    The Institute for Regulatory Science (RS) and the American Society of Mechanical Engineers (ASME) jointly estab lished a peer-review program. As the program evolved, four types of review were established. A process for stakeholder participation in peer review eetings was also developed. While a committee established by the ASME provided oversight to the peer-review process, the RSI managed the day-to-day operations of peer review panels. In addition to the reports resulting from peer review of specific projects, several documents were prepared to facilitate the review process, all of which were widely distributed.

    A Human Reliability Program (HRP) is designed to protect national security as well as worker and public safety by continuously evaluating the reliability of those who have access to sensitive materials, facilities, and programs. Some elements of a site HRP include systematic (1) supervisory reviews, (2) medical and psychological assessments, (3) management evaluations, (4) personnel security reviews, and (4) training of HRP staff and critical positions. Over the years of implementing an HRP, the Department of Energy (DOE) has faced various challenges and overcome obstacles. During this 4-day activity, participants will examine programs that mitigate threats to nuclear security and the insider threat to include HRP, Nuclear Security Culture (NSC) Enhancement, and Employee Assistance Programs. The focus will be to develop an understanding of the need for a systematic HRP and to discuss challenges and best practices associated with mitigating the insider threat.

    On June 6-10, 2011, the U.S. Department of Energy (DOE), Office of Energy Efficiency and Renewable Energy (EERE), Geothermal Technologies Program (GTP or the Program) conducted its annual program peer review in Bethesda, Maryland. In accordance with the EERE Peer Review Guide, the review provides an independent, expert evaluation of the strategic goals and direction of the program and is a forum for feedback and recommendations on future program planning. The purpose of the review was to evaluate DOE-funded projects for their contribution to the mission and goals of the Program and to assess progress made against stated objectives.

    The 2013 Building Technologies Office Program Peer Review Report summarizes the results of the 2013 Building Technologies Office (BTO) peer review, which was held in Washington, D.C., on April 2–4, 2013. The review was attended by over 300 participants and included presentations on 59 BTO-funded projects: 29 from BTO’s Emerging Technologies Program, 20 from the Commercial Buildings Integration Program, 6 from the Residential Buildings Integration Program, and 4 from the Building Energy Codes Program. This report summarizes the scores and comments provided by the independent reviewers for each project.

    Over the last 20 years the Idaho National Laboratory (INL) has adopted a number of operations and safety-related programs which has each periodically taken its turn in the limelight. As new programs have come along there has been natural competition for resources, focus and commitment. In the last few years, the INL has made real progress in integrating all these programs and are starting to realize important synergies. Contributing to this integration are both collaborative individuals and an emerging shared vision and goal of the INL fully maturing in its high reliability operations. This goal is so powerful because the concept of high reliability operations (and the resulting organizations) is a masterful amalgam and orchestrator of the best of all the participating programs (i.e. conduct of operations, behavior based safety, human performance, voluntary protection, quality assurance, and integrated safety management). This paper is a brief recounting of the lessons learned, thus far, at the INL in bringing previously competing programs into harmony under the goal (umbrella) of seeking to perform regularly as a high reliability organization. In addition to a brief diagram-illustrated historical review, the authors will share the INL’s primary successes (things already effectively stopped or started) and the gaps yet to be bridged.

    This document presents a shortened version of the procedure, models, and data for human reliability analysis (HRA) which are presented in the Handbook of Human Reliability Analysis With emphasis on Nuclear Power Plant Applications (NUREG/CR-1278, August 1983). This shortened version was prepared and tried out as part of the Accident Sequence Evaluation Program (ASEP) funded by the US Nuclear Regulatory Commission and managed by Sandia National Laboratories. The intent of this new HRA procedure, called the ''ASEP HRA Procedure,'' is to enable systems analysts, with minimal support from experts in human reliability analysis, to make estimates of human error probabilities and other human performance characteristics which are sufficiently accurate for many probabilistic risk assessments. The ASEP HRA Procedure consists of a Pre-Accident Screening HRA, a Pre-Accident Nominal HRA, a Post-Accident Screening HRA, and a Post-Accident Nominal HRA. The procedure in this document includes changes made after tryout and evaluation of the procedure in four nuclear power plants by four different systems analysts and related personnel, including human reliability specialists. The changes consist of some additional explanatory material (including examples), and more detailed definitions of some of the terms. 42 refs.

    This paper describes the introduction of stochastic linear programming into Operations DER-CAM, a tool used to obtain optimal operating schedules for a given microgrid under local economic and environmental conditions. This application follows previous work on optimal scheduling of a lithium-iron-phosphate battery given the output uncertainty of a 1 MW molten carbonate fuel cell. Both are in the Santa Rita Jail microgrid, located in Dublin, California. This fuel cell has proven unreliable, partially justifying the consideration of storage options. Several stochastic DER-CAM runs are executed to compare different scenarios to values obtained by a deterministic approach. Results indicate that using a stochastic approach provides a conservative yet more lucrative battery schedule. Lower expected energy bills result, given fuel cell outages, in potential savings exceeding 6percent.

    The Nuclear Regulatory Commission (NRC) established the Program to Assess the Reliability of Emerging Nondestructive Techniques (PARENT) to follow on from the successful Program for the Inspection of Nickel alloy Components (PINC). The goal of the PARENT is to conduct a confirmatory assessment of the reliability of nondestructive evaluation (NDE) techniques for detecting and sizing primary water stress corrosion cracks (PWSCC) and applying the lessons learned from PINC to a series of round-robin tests. These open and blind round-robin tests will comprise a new set of typical pressure boundary components including dissimilar metal welds (DMWs) and bottom-mounted instrumentation penetrations. Open round-robin tests will engage research and industry teams worldwide to investigate and demonstrate the reliability of emerging NDE techniques to detect and size flaws with a wide range of lengths, depths, orientations, and locations. Blind round-robin tests will utilize various testing organizations, whose inspectors and procedures are certified by the standards for the nuclear industry in their respective countries, to investigate the ability of established NDE techniques to detect and size flaws whose characteristics range from relatively easy to very difficult for detection and sizing. Blind and open round-robin testing started in late 2011 and early 2012, respectively. This paper will present the work scope with reports on progress, NDE methods evaluated, and project timeline for PARENT.

    The Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) is a software application developed for performing a complete probabilistic risk assessment (PRA) using a personal computer. SAPHIRE is primarily funded by the U.S. Nuclear Regulatory Commission (NRC) and developed by the Idaho National Laboratory. This report is intended to assist the user to enter PRA data into the SAPHIRE program using the built-in MAR-D ASCII-text file data transfer process. Towards this end, a small sample database is constructed and utilized for demonstration. Where applicable, the discussion includes how the data processes for loading the sample database relate to the actual processes used to load a larger PRA models. The procedures described herein were developed for use with SAPHIRE Version 6.0 and Version 7.0. In general, the data transfer procedures for version 6 and 7 are the same, but where deviations exist, the differences are noted. The guidance specified in this document will allow a user to have sufficient knowledge to both understand the data format used by SAPHIRE and to carry out the transfer of data between different PRA projects.

    This three-volume report presents the results of Phase II of the multiphase EPRI-sponsored High-Reliability Gas Turbine Combined-Cycle Development Program whose goal is to achieve a highly reliable gas turbine combined-cycle power plant, available by the mid-1980s, which would be an economically attractive baseload generation alternative for the electric utility industry. The Phase II program objective was to prepare the preliminary design of this power plant. The power plant was addressed in three areas: (1) the gas turbine, (2) the gas turbine ancillaries, and (3) the balance of plant including the steam turbine generator. To achieve the program goals, a gas turbine was incorporated which combined proven reliability characteristics with improved performance features. This gas turbine, designated the V84.3, is the result of a cooperative effort between Kraftwerk Union AG and United Technologies Corporation. Gas turbines of similar design operating in Europe under baseload conditions have demonstrated mean time between failures in excess of 40,000. The reliability characteristics of the gas turbine ancillaries and balance-of-plant equipment were improved through system simplification and component redundancy and by selection of component with inherent high reliability. A digital control system was included with logic, communications, sensor redundancy, and manual backup. An independent condition monitoring and diagnostic system was also included. Program results provide the preliminary design of a gas turbine combined-cycle baseload power plant. This power plant has a predicted mean time between failure of nearly twice the 3000-h EPRI goal. The cost of added reliability features is offset by improved performance, which results in a comparable specific cost and an 8% lower cost of electricty compared to present market offerings.

SAPHIRE is funded by the U.S. Nuclear Regulatory Commission (NRC) and developed by the Idaho National Laboratory (INL). The INL's primary role in this project is that of software developer. However, the INL also plays an important role in technology transfer by interfacing and supporting SAPHIRE users comprised of a wide range of PRA practitioners from the NRC, national laboratories, the private sector, and foreign countries. SAPHIRE can be used to model a complex system’s response to initiating events, quantify associated damage outcome frequencies, and identify important contributors to this damage (Level 1 PRA) and to analyze containment performance during a severe accident and quantify radioactive releases (Level 2 PRA). It can be used for a PRA evaluating a variety of operating conditions, for example, for a nuclear reactor at full power, low power, or at shutdown conditions. Furthermore, SAPHIRE can be used to analyze both internal and external initiating events and has special features for transforming models built for internal event analysis to models for external event analysis. It can also be used in a limited manner to quantify risk in terms of release consequences to both the public and the environment (Level 3 PRA). SAPHIRE includes a separate module called the Graphical Evaluation Module (GEM). GEM provides a highly specialized user interface with SAPHIRE that automates SAPHIRE process steps for evaluating operational events at commercial nuclear power plants. Using GEM, an analyst can estimate the risk associated with operational events in a very efficient and expeditious manner. This reference guide will introduce the SAPHIRE Version 7.0 software. A brief discussion of the purpose and history of the software is included along with general information such as installation instructions, starting and stopping the program, and some pointers on how to get around inside the program. Next, database concepts and structure are discussed. Following that discussion are nine sections, one for each of the menu options on the SAPHIRE main menu, wherein the purpose and general capabilities for each option are furnished. Next, the capabilities and limitations of the software are provided.

    The overall mission of the U.S. Department of Energy’s Office of Electricity Delivery and Energy Reliability (OE) is to lead national efforts to modernize the electric grid, enhance the security and reliability of the energy infrastructure, and facilitate recovery from disruptions to the energy supply. In support of this mission, OE conducts a portfolio of research and development (R&D) activities to advance technologies to enhance electric power delivery. Multiple benefits are anticipated to result from the deployment of these technologies, including higher quality and more reliable power, energy savings, and lower cost electricity. Many countries continue to make efforts to strengthen nuclear security by enhancing systems of nuclear material protection, control, and accounting (MPC&A). Though MPC&A systems can significantly upgrade nuclear security, they do not eliminate the "human factor." Gen. Eugene Habiger, a former "Assistant Secretary for Safeguards and Security" at the U.S. Department of Energy’s (DOE) nuclear-weapons complex and a former commander of U.S. strategic nuclear forces, has observed that "good security is 20% equipment and 80% people." Although eliminating the "human factor" is not possible, accounting for and mitigating the risk of the insider threat is an essential element in establishing an effective nuclear security culture. This paper will consider the organizational role in mitigating the risk associated with the malicious insider through monitoring and enhancing human reliability and motivation as well as enhancing the nuclear security culture.

    Utility load management programs--including direct load control and interruptible load programs--were employed by utilities in the past as system reliability resources. With electricity industry restructuring, the context for these programs has changed; the market that was once controlled by vertically integrated utilities has become competitive, raising the question: can existing load management programs be modified so that they can effectively participate in competitive energy markets? In the short run, modified and/or improved operation of load management programs may be the most effective form of demand-side response available to the electricity system today. However, in light of recent technological advances in metering, communication, and load control, utility load management programs must be carefully reviewed in order to determine appropriate investments to support this transition. This report investigates the feasibility of and options for modifying an existing utility load management system so that it might provide reliability services (i.e. ancillary services) in the competitive markets that have resulted from electricity industry restructuring. The report is a case study of Southern California Edison's (SCE) load management programs. SCE was chosen because it operates one of the largest load management programs in the country and it operates them within a competitive wholesale electricity market. The report describes a wide range of existing and soon-to-be-available communication, control, and metering technologies that could be used to facilitate the evolution of SCE's load management programs and systems to provision of reliability services. The fundamental finding of this report is that, with modifications, SCE's load management infrastructure could be transitioned to provide critical ancillary services in competitive electricity markets, employing currently or soon-to-be available load control technologies.

    The goal of this research was to investigate the potential for employing dynamic, decentralized software architectures to achieve reliability in future high-performance computing platforms. These architectures, inspired by peer-to-peer networks such as botnets that already scale to millions of unreliable nodes, hold promise for enabling scientific applications to run usefully on next-generation exascale platforms ({approx} 10{sup 18} operations per second). Traditional parallel programming techniques suffer rapid deterioration of performance scaling with growing platform size, as the work of coping with increasingly frequent failures dominates over useful computation. Our studies suggest that new architectures, in which failures are treated as ubiquitous and their effects are considered as simply another controllable source of error in a scientific computation, can remove such obstacles to exascale computing for certain applications. We have developed a simulation framework, as well as a preliminary implementation in a large-scale emulation environment, for exploration of these 'fault-oblivious computing' approaches. High-performance computing (HPC) faces a fundamental problem of increasing total component failure rates due to increasing system sizes, which threaten to degrade system reliability to an unusable level by the time the exascale range is reached ({approx} 10{sup 18} operations per second, requiring of order millions of processors). As computer scientists seek a way to scale system software for next-generation exascale machines, it is worth considering peer-to-peer (P2P) architectures that are already capable of supporting 10{sup 6}-10{sup 7} unreliable nodes. Exascale platforms will require a different way of looking at systems and software because the machine will likely not be available in its entirety for a meaningful execution time. Realistic estimates of failure rates range from a few times per day to more than once per hour for these platforms. P2P architectures give us a starting point for crafting applications and system software for exascale. In the context of the Internet, P2P applications (e.g., file sharing, botnets) have already solved this problem for 10{sup 6}-10{sup 7} nodes. Usually based on a fractal distributed hash table structure, these systems have proven robust in practice to constant and unpredictable outages, failures, and even subversion. For example, a recent estimate of botnet turnover (i.e., the number of machines leaving and joining) is about 11% per week. Nonetheless, P2P networks remain effective despite these failures: The Conficker botnet has grown to {approx} 5 x 10{sup 6} peers. Unlike today's system software and applications, those for next-generation exascale machines cannot assume a static structure and, to be scalable over millions of nodes, must be decentralized. P2P architectures achieve both, and provide a promising model for 'fault-oblivious computing'. This project aimed to study the dynamics of P2P networks in the context of a design for exascale systems and applications. Having no single point of failure, the most successful P2P architectures are adaptive and self-organizing. While there has been some previous work applying P2P to message passing, little attention has been previously paid to the tightly coupled exascale domain. Typically, the per-node footprint of P2P systems is small, making them ideal for HPC use. The implementation on each peer node cooperates en masse to 'heal' disruptions rather than relying on a controlling 'master' node. Understanding this cooperative behavior from a complex systems viewpoint is essential to predicting useful environments for the inextricably unreliable exascale platforms of the future. We sought to obtain theoretical insight into the stability and large-scale behavior of candidate architectures, and to work toward leveraging Sandia's Emulytics platform to test promising candidates in a realistic (ultimately {ge} 10{sup 7} nodes) setting.     The CONTAIN code was developed by Sandia National Laboratories under the sponsorship of the US Nuclear Regulatory Commission (NRC) to provide integrated analyses of containment phenomena. It is used to predict nuclear reactor containment loads, radiological source terms, and associated physical phenomena for a range of accident conditions encompassing both design-basis and severe accidents. The code`s targeted applications include support for containment-related experimental programs, light water and advanced light water reactor plant analysis, and analytical support for resolution of specific technical issues such as direct containment heating. The NRC decided that a broad technical review of the code should be performed by technical experts to determine its overall technical adequacy. For this purpose, a six-member CONTAIN Peer Review Committee was organized and a peer review as conducted. While the review was in progress, the NRC issued a draft ``Revised Severe Accident Code Strategy`` that incorporated revised design objectives and targeted applications for the CONTAIN code. The committee continued its effort to develop findings relative to the original NRC statement of design objectives and targeted applications.     Geothermal Technologies Office conducted its annual program peer review in April of 2013. The review provided an independent, expert evaluation of the technical progress and merit of GTO-funded projects. Further, the review was a forum for feedback and recommendations on future GTO strategic planning. During the course of the peer review, DOE-funded projects were evaluated for 1) their contribution to the mission and goals of the GTO and 2) their progress against stated project objectives. Principal Investigators (PIs) came together in sessions organized by topic “tracks” to disseminate information, progress, and results to a panel of independent experts as well as attendees.

This paper summarizes findings from a unique, interdisciplinary program of public-interest research that lays a formal foundation for evaluating aspects of FERC staff recommendations and offers early insights that should be useful in guiding policy implementation, specifically by: (1) clarifying the consumers and economic characteristics of reactive power as a basis for creating incentives to appropriately price it, (2) defining specific challenges in creating a competitive market for reactive power as well as new tools needed to help ensure such a market functions efficiently, and (3) demonstrating the importance of accounting for the physical characteristics of the transmission network in planning for reactive power and avoiding the exercise of market power by suppliers.

    On May 7-10, 2012, the U.S. Department of Energy, Office of Energy Efficiency and Renewable Energy, Geothermal Technologies Office conducted its annual program peer review in Westminster, CO. In accordance with the EERE Peer Review Guide, the review provides an independent, expert evaluation of the strategic goals and direction of the office and is a forum for feedback and recommendations on future office planning. The purpose of the review was to evaluate DOE-funded projects for their contribution to the mission and goals of the office and to assess progress made against stated objectives. The desired product of this developmental work is a tool for the system designer to use in the early design phase so that the final design will achieve the desired system reliability without lengthy testing and rework. Three computer programs were written which provide the first attempt at fulfilling this need. The programs are described and a case study is presented for each one. This is a continuing effort which will be furthered in FY-1992. 10 refs.

    Deposition of crystallographic structures should be concurrent with or prior to manuscript submission for peer review, enabling validation and increasing reliability of the PDB. Most of the macromolecular structures in the Protein Data Bank (PDB), which are used daily by thousands of educators and scientists alike, are determined by X-ray crystallography. It was examined whether the crystallographic models and data were deposited to the PDB at the same time as the publications that describe them were submitted for peer review. This condition is necessary to ensure pre-publication validation and the quality of the PDB public archive. It was found that a significant proportion of PDB entries were submitted to the PDB after peer review of the corresponding publication started, and many were only submitted after peer review had ended. It is argued that clear description of journal policies and effective policing is important for pre-publication validation, which is key in ensuring the quality of the PDB and of peer-reviewed literature.

  20. Cooperative peer-to-peer multiagent based systems

    A multiagent based model for a system of cooperative agents aiming at growth is proposed. This is based on a set of generalized Verhulst-Lotka-Volterra differential equations. In this study, strong cooperation is allowed among agents having similar sizes, and weak cooperation if agent have markedly different "sizes", thus establishing a peer-to-peer modulated interaction scheme. A rigorous analysis of the stable configurations is presented first examining the fixed points of the system, next determining their stability as a function of the model parameters. It is found that the agents are self-organizing into clusters. Furthermore, it is demonstrated that, depending on parameter values, multiple stable configurations can coexist. It occurs that only one of them always emerges with probability close to one, because its associated attractor dominates over the rest.     This report addresses Hawaii electric system reliability issues; greater emphasis is placed on short-term reliability but resource adequacy is reviewed in reference to electric consumers' views of reliability %E2%80%9Cworth%E2%80%9D and the reserve capacity required to deliver that value. The report begins with a description of the Hawaii electric system to the extent permitted by publicly available data. Electrical engineering literature in the area of electric reliability is researched and briefly reviewed. North American Electric Reliability Corporation standards and measures for generation and transmission are reviewed and identified as to their appropriateness for various portions of the electric grid and for application in Hawaii. Analysis of frequency data supplied by the State of Hawaii Public Utilities Commission is presented together with comparison and contrast of performance of each of the systems for two years, 2010 and 2011. Literature tracing the development of reliability economics is reviewed and referenced. A method is explained for integrating system cost with outage cost to determine the optimal resource adequacy given customers' views of the value contributed by reliable electric supply. The report concludes with findings and recommendations for reliability in the State of Hawaii.

    The paper and presentations compiled in this volume form the Proceedings of the fourth in a series of Workshops sponsored by Solar Energy Research Institute (SERI/DOE) under the general theme of photovoltaic module reliability during the period 1986--1990. The reliability Photo Voltaic (PV) modules/systems is exceedingly important along with the initial cost and efficiency of modules if the PV technology has to make a major impact in the power generation market, and for it to compete with the conventional electricity producing technologies. The reliability of photovoltaic modules has progressed significantly in the last few years as evidenced by warranties available on commercial modules of as long as 12 years. However, there is still need for substantial research and testing required to improve module field reliability to levels of 30 years or more. Several small groups of researchers are involved in this research, development, and monitoring activity around the world. In the US, PV manufacturers, DOE laboratories, electric utilities and others are engaged in the photovoltaic reliability research and testing. This group of researchers and others interested in this field were brought together under SERI/DOE sponsorship to exchange the technical knowledge and field experience as related to current information in this important field. The papers presented here reflect this effort.

    The Joint Architecture Standard (JAS) program at Sandia National Laboratories requires the use of a reliable data delivery protocol over SpaceWire. The National Aeronautics and Space Administration at the Goddard Spaceflight Center in Greenbelt, Maryland, developed and specified a reliable protocol for its Geostationary Operational Environment Satellite known as GOES-R Reliable Data Delivery Protocol (GRDDP). The JAS program implemented and tested GRDDP and then suggested a number of modifications to the original specification to meet its program specific requirements. This document details the full RDDP specification as modified for JAS. The JAS Reliable Data Delivery Protocol uses the lower-level SpaceWire data link layer to provide reliable packet delivery services to one or more higher-level host application processes. This document specifies the functional requirements for JRDDP but does not specify the interfaces to the lower- or higher-level processes, which may be implementation-dependent.

    The churn rate of a peer-to-peer system places direct limitations on the rate at which messages can be effectively communicated to a group of peers. These limitations are independent of the topology and message transmission latency. In this paper we consider a peer-to-peer network, based on the Engset model, where peers arrive and depart independently at random. We show how the arrival and departure rates directly limit the capacity for message streams to be broadcast to all other peers, by deriving mean field models that accurately describe the system behavior. Our models cover the unit and more general k buffer cases, i.e. where a peer can buffer at most k messages at any one time, and we give results for both single and multi-source message streams. We define coverage rate as peer-messages per unit time, i.e. the rate at which a number of peers receive messages, and show that the coverage rate is limited by the churn rate and buffer size.
