Press Release 29 May 2007 Carbon Joins the Magnetic Club summary written by Brad Plummer, SLAC Communication Office The exclusive club of magnetic elements officially has a new member-carbon. Using a proton beam and advanced x-ray techniques, SLAC researchers in collaboration with colleagues from LBNL and the University of Leipzig in Germany have finally put to rest doubts about carbon's ability to be made magnetic. "In the past, some groups thought they had discovered magnetic carbon," said Hendrik Ohldag, the paper's lead author and SSRL staff scientist. "Unfortunately, they realized later that they were misled by small amounts of iron, cobalt or nickel in their samples." In Leipzig, Ohldag's team applied a beam of protons to disrupt and align a portion of the electrons in samples of pure carbon, magnetizing tiny, measurable spots within the carbon. The team then used the x-ray microscope at ALS to obtain images of the magnetized portions-a measurement only possible with a state-of-the-art microscope that uses the brilliant x-ray beams generated when electrons accelerate around the ring of a synchrotron. The x-ray beam also enabled the team to verify beyond doubt that the sample remained free of impurities during the experiments, unlike the case in previous studies.

The Galactic gamma-ray club

The exclusive Galactic gamma-ray club has opened up to new members. Supernova remnants, pulsar wind nebulae, and massive binary systems hosting a compact object have recently joined the young pulsars as firmly established sources of gamma rays in the Milky Way. Massive young stellar clusters are on the waiting list to join the club. Only the fine imaging recently obtained at TeV energies could resolve specific sources. The samples are sparse, but raise exciting questions. The jet or pulsar-wind origin of the emission in binaries has been hotly debated, but it seems that both types of systems have been recently detected. The nature of the radiation in shock accelerators is still questioned: do nuclei contribute a lot, a little, or not to the gamma rays and what energy do they carry away from the shock budget? The acceleration process and the structural evolution of the pulsar winds are still uncertain. The magnetic field distribution in all these systems is a key, but poorly constrained, ingredient to model the multi-wavelength data, particle transport and electron ageing. It must, however, be determined in order to efficiently probe particle distributions and the acceleration mechanisms. The source samples soon to be expected from GLAST and the Cherenkov telescopes should bring new valuable test cases and they will, for the first time, shed statistical light on the collective behaviour of these different types of accelerators.

Abstract This article reviews the recent literature on the causal effect of health insurance on health outcomes. The focus is mainly on private health insurance in the US. The objective is to illustrate measurement issues for both health insurance and health outcomes, and to discuss the methodological challenges for researchers as they address endogeneity of insurance. Finally, a brief overview of methods and results found in the source studies is provided. Certain patterns emerge: in general, insurance is more pronounced for all-cause mortality and for generic health outcomes as compared with disease-specific outcomes. In addition, vulnerable and medically needy populations are more likely to benefit from health insurance than the general population. Finally, there is some support for the notion that continuous health insurance coverage benefits health more than intermittent insurance, suggesting that sporadic coverage offers limited value.

Using architectures for semantic interoperability to create journal clubs for emergency response

In certain types of 'slow burn' emergencies, careful accumulation and evaluation of information can offer a crucial advantage. The SARS outbreak in the first decade of the 21st century was such an event, and ad hoc journal clubs played a critical role in assisting scientific and technical responders in identifying and developing various strategies for halting what could have become a dangerous pandemic. This research-in-progress paper describes a process for leveraging emerging semantic web and digital library architectures and standards to (1) create a focused collection of bibliographic metadata, (2) extract semantic information, (3) convert it to the Resource Description Framework /Extensible Markup Language (RDF/XML), and (4) integrate it so that scientific and technical responders can share and explore critical information in the collections.

Abstract In this article the authors critically examine what it means to be in need of either health or health care. The authors start by considering different definitions of health and reasons for valuing health. In light of their answers to these questions the authors consider the notion of a baseline of health against which health need should be measured, and suggest that this baseline should be specified in terms of a sufficient rather than a maximally attainable level of health achievement. The authors then look at three different concepts of health care need and how needs would be ranked for the purpose of health care rationing on each of these three conceptions. It is concluded that distribution according to need names a general approach to health policy rather than a specific principle of distribution.

Abstract This article is concerned with the impact of international e-health on national health systems. It does so by first considering the different uses of e-health. It then analyzes how the health systems of both the countries which provide the e-health services and those who purchase them are affected by this type of trade. Finally, the article provides a description of how different modes of trade (multilateral, regional, and bilateral) affect the impact of e-health on national health systems, and concludes highlighting the need to consider all forms of trade when discussing international e-health services.

A government-funded study has concluded that reformulated gasoline containing methyl tert-butyl ether (MTBE) does not increase short-term health risks when compared with gasoline that does not contain the additive. The study, performed by the Health Effects Institute (Cambridge, MA), compared data from dozens of animal, human, and epidemiological studies of health effects linked to oxygenates, including MTBE and ethanol, but did not find enough evidence to warrant an immediate reduction in oxygenate use. However, the study did recommend that additional research be conducted on possible health consequences associated with the gasoline additives, including neurotoxic effects, if oxygenates continue to be used long term. Oxygenates have been used in gasoline since 1992, when EPA mandated that several municipalities use MTBE or other oxygenates in reformulated gasoline to reduce carbon monoxide emissions and meet Clean Air Act requirements. Shortly after the program began, residents in areas where the oxygenates were used complained of nausea, headaches, and dizziness. The institute says the study--funded by EPA and the Centers for Disease Control--will be used for a broader review of gasoline oxygenates by the White House Office of Science and Technology Policy.

The Imperative of Health: Public Health and the Regulated Body

The origins of this book lie in the author's consternation about the limitations of the formal training provided for public health specialists. Deborah Lupton's disappointment about the apparent failure of scholars and researchers in public health and health promotion to avail themselves of recent work in socio-cultural and political theory led her to produce this rich and highly readable critique of public health and health promotion practices, past and present. The book benefits from the author's excellent use of the literature from a variety of disciplines, and is packed with vivid examples from a wide range of contexts, which illustrate her basic argument that `the practices and discourses of public health are not value-free or neutral, but rather are highly political and socially contextual' (p.2). She begins with a brief historical tour of public health practices. Ports of call include: mediaeval responses to epidemics; the Enlightenment and the optimistic belief that once science had revealed the laws of nature which governed disease and epidemics, these could be controlled and removed; the Industrial Revolution and the rise of concerns that the poor health of workers living in dirty and inadequate conditions would limit their productivity; the introduction of vaccination; and the focus on individual and family responsibilities for health in the twentieth century. The chapter draws heavily on social constructionist history and Michel Foucault, and explores the ways in which certain phenomena such as dirt, odour, microbes and ignorance come to be identified as public health problems. She suggests that many of the ideas and practices of public health are religious or moralistic in origin, pointing out that the belief that disease was God's punishment for the sins of humankind `allowed the expression of overtly moralistic statements concerning the relative sinfulness of individuals and certain social groups such as the poor; a tendency that has pervaded public health discourses for centuries' (p.21). This theme recurs throughout the book. She moves on to examine the dominant discourses of contemporary public health. Here, Lupton argues that many of the approaches used in the so-called `new' public health in fact date back centuries. She points out the limitations of what currently passes for theory in health promotion and provides strong critiques of the arguments that health promotion achieves `empowerment' and that, with the help of the disciplines of epidemiology and health economics, it is scientific and rational. Discussing the prevailing discourse of `healthism', in which the pursuit of good health is seen as an end in itself which individuals have a responsibility to pursue, Lupton argues that moral judgements are now made against those who place themselves at risk of disease or develop a medical condition related to lifestyle factors, and that although the state intervenes less obviously than previously, the imperatives of health now mean that many people are subject to control which is mediated by self-surveillance. She then takes up risk discourses and diagnostic testing, concentrating particularly on `internally imposed' risks of developing diseases which are associated with individual behaviour. The discussion highlights discrepancies between official and personal, `expert' and lay viewpoints, for example, differences between scientists' and lay peoples' interpretations of risk. And she shows how there can be dramatic differences between the motivations and experiences of people undergoing diagnostic tests and the official rationales for and interpretations of such tests. Lupton is critical of the way that many public health discourses about risk and diagnostic testing are inappropriately simplified, ignoring as they do political, social and personal psychological perspectives. Abstract The key drivers of population health lie outside the health sector. However, decision makers outside the health sector are primarily interested in delivering sector specific outputs other than health. Economic approaches to priority setting can help align sectors to consider the intersectoral impacts of decisions within an integrated societal framework. First, economic evaluation can explicitly identify and measure all impacts including health and value them as inputs into overall social welfare. Second, priority setting tools such as program budgeting marginal analysis and multi-criteria decision analysis can then be used to help translate evidence into policy, where the full consequences of decisions are considered and the overall aim is to improve social welfare.

A majority of recent intervention studies employed strong designs and more of these studies report statistically significant improvements in health symptoms or objective health outcomes, particularly for subjects with allergies or asthma. The percentage improvement in health outcomes is typically modest, e.g., 7% to 25%. Delivery of filtered air to the breathing zone of sleeping allergic or asthmatic persons may be more consistently effective in improving health than room air filtration. Notable are two studies that report statistically significant improvements, with filtration, in markers that predict future adverse coronary events.
