    Security officer tactical training issues are discussed in relation to the possible implementation of the Tactical Improvement Package (TIP), utilizing the Engagement Simulation System (ESS) equipment, by nuclear power plant licensees for security officer tactical training. The ESS equipment provides the capability to simulate engagement conditions between adversaries armed with weapons which have harmless laser transmitters. A brief discussion of the TIP is presented, along with some concerns and considerations in the use of the TIP.

    One method used by the Department of Energy (DOE) to promote worker safety is through the Department`s accident investigation program. The objectives of the program are, among other things, to enhance safety and health of employees, to prevent the recurrence of accidents, and to reduce accident fatality rates and promote a downward trend in the number and severity of accidents. The Assistant Secretary, Office of Environment, Safety and Health (EH), through the EH Office of the Deputy Assistant Secretary for Oversight, is responsible for implementation of the Department`s accident investigation program. As part of the inspection, the authors reviewed an April 1997 EH accident investigation report regarding an accident involving a Lockheed Martin Energy Systems (LMES) welder, who suffered fatal burns when his clothing caught fire while he was using a cutting torch at the Oak Ridge K-25 Site. They also reviewed reports of other accident investigations conducted by EH and DOE field organizations. Based on the review of these reports, the authors identified issues concerning the adequacy of the examination and reporting by accident investigation boards of specific management systems and organizations as a possible accident root cause. The inspection also identified issues concerning worker safety that they determined required immediate management attention, such as whether occurrences were being reported in the appropriate management systems and whether prompt consideration was being given to implementing revisions of national standards when the revisions increased worker safety.

  17. Indoor environmental quality and ventilation in U.S. office buildings: A view of current issues

    Much of the current focus on indoor environmental quality and ventilation in US office buildings is a response to sick building syndrome and occupant complaints about building-related health symptoms, poor indoor air quality, and thermal discomfort. The authors know that serious ``sick-building`` problems occur in a significant number of US office buildings and that a significant proportion of the occupants in many normal (non-sick) buildings report building-related health symptoms. Concerns about the health effects of environmental tobacco smoke have also focused attention on the indoor environment. The major responses of industry and governments, underway at the present time, are to restrict smoking in offices, to attempt to reduce the emissions of indoor pollutants, and to improve the operation of heating, ventilating and air conditioning (HVAC) systems. Better air filtration, improved HVAC commissioning and maintenance, and increased provisions for individual control of HVAC are some of the improvements in HVAC that are currently being, evaluated. In the future, the potential for improved productivity and reduced airborne transmission of infectious disease may become the major driving force for improved indoor environments.

The project requires a broad range of environmental protection expertise, necessitating senior-level personnel as the primary project staff. Many of the tasks assigned by DOE require quick startup and performance, and several tasks may be active at any one time.

    Energy-Efficient Office Technologies. The Outlook and Market Workshop held from June 17 to 18, 1992, in San Jose, California, was planned with the goal of developing and implementing strategies to make more energy-efficient office automation equipment a part of the modern business environment. The consumption of electricity in the office environment has increased dramatically since the advent of the desktop personal computer for the mass market. During that same period, other office automation equipment, such as facsimile machines (faxes) and convenience copiers, has also proliferated, contributing to a dramatic increase in plug loads. Participants in the workshop gathered in plenary session to hear a series of stage-setting'' informal presentations, then divided into three concurrent working groups: PCs, Workstations, and Terminals; Imaging Technologies: Printers, Copiers, and Facsimile Machines; and LANS, Software, and Telecommunications. These working groups developed brief consensus statements of the state of the art and trends in energy efficiency and power management; user acceptance; and energy-efficiency implementation strategies. More complete statistics on power consumption by office equipment are needed to heighten awareness among end users and to facilitate better design for new office space. The collaboration of manufacturers, customers, and energy suppliers across international boundaries is critical to identify mechanisms to improve energy performance in the commercial office environment The most promising strategies will work best if they are voluntary, market-driven, and are truly the end result of a common vision.

    Issues in Midterm Analysis and Forecasting 1998 (Issues) presents a series of nine papers covering topics in analysis and modeling that underlie the Annual Energy Outlook 1998 (AEO98), as well as other significant issues in midterm energy markets. AEO98, DOE/EIA-0383(98), published in December 1997, presents national forecasts of energy production, demand, imports, and prices through the year 2020 for five cases -- a reference case and four additional cases that assume higher and lower economic growth and higher and lower world oil prices than in the reference case. The forecasts were prepared by the Energy Information Administration (EIA), using EIA`s National Energy Modeling System (NEMS). The papers included in Issues describe underlying analyses for the projections in AEO98 and the forthcoming Annual Energy Outlook 1999 and for other products of EIA`s Office of Integrated Analysis and Forecasting. Their purpose is to provide public access to analytical work done in preparation for the midterm projections and other unpublished analyses. Specific topics were chosen for their relevance to current energy issues or to highlight modeling activities in NEMS. 59 figs., 44 tabs.

    The Lawrence Berkeley National Laboratory (LBNL) Issues Management Program encompasses the continuous monitoring of work programs, performance and safety to promptly identify issues to determine their risk and significance, their causes, and to identify and effectively implement corrective actions to ensure successful resolution and prevent the same or similar problems from occurring. This document describes the LBNL Issues Management Program and prescribes the process for issues identification, tracking, resolution, closure, validation, and effectiveness of corrective actions. Issues that are governed by this program include program and performance deficiencies or nonconformances that may be identified through employee discovery, internal or external oversight assessment findings, suggested process improvements and associated actions that require formal corrective action. Issues may also be identified in and/or may result in Root Cause Analysis (RCA) reports, Price Anderson Amendment Act (PAAA) reports, Occurrence Reporting and Processing System (ORPS) reports, Accident Investigation reports, assessment reports, and External Oversight reports. The scope of these issues may include issues of both high and low significance as well as adverse conditions that meet the reporting requirements of the University of California (UC) Assurance Plan for LBNL or other reporting entities (e.g., U.S. Environmental Protection Agency, U.S. Department of Energy). Issues that are found as a result of a walk-around or workspace inspection that can be immediately corrected or fixed are exempt from the requirements of this document.

    The US Department of Energy (DOE) Office of Environmental Management, formerly the Office of Environmental Restoration and Waste Management (EM), was established in November 1989 as the first step toward correcting contamination problems resulting from nearly 50 years of nuclear weapons production and fuel processing activities. EM consolidates several DOE organizations previously responsible for the handling, treatment, and disposition of radioactive and hazardous waste. Within EM, the Office of Technology Development (OTD/EM-50) is responsible for developing technologies to meet DOE`s goal for environmental restoration. OTD manages an aggressive national program of applied research, development, demonstration, testing, and evaluation (RDDT and E) for environmental cleanup, waste management, and related technologies. The program is designed to resolve major technical issues, to rapidly advanced beyond current technologies for environmental restoration and waste management operations, and to expedite compliance with applicable environmental laws and regulations. This report summarizes Fiscal Year 1994 (FY94) programmatic information, accomplishments, and planned activities relevant to the individual activities within OTD`s RDDT and E.

    In March, a review team consisting of CFOs from other national laboratories, industry, and members of the University of California Office of the President (UCOP) convened for three days to conduct a comprehensive peer review of the OCFO. This was the first time in almost a decade that the financial operations of the Laboratory had been reviewed. The Committee relayed their observations on our strengths, and their very thoughtful recommendations for improvement, which we are actively pursuing. These improvements, when implemented, will benefit the entire Laboratory for many years to come. The complete report is available on the OCFO website (www.lbl.gov/Workplace/CFO). In August, the senior management team of the OCFO participated in a strategic planning retreat. The purpose of the two and a half day exercise was, of course, to update our strategic plan, but instead of spending days developing a written document, we enlisted the expertise of a seasoned journalist who also happens to be a very talented graphic artist. He listened carefully to our ideas and committed them to a visual roadmap. All members of the OCFO, Business Managers, and the Laboratory Leadership Team reviewed this draft roadmap. By having a completely visual strategic plan that is posted widely throughout the OCFO, all employees can easily see and identify with the goals that we are all working towards. FY2010 was an extraordinary year. The Laboratory welcomed its seventh Director, Dr. Paul Alivisatos, who wasted no time communicating his vision and priorities for Berkeley Lab. They include five very ambitious initiatives: Carbon Cycle 2.0, The Next Generation Light Source, a Safe and Efficient Lab, Building Community, and Space. In response, the Office of the Chief Financial Officer (OCFO) developed twelve specific initiatives that align completely with these five priorities. We will be very focused on these in the coming fiscal year, but for now, let's review what happened in FY2010. FY2010 was a pivotal year for the Procurement and Property Department. A provision of the management contract that was signed fives years ago required us to achieve cost savings of $30M. I am proud to announce that this last fiscal year we reached that goal, in large part due to the implementation of eBuy, and the negotiation of strategic sourcing contracts. Our last wall-to-wall inventory exceeded all the Department of Energy's (DOE) national targets and DOE approved the LBNL property system unconditionally. Of the total inventory, 92.3% or 25,601 assets were accounted for using barcode scanning that made the inventory process much more efficient. The effective management of the American Recovery and Reinvestment Act (ARRA) funds was strengthened by the continued successful partnership that LBNL shares with our DOE Site Office. They provided authority, support and clarity to this very complex task. The Laboratory's ARRA Stimulus Committee played a critical role in assuring internal controls, compliance with DOE regulations and quality financial management. The LBNL Budget Officer led a DOE complex-wide effort to identify and share how various DOE facilities track and report ARRA-funded projects, share best practices, evaluate issues and discuss solutions. LBNL has been recognized as a leader in this process and will continue to share knowledge and best practices with other DOE laboratories. Continuous education of our staff as well as the greater Laboratory population was still a major focus of the OCFO. With the help of many OCFO senior managers and staff that acted as subject-matter experts, the Core Financial Management Program was completely re-engineered and resulted in a revised classroom and web-based curriculum that will be formally rolled out Lab-wide in early FY2011. The Office of Sponsored Projects and Industry Partnerships (OSPIP) led the effort to select and purchase four Click Commerce software modules for the LBNL electronic Scientific Research Administration (eSRA) project.     The paper reports on the first experiments with plasma-dust formations in dc gas discharge plasma for He-Ar mixture. It is shown that under the conventional conditions of the experiments with dusty structures in plasma, the choice of light and heavy gases for the mixture suppresses electron heating in electric field and results in a supersonic jet with high Mach numbers.     Permitting is a major component of the geothermal development process. Better coordination across government agencies could reduce uncertainty of the process and the actual time of permitting. This presentation highlights various forms of coordinating permit offices at the state and federal level in the western United States, discusses inefficiencies and mitigation techniques for permitting natural resource projects, analyzes whether various approaches are easily adaptable to utility-scale geothermal development, and addresses advantages and challenges for coordinating permit offices. Key successful strategies identified include: 1. Flexibility in implementing the approach (i.e. less statutory requirements for the approach); 2. Less dependence on a final environmental review for information sharing and permit coordination; 3. State and federal partnerships developed through memorandum of understanding to define roles and share data and/or developer information. A few of the most helpful techniques include: 1. A central point of contact for the developer to ask questions surrounding the project; 2. Pre-application meetings to assist the developer in identifying all of the permits, regulatory approvals, and associated information or data required; 3. A permit schedule or timeline to set expectations for the developer and agencies; 4. Consolidating the public notice, comment, and hearing period into fewer hearings held concurrently.

    Although the government admit the benefit of construction of a nuclear facility for national electric source, related policy could be developed and carried out only if the public, especially who have some stake on it, recognize the benefit and accept the policy. For public participation, Korea has a system of public-hearing in accordance with the law. Because of the absence of the detailed way for public opinion aggregation and for the reflection of the aggregated opinion, Korean public-hearing system is only a conceptual model. Therefore, some specific system for Korean Public-Hearing should be developed and applied. In this study, to share the right of decision making, which is an ultimate concept for public participation, decision making components and the characteristics of each phase are analyzed. The criteria weight for assessment and comparison with alternatives are founded as a valuation factor of the decision making components, which should be based on the social consensus. On these foundations, a system for aggregation and reflection of the public opinion was proposed. The system named 'CPDM' (Consensus based Participatory Decision Making) has three authority groups for decision making. At first, 'advisory experts group' play a role for the technical assessment and the serve utility value on the criteria for each alternatives. Next, 'participatory deliberation group' play a role for consensus building on the relative-importance (weight) between the criteria by feedback to promote degree of consensus. Lastly including gentlemen of the long robe, 'expert group for decision making' play a role to reflect the utility and weight and make a decision with agreement for performance of it. Also, in this study, a mathematical model for the quantification of the degree of consensus was conceptualized using Ordered Weighted Averaging (OWA) aggregation operator and fuzzy similarity theory, which is a comparison concept. Since this model enables influence of each criteria and each participant on collective consensus to be analyzed, a direction to promote consensus building can be derived. That is to say, this model can support consensus building and promote public acceptance for the nuclear industry and related policy. (authors)

    Purpose: We evaluated the prognostic factors for hearing outcomes after stereotactic radiosurgery (SRS) for unilateral sporadic intracanalicular vestibular schwannomas (IC-VSs) as a clinical homogeneous group of VSs. Methods and Materials: Sixty consecutive patients with unilateral sporadic IC-VSs, defined as tumors in the internal acoustic canal, and serviceable hearing (Gardner-Roberson grade 1 or 2) were treated with SRS as an initial treatment. The mean tumor volume was 0.34 {+-} 0.03 cm{sup 3} (range, 0.03-1.00 cm{sup 3}), and the mean marginal dose was 12.2 {+-} 0.1 Gy (range, 11.5-13.0 Gy). The median follow-up duration was 62 months (range, 36-141 months). Results: The actuarial rates of serviceable hearing preservation were 70%, 63%, and 55% at 1, 2, and 5 years after SRS, respectively. In multivariate analysis, transient volume expansion of {>=}20% from initial tumor size was a statistically significant risk factor for loss of serviceable hearing and hearing deterioration (increase of pure tone average {>=}20 dB) (odds ratio = 7.638; 95% confidence interval, 2.317-25.181; P=.001 and odds ratio = 3.507; 95% confidence interval, 1.228-10.018; P=.019, respectively). The cochlear radiation dose did not reach statistical significance. Conclusions: Transient volume expansion after SRS for VSs seems to be correlated with hearing deterioration when defined properly in a clinically homogeneous group of patients.

    The evaluation and communication of the significance of environmental effects remains a critical yet poorly understood component of EIA theory and practice. Following a conceptual overview of the generic dimensions of impact significance in EIA, this paper reports upon the findings of an empirical study of recent environmental impact statements that considers the treatment of significance for impacts concerning landscape ('see no evil') and noise ('hear no evil'), focussing specifically upon the evaluation and communication of impact significance ('speak no evil') in UK practice. Particular attention is given to the use of significance criteria and thresholds, including the development of a typology of approaches applied within the context of noise and landscape/visual impacts. Following a broader discussion of issues surrounding the formulation, application and interpretation of significance criteria, conclusions and recommendations relevant to wider EIA practice are suggested.

    Pacific Northwest Laboratory (PNL) conducted this study for the Office of Energy Resources of the Bonneville Power Administration. The purpose of the report is to compare and contrast the environmental requirements and issues involving Bonneville's residential conservation programs. In addition to environmental issues that Bonneville has addressed in environmental documents, this report also briefly examines new issues that may affect residential conservation programs. The key environmental concern confronting each of the programs with measures aimed at reducing air leakage in houses (both new and existing) is indoor air quality. There are inconsistencies in how the Weatherization Program and the New Homes programs approach indoor air quality. However, these differences make sense, given the character and constraints affecting how each program operates. Newer issues that have arisen include global warming, potential health effects of mineral and glass fibers, and possible fire hazards associated with plastic foam and cellulose insulation. Bonneville staff are aware of these issues as they relate to conservation programs. No action appears necessary at this time.

    The report summarizes significant audit, inspection, and investigative accomplishments for the reporting period which facilitated Department of Energy management efforts to improve management controls and ensure efficient and effective operation of its programs. Narratives of the Office`s most significant reports are grouped by measures which the Office of Inspector General uses to gauge its performance. The common thread tying the performance measures together is their emphasis on supporting the Department efforts to produce high quality products at the lowest possible cost to the taxpayer. During this reporting period, the Office of Inspection General issued 59 reports. These reports included recommendations than, when implemented by management, could result in $211.7 million being put to better use. Furthermore, management has committed to taking corrective actions which the Office of Inspector General estimates will result in a more efficient use of funds totaling $57 million. Office of Inspector General investigations led to 7 criminal convictions, as well as criminal and civil prosecutions which resulted in fines and recoveries of approximately $1.95 million. The Office of Inspector General also provided 9 investigative reports to management for recommending positive change.
