    Nuclear Material Management (NMM) completed a comprehensive study at the request of the Department of Energy Savannah River Operations Office (DOE-SR) in 2004 (Reference 11.1). The study evaluated the feasibility of removal and/or mitigation of the Pu-238 source term in the F-Area Material Storage (FAMS) facility during on-going material storage operations. The study recommended different options to remove and/or mitigate the Pu-238 source term depending on its location within the facility. During April 2005, the Department of Energy (DOE) sent a letter of direction (LOD) to Washington Savannah River Company (WSRC) directing WSRC to implement a new program direction that would enable an accelerated shutdown and decommissioning of FAMS (Reference 11.2). Further direction in the LOD stated that effective December 1, 2006 the facility will be transitioned to begin deactivation and decommissioning (D&D) activities. To implement the LOD, Site D&D (SDD) and DOE agreed the planning end-state would be demolition of the FAMS structure to the building slab. SDD developed the D&D strategy, preliminary cost and schedule, and issued the deactivation project plan in December 2005 (Reference 11.3). Due to concerns and questions regarding the FAMS planning end-state and in support of the project's Critical Decision 1, an alternative study was performed to evaluate the various decommissioning end-states and the methods by which those end-states are achieved. This report documents the results of the alternative evaluation which was performed in a structured decision-making process as outlined in the E7 Manual, Procedure 2.15, ''Alternative Studies'' (Reference 11.4).

    We present a new x-ray Raman spectroscopy end-station recently developed, installed, and operated at the Stanford Synchrotron Radiation Lightsource. The end-station is located at wiggler beamline 6-2 equipped with two monochromators-Si(111) and Si(311) as well as collimating and focusing optics. It consists of two multi-crystal Johann type spectrometers arranged on intersecting Rowland circles of 1 m diameter. The first one, positioned at the forward scattering angles (low-q), consists of 40 spherically bent and diced Si(110) crystals with 100 mm diameters providing about 1.9% of 4{pi} sr solid angle of detection. When operated in the (440) order in combination with the Si (311) monochromator, an overall energy resolution of 270 meV is obtained at 6462.20 eV. The second spectrometer, consisting of 14 spherically bent Si(110) crystal analyzers (not diced), is positioned at the backward scattering angles (high-q) enabling the study of non-dipole transitions. The solid angle of this spectrometer is about 0.9% of 4{pi} sr, with a combined energy resolution of 600 meV using the Si (311) monochromator. These features exceed the specifications of currently existing relevant instrumentation, opening new opportunities for the routine application of this photon-in/photon-out hard x-ray technique to emerging research in multidisciplinary scientific fields, such as energy-related sciences, material sciences, physical chemistry, etc.

    Over the past two decades, continuum quantum Monte Carlo (QMC) has proved to be an invaluable tool for predicting of the properties of matter from fundamental principles. By solving the Schrodinger equation through a stochastic projection, it achieves the greatest accuracy and reliability of methods available for physical systems containing more than a few quantum particles. QMC enjoys scaling favorable to quantum chemical methods, with a computational effort which grows with the second or third power of system size. This accuracy and scalability has enabled scientific discovery across a broad spectrum of disciplines. The current methods perform very efficiently at the terascale. The quantum Monte Carlo Endstation project is a collaborative effort among researchers in the field to develop a new generation of algorithms, and their efficient implementations, which will take advantage of the upcoming petaflop architectures. Some aspects of these developments are discussed here. These tools will expand the accuracy, efficiency and range of QMC applicability and enable us to tackle challenges which are currently out of reach. The methods will be applied to several important problems including electronic and structural properties of water, transition metal oxides, nanosystems and ultracold atoms.

    The U.S. Department of Energy's (DOE) Savannah River Site (SRS) is decommissioning many of its excess facilities through removal of the facility structures leaving only the concrete-slab foundations in place. Site-specific, risk-based derived concentration guideline levels (DCGLs) for radionuclides have been determined for a future industrial worker potentially exposed to residual contamination on these concrete slabs as described in Jannik [1]. These risk-based DCGLs were estimated for an exposure area of 100 m{sup 2}. During deactivation and decommissioning (D&D) operations at SRS, the need for area factors for larger and smaller contaminated areas arose. This paper compares the area factors determined for an industrial worker exposed to a concrete slab end-state for several radionuclides of concern at SRS with (1) the illustrative area factors provided in MARSSIM [2], (2) the area correction factors provided in the U.S. Environmental Protection Agency's (EPA) Soil Screening Guidance [3], and (3) the hot spot criterion for field application provided in the RESRAD User's Manual [4].

    In August of 2009, the Nuclear Science Advisory Committee (NSAC) recommended a variable-energy, high-current multi-particle accelerator for the production of medical radioisotopes. The Oak Ridge National Laboratory is developing a technical concept for a 70 MeV dual-extraction multi-particle cyclotron that will meet the needs identified in the NSAC report. The cyclotron, which will be located at the Holifield Radioactive Ion Beam Facility (HRIBF), will operate on a 24/7 basis and will provide approximately 6000 hours per year of quality beam time for both the production R&D and production of medical and industrial radioisotopes. The proposed cyclotron will be capable of accelerating dual beams of 30 to 70 MeV H at up to 750 A, and up to 50 A of 15-35 MeV D , 35 MeV H2, and 70 MeV -particles. In dual-extraction H mode, a total of 750 A of 70 MeV protons will be provided simultaneously to both HRIBF and Isotope Production Facility. The isotope facility will consist of two target stations: a 2 water-cooled station and a 4 water-cooled high-energy-beam research station. The multi-particle capability and high beam power will enable research into new regimes of accelerator-produced radioisotopes, such as 225Ac, 211At, 68Ge, and 7B. The capabilities of the accelerator will enable the measurement of excitation functions, thick target yield measurements, research in high-power-target design, and will support fundamental research in nuclear and radiochemistry.

    The Chemical Sciences Division (CSD) is one of eleven research Divisions of the Lawrence Berkeley Laboratory, a DOE National Laboratory. In FY 1993, the Division made considerable progress on developing two end-stations and a beamline to advance combustion dynamics at the Advanced Light Source (ALS). In support of DOE`s national role in combustion research and chemical science, the beamline effort will enable researchers from around the world to make fundamental advances in understanding the structure and reactivity of critical reaction intermediates and transients, and in understanding the dynamics of elementary chemical reactions. The Division has continued to place a strong emphasis on full compliance with environmental health and safety guidelines and regulations and has made progress in technology transfer to industry. Finally, the Division has begun a new program in advanced battery research and development that should help strengthen industrial competitiveness both at home and abroad.

    The Atomic, Molecular and Optical Science (AMO) instrument at the Linac Coherent Light Source (LCLS) provides a tight soft X-ray focus into one of three experimental endstations. The flexible instrument design is optimized for studying a wide variety of phenomena requiring peak intensity. There is a suite of spectrometers and two photon area detectors available. An optional mirror-based split-and-delay unit can be used for X-ray pump–probe experiments.     The Medical University of South Carolina (MUSC) and the University of Charleston, South Carolina (UCSC) propose to offer the degree of Master of Science in Environmental Studies. The proposed starting date is August 1994. The purpose of this interdisciplinary program is to offer nationally and internationally recognized graduate level training in the areas of environmental policy, science, and health risk assessment. Special emphasis will be placed on human health. Included in this proposal are a needs assessment for environmental science professionals along with employment projections and salary expectations. The Environmental Science program is described and its relationship to other programs within MUSC and UCSC, as well as its relation to similar programs at other institutions are examined. Enrollment is discussed, admission requirements and standards outlined, and the curriculum is described. Academic and physical resources are examined and estimated costs are given.

    This report is a compilation of submitted abstracts of papers presented at the DOE-supported workshop on medical information and the right to privacy held at the National Academy of Sciences in Washington, DC, on June 9 and 10, 1994. The aim of this meeting is to provide a forum to discuss the legal, ethical and practical issues related to the computerization and use of medical data, as well as the potential impact the use of these data may have on an individual`s privacy. Topical areas include an overview of the Federal and legal requirements to collect medical data, historical experiences with worker screening programs, currently available medical surveillance technologies (both biomedical and computer technologies) and their limitations. In addition, an-depth assessment of the needs and interests of a wide spectrum of parties as they relate to the use of medical data from both a legal and privacy perspective is provided. The needs of the individual, the public (e.g., blood and tissue banks), private enterprises (e.g., industry and insurance carriers), and the government (e.g., FBI) are discussed. Finally, the practical and legal issues relating to the use of computers to carry, store and transmit this information are also examined. The abstracts are presented in the intended order of presentation as indicated in the agenda for the meeting.

    A Draft Technical Information Document (HNF-1855) is being prepared to evaluate proposed interim tritium and medical isotope production at the Fast Flux Test Facility (FFTF). This assessment examines the potential health and safety impacts of transportation operations associated with the production of medical isotopes. Incident-free and accidental impacts are assessed using bounding source terms for the shipment of nonradiological target materials to the Hanford Site, the shipment of irradiated targets from the FFTF to the 325 Building, and the shipment of medical isotope products from the 325 Building to medical distributors. The health and safety consequences to workers and the public from the incident-free transportation of targets and isotope products would be within acceptable levels. For transportation accidents, risks to works and the public also would be within acceptable levels. This assessment is based on best information available at this time. As the medical isotope program matures, this analysis will be revised, if necessary, to support development of a final revision to the Technical Information Document.

    The demand for clinical use of accelerated heavy charged-particle (proton and light-ion) beams for cancer treatment is now burgeoning worldwide. Clinical trials are underway at more than a dozen accelerators. Several hospital-based accelerator facilities dedicated to radiation treatment of human cancer have been constructed, and their number is growing. Many instruments in medical systems have been developed for modifying extracted particle beams for clinical application, monitoring the delivery of the treatment beams, and controlling the treatment processes to ensure patient safety. These in turn demand new developments of instruments in controlling beam extraction, beam tuning, and beam transportation at the medical systems.

    I will explore the similarities and differences between the process of writing science fiction and the process of 'producing' science, specifically theoretical physics. What are the ground rules for introducing unproven new ideas in science fiction, and how do they differ from the corresponding rules in physics? How predictive is science fiction? (For that matter, how predictive is theoretical physics?) I will also contrast the way in which information is presented in science fiction, as opposed to its presentation in scientific papers, and I will examine the relative importance of ideas (as opposed to the importance of the way in which these ideas are presented). Finally, I will discuss whether a background as a research scientist provides any advantage in writing science fiction.

    The Medical Applications and Biophysical Research Division of the Office of Biological and Environmental Research supports and manages research in several distinct areas of science and technology. The projects described in this book are grouped by the main budgetary areas: General Life Sciences (structural molecular biology), Medical Applications (primarily nuclear medicine) and Measurement Science (analytical chemistry instrumentation), Environmental Management Science Program, and the Small Business Innovation Research Program. The research funded by this division complements that of the other two divisions in the Office of Biological and Environmental Research (OBER): Health Effects and Life Sciences Research, and Environmental Sciences.     This report describes the design of an advanced medical facility dedicated to charged particle radiotherapy and other biomedical applications of relativistic heavy ions. Project status is reviewed and some technical aspects discussed. Clinical standards of reliability are regarded as essential features of this facility. Particular emphasis is therefore placed on the control system and on the use of technology which will maximize operational efficiency. The accelerator will produce a variety of heavy ion beams from helium to argon with intensities sufficient to provide delivered dose rates of several hundred rad/minute over large, uniform fields. The technical components consist of a linac injector with multiple PIG ion sources, a synchrotron and a versatile beam delivery system. An overview is given of both design philosophy and selected accelerator subsystems. Finally, a plan of the facility is described.

    A medical imaging system provides simultaneous rendering of visible light and fluorescent images. The system may employ dyes in a small-molecule form that remains in a subject's blood stream for several minutes, allowing real-time imaging of the subject's circulatory system superimposed upon a conventional, visible light image of the subject. The system may also employ dyes or other fluorescent substances associated with antibodies, antibody fragments, or ligands that accumulate within a region of diagnostic significance. In one embodiment, the system provides an excitation light source to excite the fluorescent substance and a visible light source for general illumination within the same optical guide that is used to capture images. In another embodiment, the system is configured for use in open surgical procedures by providing an operating area that is closed to ambient light. More broadly, the systems described herein may be used in imaging applications where a visible light image may be usefully supplemented by an image formed from fluorescent emissions from a fluorescent substance that marks areas of functional interest.

    Computational Science is an integral component of Brookhaven's multi science mission, and is a reflection of the increased role of computation across all of science. Brookhaven currently has major efforts in data storage and analysis for the Relativistic Heavy Ion Collider (RHIC) and the ATLAS detector at CERN, and in quantum chromodynamics. The Laboratory is host for the QCDOC machines (quantum chromodynamics on a chip), 10 teraflop/s computers which boast 12,288 processors each. There are two here, one for the Riken/BNL Research Center and the other supported by DOE for the US Lattice Gauge Community and other scientific users. A 100 teraflop/s supercomputer will be installed at Brookhaven in the coming year, managed jointly by Brookhaven and Stony Brook, and funded by a grant from New York State. This machine will be used for computational science across Brookhaven's entire research program, and also by researchers at Stony Brook and across New York State. With Stony Brook, Brookhaven has formed the New York Center for Computational Science (NYCCS) as a focal point for interdisciplinary computational science, which is closely linked to Brookhaven's Computational Science Center (CSC). The CSC has established a strong program in computational science, with an emphasis on nanoscale electronic structure and molecular dynamics, accelerator design, computational fluid dynamics, medical imaging, parallel computing and numerical algorithms. We have been an active participant in DOES SciDAC program (Scientific Discovery through Advanced Computing). We are also planning a major expansion in computational biology in keeping with Laboratory initiatives. Additional laboratory initiatives with a dependence on a high level of computation include the development of hydrodynamics models for the interpretation of RHIC data, computational models for the atmospheric transport of aerosols, and models for combustion and for energy utilization. The CSC was formed to bring together researchers in these areas and to provide a focal point for the development of computational expertise at the Laboratory. These efforts will connect to and support the Department of Energy's long range plans to provide Leadership class computing to researchers throughout the Nation. Recruitment for six new positions at Stony Brook to strengthen its computational science programs is underway. We expect some of these to be held jointly with BNL.

    A medical implant device having a substrate with an oxidized surface and a silane derivative coating covalently bonded to the oxidized surface. A bioactive agent is covalently bonded to the silane derivative coating. An implantable stent device including a stent core having an oxidized surface with a layer of silane derivative covalently bonded thereto. A spacer layer comprising polyethylene glycol (PEG) is covalently bonded to the layer of silane derivative and a protein is covalently bonded to the PEG. A method of making a medical implant device including providing a substrate having a surface, oxidizing the surface and reacting with derivitized silane to form a silane coating covalently bonded to the surface. A bioactive agent is then covalently bonded to the silane coating. In particular instances, an additional coating of bio-absorbable polymer and/or pharmaceutical agent is deposited over the bioactive agent.

    The Former Hanford Worker Medical Monitoring Program, directed by the Occupational and Environmental Medicine Program at the University of Washington, served former production and other non-construction workers who were potentially exposed to workplace hazards while working for the USDOE or its contractors at Hanford. The USDOE Former Workers Program arose from Congressional action in the Defense Authorization of 1993 (Public Law 102). Section 3162 stated that, “The Secretary shall establish and carry out a program for the identification and ongoing medical evaluation of current and former Department of Energy employees who are subject to significant health risks as a result of exposure of such employees to hazardous or radioactive substances during such employment.” (This also covers former employees of USDOE contractors and subcontractors.) The key objective has been to provide these former workers with medical evaluations in order to determine whether workers have experienced significant risk due to workplace exposure to hazards. Exposures to asbestos, beryllium, and noise can produce specific medical conditions: asbestosis, berylliosis, and noise-induced hearing loss (NIHL). Each of these conditions can be identified by specific, non-invasive screening tests, which are widely available. Treatments are also available for individuals affected by these conditions. This project involved two phases. Phase I involved a needs and risk assessment, characterizing the nature and extent of workplace health hazards which may have increased the risk for long-term health effects. We categorized jobs and tasks by likelihood of exposures to specific workplace health hazards; and located and established contact with former Hanford workers. Phase II involved implementation of medical monitoring programs for former workers whose individual work history indicated significant risk for adverse health effects. We identified 118,000 former workers, employed from 1943 to 1997. After excluding current workers, construction workers, and deceased workers, the total estimated number of former workers eligible for screening was 72,611. By September, 2006, 53,010 workers had been contacted, 20,298 responded, 2,835 were eligible and authorized, and 2,773 workers were ultimately screened. The cohort was 80% male, 85% white, and had a mean age of 63 years (range 24-96 years) at the time of first exam. Participants completed an occupational health history survey prior to the medical exam. Former Hanford workers were considered eligible for an exam if they reported exposure to asbestos, beryllium, or noise, or if a review of their Hanford work history indicated possible or probable exposure to one of these three hazards. We also invited any former Hanford worker who requested an exam to participate, regardless of documentation of exposure. The screening exam included a problem-focused physical exam, along with screening tests for one or more of three specific medical conditions: asbestosis (chest X-ray and spirometry), berylliosis (chest X-ray, spirometry, and beryllium-induced lymphocyte proliferation test), and NIHL (audiometry). We assisted ill workers in filing appropriate workers’ compensation claims, and facilitated appropriate follow-up medical care. This program has made an important contribution to the health of former DOE contractor workers at the Hanford defense nuclear site.

    The present invention relates to an apparatus for producing sterile ice slurries for medical cooling applications. The apparatus is capable of producing highly loaded slurries suitable for delivery to targeted internal organs of a patient, such as the brain, heart, lungs, stomach, kidneys, pancreas, and others, through medical size diameter tubing. The ice slurry production apparatus includes a slurry production reservoir adapted to contain a volume of a saline solution. A flexible membrane crystallization surface is provided within the slurry production reservoir. The crystallization surface is chilled to a temperature below a freezing point of the saline solution within the reservoir such that ice particles form on the crystallization surface. A deflector in the form of a reciprocating member is provided for periodically distorting the crystallization surface and dislodging the ice particles which form on the crystallization surface. Using reservoir mixing the slurry is conditioned for easy pumping directly out of the production reservoir via medical tubing or delivery through other means such as squeeze bottles, squeeze bags, hypodermic syringes, manual hand delivery, and the like.

    There is an increasing demand for different separated isotopes as feed material for reactor and cyclotron-produced radioisotopes used by a fast-growing radiopharmaceutical industry. One new technology that may meet future demands for medical isotopes is molecular laser isotope separation (MLIS). This method was investigated for the enrichment of uranium in the 1970`s and 1980s by Los Alamos National Laboratory, Isotope Technologies, and others around the world. While South Africa and Japan have continued the development of MLIS for uranium and are testing pilot units, around 1985 the United States dropped the LANL MLIS program in favor of AVLIS (atomic vapor LIS), which uses electron-beam-heated uranium metal vapor. AVLIS appears difficult and expensive to apply to most isotopes of medical interest, however, whereas MLIS technology, which is based on cooled hexafluorides or other gaseous molecules, can be adapted more readily. The attraction of MLIS for radiopharmaceutical firms is that it allows them to operate their own dedicated separators for small-quantity productions of critical medical isotopes, rather than having to depend on large enrichment complexes run by governments, which are only optimal for large-quantity productions. At the University of Missouri, the authors are investigating LIS of molybdenum isotopes using MoF{sub 6}, which behaves in a way similar to UF{sub 6}, studied in the past.

    An engineering network of collaborating medical laser laboratories are developing laser and optical technologies for medical diagnosis and therapy and are translating the engineering into medical centers in Portland OR, Houston TX, and Galveston TX. The Center includes the University of Texas M.D. Anderson Cancer Center, the University of Texas-Austin, Texas A and M University, Rice University, the University Texas Medical Branch-Galveston, Oregon Medical Laser Center (Providence St. Vincent Medical Center, Oregon Health Sciences University, and Oregon Graduate Institute, Portland, OR), and the University of Oregon. Diagnostics include reflectance, fluorescence, Raman IR, laser photoacoustics, optical coherence tomography, and several new video techniques for spectroscopy and imaging. Therapies include photocoagulation therapy, laser welding, pulsed laser ablation, and light-activated chemotherapy of cancer (photodynamic therapy, or PDT). Medical applications reaching the clinic include optical monitoring of hyperbilirubinemia in newborns, fluorescence detection of cervical dysplasia, laser thrombolysis of blood clots in heart attack and brain stroke, photothermal coagulant of benign prostate hyperplasia, and PDT for both veterinary and human cancer. New technologies include laser optoacoustic imaging of breast tumors and hemorrhage in head trauma and brain stroke, quality control monitoring of dosimetry during PDT for esophageal and lung cancer, polarization video reflectometry of skin cancer, laser welding of artificial tissue replacements, and feedback control of laser welding.

    This paper briefly explores the accelerator technology available for heavy ion medical accelerators in the mass range of 1 to 40 (protons through argon). Machines that are designed to produce the required intensities of a particular design ion, such as silicon (mass 28), can satisfy the intensity requirements for all lighter ions, and can produce beams with higher mass, such as argon, at somewhat reduced, but still useful intensity levels. They can also provide beams of radioactive ions, such as carbon-11 and neon-19, which are useful in diagnostic imaging and for directly verifiable treatments. These accelerators are all based on proven technology, and can be built at predictable costs. It is the conclusion of several design studies that they can be operated reliably in a hospital-based environment. 8 refs., 22 figs.

    It is my pleasure to introduce the 2009 annual report of the Advanced Photon Source. This was a very good year for us. We operated with high reliability and availability, despite growing problems with obsolete systems, and our users produced a record output of publications. The number of user experiments increased by 14% from 2008 to more than 3600. We congratulate the recipients of the 2009 Nobel Prize in Chemistry-Venkatraman Ramakrishnan (Cambridge Institute for Medical Research), Thomas Steitz (Yale University), and Ada Yonath (Weizmann Institute) - who did a substantial amount of this work at APS beamlines. Thanks to the efforts of our users and staff, and the ongoing counsel of the APS Scientific Advisory Committee, we made major progress in advancing our planning for the upgrade of the APS (APS-U), producing a proposal that was positively reviewed. We hope to get formal approval in 2010 to begin the upgrade. With advocacy from our users and the support of our sponsor, the Office of Basic Energy Sciences in the Department of Energy (DOE) Office of Science, our operating budgets have grown to the level needed to more adequately staff our beamlines. We were also extremely fortunate to have received $7.9 M in American Recovery and Reinvestment Act ('stimulus') funding to acquire new detectors and improve several of our beamlines. The success of the new Linac Coherent Light Source at Stanford, the world's first x-ray free-electron laser, made us particularly proud since the undulators were designed and built by the APS. Among other highlights, we note that more than one-quarter of the 46 Energy Frontier Research Centers, funded competitively across the U.S. in 2009 by the DOE, included the Advanced Photon Source in their proposed work, which shows that synchrotron radiation, and the APS in particular, are central to energy research. While APS research covers everything from fundamental to applied science (reflected by the highlights in this report), the challenge of sustainable energy provides an opportunity for expanded involvement with industrial research. We were privileged to recruit several outstanding new leaders at the APS. Linda Young, from Argonne's Chemical Sciences Division, became the new Director of the X-ray Science Division (XSD). Chris Jacobsen (from Stony Brook University) has been added to Linda's team as an XSD Associate Division Director, joining George Srajer. Alexander (Sasha) Zholents (formerly of Berkeley Lab) became Director of the Accelerator Systems Division. Sasha is the inventor of the short-pulse x-ray scheme that we plan to implement in the APS-U to obtain very high average brightness, broadband, 1-ps x-ray pulses. Walter Lowe (formerly of Howard University) has taken a new position as senior advisor for outreach and development of the user community. Walter's role is to increase the diversity of the user community (with diversity read broadly to include users, institutions, and technical disciplines that are underrepresented at APS). Walter is also leading an effort to increase access for industrial users. I am confident that we have in place a great team to help our users and the APS take fullest advantage of the APS-U opportunity. In planning with users for the proposed APS-U, we focused on the need to study 'real materials under real conditions in real time' on spatial and temporal scales unavailable today. Only by studying materials as they are made-or as they perform-in difficult environments can we solve the grand challenge of higher-performance, sustainable materials for energy and health. The proposed APS-U will improve the brightness of penetrating x-rays produced by the APS over 100 times, and support our efforts in developing state-of-the-art instruments to address these challenges.

    This article provides a review of hazardous medical waste production and its management in Croatia. Even though Croatian regulations define all steps in the waste management chain, implementation of those steps is one of the country's greatest issues. Improper practice is evident from the point of waste production to final disposal. The biggest producers of hazardous medical waste are hospitals that do not implement existing legislation, due to the lack of education and funds. Information on quantities, type and flow of medical waste are inadequate, as is sanitary control. We propose an integrated approach to medical waste management based on a hierarchical structure from the point of generation to its disposal. Priority is given to the reduction of the amounts and potential for harm. Where this is not possible, management includes reduction by sorting and separating, pretreatment on site, safe transportation, final treatment and sanitary disposal. Preferred methods should be the least harmful for human health and the environment. Integrated medical waste management could greatly reduce quantities and consequently financial strains. Landfilling is the predominant route of disposal in Croatia, although the authors believe that incineration is the most appropriate method. In a country such as Croatia, a number of small incinerators would be the most economical solution.

    Computer-mediated communication systems can be used to bridge the gap between doctors in underserved regions with local shortages of medical expertise and medical specialists worldwide. To this end, we describe the design of a prototype remote consultation system intended to provide the social, institutional and infrastructural context for sustained, self-organizing growth of a globally-distributed Ghanaian medical community. The design is grounded in an iterative design process that included two rounds of extended design fieldwork throughout Ghana and draws on three key design principles (social networks as a framework on which to build incentives within a self-organizing network; optional and incremental integration with existing referral mechanisms; and a weakly-connected, distributed architecture that allows for a highly interactive, responsive system despite failures in connectivity). We discuss initial experiences from an ongoing trial deployment in southern Ghana.

    Implantable medical devices are becoming more pervasive as new technologies increase their reliability and safety. Furthermore, these devices are becoming increasingly reliant on wireless communication for interaction with the device. Such technologies have the potential to leak information that could be utilized by an attacker to threaten the lives of patients. Privacy of patient information is essential; however, this information is not the only privacy issue that must be considered. In this paper, we discuss why information privacy is insufficient for protecting patients from some attacks and how information regarding the presence of individual devices can leak vulnerabilities. Furthermore, we examine existing privacy enhancing algorithms and discuss their applicability to implantable medical devices.

    Light ion beams have been used for cancer therapy for about twenty years; several dedicated facilities are presently either planned or under construction. In addition, several synchrotrons designed for other purposes are now considered for medical applications as well. A medical synchrotron needs a preaccelerator to produce and inject a range of different light ions, preferably fully stripped, into the ring. The size, cost and complexity of the preaccelerator depend on the performance of its first element, the ion source, and these features will be optimized if the source itself produces fully stripped ions. An EBIS (Electron Beam Ion Source) is capable of producing fully stripped light ions up to argon with intensities sufficient for medical applications. As it has been pointed out in the past, this source option may require just one stage of preacceleration, an RFQ linac, thus making it very simple and compact. The AGS Department has a separate project already under way to develop a very high intensity EBIS for our nuclear physics program. It is, however, our plan first to construct and test an intermediate size device and then to proceed to the design of the final, full scale device. Parameters of that intermediate model are close to those that would be needed for a medical synchrotron. This paper describes the BNL program and considers parameters of EBIS devices for possible use in synchrotron facilities serving as sources of high energy light ions for cancer therapy.

    Jackson Park Hospital completed the construction of a new Medical Office Building on its campus this spring. The new building construction has adopted the City of Chicago's recent focus on protecting the environment, and conserving energy and resources, with the introduction of green building codes. Located in a poor, inner city neighborhood on the South side of Chicago, Jackson Park Hospital has chosen green building strategies to help make the area a better place to live and work. The new green building houses the hospital's Family Medicine Residency Program and Specialty Medical Offices. The residency program has been vital in attracting new, young physicians to this medically underserved area. The new outpatient center will also help to allure needed medical providers to the community. The facility also has areas designated to women's health and community education. The Community Education Conference Room will provide learning opportunities to area residents. Emphasis will be placed on conserving resources and protecting our environment, as well as providing information on healthcare access and preventive medicine. The new Medical Office Building was constructed with numerous energy saving features. The exterior cladding of the building is an innovative, locally-manufactured precast concrete panel system with integral insulation that achieves an R-value in excess of building code requirements. The roof is a 'green roof' covered by native plantings, lessening the impact solar heat gain on the building, and reducing air conditioning requirements. The windows are low-E, tinted, and insulated to reduce cooling requirements in summer and heating requirements in winter. The main entrance has an air lock to prevent unconditioned air from entering the building and impacting interior air temperatures. Since much of the traffic in and out of the office building comes from the adjacent Jackson Park Hospital, a pedestrian bridge connects the two buildings, further decreasing the amount of unconditioned air that enters the office building. The HVAC system has an Energy Efficiency Rating 29% greater than required. No CFC based refrigerants were used in the HVAC system, thus reducing the emission of compounds that contribute to ozone depletion and global warming. In addition, interior light fixtures employ the latest energy-efficient lamp and ballast technology. Interior lighting throughout the building is operated by sensors that will automatically turn off lights inside a room when the room is unoccupied. The electrical traction elevators use less energy than typical elevators, and they are made of 95% recycled material. Further, locally manufactured products were used throughout, minimizing the amount of energy required to construct this building. The primary objective was to construct a 30,000 square foot medical office building on the Jackson Park Hospital campus that would comply with newly adopted City of Chicago green building codes focusing on protecting the environment and conserving energy and resources. The energy saving systems demonstrate a state of the-art whole-building approach to energy efficient design and construction. The energy efficiency and green aspects of the building contribute to the community by emphasizing the environmental and economic benefits of conserving resources. The building highlights the integration of Chicago's new green building codes into a poor, inner city neighborhood project and it is designed to attract medical providers and physicians to a medically underserved area.

    The National Institute of Radiological Sciences (NIRS) maintains various ion accelerators in order to study the effects of radiation of the human body and medical uses of radiation. Two electrostatic tandem accelerators and three cyclotrons delivered by commercial companies have offered various life science tools; these include proton-induced x-ray emission analysis (PIXE), micro beam irradiation, neutron exposure, and radioisotope tracers and probes. A duoplasmatron, a multicusp ion source, a penning ion source (PIG), and an electron cyclotron resonance ion source (ECRIS) are in operation for these purposes. The Heavy-Ion Medical Accelerator in Chiba (HIMAC) is an accelerator complex for heavy-ion radiotherapy, fully developed by NIRS. HIMAC is utilized not only for daily treatment with the carbon beam but also for fundamental experiments. Several ECRISs and a PIG at HIMAC satisfy various research and clinical requirements.

    Abstract The International Monitoring System (IMS) of the Comprehensive-Nuclear-Test-Ban-Treaty monitors the atmosphere for radioactive xenon leaking from underground nuclear explosions. Emissions from medical isotope production represent a challenging background signal when determining whether measured radioxenon in the atmosphere is associated with a nuclear explosion prohibited by the treaty. The Australian Nuclear Science and Technology Organisation (ANSTO) operates a reactor and medical isotope production facility in Lucas Heights, Australia. This study uses two years of release data from the ANSTO medical isotope production facility and Xe-133 data from three IMS sampling locations to estimate the annual releases of Xe-133 from medical isotope production facilities in Argentina, South Africa, and Indonesia. Atmospheric dilution factors derived from a global atmospheric transport model were used in an optimization scheme to estimate annual release values by facility. The annual releases of about 6.8×1014 Bq from the ANSTO medical isotope production facility are in good agreement with the sampled concentrations at these three IMS sampling locations. Annual release estimates for the facility in South Africa vary from 1.2×1016 to 2.5×1016 Bq and estimates for the facility in Indonesia vary from 6.1×1013 to 3.6×1014 Bq. Although some releases from the facility in Argentina may reach these IMS sampling locations, the solution to the objective function is insensitive to the magnitude of those releases.

    The Former Radiation Worker Medical Surveillance Program at Rocky Flats was conducted in Arvada, CO, by Oak Ridge Associated Universities through the Oak Ridge Institute for Science and Education under DOE Contract DE-AC05-00OR22750. Objectives of the program were to obtain information on the value of medical surveillance among at-risk former radiation workers and to provide long-term internal radiation dosimetry information to the scientific community. This program provided the former radiation workers of the Rocky Flats Environmental Technology Site (formerly Rocky Flats Plant) an opportunity to receive follow-up medical monitoring and a re-evaluation of their internal radiation dose. The former Rocky Flats radiation worker population is distinctive because it was a reasonably stable work force that received occupational exposures, at times substantial, over several decades. This report reflects the summation of health outcomes, statistical analyses, and dose assessment information on former Rocky Flats radiation workers to the date of study termination as of March 2004.

    This report describes the Sandia National Laboratories Medical Isotope Reactor and hot cell facility concepts. The reactor proposed is designed to be capable of producing 100% of the U.S. demand for the medical isotope {sup 99}Mo. The concept is novel in that the fuel for the reactor and the targets for the {sup 99}Mo production are the same. There is no driver core required. The fuel pins that are in the reactor core are processed on a 7 to 21 day irradiation cycle. The fuel is low enriched uranium oxide enriched to less than 20% {sup 235}U. The fuel pins are approximately 1 cm in diameter and 30 to 40 cm in height, clad with Zircaloy (zirconium alloy). Approximately 90 to 150 fuel pins are arranged in the core in a water pool {approx}30 ft deep. The reactor power level is 1 to 2 MW. The reactor concept is a simple design that is passively safe and maintains negative reactivity coefficients. The total radionuclide inventory in the reactor core is minimized since the fuel/target pins are removed and processed after 7 to 21 days. The fuel fabrication, reactor design and operation, and {sup 99}Mo production processing use well-developed technologies that minimize the technological and licensing risks. There are no impediments that prevent this type of reactor, along with its collocated hot cell facility, from being designed, fabricated, and licensed today.

    A medical imaging system provides simultaneous rendering of visible light and fluorescent images. The system may employ dyes in a small-molecule form that remain in the subject's blood stream for several minutes, allowing real-time imaging of the subject's circulatory system superimposed upon a conventional, visible light image of the subject. The system may provide an excitation light source to excite the fluorescent substance and a visible light source for general illumination within the same optical guide used to capture images. The system may be configured for use in open surgical procedures by providing an operating area that is closed to ambient light. The systems described herein provide two or more diagnostic imaging channels for capture of multiple, concurrent diagnostic images and may be used where a visible light image may be usefully supplemented by two or more images that are independently marked for functional interest.
