    The U.S. Department of Energy, Nevada Operations Office (DOE/NV), Completion Report provides a summary of activities conducted at the Nevada Test Site (NTS) between October 1, 1992, and September 30, 1993, associated with Operation KLAXON. (In the past, each annual Completion Report dealt with a series of underground nuclear detonations; however, because no nuclear tests were conducted during FY 1993, this Report summarizes continuing nonnuclear and nuclear test readiness activities at the NTS sponsored by DOE/NV.) The report serves as a reference for those involved with the planning and execution of Operation KLAXON and also serves as a planning guide for future operations. Information in the report covers the logistics and management of activities. Scientific information and data associated with NTS activities are presented in technical documents published by participating agencies. In September 1992, Congress legislated a nine-month moratorium on the testing of nuclear weapons. The bill also provided for a resumption of testing (with no more than five tests per year, or a total of 15 during the next three years) in July 1993, and mandated an end to nuclear testing, entirely, by 1996. President Bush signed the bill into law in October 1992.

Sixty juvenile desert tortoises were released in an undisturbed area on the southern end of the Site. The animals came from the Desert Tortoise Conservation Center (DTCC) in Las Vegas. The DTCC is managed by the San Diego Zoo for conservation research for the United States Fish and Wildlife Service (USFWS).

    The Department of Energy (Department), Nevada Operations office (Nevada) is responsible for following established policy in obtaining necessary support services through its Contract Management Division. The objective of the audit was to determine whether Nevada and its Management and Operating (M&O) contractors were following Federal and Department policies with regard to directed support service subcontracts. The audit showed that program offices in Nevada and Headquarters were directing the Nevada M&O contractor to award subcontracts to specific companies or individuals. The subcontractors reported either directly to a program office or to a national laboratory. Furthermore, the subcontractors` work products were delivered directly to the requesting program office. The M&O contractor had only administrative responsibility for the subcontracts awarded. This occurred because Nevada had not established adequate internal controls over the process of procuring support service. As a result, the M&O contractor was paid a higher award fee for managing the Department`s contracts and may have incurred additional costs in staffing its procurement office. We recommended that the Manager, Nevada Operations Office, discontinue directed support service subcontracts to its M&O contractor and act to strengthen internal controls over subcontracting. Nevada management partially concurred with the recommendations but did not believe the directed procurements cited in the report were inappropriate. Details of management`s comments and our responses are included in Part III.

    The Nevada Test Site Underground Safety and Health Standards Working Group was formed at the direction of John D. Stewart, Director, Nevada Test Site Office in April, 1990. The objective of the Working Group was to compile a safety and health standard from the California Tunnel Safety Orders and OSHA for the underground operations at the NTS, (excluding Yucca Mountain). These standards are called the NTS U/G Safety and Health Standards. The Working Group submits these standards as a RECOMMENDATION to the Director, NTSO. Although the Working Group considers these standards to be the most integrated and comprehensive standards that could be developed for NTS Underground Operations, the intent is not to supersede or replace any relevant DOE orders. Rather the intent is to collate the multiple safety and health references contained in DOE Order 5480.4 that have applicability to NTS Underground Operations into a single safety and heath standard to be used in the underground operations at the NTS. Each portion of the standard was included only after careful consideration by the Working Group and is judged to be both effective and appropriate. The specific methods and rationale used by the Working Group are outlined as follows: The letter from DOE/HQ, dated September 28, 1990 cited OSHA and the CTSO as the safety and health codes applicable to underground operations at the NTS. These mandated codes were each originally developed to be comprehensive, i.e., all underground operations of a particular type (e.g., tunnels in the case of the CTSO) were intended to be adequately regulated by the appropriate code. However, this is not true; the Working Group found extensive and confusing overlap in the codes in numerous areas. Other subjects and activities were addressed by the various codes in cursory fashion or not at all.

    DOE Order 5400.1, ``General Environmental Protection Program,`` established environmental protection program requirements, authorities, and responsibilities to assure that the Department of Energy (DOE) operations are in compliance with applicable federal, state, and local environmental protection laws and regulations, executive orders, and internal department policies. Chapter III of DOE Order 5400.1 required that each field organization prepare a plan for implementing the requirements of this order by no later than November 9, 1989, and update the plan annually. Therefore, the Department of Energy/Nevada Operations Office (DOE/NV) has prepared this fourth annual update of its Environmental Protection Implementation Plan (EPIP). The Order and corresponding guidances also require estimated budgetary resources necessary for implementation of the Order be identified in the Environmental Protection Implementation Plan. To satisfy this requirement, the estimated costs to effectuate necessary changes in existing programs or processes and to institute new programs or processes for compliance with the Order are provided in the following sections of this plan. The DOE/NV Assistant Manager for Environment, Safety, Security, & Health (AMESSH), in consultation with other organizations responsible for line management of plan implementation, is responsible for annual plan revisions.

    P- and S-body wave travel times collected from stations in and near the state of Nevada were inverted for P-wave velocity and the Vp/Vs ratio. These waves consist of Pn, Pg, Sn and Sg, but only the first arriving P and S waves were used in the inversion. Travel times were picked by University of Nevada Reno colleagues and were culled for inclusion in the tomographic inversion. The resulting tomographic model covers the entire state of Nevada to a depth of {approx}90 km; however, only the upper 40 km indicate relatively good resolution. Several features of interest are imaged including the Sierra Nevada, basin structures, and low velocities at depth below Yucca Mountain. These velocity structure images provide valuable information to aide in the interpretation of geothermal resource areas throughout the state on Nevada.

    This document establishes the U. S. Department of Energy, National Nuclear Security Administration Nevada Site Office (NNSA/NSO) waste acceptance criteria (WAC). The WAC provides the requirements, terms, and conditions under which the Nevada Test Site will accept low-level radioactive and mixed waste for disposal. Mixed waste generated within the State of Nevada by NNSA/NSO activities is accepted for disposal. It includes requirements for the generator waste certification program, characterization, traceability, waste form, packaging, and transfer. The criteria apply to radioactive waste received at the Nevada Test Site Area 3 and Area 5 Radioactive Waste Management Site for storage or disposal.

The meeting was hosted by the Department of Energy (DOE) Office of Indian Policy and Programs (DOE Office of Indian Energy) and facilitated by JR Bluehouse, Program Manager, Udall Foundations U.S. Institute for Environmental Conflict Resolution (U.S. Institute). Mr. Bluehouse was assisted by Tamara, Underwood, Program Assistant, U.S. Institute.Tribal leaders and representatives from multiple tribal governments and communities attended the roundtable. Tracey LeBeau, newly appointed Director of the Office of Indian Energy attended. LaMont Jackson from DOEs Office of Electricity attended.     This report contains the environmental monitoring and assessment results for the Nevada Test Site for 1992. Monitoring and surveillance on and around the NTS by DOE contractors and Site user organizations during 1992 indicated that underground nuclear testing operations were conducted in compliance with regulations, i.e., the dose the maximally exposed offsite individual could have received was less than 0.15 percent of the guideline for air exposure. All 1992 nuclear events took place during the first three quarters of the calendar year prior to the Congressional testing moratorium. All discharges of radioactive liquids remained onsite in containment ponds, and there was no indication of potential migration of radioactivity to the offsite area through groundwater. Surveillance around the NTS indicated that airborne radioactivity from test operations was not detectable offsite, and no measurable net exposure to members of the offsite population was detected through the offsite dosimetry program. Using the CAP88-PC model and NTS radionuclide emissions data, the calculated maximum effective dose equivalent offsite would have been 0.012 mrem. Any person receiving this dose was also exposed to 78 mrem from natural background radiation. There were no nonradiological releases to the offsite area. Hazardous wastes were shipped to EPA-approved disposal facilities. Compliance with the various regulations stemming from the National Environmental Policy Act is being achieved and, where mandated, permits for air and water discharges and waste management have been obtained from the appropriate agencies. Non-NTS support facilities complied with the requirements of air quality permits and state or local wastewater discharge and hazardous waste permits.

Surveillance around the NTS indicated that airborne radioactivity from diffusion, evaporation of effluents, or resuspension was not detectable offsite, and no measurable net exposure to members of the offsite population was detected through the offsite dosimetry program. Using the CAP88-PC model and NTS radionuclide emissions data, the calculated effective dose equivalent to the maximally exposed individual offsite would have been 0.004 mrem. Any person receiving this dose would also have received 97 mrem from natural background radiation. There were no nonradiological releases to the offsite area. Hazardous wastes were shipped offsite to approved disposal facilities. Compliance with the various regulations stemming from the National Environmental Policy Act is being achieved and, where mandated, permits for air and water discharges and waste management have been obtained from the appropriate agencies. Support facilities at off-NTS locations compiled with the requirements of air quality permits and state or local wastewater discharge and hazardous waste permits.

    The DOE Notice of Availability for this environmental impact statement was published in the Federal Register on Friday, October 18, 1996 (61 FR 54437). The final environmental impact statement identifies potential adverse effects resulting from the four use alternatives evaluated and discusses measures that DOE considered for the mitigation of these potential adverse effects. The Secretary of Energy signed the Record of Decision on the management and operation of the Nevada Test Site and other DOE sites in the state of Nevada on December 9, 1996. These decisions will result in the continuation of the multipurpose, multi-program use of the Nevada Test Site, under which DOE will pursue a further diversification of interagency, private industry, and public-education uses while meeting its Defense Program, Waste Management, and Environmental Restoration mission requirements at the Nevada Test Site and other Nevada sites, including the Tonopah Test Range, the Project Shoal Site, the Central Nevada Test Area, and on the Nellis Air Force Range Complex. The Record of Decision also identifies specific mitigation actions beyond the routine day-to-day physical and administrative controls needed for implementation of the decisions. These specific mitigation actions are focused on the transportation of waste and on groundwater availability. This Mitigation Action Plan elaborates on these mitigation commitments.
