Emergency preparedness for the small oilfield contractor

Changes in the oil and gas industry have been dramatic in recent years. Operators have consolidated field office facilities and downsized staffing at many of their remaining facilities. As a result, operators are turning to independent Contractors as a method of controlling both peak personnel demands and selected production functions. With these changes in the business environment, the role of the specialized contractor has been greatly expanded. Tasks Diane performed by operator employees are now ongoing services provided by the contractor. Their performance can Taken be exemplary until a major emergency occurs. Emergency response can no longer be considered a secondary issue. Every contractor, small or large, must be equally prepared for an emergency. Their ability to address emergency incidents will have a direct effect in their success. This paper is a presentation of the actual learning experiences of one oilfield contractor confronted with the ask of emergency preparedness, more specifically a small contractor. Oilfield service contractors commonly employ field personnel who work for extended periods of time without direct supervision. These employees may singularly or collectively float between sites or anchor one site. Operating sites might be localized or spread over a vast geographical area. Because of the uniqueness of their situation, direct supervision is minimal and the employees will shoulder greater responsibilities and authority. The contractor must evaluate operating conditions and develop an approach which will achieve the optimum results.

Characterization of emergency preparedness at DOE contractor facilities

A study of emergency preparedness capabilities at DOE facilities was initiated following the incident at the Three Mile Island (TMI) Nuclear Power Station. It was designed to parallel but expand on a study on emergency preparedness instrumentation that was conducted in 1970 by Pacific Northwest Laboratory. The 1970 survey findings led to the publication of four reports on performance criteria for radiological emergency instrumentation. Three of these reports - BNWL-1635 (Selby et al. 1972), BNWL-1742 (Anderson et al. 1974) and BNWL-1857 (Andersen et al. 1976) - addressed the criteria for emergency instrumentation at reactors, mixed oxide fuel fabrication plants, and fuel reprocessing plants, respectively. The fourth report, BNWL-1991 (Bramson et al. 1976), addressed evaluation testing and calibration methodology for these instruments. This report is presented in three parts. Part One is a review of the BNWL documents to determine whether they are applicable to state-of-the-art instrument capabilities. The Appendix to Part One provides a comparison between the instrument performance criteria established in BNWL-1991 to applicable American National Standards Institute (ANSI) standards for portable survey and contamination meters, installed radiation and area monitors, effluent monitors, calibration techniques, criticality detection systems, alarm systems, and direct reading dosimeters. Part Two compares the 1970 survey results with the 1980 survey results to identify trends in emergency preparedness. Part Three is a discussion of the results of the 1980 emergency preparedness survey and the supporting data for each of the 15 modules. 8 references. (ACR)

A Counter-IED Preparedness Methodology for Large Event Planning

Since 2009, Oak Ridge National Laboratory (ORNL) has been involved in a project sponsored by the Department of Homeland Security Science and Technology Directorate aimed at improving preparedness against Improvised Explosive Devices (IED) at large sporting events. Led by the University of Southern Mississippi (USM) as part of the Southeast Region Research Initiative, the project partners have been developing tools and methodologies for use by security personnel and first responders at sports stadiums. ORNL s contribution has been to develop an automated process to gather and organize disparate data that is usually part of an organization s security plan. The organized data informs a table-top exercise (TTX) conducted by USM using additional tools developed by them and their subcontractors. After participating in several pilot TTXs, patterns are beginning to emerge that would enable improvements to be formulated to increase the level of counter-IED preparedness. This paper focuses on the data collection and analysis process and shares insights gained to date.

As part of the Emergency Preparedness Implementation Appraisal Program conducted by the Nuclear Regulatory Commission (NRC) with the technical assistance of the Pacific Northwest Laboratory (PNL), emergency preparedness exercises are observed on an annual basis at all licensed reactor facilities. One of the significant findings to arise from these observations was that a large number of the commonly observed problems originated in the design of the scenarios used as a basis for each exercise. In an effort to help eliminate some of these problems a scenario guidance handbook has been generated by PNL for the NRC to assist nuclear power plant licensees in developing scenarios for emergency preparedness exercises.

This planning guide was developed under the direction of the U.S. Army and the Federal Emergency Management Agency (FEMA) which jointly coordinate and direct the development of the Chemical Stockpile Emergency Preparedness Program (CSEPP). It was produced to assist state, local, and Army installation planners in formulating and coordinating plans for chemical events that may occur at the chemical agent stockpile storage locations in the continental United States. This document provides broad planning guidance for use by both on-post and off-post agencies and organizations in the development of a coordinated plan for responding to chemical events. It contains checklists to assist in assuring that all important aspects are included in the plans and procedures developed at each Chemical Stockpile Disposal Program (CSDP) location. The checklists are supplemented by planning guidelines in the appendices which provide more detailed guidance regarding some issues. The planning guidance contained in this document will help ensure that adequate coordination between on-post and off-post planners occurs during the planning process. This planning guide broadly describes an adequate emergency planning base that assures that critical planning decisions will be made consistently at every chemical agent stockpile location. This planning guide includes material drawn from other documents developed by the FEMA, the Army, and other federal agencies with emergency preparedness program responsibilities. Some of this material has been developed specifically to meet the unique requirements of the CSEPP. In addition to this guidance, other location-specific documents, technical studies, and support studies should be used as needed to assist in the planning at each of the chemical agent stockpile locations to address the specific hazards and conditions at each location.

A regulatory analysis on emergency preparedness for fuel cycle and other radioactive material licensees: Final report

The question this Regulatory Analysis sought to answer is: should the NRC impose additional emergency preparedness requirements on certain fuel cycle and other radioactive material licensees for dealing with accidents that might have offsite releases of radioactive material. To answer the question, we analyzed potential accidents for 15 types of fuel cycle and other radioactive material licensees. An appropriate plan would: (1) identify accidents for which protective actions should be taken by people offsite; (2) list the licensee's responsibilities for each type of accident, including notification of local authorities (fire and police generally); and (3) give sample messages for local authorities including protective action recommendations. This approach more closely follows the approach used for research reactors than for power reactors. The low potential offsite doses (acute fatalities and injuries not possible except possibly for UF/sub 6/ releases), the small areas where actions would be warranted, the small number of people involved, and the fact that the local police and fire departments would be doing essentially the same things they normally do, are all factors that tend to make a simple plan adequate. This report discusses the potentially hazardous accidents, and the likely effects of these accidents in terms of personnel danger.

Compilation of demographic data for the chemical stockpile emergency preparedness program

There are eight installations in the continental US where lethal unitary chemical agents and munitions have been stored since the late 1950`s. In December, 1985, Congress directed the Department of Defense (DOD) to destroy these stockpiles of aging chemical warfare weapons. The destruction was to take place in such a manner as to provide: (1) maximum protection of the environment, the general public, and the personnel involved in the destruction, (2) adequate and safe facilities designed solely for the destruction of the stockpile, and (3) clean-up dismantling, and disposal of the facilities when the disposal program was complete. To help communities develop emergency response capabilities, the Army established the Chemical Stockpile Emergency Preparedness Program or CSEPP based on principals established in the Emergency Response Concept Plan (ERCP). The Army and the Federal Emergency Management Agency (FEMA) jointly oversee the CSEPP. An important part of the ERCP guidance was establishing cooperative interaction between local, state, and federal agencies and the development of emergency planning zones (EPZs) to support the emergency response concept. The purpose of this document is to describe how the population figures were derived for the population estimates for both the Chemical Stockpile Disposal Program and the CSEPP analyses. Most of the data is derived from the US Census 1990 population figures. However, the Census only counts residential populations and does not attempt to document daytime populations within commercial or residential facilities. One conclusion from this review is that there is a need for better and more consistent population data in the Emergency Planning Guides.

Jackson Park Hospital completed the construction of a new Medical Office Building on its campus this spring. The new building construction has adopted the City of Chicago's recent focus on protecting the environment, and conserving energy and resources, with the introduction of green building codes. Located in a poor, inner city neighborhood on the South side of Chicago, Jackson Park Hospital has chosen green building strategies to help make the area a better place to live and work. The new green building houses the hospital's Family Medicine Residency Program and Specialty Medical Offices. The residency program has been vital in attracting new, young physicians to this medically underserved area. The new outpatient center will also help to allure needed medical providers to the community. The facility also has areas designated to women's health and community education. The Community Education Conference Room will provide learning opportunities to area residents. Emphasis will be placed on conserving resources and protecting our environment, as well as providing information on healthcare access and preventive medicine. The new Medical Office Building was constructed with numerous energy saving features. The exterior cladding of the building is an innovative, locally-manufactured precast concrete panel system with integral insulation that achieves an R-value in excess of building code requirements. The roof is a 'green roof' covered by native plantings, lessening the impact solar heat gain on the building, and reducing air conditioning requirements. The windows are low-E, tinted, and insulated to reduce cooling requirements in summer and heating requirements in winter. The main entrance has an air lock to prevent unconditioned air from entering the building and impacting interior air temperatures. Since much of the traffic in and out of the office building comes from the adjacent Jackson Park Hospital, a pedestrian bridge connects the two buildings, further decreasing the amount of unconditioned air that enters the office building. The HVAC system has an Energy Efficiency Rating 29% greater than required. No CFC based refrigerants were used in the HVAC system, thus reducing the emission of compounds that contribute to ozone depletion and global warming. In addition, interior light fixtures employ the latest energy-efficient lamp and ballast technology. Interior lighting throughout the building is operated by sensors that will automatically turn off lights inside a room when the room is unoccupied. The electrical traction elevators use less energy than typical elevators, and they are made of 95% recycled material. Further, locally manufactured products were used throughout, minimizing the amount of energy required to construct this building. The primary objective was to construct a 30,000 square foot medical office building on the Jackson Park Hospital campus that would comply with newly adopted City of Chicago green building codes focusing on protecting the environment and conserving energy and resources. The energy saving systems demonstrate a state of the-art whole-building approach to energy efficient design and construction. The energy efficiency and green aspects of the building contribute to the community by emphasizing the environmental and economic benefits of conserving resources. The building highlights the integration of Chicago's new green building codes into a poor, inner city neighborhood project and it is designed to attract medical providers and physicians to a medically underserved area.

Finally, examples of manual calculational techniques for producing radiological data are included as an appendix.

This report presents preliminary operational guidelines and supporting work products developed through the interagency Operational Guidelines Task Group (OGT). The report consolidates preliminary operational guidelines, all ancillary work products, and a companion software tool that facilitates their implementation into one reference source document. The report is intended for interim use and comment and provides the foundation for fostering future reviews of the operational guidelines and their implementation within emergency preparedness and response initiatives in the event of a radiological dispersal device (RDD) incident. The report principally focuses on the technical derivation and presentation of the operational guidelines. End-user guidance providing more details on how to apply these operational guidelines within planning and response settings is being considered and developed elsewhere. The preliminary operational guidelines are categorized into seven groups on the basis of their intended application within early, intermediate, and long-term recovery phases of emergency response. We anticipate that these operational guidelines will be updated and refined by interested government agencies in response to comments and lessons learned from their review, consideration, and trial application. This review, comment, and trial application process will facilitate the selection of a final set of operational guidelines that may be more or less inclusive of the preliminary operational guidelines presented in this report. These and updated versions of the operational guidelines will be made available through the OGT public Web site (http://ogcms.energy.gov) as they become finalized for public distribution and comment.

CPWR is carrying out a screening program for former Hanford construction workers. This program includes continuing screening and re-screening services for the former worker population. The Program contains the following general components: Start-up planning/needs assessment: A modified exposure assessment will be conducted to identify high-risk buildings or areas, primary exposures, and worker populations at risk. Outreach: CPWR, as the research arm of the Building and Construction Trades Department, AFL-CIO, has direct access to workers. CPWR will rely on direct mailings to lists of former workers, and work through and rely on existing organizations (unions, union pension funds, employers, DOE site administrators, etc.) to reach former workers and "get the word out." CPWR will establish/maintain an outreach office at each site listed above. This office will serve as the face of the Program to workers and their communities. Communications and intake center: CPWR has two established toll-free phone numbers (1-800-866-9663 and 1-888-464-0009). There is also a dedicated website for the program (btmed.org). Workers can register with the Program by mail, telephone, or on-line. Work history: A standardized, structured work history is administered with modules that accommodate unique exposure scenarios for different occupations and different DOE sites. A work history interview is administered by a trained program interviewer. The work histories are used to determine whether a participant is eligible for the medical examination and to interpret the findings from the medical examination. Medical evaluation: The Program contracts with local medical providers qualified to deliver occupational medical screening services. All providers are credentialed. The Program contracts with a certified national laboratory and with NIOSH certified B-readers to review x-rays. Based on the work history, the participant is referred to a credentialed medical provider who is located close to the participant's home. If it is not convenient to use a credentialed provider, the Program will make arrangements, if necessary, for the participant to receive a physical exam through the National Supplemental Screening Program. Eligible participants will receive the same core medical exam (including a Beryllium Lymphocyte Proliferation Test, BeLPT), and in addition, based on their work history, they may be assigned to exposure specific modules for asbestos, silica, lead, noise, cadmium, and chromium. Lab work will be sent to a national laboratory for processing, except the blood samples for the BeLPT, which will be sent to a DOE-approved laboratory for evaluation. Determination of work-relatedness and follow-up: A letter of findings will be sent to the participant within 60 days of the exam. The letter is written and/or reviewed by occupational medical health personnel with knowledge of the DOE site(s) where the participant has worked and will include specific follow-up recommendations. Urgent findings are followed up by the provider without delay. Evaluation and quality assurance: All data are entered into the Program Data Management System (DMS). The DMS is web-based and relies on electronic submission of results, whenever possible. A de-identified data set on all participants is provided to Duke University Medical Center for evaluation and analysis. Each participant is asked to complete a satisfaction survey. The DMS will be used for quality assurance purposes and to also report summary data to the DOE. The BTMED.ORG website is encrypted using the industry-standard Secure Sockets Layer (SSL) technology (128-bit encryption keys). Each individual that accesses the website is assigned a unique login ID, password, and token. Passwords are required to meet standards for "strength," including minimum length, multi-case, and use of numbers. The website is also protected by additional security layers including additional encryption, hardware and software firewalls, Virtual Private Networks (VPNs), and virus protection.

A Draft Technical Information Document (HNF-1855) is being prepared to evaluate proposed interim tritium and medical isotope production at the Fast Flux Test Facility (FFTF). This assessment examines the potential health and safety impacts of transportation operations associated with the production of medical isotopes. Incident-free and accidental impacts are assessed using bounding source terms for the shipment of nonradiological target materials to the Hanford Site, the shipment of irradiated targets from the FFTF to the 325 Building, and the shipment of medical isotope products from the 325 Building to medical distributors. The health and safety consequences to workers and the public from the incident-free transportation of targets and isotope products would be within acceptable levels. For transportation accidents, risks to works and the public also would be within acceptable levels. This assessment is based on best information available at this time. As the medical isotope program matures, this analysis will be revised, if necessary, to support development of a final revision to the Technical Information Document.

A medical imaging system provides simultaneous rendering of visible light and diagnostic or functional images. The system may be portable, and may include adapters for connecting various light sources and cameras in open surgical environments or laparascopic or endoscopic environments. A user interface provides control over the functionality of the integrated imaging system. In one embodiment, the system provides a tool for surgical pathology.

Newberg, OR In 2002, Providence Health & Services began planning a new 188,000 square foot medical center in Newberg, Oregon to respond to the growing community's need for accessible health care. Since this was Providence's first new hospital in almost thirty years, its leaders decided to approach the project through innovative planning, design, and construction, including the achievement of lifecycle energy savings and a potential LEED certification. DOE's Technology Commercialization activities in 2009-13 have involved three broad areas of focus. The primary focus of technology commercialization has continued to be through new technologies developed at the National Laboratories and Facilities. As a second focus, to support and streamline commercialization of these DOE technologies, DOE has carried out a number of new initiatives and pilot projects. Finally, DOE's Department-wide commitment to using commercialization as one mechanism to support U.S. economic growth has led to new cross-cutting programs. U.S. Department of Energy researchers won 31 of the 100 awards in 2014, 36 awards in each of 2013, 2012 and 2011, and 46 in 2010, for a total of 185 over the period of 2009-13. A subset of these awards and other DOE developed technologies are described in Appendix E. These represent a spectrum of commercial areas including DOE mission areas of energy, efficiency, environment and security, as well as spin-off applications in the agricultural, aeronautical, medical, semiconductor and information technology industries, and broad applications in cyber security and sensing/control systems.

A medical imaging system provides simultaneous rendering of visible light and fluorescent images. The system may employ dyes in a small-molecule form that remains in a subject's blood stream for several minutes, allowing real-time imaging of the subject's circulatory system superimposed upon a conventional, visible light image of the subject. The system may also employ dyes or other fluorescent substances associated with antibodies, antibody fragments, or ligands that accumulate within a region of diagnostic significance. In one embodiment, the system provides an excitation light source to excite the fluorescent substance and a visible light source for general illumination within the same optical guide that is used to capture images. In another embodiment, the system is configured for use in open surgical procedures by providing an operating area that is closed to ambient light. More broadly, the systems described herein may be used in imaging applications where a visible light image may be usefully supplemented by an image formed from fluorescent emissions from a fluorescent substance that marks areas of functional interest.

In order to obtain an improved data base encompassing a broad spectrum of office space and with information suitable for energy analysis in greater detail than is currently available, BOMA undertook a major data-collection effort. Based on a consideration of geographic area, climate, population, and availability of data, BOMA selected twenty cities for data collection. BOMA listed all of the major office space - buildings in excess of 40,000 square feet - in each of the cities. Tax-assessment records, local maps, Chamber of Commerce data, recent industrial-development programs, results of related studies, and local-realtor input were used in an effort to assemble a comprehensive office-building inventory. In order to verify the accuracy and completeness of the building lists, BOMA assembled an Ad-Hoc Review Committee in each city to review the assembled inventory of space. A questionnaire on office-building energy use and building characteristics was developed. In each city BOMA assembled a data collection team operating under the supervision of its regional affiliate to gather the data. For each city a random sample of buildings was selected, and data were gathered. Responses for over 1000 buildings were obtained.

This document has been prepared by the Department of Energy`s (DOE) Environmental Management (EM) Office of Technology Development (OTD) to highlight its research, development, demonstration, testing, and evaluation (RDDT and E) activities funded through the Chicago Operations Office. Technologies and processes described have the potential to enhance DOE`s cleanup and waste management efforts, as well as improve US Industry`s competitiveness in global environmental markets. The information has been assembled from recently produced OTD documents which highlight technology development activities within each of the OTD program elements. OTD technologies addresses three specific problem areas: (1) groundwater and soils cleanup; (2) waste retrieval and processing; and (3) pollution prevention. These problems are not unique to DOE, but are associated with other Federal agency and industry sites as well. Thus, technical solutions developed within OTD programs will benefit DOE, and should have direct applications in outside markets.

Under the current program standards, the outdoor warning system consists of omnidirectional sirens and the new indoor system would be an enhanced tone alert (TA) radio system. This analysis identifies various tone-alert technologies, distribution options, and alternative siren configurations. It also assesses the costs and benefits of the options and analyzes what appears to best meet program needs. Given the current evidence, it is recommended that a 10-dB siren system and the special or enhanced TA radio be distributed to each residence and special institution in the immediate response zone as preferred the A N standard. This approach minimizes the cost of maintenance and cost of the TA radio system while providing a high degree of reliability for indoor alerting. Furthermore, it reaches the population (residential and institutional) in the greatest need of indoor alerting.

The Nuclear Installations Inspectorate (NII) of the United Kingdom (UK) suggested the use of an accident progression logic model method developed by Westinghouse Savannah River Company (WSRC) and Science Applications International Corporation (SAIC) for K Reactor to predict the magnitude and timing of radioactivity releases (the source term) based on an advanced logic model methodology. Predicted releases are output from the personal computer-based model in a level-of-confidence format. Additional technical discussions eventually led to a request from the NII to develop a proposal for assembling a similar technology to predict source terms for the UK`s advanced gas-cooled reactor (AGR) type. To respond to this request, WSRC is submitting a proposal to provide contractual assistance as specified in the Scope of Work. The work will produce, document, and transfer technology associated with a Decision-Oriented Source Term Estimator for Emergency Preparedness (DOSE-EP) for the NII to apply to AGRs in the United Kingdom. This document, Appendix A is a part of this proposal.

A medical implant device having a substrate with an oxidized surface and a silane derivative coating covalently bonded to the oxidized surface. A bioactive agent is covalently bonded to the silane derivative coating. An implantable stent device including a stent core having an oxidized surface with a layer of silane derivative covalently bonded thereto. A spacer layer comprising polyethylene glycol (PEG) is covalently bonded to the layer of silane derivative and a protein is covalently bonded to the PEG. A method of making a medical implant device including providing a substrate having a surface, oxidizing the surface and reacting with derivitized silane to form a silane coating covalently bonded to the surface. A bioactive agent is then covalently bonded to the silane coating. In particular instances, an additional coating of bio-absorbable polymer and/or pharmaceutical agent is deposited over the bioactive agent.

Office of the Chief Financial Officer Annual Report 2010

In March, a review team consisting of CFOs from other national laboratories, industry, and members of the University of California Office of the President (UCOP) convened for three days to conduct a comprehensive peer review of the OCFO. This was the first time in almost a decade that the financial operations of the Laboratory had been reviewed. The Committee relayed their observations on our strengths, and their very thoughtful recommendations for improvement, which we are actively pursuing. These improvements, when implemented, will benefit the entire Laboratory for many years to come. The complete report is available on the OCFO website (www.lbl.gov/Workplace/CFO). In August, the senior management team of the OCFO participated in a strategic planning retreat. The purpose of the two and a half day exercise was, of course, to update our strategic plan, but instead of spending days developing a written document, we enlisted the expertise of a seasoned journalist who also happens to be a very talented graphic artist. He listened carefully to our ideas and committed them to a visual roadmap. All members of the OCFO, Business Managers, and the Laboratory Leadership Team reviewed this draft roadmap. By having a completely visual strategic plan that is posted widely throughout the OCFO, all employees can easily see and identify with the goals that we are all working towards. FY2010 was an extraordinary year. The Laboratory welcomed its seventh Director, Dr. Paul Alivisatos, who wasted no time communicating his vision and priorities for Berkeley Lab. They include five very ambitious initiatives: Carbon Cycle 2.0, The Next Generation Light Source, a Safe and Efficient Lab, Building Community, and Space. In response, the Office of the Chief Financial Officer (OCFO) developed twelve specific initiatives that align completely with these five priorities. We will be very focused on these in the coming fiscal year, but for now, let's review what happened in FY2010. FY2010 was a pivotal year for the Procurement and Property Department. A provision of the management contract that was signed fives years ago required us to achieve cost savings of $30M. I am proud to announce that this last fiscal year we reached that goal, in large part due to the implementation of eBuy, and the negotiation of strategic sourcing contracts. Our last wall-to-wall inventory exceeded all the Department of Energy's (DOE) national targets and DOE approved the LBNL property system unconditionally. Of the total inventory, 92.3% or 25,601 assets were accounted for using barcode scanning that made the inventory process much more efficient. The effective management of the American Recovery and Reinvestment Act (ARRA) funds was strengthened by the continued successful partnership that LBNL shares with our DOE Site Office. They provided authority, support and clarity to this very complex task. The Laboratory's ARRA Stimulus Committee played a critical role in assuring internal controls, compliance with DOE regulations and quality financial management. The LBNL Budget Officer led a DOE complex-wide effort to identify and share how various DOE facilities track and report ARRA-funded projects, share best practices, evaluate issues and discuss solutions. LBNL has been recognized as a leader in this process and will continue to share knowledge and best practices with other DOE laboratories. Continuous education of our staff as well as the greater Laboratory population was still a major focus of the OCFO. With the help of many OCFO senior managers and staff that acted as subject-matter experts, the Core Financial Management Program was completely re-engineered and resulted in a revised classroom and web-based curriculum that will be formally rolled out Lab-wide in early FY2011. The Office of Sponsored Projects and Industry Partnerships (OSPIP) led the effort to select and purchase four Click Commerce software modules for the LBNL electronic Scientific Research Administration (eSRA) project. Medical Surveillance for Former Workers

The Former Hanford Worker Medical Monitoring Program, directed by the Occupational and Environmental Medicine Program at the University of Washington, served former production and other non-construction workers who were potentially exposed to workplace hazards while working for the USDOE or its contractors at Hanford. The USDOE Former Workers Program arose from Congressional action in the Defense Authorization of 1993 (Public Law 102). Section 3162 stated that, ďThe Secretary shall establish and carry out a program for the identification and ongoing medical evaluation of current and former Department of Energy employees who are subject to significant health risks as a result of exposure of such employees to hazardous or radioactive substances during such employment.Ē (This also covers former employees of USDOE contractors and subcontractors.) The key objective has been to provide these former workers with medical evaluations in order to determine whether workers have experienced significant risk due to workplace exposure to hazards. Exposures to asbestos, beryllium, and noise can produce specific medical conditions: asbestosis, berylliosis, and noise-induced hearing loss (NIHL). Each of these conditions can be identified by specific, non-invasive screening tests, which are widely available. Treatments are also available for individuals affected by these conditions. This project involved two phases. Phase I involved a needs and risk assessment, characterizing the nature and extent of workplace health hazards which may have increased the risk for long-term health effects. We categorized jobs and tasks by likelihood of exposures to specific workplace health hazards; and located and established contact with former Hanford workers. Phase II involved implementation of medical monitoring programs for former workers whose individual work history indicated significant risk for adverse health effects. We identified 118,000 former workers, employed from 1943 to 1997. After excluding current workers, construction workers, and deceased workers, the total estimated number of former workers eligible for screening was 72,611. By September, 2006, 53,010 workers had been contacted, 20,298 responded, 2,835 were eligible and authorized, and 2,773 workers were ultimately screened. The cohort was 80% male, 85% white, and had a mean age of 63 years (range 24-96 years) at the time of first exam. Participants completed an occupational health history survey prior to the medical exam. Former Hanford workers were considered eligible for an exam if they reported exposure to asbestos, beryllium, or noise, or if a review of their Hanford work history indicated possible or probable exposure to one of these three hazards. We also invited any former Hanford worker who requested an exam to participate, regardless of documentation of exposure. The screening exam included a problem-focused physical exam, along with screening tests for one or more of three specific medical conditions: asbestosis (chest X-ray and spirometry), berylliosis (chest X-ray, spirometry, and beryllium-induced lymphocyte proliferation test), and NIHL (audiometry). We assisted ill workers in filing appropriate workersí compensation claims, and facilitated appropriate follow-up medical care. This program has made an important contribution to the health of former DOE contractor workers at the Hanford defense nuclear site.

The present invention relates to an apparatus for producing sterile ice slurries for medical cooling applications. The apparatus is capable of producing highly loaded slurries suitable for delivery to targeted internal organs of a patient, such as the brain, heart, lungs, stomach, kidneys, pancreas, and others, through medical size diameter tubing. The ice slurry production apparatus includes a slurry production reservoir adapted to contain a volume of a saline solution. A flexible membrane crystallization surface is provided within the slurry production reservoir. The crystallization surface is chilled to a temperature below a freezing point of the saline solution within the reservoir such that ice particles form on the crystallization surface. A deflector in the form of a reciprocating member is provided for periodically distorting the crystallization surface and dislodging the ice particles which form on the crystallization surface. Using reservoir mixing the slurry is conditioned for easy pumping directly out of the production reservoir via medical tubing or delivery through other means such as squeeze bottles, squeeze bags, hypodermic syringes, manual hand delivery, and the like.
