    The Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) is a software application developed for performing a complete probabilistic risk assessment (PRA) using a personal computer. SAPHIRE is funded by the U.S. Nuclear Regulatory Commission (NRC) and developed by the Idaho National Laboratory (INL). The INL's primary role in this project is that of software developer. However, the INL also plays an important role in technology transfer by interfacing and supporting SAPHIRE users comprised of a wide range of PRA practitioners from the NRC, national laboratories, the private sector, and foreign countries. SAPHIRE can be used to model a complex system’s response to initiating events, quantify associated damage outcome frequencies, and identify important contributors to this damage (Level 1 PRA) and to analyze containment performance during a severe accident and quantify radioactive releases (Level 2 PRA). It can be used for a PRA evaluating a variety of operating conditions, for example, for a nuclear reactor at full power, low power, or at shutdown conditions. Furthermore, SAPHIRE can be used to analyze both internal and external initiating events and has special features for transforming models built for internal event analysis to models for external event analysis. It can also be used in a limited manner to quantify risk in terms of release consequences to both the public and the environment (Level 3 PRA). This reference guide will introduce the SAPHIRE Version 8.0 software. A brief discussion of the purpose and history of the software is included along with general information such as installation instructions, starting and stopping the program, and some pointers on how to get around inside the program. Next, database concepts and structure are discussed. Following that discussion are nine sections, one for each of the menu options on the SAPHIRE main menu, wherein the purpose and general capabilities for each option are furnished

Volume I briefly describes the Epidemiologic Records Project and provides information on the methodology used to inventory and describe the records series contained in subsequent volumes.     MELCOR is a fully integrated, engineering-level computer code that models the progression of severe accidents in light water reactor nuclear power plants. MELCOR is being developed at Sandia National Laboratories for the US Nuclear Regulatory Commission as a second-generation plant risk assessment tool and the successor to the Source Term Code Package. A broad spectrum of severe accident phenomena in both boiling and pressurized water reactors is treated in MELCOR in a unified framework. These include: thermal-hydraulic response in the reactor coolant system, reactor cavity, containment, and confinement buildings; core heatup, degradation, and relocation; core-concrete attack; hydrogen production, transport, and combustion; fission product release and transport; and the impact of engineered safety features on thermal-hydraulic and radionuclide behavior. Current uses of MELCOR include estimation of severe accident source terms and their sensitivities and uncertainties in a variety of applications. This publication of the MELCOR computer code manuals corresponds to MELCOR 1.8.3, released to users in August, 1994. Volume 1 contains a primer that describes MELCOR`s phenomenological scope, organization (by package), and documentation. The remainder of Volume 1 contains the MELCOR Users` Guides, which provide the input instructions and guidelines for each package. The report also provides decision trees and tables of useful information for homeowners who are making decisions about adding, replacing, or upgrading existing HVAC equipment in their homes. Information regarding home energy performance assessments (audits) and combustion safety issues when replacing HVAC equipment are also provided.

    The DOE has conducted economic analyses of alternative energy projects over the past several years which compared emerging technologies to conventional technologies and also to competing emerging technologies. The method used was to determine and compare the product costs of the technologies being assessed. A review of the application of this product cost technique over the past few years revealed that slightly different financial assumptions, economic factors, and scope were used. Therefore, it is not meaningful to compare results. Consistent comparison of the economic benefits of programs and projects being evaluated by DOE is essential to accomplish the assigned missions and functions. The process of reviewing much of the work accomplished by DOE and its support contractors failed to disclose a single source document or methodology which had potential application across the full range of DOE interests. However, the 1981 version of the Technical Assessment Guide published by the Electric Power Research Institute (EPRI) appears to have all of the necessary elements, economic factors, assumptions, and cost performance information necessary to meet the DOE's needs. A review of the document indicates that the methodology described by EPRI can be applied to almost any energy project so long as appropriate business and financial assumptions are made.The financial and business assumptions presented in EPRI technical report P-2410-SR are typical of the electric utility industry. Moreover, using a methodology already widely accepted by the utility industry, DOE can help reduce proliferation of different methods and promote the adoption of a standard. This guide presents a summary of the methodology, data on fuel price projections, and cost and performance information for transmission and generation technologies, and reflects an explanation of the economic methodology to include sample problems.

    The RELAP5 code has been developed for best estimate transient simulation of light water reactor coolant systems during postulated accidents. The code models the coupled behavior of the reactor coolant system and the core for loss-of-coolant accidents, and operational transients, such as anticipated transient without scram, loss of offsite power, loss of feedwater, and loss of flow. A generic modeling approach is used that permits simulating a variety of thermal hydraulic systems. Control system and secondary system components are included to permit modeling of plant controls, turbines, condensers, and secondary feedwater systems. Volume II contains detailed instructions for code application and input data preparation.

    This report describes the use of the MACCS2 code. The document is primarily a user`s guide, though some model description information is included. MACCS2 represents a major enhancement of its predecessor MACCS, the MELCOR Accident Consequence Code System. MACCS, distributed by government code centers since 1990, was developed to evaluate the impacts of severe accidents at nuclear power plants on the surrounding public. The principal phenomena considered are atmospheric transport and deposition under time-variant meteorology, short- and long-term mitigative actions and exposure pathways, deterministic and stochastic health effects, and economic costs. No other U.S. code that is publicly available at present offers all these capabilities. MACCS2 was developed as a general-purpose tool applicable to diverse reactor and nonreactor facilities licensed by the Nuclear Regulatory Commission or operated by the Department of Energy or the Department of Defense. The MACCS2 package includes three primary enhancements: (1) a more flexible emergency-response model, (2) an expanded library of radionuclides, and (3) a semidynamic food-chain model. Other improvements are in the areas of phenomenological modeling and new output options. Initial installation of the code, written in FORTRAN 77, requires a 486 or higher IBM-compatible PC with 8 MB of RAM.

    The Hanford Environmental Information System (HEIS) is a consolidated set of automated resources that effectively manage the data gathered during environmental monitoring and restoration of the Hanford Site. HEIS includes an integrated database that provides consistent and current data to all users and promotes sharing of data by the entire user community. HEIS is an information system with an inclusive database. Although the database is the nucleus of the system, HEIS also provides user access software: query-by-form data entry, extraction, and browsing facilities; menu-driven reporting facilities; an ad hoc query facility; and a geographic information system (GIS). These features, with the exception of the GIS, are described in this manual set. Because HEIS contains data from the entire Hanford Site, many varieties of data are included and have.been divided into subject areas. Related subject areas comprise several volumes of the manual set.     Purpose: To identify an optimal margin about the gross target volume (GTV) for stereotactic radiosurgery (SRS) of brain metastases, minimizing toxicity and local recurrence. Methods and Materials: Adult patients with 1 to 3 brain metastases less than 4 cm in greatest dimension, no previous brain radiation therapy, and Karnofsky performance status (KPS) above 70 were eligible for this institutional review board–approved trial. Individual lesions were randomized to 1- or 3- mm uniform expansion of the GTV defined on contrast-enhanced magnetic resonance imaging (MRI). The resulting planning target volume (PTV) was treated to 24, 18, or 15 Gy marginal dose for maximum PTV diameters less than 2, 2 to 2.9, and 3 to 3.9 cm, respectively, using a linear accelerator–based image-guided system. The primary endpoint was local recurrence (LR). Secondary endpoints included neurocognition Mini-Mental State Examination, Trail Making Test Parts A and B, quality of life (Functional Assessment of Cancer Therapy-Brain), radionecrosis (RN), need for salvage radiation therapy, distant failure (DF) in the brain, and overall survival (OS). Results: Between February 2010 and November 2012, 49 patients with 80 brain metastases were treated. The median age was 61 years, the median KPS was 90, and the predominant histologies were non–small cell lung cancer (25 patients) and melanoma (8). Fifty-five, 19, and 6 lesions were treated to 24, 18, and 15 Gy, respectively. The PTV/GTV ratio, volume receiving 12 Gy or more, and minimum dose to PTV were significantly higher in the 3-mm group (all P<.01), and GTV was similar (P=.76). At a median follow-up time of 32.2 months, 11 patients were alive, with median OS 10.6 months. LR was observed in only 3 lesions (2 in the 1 mm group, P=.51), with 6.7% LR 12 months after SRS. Biopsy-proven RN alone was observed in 6 lesions (5 in the 3-mm group, P=.10). The 12-month DF rate was 45.7%. Three months after SRS, no significant change in

    Purpose: Magnetic resonance imaging/diffusion weighted-imaging (MRI/DWI)-guided high-dose-rate (HDR) brachytherapy and {sup 18}F-fluorodeoxyglucose (FDG) — positron emission tomography/computed tomography (PET/CT)-guided intensity modulated radiation therapy (IMRT) for the definitive treatment of cervical cancer is a novel treatment technique. The purpose of this study was to report our analysis of dose-volume parameters predicting gross tumor volume (GTV) control. Methods and Materials: We analyzed the records of 134 patients with International Federation of Gynecology and Obstetrics stages IB1-IVB cervical cancer treated with combined MRI-guided HDR and IMRT from July 2009 to July 2011. IMRT was targeted to the metabolic tumor volume and lymph nodes by use of FDG-PET/CT simulation. The GTV for each HDR fraction was delineated by use of T2-weighted or apparent diffusion coefficient maps from diffusion-weighted sequences. The D100, D90, and Dmean delivered to the GTV from HDR and IMRT were summed to EQD2. Results: One hundred twenty-five patients received all irradiation treatment as planned, and 9 did not complete treatment. All 134 patients are included in this analysis. Treatment failure in the cervix occurred in 24 patients (18.0%). Patients with cervix failures had a lower D100, D90, and Dmean than those who did not experience failure in the cervix. The respective doses to the GTV were 41, 58, and 136 Gy for failures compared with 67, 99, and 236 Gy for those who did not experience failure (P<.001). Probit analysis estimated the minimum D100, D90, and Dmean doses required for ≥90% local control to be 69, 98, and 260 Gy (P<.001). Conclusions: Total dose delivered to the GTV from combined MRI-guided HDR and PET/CT-guided IMRT is highly correlated with local tumor control. The findings can be directly applied in the clinic for dose adaptation to maximize local control.

    Purpose: The purpose of this study was to analyze the relationship between prostate, bladder, and rectum volumes on treatment planning CT day and prostate shifts in the XYZ directions on treatment days. Methods: Prostate, seminal vesicles, bladder, and rectum were contoured on CT images obtained in supine position. Intensity modulated radiation therapy plans was prepared. Contours were exported to BAT-ultrasound imaging system. Patients were positioned on the couch using skin marks. An ultrasound probe was used to obtain ultrasound images of prostate, bladder, and rectum, which were aligned with CT images. Couch shifts in the XYZ directions as recommended by BAT system were made and recorded. 4698 couch shifts for 42 patients were analyzed to study the correlations between interfraction prostate shifts vs bladder, rectum, and prostate volumes on planning CT. Results: Mean and range of volumes (cc): Bladder: 179 (42-582), rectum: 108 (28-223), and prostate: 55 (21-154). Mean systematic prostate shifts were (cm, {+-}SD) right and left lateral: -0.047{+-}0.16 (-0.361-0.251), anterior and posterior: 0.14{+-}0.3 (-0.466-0.669), and superior and inferior: 0.19{+-}0.26 (-0.342-0.633). Bladder volume was not correlated with lateral, anterior/posterior, and superior/inferior prostate shifts (P>0.2). Rectal volume was correlated with anterior/posterior (P<0.001) but not with lateral and superior/inferior prostate shifts (P>0.2). The smaller the rectal volume or cross sectional area, the larger was the prostate shift anteriorly and vice versa (P<0.001). Prostate volume was correlated with superior/inferior (P<0.05) but not with lateral and anterior/posterior prostate shifts (P>0.2). The smaller the prostate volume, the larger was prostate shift superiorly and vice versa (P<0.05).     The Simulation Package for the Analysis of Health Risk (SPAHR) is a computer software package based upon a demographic model for health risk projections. The model extends several health risk projection models by making realistic assumptions about the population at risk, and thus represents a distinct improvement over previous models. Complete documentation for use of SPAHR is contained in this five-volume publication. The demographic model in SPAHR estimates population response to environmental toxic exposures. Latency of responses, changing dose level over time, competing risks from other causes of death, and population structure can be incorporated into SPAHR to project health risks. Risks are measured by morbid years, number of deaths, and loss of life expectancy. Comparisons of estimates of excess deaths demonstrate that previous health risk projection models may have underestimated excess deaths by a factor of from 2 to 10, depending on the pollutant and the exposure scenario. The software supporting the use of the demographic model is designed to be user oriented. Complex risk projects are made by responding to a series of prompts generated by the package. The flexibility and ease of use of SPAHR make it an important contribution to existing models and software packages. This volume gives the user of the SPAHR program the information required to operate the program when it is up and running on the computer. It assumes that the user is familiar with the concepts and terms relating to demography and health risk assessment. It contains a brief description of all commands and options available in SPAHR, as well as a user-oriented description of the structure and operation of the control system and language processor.

    Purpose: To investigate the feasibility of improved normal tissue sparing for head-and-neck (H'N) image-guided radiotherapy (IGRT) by employing tighter CTV-to-PTV margins for target level II/III though a GPU-based deformable image registration and dose accumulation framework. Methods: Ten H'N simultaneous integrated boost cases treated on TomoTherapy were retrospectively analyzed. Weekly kVCT scans in addition to daily MVCT scans were acquired for each patient. Reduced margin plans were generated with 0- mm margin for level II and III PTV (while 3-5 mm margin for PTV1) and compared with the standard margin plan using 3-5mm margin to all CTV1-3 (reference plan). An in-house developed GPU-based 3D image deformation tool was used to register and deform the weekly KVCTs with the planning CT and determine the delivered mean/minimum/maximum dose, dose volume histograms (DVHs), etc. Results: Compared with the reference plans, the averaged cord maximum, the right and left parotid doses reduced by 22.7 %, 16.5 %, and 9 % respectively in the reduced margin plans. The V95 for PTV2 and PTV3 were found within 2 and 5% between the reference and tighter margin plans. For the reduced margin plans, the averaged cumulative mean doses were consistent with the planned dose for PTV1, PTV2 and PTV3 within 1.5%, 1.7% and 1.4%. Similar dose variations of the delivered dose were seen for the reference and tighter margin plans. The delivered maximum and mean doses for the cord were 3.55 % and 2.37% higher than the planned doses; a 5 % higher cumulative mean dose for the parotids was also observed for the delivered dose than the planned doses in both plans. Conclusion: By imposing tighter CTV-to-PTV margins for level II and III targets for H'N irradiation, acceptable cumulative doses were achievable when coupled with weekly kVCT guidance while improving normal structure sparing.

With the measures described in this guide, builders in the marine climate (portions of Washington, Oregon, and California) can achieve homes that have whole house energy savings of 40% over the Building America benchmark (a home built to mid-1990s building practices roughly equivalent to the 1993 Model Energy Code) with no added overall costs for consumers. These best practices are based on the results of research and demonstration projects conducted by Building America’s research teams. The guide includes information for managers, designers, marketers, site supervisors, and subcontractors, as well as case studies of builders who are successfully building homes that cut energy use by 40% in the marine climate. This document is available on the web at www.buildingamerica.gov. This report was originally cleared 06-29-2010. This version is Rev 1 cleared in Nov 2010. The only change is the reference to the Energy Star Windows critieria shown on pg 8.25 was updated to match the criteria - Version 5.0, 04/07/2009, effective 01/04/2010.

This guide book is a resource to help builders design and construct homes that are among the most energy-efficient available, while addressing issues such as building durability, indoor air quality, and occupant health, safety, and comfort. With the measures described in this guide, builders in the cold and very cold climates can build homes that have whole-house energy savings of 40% over the Building America benchmark with no added overall costs for consumers. The best practices described in this document are based on the results of research and demonstration projects conducted by Building America’s research teams. Building America brings together the nation’s leading building scientists with over 300 production builders to develop, test, and apply innovative, energy-efficient construction practices. Building America builders have found they can build homes that meet these aggressive energy-efficiency goals at no net increased costs to the homeowners. Currently, Building America homes achieve energy savings of 40% greater than the Building America benchmark home (a home built to mid-1990s building practices roughly equivalent to the 1993 Model Energy Code). The recommendations in this document meet or exceed the requirements of the 2009 IECC and 2009 IRC and thos erequirements are highlighted in the text. This document will be distributed via the DOE Building America website: www.buildingamerica.gov.

  1. Oak Ridge Reservation volume I. Y-12 mercury task force files: A guide to record series of the Department of Energy and its contractors

    The purpose of this guide is to describe each of the series of records identified in the documents of the Y-12 Mercury Task Force Files that pertain to the use of mercury in the separation and enrichment of lithium isotopes at the Department of Energy`s (DOE) Y-12 Plant in Oak Ridge, Tennessee. History Associates Incorporated (HAI) prepared this guide as part of DOE`s Epidemiologic Records Inventory Project, which seeks to verify and conduct inventories of epidemiologic and health-related records at various DOE and DOE contractor sites. This introduction briefly describes the Epidemiologic Records Inventory Project and HAI`s role in the project. Specific attention will be given to the history of the DOE-Oak Ridge Reservation, the development of the Y-12 Plant, and the use of mercury in the production of nuclear weapons during the 1950s and early 1960s. This introduction provides background information on the Y-12 Mercury Task Force Files, an assembly of documents resulting from the 1983 investigation of the Mercury Task Force into the effects of mercury toxicity upon workplace hygiene and worker health, the unaccountable loss of mercury, and the impact of those losses upon the environment. This introduction also explains the methodology used in the selection and inventory of these record series. Other topics include the methodology used to produce this guide, the arrangement of the detailed record series descriptions, and information concerning access to the collection.

    Environmental and occupational health effects associated with exposures to various chemicals are a subject of increasing concern. One recently developed methodology for assessing the health impacts of various chemical compounds involves the classification of similar chemicals into risk-assessment categories (RACs). This report reviews documented human health effects for a broad range of pollutants, classified by RACs. It complements other studies that have estimated human health effects by RAC based on analysis and extrapolation of data from animal research.

    The purpose of this Corrective Action Unit (CAU) Work Plan is to provide a strategy to be used by the US Department of Energy Nevada Operations Office (DOE/NV), the US Department of Defense (DoD) Defense Special Weapons Agency (DSWA) (formerly the Defense Nuclear Agency), and contractor personnel for conducting corrective actions at the Nevada Test Site (NTS) and Nevada off-site locations including the Tonopah Test Range (TTR), the Project Shoal Area, and the Central Nevada Test Area. This Work Plan applies to housekeeping category CAUs already listed in the Federal Facility Agreement and Consent Order (FFACO) Appendices (FFACO, 1996) as well as newly identified Corrective Action Sites (CASs) that will follow the housekeeping process.
