In this paper we identified the Grid services that could facilitate the integration and interoperation of Health Care data and frameworks world-wide. While many of the current Health Care Grid projects address issues such as data location and description on the Grid and the security aspects, the problems connected to data storage, integrity, preservation and distribution have been neglected. We describe the currently available Grid storage services and protocols that can come in handy when dealing with those problems. Indoor air quality can be impacted by hundreds of different chemicals. More than 900 different organic compounds alone have been identified in indoor air. Health effects that could arise from exposure to individual pollutants or mixtures of pollutants cover the full range of acute and chronic effects, including largely reversible responses, such as rashes and irritations, to the irreversible toxic and carcinogenic effects. These indoor contaminants are emitted from a large variety of materials and substances that are widespread components of everyday life. Pacific Northwest Laboratory conducted a search of the peer-reviewed literature on health effects associated with indoor air contaminants for the Bonneville Power Administration to aid the agency in the preparation of environmental documents. Results are reported in two volumes. Volume 1 summarizes the results of the search of the peer-reviewed literature on health effects associated with a selected list of indoor air contaminants. In addition, the report discusses potential health effects of polychlorinated biphenyls and chlorofluorocarbons. All references to the literature reviewed are found in this document Volume 2. Volume 2 provides detailed information from the literature reviewed, summarizes potential health effects, reports health hazard ratings, and discusses quantitative estimates of carcinogenic risk in humans and animals. Contaminants discussed in this report are those that; have been measured in the indoor air of a public building; have been measured (significant concentrations) in test situations simulating indoor air quality (as presented in the referenced literature); and have a significant hazard rating. 38 refs., 7 figs., 23 tabs.

Sandia National Laboratories applied a systems approach to identifying innovative biomedical technologies with the potential to reduce U.S. health care delivery costs while maintaining care quality. This study was conducted by implementing both top-down and bottom-up strategies. The top-down approach used prosperity gaming methodology to identify future health care delivery needs. This effort provided roadmaps for the development and integration of technology to meet perceived care delivery requirements. The bottom-up approach identified and ranked interventional therapies employed in existing care delivery systems for a host of health-related conditions. Economic analysis formed the basis for development of care pathway interaction models for two of the most pervasive, chronic disease/disability conditions: coronary artery disease (CAD) and benign prostatic hypertrophy (BPH). Societal cost-benefit relationships based on these analyses were used to evaluate the effect of emerging technology in these treatment areas. 17 figs., 48 tabs.

In Phase I of this project, reported in SAND97-1922, Sandia National Laboratories applied a systems approach to identifying innovative biomedical technologies with the potential to reduce U.S. health care delivery costs while maintaining care quality. The effort provided roadmaps for the development and integration of technology to meet perceived care delivery requirements and an economic analysis model for development of care pathway costs for two conditions: coronary artery disease (CAD) and benign prostatic hypertrophy (BPH). Phases II and III of this project, which are presented in this report, were directed at detailing the parameters of telemedicine that influence care delivery costs and quality. These results were used to identify and field test the communication, interoperability, and security capabilities needed for cost-effective, secure, and reliable health care via telemedicine.

A total of 356 indoor-air-quality health-hazard evaluations were completed by NIOSH from 1971 through December of 1985. Most of these studies concerned government and private office buildings where there were worker complaints. Worker complaints resulted from contamination from inside the building (19% of the cases), contamination from outside (11 percent), contamination from the building fabric (4%), biological contamination (5%), inadequate ventilation (50%), and unknown causes (11%). Health complaints addressed by investigative efforts included eye irritation, dry throat, headache, fatigue, sinus congestion, skin irritation, shortness of breath, cough, dizziness, and nausea.

In the United States, there is no standardized method for training nurses in manual patient handling, despite the high incidence of injury. The objective of this research was to evaluate several training protocols, including the use of simulated emergency situations and the use of realistic context during course delivery, to evaluate their impact on nurses postures and compliance with trained procedures during post-training tests. The results show that training on body mechanics and proper use of equipment can significantly reduce the amount of torso flexion and rotation during patient handling and compliance with safe practices. Implications for health care training are provided.

The West Haven (Connecticut) Campus of the Veterans Affairs Connecticut Health Care System was the first Veteran's Hospital to award a shared energy savings (SES) contract (now known as energy savings performance contracts). The project involves replacement of the lighting system, installation of a cooling system, maintenance of the new chiller equipment, and several smaller efforts. Up-front costs are being provided through a $3.9 million investment by the contractor, EUA Cogenex, about $400,000 in rebates from the local utilities for gas and electric service (Southern Connecticut Gas and United Illuminating Company, respectively), and guaranteed energy cost savings over the life of the contract. The Government's share of energy savings over the 16 year contract is expected to be $880,000.

Black-Right-Pointing-Pointer HCW treatment using microwave oven had the lowest environmental impacts and costs in comparison with autoclave and lime. Black-Right-Pointing-Pointer Lime had the worst environmental and economic results for HCW treatment, in comparison with autoclave and microwave. - Abstract: The establishment of rules to manage Health Care Waste (HCW) is a challenge for the public sector. Regulatory agencies must ensure the safety of waste management alternatives for two very different profiles of generators: (1) hospitals, which concentrate the production of HCW and (2) small establishments, such as clinics, pharmacies and other sources, that generate dispersed quantities of HCW and are scattered throughout the city. To assist in developing sector regulations for the small generators, we evaluated three management scenarios using decision-making tools. They consisted of a disinfection technique (microwave, autoclave and lime) followed by landfilling, where transportation was also included. The microwave, autoclave and lime techniques were tested at the laboratory to establish the operating parameters to ensure their efficiency in disinfection. Using a life cycle assessment (LCA) and cost analysis, the decision-making tools aimed to determine the technique with the best environmental performance. This consisted of evaluating the eco-efficiency of each scenario. Based on the life cycle assessment, microwaving had the lowest environmental impact (12.64 Pt) followed by autoclaving (48.46 Pt). The cost analyses indicated values of US$ 0.12 kg{sup -1} for the waste treated with microwaves, US$ 1.10 kg{sup -1} for the waste treated by the autoclave and US$ 1.53 kg{sup -1} for the waste treated with lime. The microwave disinfection presented the best eco-efficiency performance among those studied and provided a feasible alternative to subsidize the formulation of the policy for small generators of HCW.

This human health risk assessment has been prepared for the Environmental Restoration (ER) Program at the Oak Ridge National Laboratory (ORNL), Oak Ridge, Tennessee. The objectives of this risk assessment are to evaluate the alternatives for interim closure of the Building 3001 Storage Canal and to identify the potential health risk from an existing leak in the canal. The Building 3001 Storage Canal connects Buildings 3001 and 3019. The volume of water in the canal is monitored and kept constant at about 62,000 gal. The primary contaminants of the canal water are the radionuclides {sup 137}Cs, {sup 60}Co, and {sup 90}Sr; a layer of sediment on the canal floor also contains radionuclides and metals. The prime medium of contaminant transport has been identified as groundwater. The primary route for occupational exposure at the canal is external exposure to gamma radiation from the canal water and the walls of the canal. Similarly, the primary exposure route at the 3042 sump is external exposure to gamma radiation from the groundwater and the walls of the sump. Based on the exposure rates in the radiation work permits (Appendix C) and assuming conservative occupational work periods, the annual radiation dose to workers is considerably less than the relevant dose limits. The potential risk to the public using the Clinch River was determined for three significant exposure pathways: ingestion of drinking water; ingestion of contaminated fish; and external exposure to contaminated sediments on the shoreline, the dominant exposure pathway.

The ability to ensure adequate numbers of medical staff represents a crucial part of the medical response to any disaster. However, healthcare worker absenteeism during disasters, especially in the event of an attack of biological terrorism or an epidemic such as pandemic influenza, is a serious concern. Though a significant rate of absenteeism is often included as a baseline assumption in emergency planning, published reports on strategies to minimize absenteeism are comparatively few. This report documents interviews with managers and emergency response planners at hospitals and public health agencies and reviews existing survey data on healthcare worker absenteeism and studies of disasters to glean lessons about the needs of healthcare workers during those disasters. Based on this research, expected rates of absenteeism and individual determinants of absenteeism are presented along with recommendations of steps that hospitals, emergency medical services departments, public health organizations, and government agencies can take to meet the needs of healthcare workers and minimize absenteeism during a biological event.

The Commercial Buildings Energy Consumption Survey (CBECS) collects basic statistical information on energy consumption and energy-related characteristics of commercial buildings in the US. The first CBECS was conducted in 1979 and the most recent was completed in 1995. Over that period, the number of commercial bindings and total amount of floorspace increased, total consumption remained flat, and total energy intensity declined. By 1995, there were 4.6 million commercial buildings and 58.8 billion square feet of floorspace. The buildings consumed a total of 5.3 quadrillion Btu (site energy), with a total intensity of 90.5 thousand Btu per square foot per year. Electricity consumption exceeded natural gas consumption (2.6 quadrillion and 1.9 quadrillion Btu, respectively). In 1995, the two major users of energy were space heating (1.7 quadrillion Btu) and lighting (1.2 quadrillion Btu). Over the period 1979 to 1995, natural gas intensity declined from 71.4 thousand to 51.0 thousand Btu per square foot per year. Electricity intensity did not show a similar decline (44.2 thousand Btu per square foot in 1979 and 45.7 thousand Btu per square foot in 1995). Two types of commercial buildings, office buildings and mercantile and service buildings, were the largest consumers of energy in 1995 (2.0 quadrillion Btu, 38% of total consumption). Three building types, health care, food service, and food sales, had significantly higher energy intensities. Buildings constructed since 1970 accounted for half of total consumption and a majority (59%) of total electricity consumption.
