    The UPAFC Records Management Plan is based on the life cycle of a record - the evolution of a record from creation until final disposition. There are three major phases in the life cycle of a record: (1) creation and receipt, (2) maintenance and use, and (3) disposition. Accordingly, the Records Management Plan is structured to follow each of those phases. During each of the three phases, some kind of control is mandatory.     The Office of Legacy Management (LM) is an integral part of the U.S. Department of Energy’s (DOE’s) strategy to ensure that legacy liabilities of former nuclear weapons production sites are properly managed following the completion of environmental cleanup activities. LM will work with each site using an integrated team approach to ensure a successful transition. Part of this process will include transition of Government records and information. The Office of Legacy Management Information and Records Management Transition Guidance focuses on LM’s goal to preserve and protect legacy records and information. This guidance document establishes a framework for the transfer of records management responsibilities for sites transferring to LM. It describes the requirements, responsibilities, and procedures for the efficient and cost-effective transfer of custody, ownership, and management of records and other information products from the transfer site to LM. Records management practices are critical to the functions of Federal agencies because records provide information about, or evidence of, the organization, functions, policies, decisions, procedures, operations, or other activities. Therefore, the information generated by an agency is created, maintained, and dispositioned through records management processes that ensure the appropriate preservation and retrieval of essential information. Because of their intrinsic value, best practices to preserve information and records should be utilized when records are transferred from one organization to another. As the transfer program completes cleanup activities at closure sites, a transitional process will facilitate the transparent shift in the management of site records activities to LM. The roles and responsibilities of the transfer site and/or program and LM described in this document are a necessary foundation for cooperation and coordination and are essential to the successful transition of records and information responsibilities. The DOE Office of the Chief Information Officer (OCIO) has a central role in DOE records management by providing guidance, expertise, and coordination to all DOE offices and organizations and coordination with the National Archives and Records Administration (NARA). LM and the transfer site will complete an integrated transition plan which will integrate all transition elements including information and records. As part of the overall transition plan, an Information and Records Transition Plan will be developed consistent with the integrated transition plan for the site transfer and included as an attachment. The Information and Records Management Transition Plan will be developed to assist both organizations in organizing the tasks; establishing a timetable and milestones for their completion; and identifying manpower, funding and other resources that will be needed to complete the ownership transfer. In addition, the plan will provide a valuable exchange of institutional knowledge that will assist LM in meeting the obligations of responsibly managing legacy records. Guidance for the development of the plan is included in this document. Records management concerns that may arise during site closure, such as management support, contract language and agreements, interactions with the OCIO and NARA, resource and budget considerations, and procedures to safeguard records are addressed. Guidelines and criteria for records management transition activities are also provided. These include LM expectations for the inventory, scheduling, and disposition of records; the management and transfer of electronic files, including databases and software; records finding aids, indices, and recordkeeping systems; and the process for the transfer of hard copy and electronic records to LM.

    The Nez Perce Tribe (NPT) currently manages a 15,325 acre parcel of land known as the Precious Lands Wildlife Management Area that was purchased as mitigation for losses incurred by construction of the four lower Snake River dams. The Management Area is located in northern Wallowa County, Oregon and southern Asotin County, Washington (Figure 1). It is divided into three management parcels--the Buford parcel is located on Buford Creek and straddles the WA-OR state line, and the Tamarack and Basin parcels are contiguous to each other and located between the Joseph Creek and Cottonwood Creek drainages in Wallowa County, OR. The project was developed under the Pacific Northwest Electric Power Planning and Conservation Act of 1980 (P.L. 96-501), with funding from the Bonneville Power Administration (BPA). The acreage protected under this contract will be credited to BPA as habitat permanently dedicated to wildlife and wildlife mitigation. A modeling strategy known as Habitat Evaluation Procedure (HEP) was developed by the U.S. Fish and Wildlife Service and adopted by BPA as a habitat equivalency accounting system. Nine wildlife species models were used to evaluate distinct cover type features and provide a measure of habitat quality. Models measure a wide range of life requisite variables for each species and monitor overall trends in vegetation community health and diversity. One product of HEP is an evaluation of habitat quality expressed in Habitat Units (HUs). This HU accounting system is used to determine the amount of credit BPA receives for mitigation lands. After construction of the four lower Snake River dams, a HEP loss assessment was conducted to determine how many Habitat Units were inundated behind the dams. Twelve target species were used in that evaluation: Canada goose, mallard, river otter, downy woodpecker, song sparrow, yellow warbler, marsh wren, western meadowlark, chukar, ring-necked pheasant, California quail, and mule deer. The U.S. Army Corp of Engineers and the Washington Department of fish and Wildlife subsequently purchased numerous properties to mitigate for the identified Snake River losses. These projects, however, were not sufficient to mitigate for all the HU's lost. The Northwest Power Planning Council amended the remaining 26,774 HU's into their 1994-1995 Fish and Wildlife Program as being unmitigated (NPPC 2000), which allowed the Nez Perce Tribe to contract with BPA to provide HU's through the Precious Lands Project. The Precious Lands project contains a different composition of cover types than those assessed during the lower Snake loss assessment. For example, no mallard or Canada goose habitat exists on Precious Lands but the area does contain conifer forest, which was not present on the area inundated by dam construction. These cover type differences have resulted in a slightly different suite of species for the current HEP assessment. Target species for Precious Lands are downy woodpecker, yellow warbler, song sparrow, California Quail, mule deer, sharp-tailed grouse (brood rearing), west em meadowlark, beaver, and black-capped chickadee. This list is a reflection of the available cover types and the management objectives of the Nez Perce Tribe. For example, chukar was not used in the present assessment because it is an introduced Eurasian game bird that does not provide an accurate representation of the ecological health of the native grasslands it was supposed to represent. Initial model runs using the chukar confirmed this suspicion so the brood-rearing section of the sharp-tailed grouse model was used instead. Additionally, the beaver model was used in place of the river otter model because the otter model used in the loss assessment was not a published model, was overly simplistic, and did not provide an accurate assessment of riparian condition. The beaver model, however, provides a detailed evaluation of overstory class structure that the NPT felt was a good compliment to the yellow warbler and song sparrow models that evaluated understory shrub layers. Overall, such substitutions should result in a more accurate evaluation of the ecological conditions on Precious Lands, and provide better information for decision making. A baseline HEP analysis was initiated on the Precious Lands in 2000, and data collection continued throughout the 2001 and 2002 field seasons. In the future, HEP analysis will be used to evaluate habitat changes resulting from management activities. Repeat surveys will be useful in assessing long-term trends in plant community health, weed encroachment, wildlife limiting factors, habitat degradation, and establishing desired future condition guidelines for the management program.

    In 2000 and 2001, the Kalispel Natural Resource Department (KNRD) continued to mitigate the wildlife habitat losses as part of the Albeni Falls Wildlife Mitigation Project. Utilizing Bonneville Power Administration (BPA) funds, the Kalispel Tribe of Indians (Tribe) purchased three projects totaling nearly 1,200 acres. The Tacoma/Trimble Wildlife Management Area is a conglomeration of properties now estimated at 1,700 acres. It is the Tribe's intent to manage these properties in cooperation and collaboration with the Pend Oreille County Public Utility District (PUD) No. 1 and the U.S. Fish and Wildlife Service (USFWS) to benefit wildlife habitats and associated species, populations, and guilds.

    The Air Quality Utility Information System (AQUIS) is a database management system that operates under dBASE IV. It runs on an IBM-compatible personal computer (PC) with MS DOS 5.0 or later, 4 megabytes of memory, and 30 megabytes of disk space. AQUIS calculates emissions for both traditional and toxic pollutants and reports emissions in user-defined formats. The system was originally designed for use at 7 facilities of the Air Force Materiel Command, and now more than 50 facilities use it. Within the last two years, the system has been used in support of Title V permit applications at Department of Defense facilities. Growth in the user community, changes and additions to reference emission factor data, and changing regulatory requirements have demanded additions and enhancements to the system. These changes have ranged from adding or updating an emission factor to restructuring databases and adding new capabilities. Quality assurance (QA) procedures have been developed to ensure that emission calculations are correct even when databases are reconfigured and major changes in calculation procedures are implemented. This paper describes these QA and updating procedures. Some user facilities include light industrial operations associated with aircraft maintenance. These facilities have operations such as fiberglass and composite layup and plating operations for which standard emission factors are not available or are inadequate.     On projects that require Earned Value Management (EVMS) Certification, it is critical to quickly prepare for and then successfully obtain certification. This is especially true for government contracts. Projects that do poorly during the review are subject to financial penalties to their company and they lose creditability with their customer creating problems with the project at the outset. At East Tennessee Technology Park (ETTP), we began preparing for Department of Energy (DOE) certification early during proposal development. Once the contract was awarded, while still in transition phase from the previous contractor to our new company, we immediately began reviewing the project controls systems that were in place on the project and determined if any replacements needed to be made immediately. The ETTP contract required the scheduling software to be upgraded to Primavera P6 and we determined that no other software changes would be done prior to certification. Next, preparation of the Project Controls System Description (PCSD) and associated procedures began using corporate standards as related to the project controls systems. During the transition phase, development was started on the Performance Measurement Baseline which is the resource loaded schedule used to measure our performance on the project and which is critical to good Earned Value Management of the project. Early on, and throughout the baseline review, there was positive feedback from the Department of Energy that the quality of the new baseline was good. Having this superior baseline also contributed to our success in EVMS certification. The combined companies of URS and CH2M Hill had recent experience with certifications at other Department of Energy sites and we were able to capitalize on that knowledge and experience. Generic PCSD and procedures consistent with our co-operations approach to Earned Value Management were available to us and were easily tailorable to the specifics of our contract and site. We also had corporate EVMS experts available to us so as to draw upon their recent certification experiences with lessons learned. This knowledge was especially helpful for training of personnel that were involved in the certification which included Project Controls, Project Management and Control Account Managers. We were also able to bring in these corporate experts to assist with our training efforts. To assure our readiness for the review, we conducted a 'White Hat' review. The 'White Hat' team consisted of corporate experts in EVMS along with an industry expert in EVMS from Humphrey and Associates. This review identified early any weaknesses that we had so corrections could be enacted prior to the EVMS Certification Readiness Review. It also helped give the evaluators confidence that we had done proper due diligence prior to their arrival. Also critical to our success, was early communication with our evaluators. It is important to start the communications early to ensure you understand the expectations of the certification team and the process that will be used during the certification. Communication through the entire process is critical to understand expectations and issues along the way. Very important to the overall process was management commitment, support and reinforcement. Management made sure that all personnel involved knew the importance and made preparations a priority. This was noted as a key strength by the evaluators during the out-brief. As a result of our preparation, our review yielded one Corrective Action Report (CAR) and two Continuous Improvement Opportunities (CIOs). The Certification team in their out-brief explained that this was the lowest number of CARs and CIOs in the history of EVMS certifications in the DOE Complex. (authors)

    The primary goal of this project was to demonstrate a new and novel approach for high resolution, 3D reservoir characterization that can enable better management of CO{sub 2} enhanced oil recovery (EOR) projects and, looking to the future, carbon sequestration projects. The approach adopted has been the subject of previous research by the DOE and others, and relies primarily upon data-mining and advanced pattern recognition approaches. This approach honors all reservoir characterization data collected, but accepts that our understanding of how these measurements relate to the information of most interest, such as how porosity and permeability vary over a reservoir volume, is imperfect. Ideally the data needed for such an approach includes surface seismic to provide the greatest amount of data over the entire reservoir volume of interest, crosswell seismic to fill the resolution gap between surface seismic and wellbore-scale measurements, geophysical well logs to provide the vertical resolution sought, and core data to provide the tie to the information of most interest. These data are combined via a series of one or more relational models to enable, in its most successful application, the prediction of porosity and permeability on a vertical resolution similar to logs at each surface seismic trace location. In this project, the procedure was applied to the giant (and highly complex) SACROC unit of the Permian basin in West Texas, one of the world's largest CO{sub 2}-EOR projects and a potentially world-class geologic sequestration site. Due to operational scheduling considerations on the part of the operator of the field, the crosswell data was not obtained during the period of project performance (it is currently being collected however as part of another DOE project). This compromised the utility of the surface seismic data for the project due to the resolution gap between it and the geophysical well logs. An alternative approach was adopted that utilized a relational model to predict porosity and permeability profiles from well logs at each well location, and a 3D geostatistical variogram to generate the reservoir characterization over the reservoir volume of interest. A reservoir simulation model was built based upon this characterization and history-matched without making significant changes to it, thus validating the procedure. While not the same procedure as originally planned, the procedure ultimately employed proved successful and demonstrated that the general concepts proposed (i.e., data mining and advanced pattern recognition methods) have the flexibility to achieve the reservoir characterization objectives sought even with imperfect or incomplete data.

    Computerized procedures (CPs) are an emerging technology within nuclear power plant control rooms. While CPs have been implemented internationally in advanced control rooms, to date no US nuclear power plant has implemented CPs in its main control room (Fink et al., 2009). Yet, CPs are a reality of new plant builds and are an area of considerable interest to existing plants, which see advantages in terms of enhanced ease of use and easier records management by omitting the need for updating hardcopy procedures. The overall intent of this paper is to provide a characterization of human reliability analysis (HRA) issues for computerized procedures. It is beyond the scope of this document to propose a new HRA approach or to recommend specific methods or refinements to those methods. Rather, this paper serves as a review of current HRA as it may be used for the analysis and review of computerized procedures.

    This book explores and supports the argument that effective environmental management must be based on a multimedia approach, which focuses simultaneously on air, water, and waste and enables managers to assess the resulting financial, operation, and management benefits. The multimedia approach, which can be used to design an effective compliance program, includes proper waste and material handling management, systematic monitoring, and record keeping requirements. This approach integrates a wide array of environmental requirements and decision processes, which the authors examine in sixteen chapters, organized into four parts: the role of environmental management; environmental aspects of business operation, environmental processes; and environmental management trends. Within these parts, the authors highlight the development of modern environmental management and provide an overview of federal laws pertinent to multimedia environmental management.     EG&G at Rocky Flats, Golden, Colorado, experienced a high amount of low morale, due to the plant site having been designated to close, and the uneasiness of the working force was very visible. Some employees accepted early retirement in October 1992, however, all received letters of 120 days notice in March 1993, and were advised several cuts Would be made by October 1, 1993. This information alone caused many insecurities in employees, and caused morale to decrease even more. This is an in depth study of morale, which was upgraded in Plant Procedures Group (PPG), through the effect of TQM. The primary research included a survey of employees with results included. The study also increased additional questions in PPG, some of which were added to the agenda of the Process Improvement Team (PIT) to improve PPG in the eyes of customers. Statistics did show that morale improved, not necessarily because of TQM or the progress of the Process Improvement Team (PIT), but due to efforts of the staff implementing the principles of TQM the best they knew how.

    An online data driven computerized procedures system that guides an operator through a complex process facility's operating procedures. The system monitors plant data, processes the data and then, based upon this processing, presents the status of the current procedure step and/or substep to the operator. The system supports multiple users and a single procedure definition supports several interface formats that can be tailored to the individual user. Layered security controls access privileges and revisions are version controlled. The procedures run on a server that is platform independent of the user workstations that the server interfaces with and the user interface supports diverse procedural views.

    This Configuration Management Plan (CMP) describes the technical and administrative management process for controlling the National Ignition Facility (NIF) Project configuration. Detailed procedures that make up the overall process are referenced. This CMP is consistent with guidance for managing a project's configuration provided in Department of Energy (DOE) Order 430.1, Guide PMG 10, ''Project Execution and Engineering Management Planning''. Configuration management is a formal discipline comprised of the following four elements: (1) Identification--defines the functional and physical characteristics of a Project and uniquely identifies the defining requirements. This includes selection of components of the end product(s) subject to control and selection of the documents that define the project and components. (2) Change management--provides a systematic method for managing changes to the project and its physical and functional configuration to ensure that all changes are properly identified, assessed, reviewed, approved, implemented, tested, and documented. (3) Data management--ensures that necessary information on the project and its end product(s) is systematically recorded and disseminated for decision-making and other uses. Identifies, stores and controls, tracks status, retrieves, and distributes documents. (4) Assessments and validation--ensures that the planned configuration requirements match actual physical configurations and approved changes are implemented according to the change requirements documents.

    The paper is based on a feasibility study investigating the possibilities of using a HIPPS (High Integrity Pressure Protection System) to protect a subsea pipeline that is not rated for full wellhead shut-in pressure. The study was called the Subsea OPPS Feasibility Study, and was performed by SINTEF, Norway. Here, OPPS is an acronym for Overpressure Pipeline Protection System. A design procedure for a subsea HIPPS is described, based on the experience and knowledge gained through the ``Subsea OPPS Feasibility Study``. Before a subsea HIPPS can be applied, its technical feasibility, reliability and profitability must be demonstrated. The subsea HIPPS design procedure will help to organize and plan the design activities both with respect to development and verification of a subsea HIPPS. The paper also gives examples of how some of the discussed design steps were performed in the Subsea OPPS Feasibility Study. Finally, further work required to apply a subsea HIPPS is discussed.

    The readiness of the Data Acquisition and Control System (DACS) to provide monitoring and control of the mixer pump, directional drive system, and the instrumentation associated with the SY-101 tank and support systems, and the proper functioning of the DACS with new Model 984-785 Programmable Logic Controllers (PLCs), new MODBUS PLUS version 2.01 software for the PLCs, and version 3.72 of the GENESIS software will be systematically evaluated by performance of this procedure. The DACS, which is housed in a trailer located just outside of the north fence at the SY tank farm, receives input signals from a variety of sensors located in and around the SY-101 tanks. These sensors provide information such as: tank vapor space and ventilation system H{sub 2} concentration; tank waste temperature; tank pressure; waste density; operating pump parameters such as speed, flow, rotational position, discharge pressure, and internal temperature; strain (for major equipment); and waste level. The output of these sensors is conditioned and transmitted to the DACS computers where these signals are displayed, recorded, and monitored for out-of-specification conditions. If abnormal conditions are detected, then, in certain situations, the DACS automatically generates alarms and causes the system to abort pump operations.

    A procedure and tools for quantifying surface cleanliness are described. Cleanliness of a target surface is quantified by wiping a prescribed area of the surface with a flexible, bright white cloth swatch, preferably mounted on a special tool. The cloth picks up a substantial amount of any particulate surface contamination. The amount of contamination is determined by measuring the reflectivity loss of the cloth before and after wiping on the contaminated system and comparing that loss to a previous calibration with similar contamination. In the alternative, a visual comparison of the contaminated cloth to a contamination key provides an indication of the surface cleanliness.

    A robotic optical sedimentation recorder is described for the recordation of carbon flux in the oceans wherein both POC and PIC particles are captured at the open end of a submersible sampling platform, the captured particles allowed to drift down onto a collection plate where they can be imaged over time. The particles are imaged using three separate light sources, activated in sequence, one source being a back light, a second source being a side light to provide dark field illumination, and a third source comprising a cross polarized light source to illuminate birefringent particles. The recorder in one embodiment is attached to a buoyancy unit which is capable upon command for bringing the sedimentation recorder to a programmed depth below the ocean surface during recordation mode, and on command returning the unit to the ocean surface for transmission of recorded data and receipt of new instructions. The combined unit is provided with its own power source and is designed to operate autonomously in the ocean for extended periods of time.

    An electromagetic recording head is described for simultaneous recording of a plurality of signals within a small space on a magnetically semsitized medium.     A device is described for automatica1ly recording pulse annplitude distribution received from a counter. The novelty of the device consists of the over-all arrangement of conventional circuit elements to provide an easy to read permanent record of the pulse amplitude distribution during a certain time period. In the device a pulse analyzer separates the pulses according to annplitude into several channels. A scaler in each channel counts the pulses and operates a pen marker positioned over a drivable recorder sheet. Since the scalers in each channel have the sanne capacity, the control circuitry permits counting of the incoming pulses until one scaler reaches capacity, whereupon the input is removed and an internal oscillator supplies the necessary pulses to fill up the other scalers. Movement of the chart sheet is initiated wben the first scaler reaches capacity to thereby give a series of marks at spacings proportional to the time required to fill the remaining scalers, and accessory equipment marks calibration points on the recorder sheet to facilitate direct reading of the number of external pulses supplied to each scaler.

    Perry Nuclear Power Plant began developing a waste minimization plan early in 1991. In March of 1991 the plan was documented following a similar format to that described in the EPA Waste Minimization Opportunity Assessment Manual. Initial implementation involved obtaining management's commitment to support a waste minimization effort. The primary assessment goal was to identify all hazardous waste streams and to evaluate those streams for minimization opportunities. As implementation of the plan proceeded, non-hazardous waste streams routinely generated in large volumes were also evaluated for minimization opportunities. The next step included collection of process and facility data which would be useful in helping the facility accomplish its assessment goals. This paper describes the resources that were used and which were most valuable in identifying both the hazardous and non-hazardous waste streams that existed on site. For each material identified as a waste stream, additional information regarding the materials use, manufacturer, EPA hazardous waste number and DOT hazard class was also gathered. Once waste streams were evaluated for potential source reduction, recycling, re-use, re-sale, or burning for heat recovery, with disposal as the last viable alternative.

(LMES) has established and maintains Document Management Centers (DMCs) to support Environmental Restoration Program (ER) activities undertaken at three Oak Ridge facilities: Oak Ridge National Laboratory, Oak Ridge K-25 Site, Oak Ridge Y-12 Plant; and two sister sites: Portsmouth Gaseous Diffusion Plant in Portsmouth, Ohio, and Paducah Gaseous Diffusion Plant in Paducah, Kentucky. The role of the DMCs is to receive, store, retrieve, and properly dispose of records. In an effort to make the DMCs run more efficiently and to more proactively manage the records` life cycles from cradle to grave, ER has decided to investigate ways in which Electronic Document Management System (EDMS) technologies can be used to redefine the DMCs and their related processes. Specific goals of this study are tightening control over the ER documents, establishing and enforcing record creation and retention procedures, speeding up access to information, and increasing the accessibility of information. A working pilot of the solution is desired within the next six months. Based on a series of interviews conducted with personnel from each of the DMCs, key management, and individuals representing related projects, it is recommended that ER utilize document management, full-text retrieval, and workflow technologies to improve and automate records management for the ER program. A phased approach to solution implementation is suggested starting with the deployment of an automated storage and retrieval system at Portsmouth. This should be followed with a roll out of the system to the other DMCs, the deployment of a workflow-enabled authoring system at Portsmouth, and a subsequent roll out of this authoring system to the other sites.

The Specified Assurance Level (SAL) Sampling Procedure has been developed to permit the direct selection of attribute sampling plans which can meet commonly used assurance levels. The SAL procedure contains sampling plans which yield the minimum sample size at stated assurance levels. The SAL procedure also provides sampling plans with acceptance numbers ranging from 0 to 10, thus, making available to the user a wide choice of plans all designed to comply with a stated assurance level.

    The US Department of Energy (DOE) Idaho National Laboratory (INL) site located near Idaho Falls, ID USA, has developed a comprehensive waste management and tracking tool that integrates multiple operational activities with characterization data from waste declaration through final waste disposition. The Integrated Waste Tracking System (IWTS) provides information necessary to help facility personnel properly manage their waste and demonstrate a wide range of legal and regulatory compliance. As a client?server database system, the IWTS is a proven tracking, characterization, compliance, and reporting tool that meets the needs of both operations and management while providing a high level of flexibility. This paper describes some of the history involved with the development and current use of IWTS as a comprehensive waste management tool as well as a discussion of IWTS deployments performed by the INL for outside clients. Waste management spans a wide range of activities including: work group interactions, regulatory compliance management, reporting, procedure management, and similar activities. The IWTS documents these activities and performs tasks in a computer-automated environment. Waste characterization data, container characterization data, shipments, waste processing, disposals, reporting, and limit compliance checks are just a few of the items that IWTS documents and performs to help waste management personnel perform their jobs. Throughout most hazardous and radioactive waste generating, storage and disposal sites, waste management is performed by many different groups of people in many facilities. Several organizations administer their areas of waste management using their own procedures and documentation independent of other organizations. Files are kept, some of which are treated as quality records, others not as stringent. Quality records maintain a history of: changes performed after approval, the reason for the change(s), and a record of whom and when the changes were made. As regulations and permits change, and as the proliferation of personal computers flourish, procedures and data files begin to be stored in electronic databases. With many different organizations, contractors, and unique procedures, several dozen databases are used to track and maintain aspects of waste management. As one can see, the logistics of collecting and certifying data from all organizations to provide comprehensive information would not only take weeks to perform, but usually presents a variety of answers that require an immediate unified resolution. A lot of personnel time is spent scrubbing the data in order to determine the correct information. The issue of disparate data is a concern in itself, and is coupled with the costs associated with maintaining several separate databases. In order to gain waste management efficiencies across an entire facility or site, several waste management databases located among several organizations would need to be consolidated. The IWTS is a system to do just that, namely store and track containerized waste information for an entire site. The IWTS has proven itself at the INL since 1995 as an efficient, successful, time saving management tool to help meet the needs of both operations and management for hazardous and radiological containerized waste.     The U.S. Department of Energy (DOE) Office of Legacy Management (LM) is an integral part of DOE's strategy to ensure that legacy liabilities of former nuclear weapons production sites are properly managed following the completion of environmental cleanup activities. In the area of environmental legacy management, records management is crucial to the protection of health, environmental, and legal interests of the Department and the public. LM is responsible for maintaining long-term surveillance and maintenance (LTS and M) records in performance of its mission. Maintaining access to the knowledge contained in these record collections is one of LM's primary responsibilities. To fulfill this responsibility, LM established a consolidated records management facility, the LM Business Center (LMBC), to house physical media records and electronic records. A new electronic record keeping system (ERKS) was needed to replace an obsolete system while helping to ensure LM is able to meet ongoing responsibilities to maintain access to knowledge and control the life cycle management of records.     Neither the National Environmental Policy Act (NEPA) nor the Council on Environmental Quality (CEQ) Regulations for implementing NEPA address the contents of an administrative record (AR). The AR typically contains the documents and information used in the development of NEPA documents and supports the decisions defined in them. The AR also should include all records pertaining to public comments and all records demonstrating the project`s efforts to involve the public. This paper will attempt to establish comprehensive guidelines to be used in assembling an AR in support of a NEPA document. While the AR is created to support an agency`s decisions, its main purpose is to demonstrate that an agency has adhered to NEPA`s procedural requirements. The CEQ requires that relevant environmental documents, comments and responses be part of the record in formal rulemaking or adjudicatory proceedings. Other Federal agency NEPA implementing procedures generally do not provide additional guidance on the contents of an AR. The CEQ and DOE guidelines make reference to the inclusion of relevant NEPA documents. The guidelines established in this paper will aid the NEPA practitioner in determining what constitutes a relevant NEPA document.

    The nuclear power industry is very procedure driven, i.e. almost all activities that take place at a nuclear power plant are conducted by following procedures. The paper-based procedures (PBPs) currently used by the industry do a good job at keeping the industry safe. However, these procedures are most often paired with methods and tools put in place to anticipate, prevent, and catch errors related to hands-on work. These tools are commonly called human performance tools. The drawback with the current implementation of these tools is that the task of performing one procedure becomes time and labor intensive. For example, concurrent and independent verification of procedure steps are required at times, which essentially means that at least two people have to be actively involved in the task. Even though the current use of PBPs and human performance tools are keeping the industry safe, there is room for improvement. The industry could potentially increase their efficiency and safety by replacing their existing PBPs with CBPs. If implemented correctly, the CBP system could reduce the time and focus spent on using the human performance tools. Some of the tools can be completely incorporated in the CBP system in a manner that the performer does not think about the fact that these tools are being used. Examples of these tools are procedure use and adherence, placekeeping, and peer checks. Other tools can be partly integrated in a fashion that reduce the time and labor they require, such as concurrent and independent verification. The incorporation of advanced technology, such as CBP systems, may help to manage the effects of aging systems, structures, and components. The introduction of advanced technology may also make the existing LWR fleet more attractive to the future workforce, which will be of importance when the future workforce will chose between existing fleet and the newly built nuclear power plants.

    This research effort is a part of the Light-Water Reactor Sustainability (LWRS) Program, which is a research and development (R&D) program sponsored by Department of Energy (DOE), performed in close collaboration with industry R&D programs, to provide the technical foundations for licensing and managing the long-term, safe, and economical operation of current nuclear power plants. The LWRS program serves to help the U.S. nuclear industry adopt new technologies and engineering solutions that facilitate the continued safe operation of the plants and extension of the current operating licenses. The introduction of advanced technology in existing nuclear power plants may help to manage the effects of aging systems, structures, and components. In addition, the incorporation of advanced technology in the existing LWR fleet may entice the future workforce, who will be familiar with advanced technology, to work for these utilities rather than more newly built nuclear power plants. Advantages are being sought by developing and deploying technologies that will increase safety and efficiency. One significant opportunity for existing plants to increase efficiency is to phase out the paper-based procedures (PBPs) currently used at most nuclear power plants and replace them, where feasible, with computer-based procedures (CBPs). PBPs have ensured safe operation of plants for decades, but limitations in paper-based systems do not allow them to reach the full potential for procedures to prevent human errors. The environment in a nuclear power plant is constantly changing depending on current plant status and operating mode. PBPs, which are static by nature, are being applied to a constantly changing context. This constraint often results in PBPs that are written in a manner that is intended to cover many potential operating scenarios. Hence, the procedure layout forces the operator to search through a large amount of irrelevant information to locate the pieces of information relevant for the task and situation at hand, which has potential consequences of taking up valuable time when operators must be responding to the situation, and potentially leading operators down an incorrect response path. Other challenges related to PBPs are the management of multiple procedures, place-keeping, finding the correct procedure for the task at hand, and relying on other sources of additional information to ensure a functional and accurate understanding of the current plant status (Converse, 1995; Fink, Killian, Hanes, & Naser, 2009; Le Blanc & Oxstrand, 2012). The main focus of this report is to describe the research activities conducted to address the remaining two objectives; Develop a prototype CBP system based on requirements identified and Evaluate the CBP prototype. The emphasis will be on the evaluation of an initial CBP prototype in at a Nuclear Power Plant.

    The Project Records Information System (PRIS) is an interactive system developed for the Information Services Division (ISD) of Martin Marietta Energy Systems, Inc., to perform indexing, maintenance, and retrieval of information about Engineering project record documents for which they are responsible. This PRIS User's Manual provides instruction on the use of this system. This manual presents an overview of PRIS, describing the system's purpose; the data that it handles; functions it performs; hardware, software, and access; and help and error functions. This manual describes the interactive menu-driven operation of PRIS. Appendixes A, B, C, and D contain the data dictionary, help screens, report descriptions, and a primary menu structure diagram, respectively.

    TRAIN-UNIX is used to track training requirements, qualifications, training completion and schedule training, classrooms and instructors. TRAIN-UNIX is a requirements-based system. When the identified training requirements for specific jobs are entered into the system, the employees manager or responsible training person assigns jobs to an employee. TRAIN-UNIX will then assemble an Individual Training Plan (ITP) with all courses required. ITP''s can also be modified to add any special training directed or identified by management, bestmore » business practices, procedures, etc. TRAIN-UNIX also schedules and tracks conferences, seminars, and required reading. TRAIN-UNIX is a secure database system on a server accessible via the network. Access to the user functions (scheduling, data entry, ITP modification etc.) within TRAIN-UNIX are granted by function, as needed, by the system administrator. An additional level of security allows those who access TRAIN-UNIX to only add, modify or view information for the organizations to which they belong. TRAIN-UNIX scheduling function allows network access to scheduling of students. As a function of the scheduling process, TRAIN-UNIX checks to insure that the student is a valid employee, not double booked, and the instructor and classroom are not double booked. TRAIN-UNIX will report pending lapse of courses or qualifications. This ability to know the lapse of training along with built in training requesting function allows the training deliverers to forecast training needs.« less

    This paper provides a characterization of human reliability analysis (HRA) issues for computerized procedures in nuclear power plant control rooms. It is beyond the scope of this paper to propose a new HRA approach or to recommend specific methods or refinements to those methods. Rather, this paper provides a review of HRA as applied to traditional paper-based procedures, followed by a discussion of what specific factors should additionally be considered in HRAs for computerized procedures. Performance shaping factors and failure modes unique to computerized procedures are highlighted. Since there is no definitive guide to HRA for paper-based procedures, this paper also serves to clarify the existing guidance on paper-based procedures before delving into the unique aspects of computerized procedures.

    ''Legacy Management Requires Information'' describes the goal(s) of the US Department of Energy's Office of Legacy Management (LM) relative to maintaining critical records and the way those goals are being addressed at Hanford. The paper discusses the current practices for document control, as well as the use of modern databases for both storing and accessing the data to support cleanup decisions. In addition to the information goals of LM, the Hanford Federal Facility Agreement and Consent Order, known as the ''Tri-Party Agreement'' (TPA) is one of the main drivers in documentation and data management. The TPA, which specifies discrete milestones for cleaning up the Hanford Site, is a legally binding agreement among the US Department of Energy (DOE), the Washington State Department of Ecology (Ecology), and the US Environmental Protection Agency (EPA). The TPA requires that DOE provide the lead regulatory agency with the results of analytical laboratory and non-laboratory tests/readings to help guide them in making decisions. The Agreement also calls for each signatory to preserve--for at least ten years after the Agreement has ended--all of the records in its or its contractors, possession related to sampling, analysis, investigations, and monitoring conducted. The tools used at Hanford to meet TPA requirements are also the tools that can satisfy the needs of LM.

    This article describes software being developed to address the need for computerized planning and documentation programs that can help manage outages. Downsized power-utility companies and the growing demand for independent, competitive engineering and maintenance services have created a need for a computer-assisted planning and technical-direction program for turbine-generator outages. To meet this need, a software tool is now under development that can run on a desktop or laptop personal computer to assist utility personnel and technical directors in outage planning. Total Outage Planning Software (TOPS), which runs on Windows, takes advantage of the mass data storage available with compact-disc technology by archiving the complete outage documentation on CD. Previous outage records can then be indexed, searched, and viewed on a computer with the click of a mouse. Critical-path schedules, parts lists, parts order tracking, work instructions and procedures, custom data sheets, and progress reports can be generated by computer on-site during an outage.

    CH2M HILL Plateau Remediation Company (CH2M HILL) is the U.S. Department of Energy (DOE) prime contractor responsible for the environmental cleanup of the Hanford Site Central Plateau. As part of this responsibility, the CH2M HILL is faced with the task of complying with thousands of environmental requirements which originate from over 200 federal, state, and local laws and regulations, DOE Orders, waste management and effluent discharge permits, Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) response and Resource Conservation and Recovery Act (RCRA) corrective action documents, and official regulatory agency correspondence. The challenge is to manage this vast number of requirements to ensure they are appropriately and effectively integrated into CH2M HILL operations. Ensuring compliance with a large number of environmental requirements relies on an organization’s ability to identify, evaluate, communicate, and verify those requirements. To ensure that compliance is maintained, all changes need to be tracked. The CH2M HILL identified that the existing system used to manage environmental requirements was difficult to maintain and that improvements should be made to increase functionality. CH2M HILL established an environmental requirements management procedure and tools to assure that all environmental requirements are effectively and efficiently managed. Having a complete and accurate set of environmental requirements applicable to CH2M HILL operations will promote a more efficient approach to: • Communicating requirements • Planning work • Maintaining work controls • Maintaining compliance

    This document is the distribution integrity management plan (Plan) for the Los Alamos National Laboratory (LANL) Natural Gas Distribution System. This Plan meets the requirements of 49 CFR Part 192, Subpart P Distribution Integrity Management Programs (DIMP) for the LANL Natural Gas Distribution System. This Plan was developed by reviewing records and interviewing LANL personnel. The records consist of the design, construction, operation and maintenance for the LANL Natural Gas Distribution System. The records system for the LANL Natural Gas Distribution System is limited, so the majority of information is based on the judgment of LANL employees; the maintenance crew, the Corrosion Specialist and the Utilities and Infrastructure (UI) Civil Team Leader. The records used in this report are: Pipeline and Hazardous Materials Safety Administration (PHMSA) 7100.1-1, Report of Main and Service Line Inspection, Natural Gas Leak Survey, Gas Leak Response Report, Gas Leak and Repair Report, and Pipe-to-Soil Recordings. The specific elements of knowledge of the infrastructure used to evaluate each threat and prioritize risks are listed in Sections 6 and 7, Threat Evaluation and Risk Prioritization respectively. This Plan addresses additional information needed and a method for gaining that data over time through normal activities. The processes used for the initial assessment of Threat Evaluation and Risk Prioritization are the methods found in the Simple, Handy Risk-based Integrity Management Plan (SHRIMP{trademark}) software package developed by the American Pipeline and Gas Agency (APGA) Security and Integrity Foundation (SIF). SHRIMP{trademark} uses an index model developed by the consultants and advisors of the SIF. Threat assessment is performed using questions developed by the Gas Piping Technology Company (GPTC) as modified and added to by the SHRIMP{trademark} advisors. This Plan is required to be reviewed every 5 years to be continually refined and improved. Records for all piping system installed after the effective date of this Plan will be captured and retained in the UI records documentation system. Primary Utility Asbuilts are maintained by Utilities Mapping (UMAP) and additional records are maintained on the N drive. Engineering Change Notices (ECNs) are stored on the N drive under configuration management and kept up by Utilities and Infrastructure Division Office (UI-DO). Records include, at a minimum, the location where new piping and appurtenances are installed and the material of which they are constructed.

    Many companies are developing and implementing Process Safety Management (PSM) systems. Various PSM models, including those by the Center for Chemical Process Safety (CCPS), the American Petroleum Institute (API), the Chemical Manufacturers Association (CMA) and OSHA have emerged to guide the design, development and installation of these systems. These models represent distillations of the practices, methods and procedures successfully used by those who believed that a strong correlation exists between sound PSM practices and achieving reductions in the frequency and severity of process incidents. This paper describes the progress of CCPS research toward developing a PSM performance measurement model. It also provides a vision for future CCPS research to define effectiveness indices.

    Following graduation from a college or university with a technical degree, or through years of experience, an individual's training and career development activities typically focus on enhancing technical problem-solving skills. However, as these technical professionals, herein referred to as 'Techies', advance throughout their careers, they may be required to accept and adapt to the role of being a manager, and must undergo a transition to learn and rely on new problem-solving skills. However, unless a company has a specific manager-trainee class to address this subject and develop talent from within, an employee's management style is learned and developed 'on the job'. Both positive and negative styles are nurtured by those managers having similar qualities. Unfortunately, a negative style often contributes to the deterioration of employee morale and ultimate closing of a department or company. This paper provides the core elements of an effective management training program for 'Teaching Managers How to Manage' derived from the Department of Energy's Integrated Safety Management System and the Occupational Safety and Health Administration 's Voluntary Protection Program. Discussion topics and real-life examples concentrate on transitioning an employee from a 'Techie' to a manager; common characteristics of being a manager; the history and academic study of management; competition, change and the business of waste management; what to do after taking over a department by applying Hylko's Star of Success; command media; the formal and informal organizational charts; chain of command; hiring and developing high-degree, autonomous employees through effective communication and delegation; periodic status checks; and determining if the program is working successfully. These common characteristics of a strong management/leadership culture and practical career tips discussed herein provide a solid foundation for any company or department that is serious about developing an effective management training program for its employees. In turn, any employee in any work environment can begin using this information immediately if they want to become a better manager. (authors)
