    The Environmental Protection Agency and Nuclear Regulatory Commission have established environmental radiation standards for specific practices which correspond to limits on risk to the public that vary by several orders of magnitude and often are much less than radiation risks that are essentially unregulated, e.g., risks from radon in homes. This paper discusses a proposed framework for environmental radiation standards that would improve the correspondence with limitation of risk. This framework includes the use of limits on annual effective dose equivalent averaged over a lifetime, rather than limits on dose equivalent to whole body or any organ for each year of exposure, and consideration of exposures of younger age groups as well as adults; limits on annual effective dose equivalent averaged over a lifetime no lower than 0.25 mSv (25 mrem) per practice; maintenance of all exposures as low as reasonably achievable (ALARA); and establishment of a generally applicable de minimis dose for public exposures. Implications of the proposed regulatory framework for the current system of standards for limiting public exposures are discussed. 20 refs.

    The US Environmental Protection Agency`s (EPA) proposed rule to certify that the Waste Isolation Pilot Plant (WIPP) meets compliance with the long-term radiation protection standards for geologic repositories (40CFR191 Subparts B and C), is one of the most significant milestones to date for the WIPP project in particular, and for the nuclear waste issue in general. The Environmental Evaluation Group (EEG) has provided an independent technical oversight for the WIPP project since 1978, and is responsible for many improvements in the location, design, and testing of various aspects of the project, including participation in the development of the EPA standards since the early 1980s. The EEG reviewed the development of documentation for assessing the WIPP`s compliance by the Sandia National Laboratories following the 1985 promulgation by EPA, and provided many written and verbal comments on various aspects of this effort, culminating in the overall review of the 1992 performance assessment. For the US Department of Energy`s (DOE) compliance certification application (CCA), the EEG provided detailed comments on the draft CCA in March, 1996, and additional comments through unpublished letters in 1997 (included as Appendices 8.1 and 8.2 in this report). Since the October 30, 1997, publication of the EPA`s proposed rule to certify WIPP, the EEG gave presentations on important issues to the EPA on December 10, 1997, and sent a December 31, 1997 letter with attachments to clarify those issues (Appendix 8.3). The EEG has raised a number of questions that may have an impact on compliance. In spite of the best efforts by the EEG, the EPA reaction to reviews and suggestions has been slow and apparently driven by legal considerations. This report discusses in detail the questions that have been raised about containment requirements. Also discussed are assurance requirements, groundwater protection, individual protection, and an evaluation of EPA`s responses to EEG`s comments.

    The procedures developed previously to simulate the radiatively induced tokamak density limit are used to examine in more detail the scaling of the density limit. It is found that the maximum allowable density increases with auxiliary power and decreases with impurity concentration. However, it is demonstrated that there is little dependence of the density limit on plasma elongation. These trends are consistent with experimental results. Our previous work used coronal equilibrium impurities; the primary result of that paper was that the maximum density increases with current when peaked profiles are assumed. Here, this behavior is shown to occur with a coronal nonequilibrium impurity as well. 26 refs., 4 figs.

  16. Physics of Radiation-driven Islands Near the Tokamak Density Limit

    In previous work [1], the onset criterion for radiation driven islands [2] in combination with a simple cylindrical model of tokamak current channel behavior was shown to be consistent with the empirical scaling of the tokamak density limit [3]. A number of the unexplained phenomena at the density limit are consistent with this novel physics mechanism. In this work, a more formal theoretical underpinning, consistent with cylindrical tearing mode theory, is developed for the onset criteria of these modes. The appropriate derivation of the radiation-driven addition to the modified Rutherford equation is discussed. Additionally, the ordering of the terms in the MRE is examined in a regime near the density limit. It is hoped that given the apparent success of this simple model in explaining the observed global scalings will lead to a more comprehensive analysis of the possibility that radiation driven islands are the physics mechanism responsible for the density limit. In particular, with modern diagnostic capabilities detailed measurements of current densities, electron densities and impurity concentrations at rational surfaces should be possible, enabling verification of the concepts described above.

    This paper attempts to unify the asymptotic diffusion limit analysis of thermal radiation transport schemes, for a linear-discontinuous representation of the material temperature reconstructed from cell centred temperature unknowns, in a process known as ‘source tilting’. The asymptotic limits of both Monte Carlo (continuous in space) and deterministic approaches (based on linear-discontinuous finite elements) for solving the transport equation are investigated in slab geometry. The resulting discrete diffusion equations are found to have nonphysical terms that are proportional to any cell-edge discontinuity in the temperature representation. Based on this analysis it is possible to design accurate schemes for representing the material temperature, for coupling thermal radiation transport codes to a cell centred representation of internal energy favoured by ALE (arbitrary Lagrange–Eulerian) hydrodynamics schemes.

    We present an analysis of the general relativistic Boltzmann equation for radiation, appropriate to the case where particles and photons interact through Thomson scattering, and derive the radiation energy-momentum tensor in the diffusion limit with viscous terms included. Contrary to relativistic generalizations of the viscous stress tensor that appear in the literature, we find that the stress tensor should contain a correction to the comoving energy density proportional to the divergence of the four-velocity, as well as a finite bulk viscosity. These modifications are consistent with the framework of radiation hydrodynamics in the limit of large optical depth, and do not depend on thermodynamic arguments such as the assignment of a temperature to the zeroth-order photon distribution. We perform a perturbation analysis on our equations and demonstrate that as long as the wave numbers do not probe scales smaller than the mean free path of the radiation, the viscosity contributes only decaying, i.e., stable, corrections to the dispersion relations. The astrophysical applications of our equations, including jets launched from super-Eddington tidal disruption events and those from collapsars, are discussed and will be considered further in future papers.

    The quantitative procedures associated with noncancer risk assessment include reference dose (RfD), benchmark dose, and severity modeling. The RfD, which is part of the EPA risk assessment guidelines, is an estimation of a level that is likely to be without any health risk to sensitive individuals.     What the EPA, working jointly with the states, hopes to accomplish over the next ten years in order to integrate and coordinate all the groundwater programs within the agency is discussed. Although many other EPA programs such as Superfund, Clean Air Act, and Wetlands Management are often highlighted in the media, EPA does not down rate the importance of groundwater protection. Indeed as a resource, it is one of the most important commodities. Groundwater is the basis for life in this country. Recharge rates are no where near what the withdrawal rates are in many areas of the country. Twenty-five percent of all the potable water comes from groundwater. Groundwater supplies 50 percent of the needs for all the US population. If you include strictly rural areas, it supplies 95 percent of all the use. Something that most people who are not groundwater hydrologists would not think about is the fact that groundwater is a recharge mechanism that provides over 30 percent of the flow in streams and major rivers.

    Large discrepancies between the laminar flame speeds and Markstein lengths measured in experiments and those predicted by simulations for ultra-lean methane/air mixtures bring a great concern for kinetic mechanism validation. In order to quantitatively explain these discrepancies, a computational study is performed for propagating spherical flames of lean methane/air mixtures in different spherical chambers using different radiation models. The emphasis is focused on the effects of radiation and compression. It is found that the spherical flame propagation speed is greatly reduced by the coupling between thermal effect (change of flame temperature or unburned gas temperature) and flow effect (inward flow of burned gas) induced by radiation and/or compression. As a result, for methane/air mixtures near the lean flammability limit, the radiation and compression cause large amounts of under-prediction of the laminar flame speeds and Markstein lengths extracted from propagating spherical flames. Since radiation and compression both exist in the experiments on ultra-lean methane/air mixtures reported in the literature, the measured laminar flame speeds and Markstein lengths are much lower than results from simulation and thus cannot be used for kinetic mechanism validation.     The effects of active galactic nuclei (AGNs) on their host galaxies depend on the coupling between the injected energy and the interstellar medium (ISM). Here, we model and quantify the impact of long-range AGN ionizing radiation—in addition to the often considered small-scale energy deposition—on the physical state of the multi-phase ISM of the host galaxy and on its total star formation rate (SFR). We formulate an AGN spectral energy distribution matched with observations, which we use with the radiative transfer (RT) code Cloudy to compute AGN ionization in a simulated high-redshift disk galaxy. We use a high-resolution (∼6 pc) simulation including standard thermal AGN feedback and calculate RT in post-processing. Surprisingly, while these models produce significant AGN-driven outflows, we find that AGN ionizing radiation and heating reduce the SFR by a few percent at most for a quasar luminosity (L {sub bol} = 10{sup 46.5} erg s{sup –1}). Although the circumgalactic gaseous halo can be kept almost entirely ionized by the AGN, most star-forming clouds (n ≳ 10{sup 2} {sup –} {sup 3} cm{sup –3}) and even the reservoirs of cool atomic gas (n ∼ 0.3-10 cm{sup –3})—which are the sites of future star formation (SF; 100-200 Myr), are generally too dense to be significantly affected. Our analysis ignores any absorption from a putative torus, making our results upper limits on the effects of ionizing radiation. Therefore, while the AGN-driven outflows can remove substantial amounts of gas in the long term, the impact of AGN feedback on the SF efficiency in the interstellar gas in high-redshift galaxies is marginal, even when long-range radiative effects are accounted for.

    X-ray diffraction microscopy (XDM) is a new form of x-ray imaging that is being practiced at several third-generation synchrotron-radiation x-ray facilities. Nine years have elapsed since the technique was first introduced and it has made rapid progress in demonstrating high-resolution three-dimensional imaging and promises few-nm resolution with much larger samples than can be imaged in the transmission electron microscope. Both life- and materials-science applications of XDM are intended, and it is expected that the principal limitation to resolution will be radiation damage for life science and the coherent power of available x-ray sources for material science. In this paper wemore » address the question of the role of radiation damage. We use a statistical analysis based on the so-called "dose fractionation theorem" of Hegerl and Hoppe to calculate the dose needed to make an image of a single life-science sample by XDM with a given resolution. We find that for simply-shaped objects the needed dose scales with the inverse fourth power of the resolution and present experimental evidence to support this finding. To determine the maximum tolerable dose we have assembled a number of data taken from the literature plus some measurements of our own which cover ranges of resolution that are not well covered otherwise. The conclusion of this study is that, based on the natural contrast between protein and water and "Rose-criterion" image quality, one should be able to image a frozen-hydrated biological sample using XDM at a resolution of about 10 nm.« less

    Today's rapidly developing and changing technologies and industrial practices frequently carry with them the increased generation of wastes and materials which, if improperly managed, may threaten public health and the environment. The US Environmental Protection Agency is charged with the mission of protecting public health and the environment from the hazards posed by these wastes and materials. As part of its effort to achieve this mandate, it has recently adopted a Pollution Prevention Program. Among other things, the program encourages the development and adoption of processing technologies and products that will lead to reducing the aggregate generation rates for pollutants entering the environment. This paper will address EPA's efforts under its Pollution Prevention Program. The paper will address regulatory and non-regulatory action EPA has taken pursuant to the Resource Conservation and Recovery Act (RCRA), the Emergency Planning and Community Right-to-Know Act, the Pollution Prevention Act of 1990, and other Federal statutes. In addition, the paper will present case studies in pollution prevention.

    The Environmental Protection Agency (EPA) Office of Superfund Remediation and Technology Innovation (OSRTI) and the Nuclear Regulatory Commission (NRC) are responsible for implementing the 'Memorandum of Understanding Between the Environmental Protection Agency and the Nuclear Regulatory Commission: Consultation and Finality on Decommissioning and Decontamination of Contaminated Sites'. This paper provides a brief overview of the origin of the Memorandum of Understanding (MOU), the major features of the MOU, and how the MOU has been implemented site specifically. EPA and NRC developed the MOU in response to direction from the House Committee on Appropriations to EPA and NRC to work together to address the potential for dual regulation. The MOU was signed by EPA on September 30, 2002 and NRC on October 9, 2002. The two agencies had worked on the MOU since March 2000. While both EPA and NRC have statutory authority to clean up these sites, the MOU provides consultation procedures between EPA and NRC to eliminate dual regulation. Under the MOU, EPA and NRC identified the interactions of the two agencies for the decommissioning and decontamination of NRC-licensed sites and the ways in which those responsibilities will be exercised. Except for Section VI, which addresses corrective action under the Resource Conservation and Recovery Act (RCRA), this MOU is limited to the coordination between EPA, when acting under its CERCLA authority, and NRC, when a facility licensed by the NRC is undergoing decommissioning, or when a facility has completed decommissioning, and the NRC has terminated its license. EPA believes that implementation of the MOU between the two agencies will ensure that future confusion about dual regulation does not occur regarding the cleanup and reuse of NRC-licensed sites. NRC and EPA have so far exchanged MOU consultation letters on eight NRC-licensed sites. EPA has responded to each consultation request with a letter expressing its views on actions

    The U.S. Environmental Protection Agency (EPA) Superfund program's six Preliminary Remediation Goal (PRG) and Dose Compliance Concentration (DCC) internet based calculators for risk and dose assessment at Superfund sites are being revised to reflect better science, revisions to existing exposure scenarios and new scenarios, and changes to match up more closely with the EPA chemical regional screening level calculator. A revised version of the 1999 guidance document that provides an overview for the Superfund risk assessment process at radioactively contaminated sites, 'Radiation Risk Assessment At CERCLA Sites: Q and A', is being completed that will reflect Superfund recommended guidance and other technical documents issued over the past 13 years. EPA is also issuing a series of fact sheets in the document 'Superfund Radiation Risk Assessment: A Community Tool-kit'.     In Citizens Coal Council v. EPA, an en banc panel for the Sixth Circuit Court of Appeals considered a challenge to EPA regulations promulgated pursuant to the Clean Water Act (CWA). The EPA promulgated the regulations in an attempt to incentivize coal companies to remine once abandoned mine sites. Petitioners, two nonprofit environmental organizations, claimed that the regulations violated the Clean Water Act and Administrative Procedure Act by allowing coal companies to remine without adhering to any enforceable pollution limitations. The EPA countered that more remining would improve water quality at abandoned sites. The Sixth Circuit rejected Petitioners' claims, finding that the EPA's regulations were reasonably consistent with the CWA's goal of restoring the integrity of the nation's waters. In so holding, the court struggled to understand the meaning of the CWA's complex procedural and technical language, and allowed the EPA to justify the rule based on the CWA's broad statement of purpose. Such superficial judicial review sets a dangerous precedent in environmental law, because it exacerbates the risk of agency capture. A captured agency promulgates regulations that benefit-industry, not the environment. Without the judiciary acting as a meaningful check against agency capture, the public loses a valuable tool in the fight against major-industrial polluters like the domestic coal industry. Citizens Coal Council therefore stands as a cautionary tale, a warning sign that the judiciary may be unable to identify agency capture where the regulations at issue are promulgated pursuant to a complex statute like the Clean Water Act.

    Purpose: Challenges for radiation therapy in developing countries include unreliable infrastructure and high patient load. We propose a system to treat whole breast in the prone position without computed tomography and/or planning software. Methods: Six parameters are measured using calipers and levels with the patient prone in the treatment position. (1) The largest separation; (2) the angle that separation makes with the horizontal; (3) the separation 2 cm posterior to the nipple; (4) the vertical distance between these two separations; (5) the sup/inf length and (6) angle of the desired posterior field edge. The data in (5) (6) and (2) provide field length, collimator and gantry angles. Isocenter is set to the midpoint of (1), anterior jaw setting is 20cm (half-beam setup), and the dose is prescribed to a point 1.5 cm anterior to isocenter. MUs and wedge angles are calculated using an MU calculator and by requiring 100% dose at that point and 100-105% at the midpoint of (3). Measurements on 30 CT scans were taken to obtain the data 1-6. To test the resulting MU/wedge combinations, they were entered into Eclipse (Varian) and dose distributions were calculated. The MU/wedge combinations were recorded and tabulated. Results: Performing a dose volume histogram analysis, the contoured breast V95 was 90.5%, and the average V90 was 94.1%. The maximum dose never exceeded 114.5%, (average 108%). The lung V20 was <5% for 96.7%, and the heart V5 was <10% for 93.3% of our sample. Conclusion: A method to provide prone whole breast treatment without CT-planning was developed. The method provides reasonable coverage and normal tissue sparing. This approach is not recommended if imaging and planning capabilities are available; it was designed to specifically avoid the need for CT planning and should be reserved to clinics that need to avoid that step.

    As a planet ages, it cools and its radius shrinks at a rate set by the efficiency with which heat is transported from the interior out to space. The bottleneck for this transport is at the boundary between the convective interior and the radiative atmosphere; the opacity there sets the global cooling rate. Models of planetary evolution are often one dimensional (1D), such that the radiative-convective boundary (RCB) is defined by a single temperature, pressure, and opacity. In reality the spatially inhomogeneous stellar heating pattern and circulation in the atmosphere could deform the RCB, allowing heat from the interior to escape more efficiently through regions with lower opacity. We present an analysis of the degree to which the RCB could be deformed and the resultant change in the evolutionary cooling rate. In this initial work we calculate the upper limit for this effect by comparing an atmospheric structure in local radiative equilibrium to its 1D equivalent. We find that the cooling through an uneven RCB could be enhanced over cooling through a uniform RCB by as much as 10%-50%. We also show that the deformation of the RCB (and the enhancement of the cooling rate) increases with a greater incident stellar flux or a lower inner entropy. Our results indicate that this mechanism could significantly change a planet's thermal evolution, causing it to cool and shrink more quickly than would otherwise be expected. This may exacerbate the well-known difficulty in explaining the very large radii observed for some hot Jupiters.

  4. SU-F-BRE-01: A Rapid Method to Determine An Upper Limit On a Radiation Detector's Correction Factor During the QA of IMRT Plans

    Purpose: Discrepancies in the verification of the absorbed dose to water from an IMRT plan using a radiation dosimeter can be wither caused by 1) detector specific nonstandard field correction factors as described by the formalism of Alfonso et al. 2) inaccurate delivery of the DQA plan. The aim of this work is to develop a simple/fast method to determine an upper limit on the contribution of composite field correction factors to these discrepancies. Methods: Indices that characterize the non-flatness of the symmetrised collapsed delivery (VSC) of IMRT fields over detector-specific regions of interest were shown to be correlated with IMRT field correction factors. The indices introduced are the uniformity index (UI) and the mean fluctuation index (MF). Each one of these correlation plots have 10 000 fields generated with a stochastic model. A total of eight radiation detectors were investigated in the radial orientation. An upper bound on the correction factors was evaluated by fitting values of high correction factors for a given index value. Results: These fitted curves can be used to compare the performance of radiation dosimeters in composite IMRT fields. Highly water-equivalent dosimeters like the scintillating detector (Exradin W1) and a generic alanine detector have been found to have corrections under 1% over a broad range of field modulations (0 – 0.12 for MF and 0 – 0.5 for UI). Other detectors have been shown to have corrections of a few percent over this range. Finally, a full Monte Carlo simulations of 18 clinical and nonclinical IMRT field showed good agreement with the fitted curve for the A12 ionization chamber. Conclusion: This work proposes a rapid method to evaluate an upper bound on the contribution of correction factors to discrepancies found in the verification of DQA plans.

    Purpose: Severe (Radiation Therapy Oncology Group [RTOG] grade 3 or greater) esophagitis generally occurs in 15% to 25% of non–small cell lung cancer (NSCLC) patients undergoing concurrent chemotherapy and radiation therapy (CCRT), which may result in treatment breaks that compromise local tumor control and pose a barrier to dose escalation. Here, we report a novel contralateral esophagus-sparing technique (CEST) that uses intensity modulated radiation therapy (IMRT) to reduce the incidence of severe esophagitis. Methods and Materials: We reviewed consecutive patients with thoracic malignancies undergoing curative CCRT in whom CEST was used. The esophageal wall contralateral (CE) to the tumor was contoured as an avoidance structure, and IMRT was used to guide a rapid dose falloff gradient beyond the target volume in close proximity to the esophagus. Esophagitis was recorded based on the RTOG acute toxicity grading system. Results: We identified 20 consecutive patients treated with CCRT of at least 63 Gy in whom there was gross tumor within 1 cm of the esophagus. The median radiation dose was 70.2 Gy (range, 63-72.15 Gy). In all patients, ≥99% of the planning and internal target volumes was covered by ≥90% and 100% of prescription dose, respectively. Strikingly, no patient experienced grade ≥3 esophagitis (95% confidence limits, 0%-16%) despite the high total doses delivered. The median maximum dose, V45, and V55 of the CE were 60.7 Gy, 2.1 cc, and 0.4 cc, respectively, indicating effective esophagus cross-section sparing by CEST. Conclusion: We report a simple yet effective method to avoid exposing the entire esophagus cross-section to high doses. By using proposed CE dose constraints of V45 <2.5 cc and V55 <0.5 cc, CEST may improve the esophagus toxicity profile in thoracic cancer patients receiving CCRT even at doses above the standard 60- to 63-Gy levels. Prospective testing of CEST is warranted.

  9. Additional Survival Benefit of Involved-Lesion Radiation Therapy After R-CHOP Chemotherapy in Limited Stage Diffuse Large B-Cell Lymphoma

    Purpose: The purpose of this study was to evaluate the role of involved-lesion radiation therapy (ILRT) after rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP) chemotherapy in limited stage diffuse large B-cell lymphoma (DLBCL) by comparing outcomes of R-CHOP therapy alone with R-CHOP followed by ILRT. Methods and Materials: We identified 198 patients treated with R-CHOP (median, 6 cycles) for pathologically confirmed DLBCL of limited stage from July 2004 to December 2012. Clinical characteristics of these patients were 33% with stage I and 66.7% with stage II; 79.8% were in the low or low-intermediate risk group; 13.6% had B symptoms; 29.8% had bulky tumors (≥7 cm); and 75.3% underwent ≥6 cycles of R-CHOP therapy. RT was given to 43 patients (21.7%) using ILRT technique, which included the prechemotherapy tumor volume with a median margin of 2 cm (median RT dose: 36 Gy). Results: After a median follow-up of 40 months, 3-year progression-free survival (PFS) and overall survival (OS) were 85.8% and 88.9%, respectively. Multivariate analysis showed ≥6 cycles of R-CHOP (PFS, P=.004; OS, P=.004) and ILRT (PFS, P=.021; OS, P=.014) were favorable prognosticators of PFS and OS. A bulky tumor (P=.027) and response to R-CHOP (P=.012) were also found to be independent factors of OS. In subgroup analysis, the effect of ILRT was prominent in patients with a bulky tumor (PFS, P=.014; OS, P=.030) or an elevated level of serum lactate dehydrogenase (LDH; PFS, P=.004; OS, P=.012). Conclusions: Our results suggest that ILRT after R-CHOP therapy improves PFS and OS in patients with limited stage DLBCL, especially in those with bulky disease or an elevated serum LDH level.

    In December 1993, the US Environmental Protection Agency (EPA) released its final rule that sets for the details for requirements to sell reformulated gasoline (RFG) in certain ozone non-attainment areas. At the same time, EPA also issued a proposed rule to require that 30% of the oxygen required in RFG be based on a renewable oxygenate. Renewables include ethanol and its ether derivatives such as ethyl tertiary butyl ether (ETBE). The RFG rule is a final rule, while the Renewable Oxygenate Requirement (ROR) rule is a proposed rule yet to be finalized and subject to revision. Included in this paper are brief reviews of Ashland petroleum Company`s ethanol usage, oxygenated fuel and reformulated gasoline blending economics, and some comments on the EPA proposed renewable oxygenate requirement.

    Purpose: To investigate whether high-dose thoracic radiation given twice daily during cisplatin-etoposide chemotherapy for limited small-cell lung cancer (LSCLC) improves survival, acute esophagitis, and local control rates relative to findings from Intergroup trial 0096 (47%, 27%, and 64%). Patients and Methods: Patients were accrued over a 3-year period from 22 US and Canadian institutions. Patients with LSCLC and good performance status were given thoracic radiation to 61.2 Gy over 5 weeks (daily 1.8-Gy fractions on days 1-22, then twice-daily 1.8-Gy fractions on days 23-33). Cisplatin (60 mg/m{sup 2} IV) was given on day 1 and etoposide (120 mg/m{sup 2} IV) on days 1-3 and days 22-24, followed by 2 cycles of cisplatin plus etoposide alone. Patients who achieved complete response were offered prophylactic cranial irradiation. Endpoints included overall and progression-free survival; severe esophagitis (Common Toxicity Criteria v 2.0) and treatment-related fatalities; response (Response Evaluation Criteria in Solid Tumors); and local control. Results: Seventy-two patients were accrued from June 2003 through May 2006; 71 were evaluable (median age 63 years; 52% female; 58% Zubrod 0). Median survival time was 19 months; at 2 years, the overall survival rate was 36.6% (95% confidence interval [CI] 25.6%-47.7%), and progression-free survival 19.7% (95% CI 11.4%-29.6%). Thirteen patients (18%) experienced severe acute esophagitis, and 2 (3%) died of treatment-related causes; 41% achieved complete response, 39% partial response, 10% stable disease, and 6% progressive disease. The local control rate was 73%. Forty-three patients (61%) received prophylactic cranial irradiation. Conclusions: The overall survival rate did not reach the projected goal; however, rates of esophagitis were lower, and local control higher, than projected. This treatment strategy is now one of three arms of a prospective trial of chemoradiation for LSCLC (Radiation Therapy Oncology Group 0538

    The goal of the Environmental Protection Agency`s (EPA) Office of Radiation and Indoor Air is to protect the public and the environment from exposures to radiation and indoor air pollutants. The Office develops protection criteria, standards, and policies and works with other programs within EPA and other agencies to control radiation and indoor air pollution exposures; provides technical assistance to states through EPA`s regional offices and other agencies having radiation and indoor air protection programs; directs an environmental radiation monitoring program; responds to radiological emergencies; and evaluates and assesses the overall risk and impact of radiation and indoor air pollution. The Office is EPA`s lead office for intra- and interagency activities coordinated through the Committee for Indoor Air Quality. It coordinates with and assists the Office of Enforcement in enforcement activities where EPA has jurisdiction. The Office disseminates information and works with state and local governments, industry and professional groups, and citizens to promote actions to reduce exposures to harmful levels of radiation and indoor air pollutants.

    The EPA Draft Document on EMF and Cancers grew out of an earlier effort by EPA to track biological effects literature relative to radio-frequency (RF) exposure. Scope of the document was broadened to include extremely low frequency electric and magnetic fields prior to an announcement in 1986 that EPA would formally review the whole area of non-ionizing radiation. An extensive survey of the relevant bioeffects and epidemiologic literature was carried out, and writing on the document began in earnest on the document sometime in 1989. In its draft form, the document reviewed the literature on mechanisms of Interaction between electromagnetic fields and biological tissue, EMF epidemiologic studies, supporting evidence for carcinogenicity and research needs. In the early summer of 1990, a draft of the document was reviewed by some 22 individuals within the EPA and other government agencies. It was also sent out for external review to an additional 9 qualified scientists who had worked in the area and were familiar with EMF-related literature in epidemiology, biology, and physics. Many of the comments sent to EPA from this first review, prior to release of the draft for public comment, were strikingly similar to those resulting from the second (public) review process. Thus, it appears that much of the controversy that was later associated with the document could have been avoided had the authors been diligent in following the recommendations of the initial set of reviewers.

  10. Single-Fraction Versus 5-Fraction Radiation Therapy for Metastatic Epidural Spinal Cord Compression in Patients With Limited Survival Prognoses: Results of a Matched-Pair Analysis

    Purpose: This study compared single-fraction to multi-fraction short-course radiation therapy (RT) for symptomatic metastatic epidural spinal cord compression (MESCC) in patients with limited survival prognosis. Methods and Materials: A total of 121 patients who received 8 Gy × 1 fraction were matched (1:1) to 121 patients treated with 4 Gy × 5 fractions for 10 factors including age, sex, performance status, primary tumor type, number of involved vertebrae, other bone metastases, visceral metastases, interval between tumor diagnosis and MESCC, pre-RT ambulatory status, and time developing motor deficits prior to RT. Endpoints included in-field repeated RT (reRT) for MESCC, overall survival (OS), and impact of RT on motor function. Univariate analyses were performed with the Kaplan-Meier method and log-rank test for in-field reRT for MESCC and OS and with the ordered-logit model for effect of RT on motor function. Results: Doses of 8 Gy × 1 fraction and 4 Gy × 5 fractions were not significantly different with respect to the need for in-field reRT for MESCC (P=.11) at 6 months (18% vs 9%, respectively) and 12 months (30% vs 22%, respectively). The RT regimen also had no significant impact on OS (P=.65) and post-RT motor function (P=.21). OS rates at 6 and 12 months were 24% and 9%, respectively, after 8 Gy × 1 fraction versus 25% and 13%, respectively, after 4 Gy × 5 fractions. Improvement of motor function was observed in 17% of patients after 8 Gy × 1 fraction and 23% after 4 Gy × 5 fractions, respectively. Conclusions: There were no significant differences with respect to need for in-field reRT for MESCC, OS, and motor function by dose fractionation regimen. Thus, 8 Gy × 1 fraction may be a reasonable option for patients with survival prognosis of a few months.

  11. New Stochastic Annual Limits on Intake for Selected Radionuclides

    Annual limits on intake (ALI) have historically been tabulated by the International Commission on Radiological Protection (e.g., ICRP 1979, 1961) and also by the Environmental Protection Agency (EPA 1988). These compilations have been rendered obsolete by more recent ICRP dosimetry methods, and, rather than provide new ALIs, the ICRP has opted instead to provide committed dose coefficients from which an ALI can be determined by a user for a specific set of conditions. The U.S. Department of Energy historically has referenced compilations of ALIs and has defined their method of calculation in its radiation protection regulation (10 CFDR 835), but has never provided a specific compilation. Under June 2007 amendments to 10 CFR 835, ALIs can be calculated by dividing an appropriate dose limit, either 5-rem (0.05 Sv) effective dose or 50 rem (0.5 Sv) equivalent dose to an individual organ or tissue, by an appropriate committed dose coefficient. When based on effective dose, the ALI is often referred to as a stochastic annual limit on intake (SALI), and when based on the individual organ or tissue equivalent limit, it has often been called a deterministic annual limit on intake (DALI).

    The purpose of this report is two-fold. First, the risk assessment methodology for both RESRAD and the EPA’s tools is reviewed. This includes a review of the EPA’s justification for 2 using a dose-to-risk conversion factor to reduce the dose-based protective ARAR from 15 to 12 mrem/yr. Second, the models and parameters used in RESRAD and the EPA PRG and DCC Calculators are compared in detail, and the results are summarized and discussed. Although there are suites of software tools in the RESRAD family of codes and the EPA Calculators, the scope of this report is limited to the RESRAD (onsite) code for soil contamination and the EPA’s PRG and DCC Calculators also for soil contamination.

  13. Cost analysis for compliance with EPA's regional NOx emissions reductions for fossil-fired power generation

    To achieve a more stringent ambient-air ozone standard promulgated in 1997, the U.S. EPA has established summer NOx emissions limits for fossil-fired electric power generating units in the Ozone Transport Rulemaking region, consisting of 22 eastern and midwestern states and the District of Columbia. These jurisdictions are required to submit State Implementation Plans by September 1999 in response to EPA's rule, with compliance required by 2007. There are 1757 affected units in this region. In the present study, projected state-by-state growth rates for power production are used to estimate power production and NOx emissions by unit in the year 2007. NOx emissions reductions expected by January 1, 2000 due to Title IV compliance are estimated, leaving a substantial balance of emissions reductions to be achieved by post-combustion NOx control. Cost estimates are developed for achieving these remaining reductions.

    With the advent and encouragement of recent initiatives by EPA and state environmental regulators for the development and use by business entities of voluntary environmental compliance and audit programs has come an increased concern over the potential for criminal prosecution of individuals charged with the responsibility for ensuring environmental compliance. With the EPA avoiding the issue of whether audit findings will be used as evidence in a joint civil and criminal investigation (i.e., multi-media inspection), a sense of heightened concern has been prevalent in the regulated community. This paper will address the use of audit methodologies that incorporate law enforcement/criminal investigatory techniques as well as suggested attorney-client reporting structure that should be applied in auditing in order to prevent/mitigate the potential for criminal enforcement actions. The goals of the paper are to enable both corporate compliance managers and environmental auditors to be aware of the potential pitfalls and/or liabilities associated with compliance audit findings. Additionally, to educate auditors on when and how to take further steps, through the use of investigatory interviewing techniques, in order to develop answers to questionable data and/or confirm findings of the environmental audit to avoid civil and criminal penalties. The goals of an environment audit should be to avoid both civil and criminal prosecution through the advanced identification of liabilities and the subsequent development of protocols to achieve compliance. However, oftentimes the use of a pre-established {open_quotes}checklist.

  4. Corporate planning for compliance with EPA`s proposed risk management program rule

    On October 20, 1993, the US Environmental Protection Agency (EPA) published in the Federal Register the proposed rule entitled Risk Management Programs for Chemical Accidental Release Prevention (40 CFR 68). Subsequently, on January 31, 1994, EPA published in the Federal Register the finalized list of 77 regulated toxic substances and 63 regulated flammable substances that are to be covered under the rule along with the associated threshold quantities for each substance. This list of substances will dictate which stationary sources will have to comply with the requirements of the proposed risk management program rule. The risk management program rule will most likely be finalized sometime after mid-1995. Covered facilities will then have a total of three years to achieve complete compliance with the requirements of the rule. This paper presents an approach for corporations with multiple sites to develop action plans for implementation of the risk management program rule. The process starts with a determination of which facilities are potentially covered, development of a matrix of requirements that each facility must comply with, and finally, common strategies that may be used in achieving compliance. Thus, a multi-facility corporation can develop a baseline compliance guideline document that individual plants can use in developing and implementing their risk management programs.
