    A major component of reliability, safety, and mission success for space missions is ensuring that the humans involved (flight crew, ground crew, mission control, etc.) perform their tasks and functions as required. This includes compliance with training and procedures during normal conditions, and successful compensation when malfunctions or unexpected conditions occur. A very significant issue that affects human performance in space flight is human error. Human errors can invalidate carefully designed equipment and procedures. If certain errors combine with equipment failures or design flaws, mission failure or loss of life can occur. The control of human error during operation of the International Space Station (ISS) will be critical to the overall success of the program. As experience from Mir operations has shown, human performance plays a vital role in the success or failure of long duration space missions. The Department of Energy`s Idaho National Engineering and Environmental Laboratory (INEEL) is developed a systematic approach to enhance human performance and reduce human errors for ISS operations. This approach is based on the systematic identification and evaluation of lessons learned from past space missions such as Mir to enhance the design and operation of ISS. This paper describes previous INEEL research on human error sponsored by NASA and how it can be applied to enhance human reliability for ISS.

    Inquiries and orders for combined cycle plant have increased recently because of the better efficiency of combined cycle plant in comparison with the usual fossil fuel power plant. The typical features of the steam turbine for combined cycle plant are the lower inlet steam conditions and the more driving steam flow quantity compared with the steam turbine for usual fossil fuel plants. This paper introduces the design and results of operation about 122 MW single cylinder steam turbine. Furthermore, the results of periodical overhaul inspection carried out after one year`s commercial operation is also presented.

    Any quantum operation applied on a physical system is performed as a unitary transformation on a larger extended system. If the extension used is a heat bath in thermal equilibrium, the concomitant change in the state of the bath necessarily implies a heat exchange with it. The dependence of the average heat transferred to the bath on the initial state of the system can then be found from the expectation value of a hermitian operator, which is named as the heat transfer operator (HTO). The purpose of this article is the investigation of the relation between the HTOs and the associated quantum operations. Since, any given quantum operation on a system can be realized by different baths and unitaries, many different HTOs are possible for each quantum operation. On the other hand, there are also strong restrictions on the HTOs which arise from the unitarity of the transformations. The most important of these is the Landauer erasure principle. This article is concerned with the question of finding a complete set of restrictions on the HTOs that are associated with a given quantum operation. An answer to this question has been found only for a subset of quantum operations. For erasure operations, these characterizations are equivalent to the generalized Landauer erasure principle. For the case of generic quantum operations however, it appears that the HTOs obey further restrictions which cannot be obtained from the entropic restrictions of the generalized Landauer erasure principle.

One can consider a pencil of operators ?-circumflex(?)=(?{sub ?}) passing through the operator ? such that any ?{sub ?} is a linear differential operator acting on densities of weight ?. This pencil can be identified with a linear differential operator ?-circumflex acting on the algebra of densities of all weights. The existence of an invariant scalar product in the algebra of densities implies a natural decomposition of operators, i.e., pencils of self-adjoint and anti-self-adjoint operators. We study lifting maps that are on one hand equivariant with respect to divergenceless vector fields, and, on the other hand, with values in self-adjoint or anti-self-adjoint operators. In particular, we analyze the relation between these two concepts, and apply it to the study of diff?(M)-equivariant liftings. Finally, we briefly consider the case of liftings equivariant with respect to the algebra of projective transformations and describe all regular self-adjoint and anti-self-adjoint liftings. Our constructions can be considered as a generalisation of equivariant quantisation.

The bottom line is that measurement errors are a fact of life and, since we can`t eliminate them, we have to find a rational way to deal with them. Which leads to the world of statistics. Beyond dealing with measurement errors, another area of statistical application involves the sampling of items for verification. Inspectors from the IAEA and domestic regulatory agencies periodically visit operating facilities and make measurements of selected items. By comparing their own measured values to those declared by the facilities, increased confidence is obtained. If verification measurements were not expensive, time consuming, and disruptive to operations, perhaps verification of 100% of the inventories would be desirable. In reality, many constraints lead to inspection of only a portion of those inventories. Drawing inferences about a larger ``population`` of declared items in a facility based on verification information obtained from a sample of those items is a statistical problem. There are few texts on statistics in safeguards. The lengthy exposition ``IAEA Safeguards: Statistical Concepts and Techniques`` and the US NRC book edited by Bowen and Bennet are two good sources of general information. In the next section, the subject of measurement quality is addressed. The third section covers the evaluation of MUFs, and discusses the related subjects of error propagation and sequential analysis. The final section covers verification, inspection sample size calculations, and the D statistic. The text is written at an elementary level, with references to the safeguards literature for more detailed treatment.

    The present study is a continuation of a study focusing on introduction and analysis of operations on transrecursive operators, which are similar to the operators considered in the theory of algorithms. Such operations may be standard product of algorithms) or special ({alpha}-disjunction and {alpha}-iteration. In some respects, these operations in the class of transrecursive operators are similar to their traditional analogs. In other respects, however, they are essentially different. For instance, they may have different types and modes. Specific features of operations on transrecursive operators are attributable to explicit omission of some restrictions on their construction compared with the construction of algorithms. The standard approaches to mathematical modeling of the concept of algorithm assume a number of essential restrictions. Specifically, they assume finiteness of (1) the input data arriving during a finite time interval; (2) the list of rules underlying the operations of the algorithm; (3) the transformations executed by a single operation; (4) the description of each rule; (5) the time to execute one operation; (6) the time to execute one operation; (7) the number of cycles that the algorithm executes in order to realize the mapping. In application to Turing machines, these restrictions imply finiteness of the initial word, finiteness of the command table of the read head, and ultimate stopping when the result is obtained after finitely many steps. Various classes of transrecursive operators are constructed by dropping various combinations of the {open_quotes}finiteness{close_quotes} restrictions. Two classes of transrecursive operators - the limit (inductive) Turing machines and transalgorithms - have been obtained by dropping respectively the assumption of finiteness of the number of execution cycles and the assumption of finiteness of the list of rules.

    This paper reports on reliability centered maintenance (RCM) which is considered a crucial part of future reliability engineering. RCM determines the maintenance requirements of plants and equipment in their operating context. The RCM method has been applied to the management of critical sealless pumps in fire/toxic risk services, typical of the petrochemical industry. The method provides advantages from a detailed study of any critical engineering system. RCM is a team exercise and fosters team spirit in the plant environment. The maintenance strategy that evolves is based on team decisions and relies on maximizing the inherent reliability built into the equipment. RCM recommends design upgrades where this inherent reliability is being questioned. Sealless pumps of canned motor design are used as main reactor charge pumps in PVC plants. These pumps handle fresh vinyl chloride monomer (VCM), which is both carcinogenic and flammable.

    Increased deployment of new technologies, e.g., renewable generation and electric vehicles, is rapidly transforming electrical power networks by crossing previously distinct spatiotemporal scales and invalidating many traditional approaches for designing, analyzing, and operating power grids. This trend is expected to accelerate over the coming years, bringing the disruptive challenge of complexity, but also opportunities to deliver unprecedented efficiency and reliability. Our Applied Mathematics for Power Systems (AMPS) Center will discover, enable, and solve emerging mathematics challenges arising in power systems and, more generally, in complex engineered networks. We will develop foundational applied mathematics resulting in rigorous algorithms and simulation toolboxes for modern and future engineered networks. The AMPS Center deconstruction/reconstruction approach 'deconstructs' complex networks into sub-problems within non-separable spatiotemporal scales, a missing step in 20th century modeling of engineered networks. These sub-problems are addressed within the appropriate AMPS foundational pillar - complex systems, control theory, and optimization theory - and merged or 'reconstructed' at their boundaries into more general mathematical descriptions of complex engineered networks where important new questions are formulated and attacked. These two steps, iterated multiple times, will bridge the growing chasm between the legacy power grid and its future as a complex engineered network.

    The Nuclear Waste Policy Act of 1982 assigned to the Department of Energy's (DOE) Office of Civilian Waste Management the responsibility for disposing of high-level waste and spent fuel. A significant part of that responsibility involves transporting nuclear waste materials within the federal waste management system; that is, from the waste generator to the repository. The lead responsibility for transportation operations has been assigned to Oak Ridge Operations, with Oak Ridge National Laboratory (ORNL) providing technical support through the Transportation Operations Support Task Group. One of the ORNL support activities involves assessing what facilities, equipment and services are required to assure that an acceptable, cost-effective and safe transportation operations system can be designed, operated and maintained.     Motivation Investigations of large-scale outages in the North American interconnected electric system often attribute the causes to three Ts: Trees, Training and Tools. To document and understand the mental processes used by expert operators when making critical decisions, a naturalistic decision making (NDM) model was developed. Transcripts of conversations were analyzed to reveal and assess NDM-based performance criteria. Findings/Design An item analysis indicated that the operators Situation Awareness Levels, mental models, and mental simulations can be mapped at different points in the training scenario. This may identify improved training methods or analytical/ visualization tools. Originality/Value This study applies for the first time, the concepts of Recognition Primed Decision Making, Situation Awareness Levels and Cognitive Task Analysis to training of electric power system operators. Take away message The NDM approach provides a viable framework for systematic training management to accelerate learning in simulator-based training scenarios for power system operators and teams.

    The asymptotic expansion of the heat-kernel for small values of its argument has been studied in many different cases and has been applied to 1-loop calculations in Quantum Field Theory. In this thesis we consider this asymptotic behavior for certain singular differential operators which can be related to quantum fields on manifolds with conical singularities. Our main result is that, due to the existence of this singularity and of infinitely many boundary conditions of physical relevance related to the admissible behavior of the fields on the singular point, the heat-kernel has an "unusual" asymptotic expansion. We describe examples where the heat-kernel admits an asymptotic expansion in powers of its argument whose exponents depend on "external" parameters. As far as we know, this kind of asymptotics had not been found and therefore its physical consequences are still unexplored.

    We study the effective operators of the standard model fields which would yield an observable rate of neutrinoless double beta decay. We particularly focus on the possibility that neutrinoless double beta decay is dominantly induced by lepton-number-violating higher dimensional operators other than the Majorana neutrino mass. Our analysis can be applied to models in which neutrinoless double beta decay is induced either by a strong dynamics or by quantum gravity effects at a fundamental scale near the TeV scale as well as the conventional models in which neutrinoless double beta decay is induced by perturbative renormalizable interactions.

    I show how applying a symplectic Gram-Schmidt orthogonalization to the normalizer of a quantum code gives a different way of determining the code's logical operators. This approach may be more natural in the setting where we produce a quantum code from classical codes because the generator matrices of the classical codes form the normalizer of the resulting quantum code. This technique is particularly useful in determining the logical operators of an entanglement-assisted code produced from two classical binary codes or from one classical quaternary code. Finally, this approach gives additional formulas for computing the amount of entanglement that an entanglement-assisted code requires.
