit of GTO-funded projects. Further, the review was a forum for feedback and recommendations on future GTO strategic planning. During the course of the peer review, DOE-funded projects were evaluated for 1) their contribution to the mission and goals of the GTO and 2) their progress against stated project objectives. Principal Investigators (PIs) came together in sessions organized by topic “tracks” to disseminate information, progress, and results to a panel of independent experts as well as attendees.
