    The Appalachian States Low-Level Radioactive Waste Commission requested the US Department of Energy`s National Low-Level Waste Management Program (NLLWMP) to assist the biomedical community in becoming more knowledgeable about its mixed waste streams, to help minimize the mixed waste stream generated by the biomedical community, and to identify applicable treatment technologies for these mixed waste streams. As the first step in the waste minimization process, liquid low-level radioactive mixed waste (LLMW) streams generated at the National Institutes of Health (NIH) were characterized and combined into similar process categories. This report identifies possible waste minimization and treatment approaches for the LLMW generated by the biomedical community identified in DOE/LLW-208. In development of the report, on site meetings were conducted with NIH personnel responsible for generating each category of waste identified as lacking disposal options. Based on the meetings and general waste minimization guidelines, potential waste minimization options were identified.

    Currently, pay lines for National Institutes of Health (NIH) grants are at a historical low. In this climate of fierce competition, knowledge about the funding situation in a small field like radiation oncology becomes very important for career planning and recruitment of faculty. Unfortunately, these data cannot be easily extracted from the NIH's database because it does not discriminate between radiology and radiation oncology departments. At the start of fiscal year 2013 we extracted records for 952 individual grants, which were active at the time of analysis from the NIH database. Proposals originating from radiation oncology departments were identified manually. Descriptive statistics were generated using the JMP statistical software package. Our analysis identified 197 grants in radiation oncology. These proposals came from 134 individual investigators in 43 academic institutions. The majority of the grants (118) were awarded to principal investigators at the full professor level, and 122 principal investigators held a PhD degree. In 79% of the grants, the research topic fell into the field of biology, 13% in the field of medical physics. Only 7.6% of the proposals were clinical investigations. Our data suggest that the field of radiation oncology is underfunded by the NIH and that the current level of support does not match the relevance of radiation oncology for cancer patients or the potential of its academic work force.

    A total of 356 indoor-air-quality health-hazard evaluations were completed by NIOSH from 1971 through December of 1985. Most of these studies concerned government and private office buildings where there were worker complaints. Worker complaints resulted from contamination from inside the building (19% of the cases), contamination from outside (11 percent), contamination from the building fabric (4%), biological contamination (5%), inadequate ventilation (50%), and unknown causes (11%). Health complaints addressed by investigative efforts included eye irritation, dry throat, headache, fatigue, sinus congestion, skin irritation, shortness of breath, cough, dizziness, and nausea.

    Purpose: To evaluate the outcomes for patients with Grade III meningiomas as defined by the 2007 World Health Organization standards. Methods and Materials: The slides from patients who had been treated at the Cleveland Clinic for malignant meningiomas were reviewed by a single neuropathologist. The data from 13 patients treated between 1984 and 2006 satisfied the World Health Organization 2007 definition of Grade III meningioma. A total of 24 surgeries were performed, including 13 primary, 7 salvage, and 4 second salvage. Also, 14 courses of radiotherapy (RT) were administered, including fractionated RT in 3 patients after primary surgery, fractionated RT in 4 patients after salvage surgery, salvage stereotactic radiosurgery to six separate areas in 3 patients, and salvage intensity-modulated RT in 1 patient. Results: From the primary surgery, the median survival was 3.4 years, the 5-year survival rate was 47.2%, and the 8-year survival rate was 12.2%. The median time to recurrence was 9.6 months. A trend was seen toward longer survival for patients who had received adjuvant RT after initial surgery compared with those treated with surgery alone. Two patients developed radiation necrosis, and three had surgical complications. Conclusion: This is one of the few studies reporting the outcomes for malignant meningioma patients according to recent definitions. Our results are consistent with existing reports of the overall poor outcomes for atypical and malignant meningioma patients. From the available data, surgical resection followed by RT and salvage therapy can lead to extended survival.

    The aim of this paper is to define in theoretical terms and summarise the available experimental evidence that physical and mental "objects", if considered "information units", may present similar classical and quantum models of communication beyond their specific characteristics. Starting with the Remote State Preparation protocol, a variant of the Teleportation protocol, for which formal models and experimental evidence are already available in quantum mechanics, we outline a formal model applied to mental information we defined Remote State Preparation of Mental Information (RSPMI), and we summarise the experimental evidence supporting the feasibility of a RSPMI protocol. The available experimental evidence offers strong support to the possibility of real communication at distance of mental information promoting the integration between disciplines that have as their object of knowledge different aspects of reality, both physical and the mental, leading to a significant paradigm shift in cognitive and information science.

    Purpose: To evaluate the early and late health-related quality of life (QOL) outcomes among prostate cancer patients following stereotactic body radiation therapy (SBRT). Methods and Materials: Patient self-reported QOL was prospectively measured among 864 patients from phase 2 clinical trials of SBRT for localized prostate cancer. Data from the Expanded Prostate Cancer Index Composite (EPIC) instrument were obtained at baseline and at regular intervals up to 6 years. SBRT delivered a median dose of 36.25 Gy in 4 or 5 fractions. A short course of androgen deprivation therapy was given to 14% of patients. Results: Median follow-up was 3 years and 194 patients remained evaluable at 5 years. A transient decline in the urinary and bowel domains was observed within the first 3 months after SBRT which returned to baseline status or better within 6 months and remained so beyond 5 years. The same pattern was observed among patients with good versus poor baseline function and was independent of the degree of early toxicities. Sexual QOL decline was predominantly observed within the first 9 months, a pattern not altered by the use of androgen deprivation therapy or patient age. Conclusion: Long-term outcome demonstrates that prostate SBRT is well tolerated and has little lasting impact on health-related QOL. A transient and modest decline in urinary and bowel QOL during the first few months after SBRT quickly recovers to baseline levels. With a large number of patients evaluable up to 5 years following SBRT, it is unlikely that unexpected late adverse effects will manifest themselves.

    The testimony addressed the proposed rule on control of hazardous energy sources and was offered in support of the position of the Occupational Safety and Health Administration on this issue. Provisions already in existence for cranes, derricks, and power presses require lockout provisions for electrical connections. The proposed rule will extend these protections to nonelectric power sources and add a requirement for isolating nonelectric hazards. The new rule requires a written procedure and training program. NIOSH opposed the use of tags instead of locks, as tags only provide a warning and are subject to several abuses including removal before maintenance is complete and negligence in removing the tag by the service operator when maintenance is completed. Over 20 electrically related fatalities were noted where a deenergized locked-out electrical circuit would have prevented the fatality. In a review of 160 responses concerning injuries where the equipment was turned off, six indicated the equipment was tagged out. Concern was also expressed over the simple tagout permitted for mechanical power transmission systems. NIOSH recommends that each worker should apply and remove his or her own lock.

    The increasing popularity of web-based distance education places high demand on distance educators to format web pages to facilitate learning. However, limited guidelines exist regarding appropriate writing styles for web-based distance education. This study investigated the effect of four different writing styles on reader’s mental representation of hypertext. Participants studied hypertext written in one of four web-writing styles (e.g., concise, scannable, objective, and combined) and were then administered a cued association task intended to measure their mental representations of the hypertext. It is hypothesized that the scannable and combined styles will bias readers to scan rather than elaborately read, which may result in less dense mental representations (as identified through Pathfinder analysis) relative to the objective and concise writing styles. Further, the use of more descriptors in the objective writing style will lead to better integration of ideas and more dense mental representations than the concise writing style.

    A government-funded study has concluded that reformulated gasoline containing methyl tert-butyl ether (MTBE) does not increase short-term health risks when compared with gasoline that does not contain the additive. The study, performed by the Health Effects Institute (Cambridge, MA), compared data from dozens of animal, human, and epidemiological studies of health effects linked to oxygenates, including MTBE and ethanol, but did not find enough evidence to warrant an immediate reduction in oxygenate use. However, the study did recommend that additional research be conducted on possible health consequences associated with the gasoline additives, including neurotoxic effects, if oxygenates continue to be used long term. Oxygenates have been used in gasoline since 1992, when EPA mandated that several municipalities use MTBE or other oxygenates in reformulated gasoline to reduce carbon monoxide emissions and meet Clean Air Act requirements. Shortly after the program began, residents in areas where the oxygenates were used complained of nausea, headaches, and dizziness. The institute says the study--funded by EPA and the Centers for Disease Control--will be used for a broader review of gasoline oxygenates by the White House Office of Science and Technology Policy.

    The Cyclotron Institute at Texas A&M University has upgraded its accelerator facilities to extend research capabilities with both stable and radioactive beams. The upgrade is divided into three major tasks: (1) re-commission the K-150 (88”) cyclotron, couple it to existing beam lines to provide intense stable beams into the K-500 experimental areas and use it as a driver to produce radioactive beams; (2) develop light ion and heavy ion guides for stopping radioactive ions created with the K-150 beams; and (3) transport 1+ ions from the ion guides into a charge-breeding electron-cyclotron-resonance ion source (CB-ECR) to produce highly-charged radioactive ions for acceleration in the K-500 cyclotron. When completed, the upgraded facility will provide high-quality re-accelerated secondary beams in a unique energy range in the world.

    On November 9, 2002, the U.S. Environmental Protection Agency (EPA), the U.S. Department of Energy (DOE), and the Idaho Department of Environmental Quality approved the Record of Decision Experimental Breeder Reactor-I/Boiling Water Reactor Experiment Area and Miscellaneous Sites, which requires a Sitewide Institutional Controls Plan for the then Idaho National Engineering and Environmental Laboratory (now known as the Idaho National Laboratory). This document, first issued in June 2004, fulfilled that requirement. The revision is needed to provide an update as remedial actions are completed and new areas of concern are found. This Sitewide Institutional Controls Plan is based on guidance in the May 3, 1999, EPA Region 10 Final Policy on the Use of Institutional Controls at Federal Facilities; the September 29, 2000, EPA guidance Institutional Controls: A Site Manager's Guide to Identifying, Evaluating, and Selecting Institutional Controls at Superfund and RCRA Corrective Action Cleanups; and the April 9, 2003, DOE Policy 454.1, "Use of Institutional Controls." These policies establish measures that ensure short- and long-term effectiveness of institutional controls that protect human health and the environment at federal facility sites undergoing remedial action pursuant to the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) and/or corrective action pursuant to the Resource Conservation and Recovery Act (RCRA). The site-specific institutional controls currently in place at the Idaho National Laboratory are documented in this Sitewide Institutional Controls Plan. This plan is being updated, along with the Idaho National Engineering and Environmental Laboratory Comprehensive Facilities and Land Use Plan, to reflect the progress of remedial activities and changes in CERCLA sites.

    Microfabrication enables many exciting experimental possibilities for medicine and biology that are not attainable through traditional methods. However, in order for microfabricated devices to have an impact they must not only provide a robust solution to a current unmet need, but also be simple enough to seamlessly integrate into standard protocols. Broad dissemination of bioMEMS has been stymied by the common aim of replacing established and well accepted protocols with equally or more complex devices, methods, or materials. The marriage of a complex, difficult to fabricate bioMEMS device with a highly variable biological system is rarely successful. Instead, the design philosophy of my lab aims to leverage a beneficial microscale phenomena (e.g. fast diffusion at the microscale) within a bioMEMS device and adapt to established methods (e.g. multiwell plate cell culture) and demonstrate a new paradigm for the field (adapt instead of replace). In order for the field of bioMEMS to mature beyond novel proof-of-concept demonstrations, researchers must focus on developing systems leveraging these phenomena and integrating into standard labs, which have largely been ignored. Towards this aim, the Integrative Bioengineering Institute has been established.

    Internationally the inclusion of health within environmental impact assessment (EIA) has been shown to be limited. While Australian EIA documentation has not been studied empirically to date, deficiencies in practice have been documented. This research developed an audit tool to undertake a qualitative descriptive analysis of 22 Major Project EAs in New South Wales, Australia. Results showed that health and wellbeing impacts were not considered explicitly. They were, however, included indirectly in the identification of traditional public health exposures associated with the physical environment and to a lesser extent the inclusion of social and economic impacts. However, no health data was used to inform any of the assessments, there was no reference to causal pathways between exposures or determinants and physical or mental health effects, and there was no inclusion of the differential distribution of exposures or health impacts on different populations. The results add conceptually and practically to the long standing integration debate, showing that health is in a position to add value to the EIA process as an explicit part of standard environmental, social and economic considerations. However, to overcome the consistently documented barriers to integrating health in EIA, capacity must be developed amongst EIA professionals, led by the health sector, to progress health related knowledge and tools.

    This Institutional Plan for Argonne National Laboratory contains central elements of Argonne`s strategic plan. Chapter II of this document discusses the Laboratory`s mission and core competencies. Chapter III presents the Science and Technology Strategic Plan, which summarizes key features of the external environment, presents Argonne`s vision, and describes how the Laboratory`s strategic goals and objectives map onto and support DOE`s four business lines. The balance of the chapter comprises the science and technology area plans, organized by the four DOE business lines. Chapter IV describes the Laboratory`s ten major initiatives, which cover a broad spectrum of science and technology. Our proposal for an Exotic Beam Facility aims at, among other things, increased understanding of the processes of nuclear synthesis during and shortly after the Big Bang. Our Advanced Transportation Technology initiative involves working with US industry to develop cost-effective technologies to improve the fuel efficiency and reduce the emissions of transportation systems. The Laboratory`s plans for the future depend significantly on the success of its major initiatives. Chapter V presents our Operations and Infrastructure Strategic Plan. The main body of the chapter comprises strategic plans for human resources; environmental protection, safety, and health; site and facilities; and information management. The chapter concludes with a discussion of the business and management practices that Argonne is adopting to improve the quality and cost-effectiveness of its operations. The structure and content of this document depart from those of the Institutional Plan in previous years. Emphasis here is on directions for the future; coverage of ongoing activities is less detailed. We hope that this streamlined plan is more direct and accessible.

    As the Department of Energy stated in its 20% Wind Energy by 2030 report, there will need to be enhanced outreach efforts on a national, state, regional, and local level to communicate wind development opportunities, benefits and challenges to a diverse set of stakeholders. To help address this need, PennFuture was awarded funding to create the Mid-Atlantic Regional Wind Energy Institute to provide general education and outreach on wind energy development across Maryland, Virginia, Delaware, Pennsylvania and West Virginia. Over the course of the two-year grant period, PennFuture used its expertise on wind energy policy and development in Pennsylvania and expanded it to other states in the Mid-Atlantic region. PennFuture accomplished this through reaching out and establishing connections with policy makers, local environmental groups, health and economic development organizations, and educational institutions and wind energy developers throughout the Mid-Atlantic region. PennFuture conducted two regional wind educational forums that brought together wind industry representatives and public interest organizations from across the region to discuss and address wind development in the Mid-Atlantic region. PennFuture developed the agenda and speakers in collaboration with experts on the ground in each state to help determine the critical issue to wind energy in each location. The sessions focused on topics ranging from the basics of wind development; model ordinance and tax issues; anti-wind arguments and counter points; wildlife issues and coalition building. In addition to in-person events, PennFuture held three webinars on (1) Generating Jobs with Wind Energy; (2) Reviving American Manufacturing with Wind Power; and (3) Wind and Transmission. PennFuture also created a web page for the institute (http://www.midatlanticwind.org) that contains an online database of fact sheets, research reports, sample advocacy letters, top anti-wind claims and information on how to address them, wind and wildlife materials and sample model ordinances. Video and presentations from each in-person meeting and webinar recordings are also available on the site. At the end of the two-year period, PennFuture has accomplished its goal of giving a unified voice and presence to wind energy advocates in the Mid-Atlantic region. We educated a broad range of stakeholders on the benefits of wind energy and gave them the tools to help make a difference in their states. We grew a database of over 500 contacts and hope to continue the discussion and work around the importance of wind energy in the region.
