    Experience makes a superior teacher. Sharing the details surrounding safety events is one of the best ways to help prevent their recurrence elsewhere. This approach requires an open, non-punitive environment to achieve broad benefits. The Hydrogen Incident Reporting Tool (www.h2incidents.org) is intended to facilitate the sharing of lessons learned and other relevant information gained from actual experiences using and working with hydrogen and hydrogen systems. Its intended audience includes those involved in virtually any aspect of hydrogen technology, systems and use, with an emphasis towards energy and transportation applications. The database contains records of safety events both publicly available and/or voluntarily submitted. Typical records contain a general description of the occurrence, contributing factors, equipment involved, and some detailing of consequences and changes that have been subsequently implemented to prevent recurrence of similar events in the future. The voluntary and confidential nature and other characteristics surrounding the database mean that any analysis of apparent trends in its contents cannot be considered statistically valid for a universal population. A large portion of reported incidents have occurred in a laboratory setting due to the typical background of the reporting projects, for example. Yet some interesting trends are becoming apparent even at this early stage of the database’s existence and general lessons can already be taken away from these experiences. This paper discusses the database and a few trends that have already become apparent for the reported incidents. Anticipated future uses of this information are also described. This paper is intended to encourage wider participation and usage of the incidents reporting database and to promote the safety benefits offered by its contents.

    This report describes the Data Archive and Portal (DAP), a key capability of the U.S. Department of Energy's Atmosphere to Electron (A2e) initiative. The DAP Thrust Area Planning Group was organized to develop a plan for deploying this capability. Primarily, the report focuses on a distributed system--a DOE Wind Cloud--that functions as a repository for all A2e data. The Wind Cloud will be accessible via an open, easy-to-navigate user interface that facilitates community data access, interaction, and collaboration. DAP management will work with the community, industry, and international standards bodies to develop standards for wind data and to capture important characteristics of all data in the Wind Cloud.

    Purpose: Describe a Web-based Radiation Oncology Incident Reporting and Learning system that has the potential to improve quality of care for radiation therapy patients. This system is an important facet of continuing effort by our community to maintain and improve safety of radiotherapy.Material and Methods: The VA National Radiation Oncology Program office has embarked on a program to electronically collect adverse events and near miss data of radiation treatment of over 25,000 veterans treated with radiotherapy annually. Software used for this program is deployed on the VAs intranet as a Website. All data entry forms (adverse event or near miss reports, work product reports) utilize standard causal, RT process step taxonomies and data dictionaries defined in AAPM and ASTRO reports on error reporting (AAPM Work Group Report on Prevention of Errors and ASTROs safety is no accident report). All reported incidents are investigated by the radiation oncology domain experts. This system encompasses the entire feedback loop of reporting an incident, analyzing it for salient details, and developing interventions to prevent it from happening again. The operational workflow is similar to that of the Aviation Safety Reporting System. This system is also synergistic with ROSIS and SAFRON. Results: The ROIRLS facilitates the collection of data that help in tracking adverse events and near misses and develop new interventions to prevent such incidents. The ROIRLS electronic infrastructure is fully integrated with each registered facility profile data thus minimizing key strokes and multiple entries by the event reporters. Conclusions: OIRLS is expected to improve the quality and safety of a broad spectrum of radiation therapy patients treated in the VA and fulfills our goal of Effecting Quality While Treating Safely The Radiation Oncology Incident Reporting and Learning System software used for this program has been developed, conceptualized and maintained by TSG Innovations

    This report describes the results of Phase 2 efforts to develop a Rapid Surface Sampling and Archival Record (RSSAR) System for the detection of semivolatile organic contaminants on concrete, transite, and metal surfaces. The characterization of equipment and building surfaces for the presence of contaminants as part of building decontamination and decommissioning activities is an immensely large task of concern to both government and industry. Because of the high cost of hazardous waste disposal, old, contaminated buildings cannot simply be demolished and scrapped. Contaminated and clean materials must be clearly identified and segregated so that the clean material can be recycled or reused, if possible, or disposed of more cheaply as nonhazardous waste. DOE has a number of sites requiring surface characterization. These sites are large, contain very heterogeneous patterns of contamination (requiring high sampling density), and will thus necessitate an enormous number of samples to be taken and analyzed. Characterization of building and equipment surfaces will be needed during initial investigations, during cleanup operations, and during the final confirmation process, increasing the total number of samples well beyond that needed for initial characterization. This multiplicity of information places a premium on the ability to handle and track data as efficiently as possible.

    During the Hot Commissioning tests in preparation for Test Series 2, a significant degree of metal wastage occurred on the external surfaces of Tube Bank 'C'. The extent of the metal wastage was unexpected and has been a matter of concern to the Project. The report provides a history of the operational period for Tube Bank 'C' and details the measurements of metal wastage that have been recorded. The project has initiated an extensive study of the possible cause, or causes, of the metal wastage and these are discussed. Operational tests are to be carried out at Grimethorpe, CURL and CRE with a view to determining the mechanism of metal wastage that exists within the fluidized bed. The results of these tests will be reported and will form the basis for the design of a new bank, which will be installed in the combustor in due course, if required. The Project is taking necessary steps to ensure that the cause of the metal wastage is fully investigated and that the resultant slippage in the Test Series 2 schedule is minimised.

    The importance and need for full compliant implementation of NIMS nationwide was clearly demonstrated during the Hurricane Katrina event, which was clearly expressed in Secretary Chertoff's October 4, 2005 letter addressed to the State's governors. It states, ''Hurricane Katrina was a stark reminder of how critical it is for our nation to approach incident management in a coordinated, consistent, and efficient manner. We must be able to come together, at all levels of government, to prevent, prepare for, respond to, and recover from any emergency or disaster. Our operations must be seamless and based on common incident management doctrine, because the challenges we face as a nation are far greater than capabilities of any one jurisdiction.'' The NIMS is a system/architecture for organizing response on a ''national'' level. It incorporations ICS as a main component of that structure (i.e., it institutionalizes ICS in NIMS). In a paper published on the NIMS Website, the following statements were made: ''NIMS represents a core set of doctrine, principles, terminology, and organizational processes to enable effective, efficient and collaborative incident management at all levels. To provide the framework for interoperability and compatibility, the NIMS is based on a balance between flexibility and standardization.'' Thus the NIC is challenged with the need to adopt quality SDO generated standards to support NIMS compliance, but in doing so maintain the flexibility necessary so that response operations can be tailored for the specific jurisdictional and geographical needs across the nation. In support of this large and complex challenge facing the NIC, the Pacific Northwest National Laboratory (PNNL) was asked to provide technical support to the NIC, through their DHS Science and Technology ? Standards Portfolio Contract, to help identify, review, and develop key standards for NIMS compliance. Upon examining the challenge, the following general process appears to be a

    The importance and necessity for a fully developed and implemented National Incident Management System (NIMS) has been demonstrated in recent years by the impact of national events such as Hurricane Katrina in 2005. Throughout the history of emergency response to major disasters, especially when multiple response organizations are involved, there have been systemic problems in the consistency and uniformity of response operations. Identifying national standards that support the development and implementation of NIMS is key to helping solve these systemic problems. The NIMS seeks to provide uniformity and consistency for incident management by using common terminology and protocols that will enable responders to coordinate their efforts to ensure an efficient response.

    We have established archives for programs, data, papers etc. in physics (mainly for plasma physics). The archives are located at computer ftp.muni.cz in the directory pub/muni.cz/physics. These archives can be reached by anonymous FTP or by gopher server gopher.muni.cz (147.251.4.33). At the present time, programs for PC, cross sections for electrons, swarm parameters and rate constants stored are in the archives. We would like to collect the programs for calculations in physics (mainly for PC). We suppose that each program should have a testing example and some description. We would also like to collect physical constants and experimental or theoretical data (e.g. cross sections, swarm parameters and rate constants), which are important for other calculation or for comparison with the results of others studies. Interested scholars are invited to sent us their programs, data, preprints and reports for these archives. All files in the archives are in public domain and can be obtained using computer network Internet.

    This report presents preliminary operational guidelines and supporting work products developed through the interagency Operational Guidelines Task Group (OGT). The report consolidates preliminary operational guidelines, all ancillary work products, and a companion software tool that facilitates their implementation into one reference source document. The report is intended for interim use and comment and provides the foundation for fostering future reviews of the operational guidelines and their implementation within emergency preparedness and response initiatives in the event of a radiological dispersal device (RDD) incident. The report principally focuses on the technical derivation and presentation of the operational guidelines. End-user guidance providing more details on how to apply these operational guidelines within planning and response settings is being considered and developed elsewhere. The preliminary operational guidelines are categorized into seven groups on the basis of their intended application within early, intermediate, and long-term recovery phases of emergency response. We anticipate that these operational guidelines will be updated and refined by interested government agencies in response to comments and lessons learned from their review, consideration, and trial application. This review, comment, and trial application process will facilitate the selection of a final set of operational guidelines that may be more or less inclusive of the preliminary operational guidelines presented in this report. These and updated versions of the operational guidelines will be made available through the OGT public Web site (http://ogcms.energy.gov) as they become finalized for public distribution and comment.

    Luminous phenomena and anomalous physical forces have been hypothesized to be generated by focal tectonic strain fields that precede earthquakes. If these geophysical processes exist, then their spatial and temporal density should be greatest during periods of protracted, localized UFO reports; they might be used as dosimetric indicators. Contemporary epidemiological data concerning the health risks of power frequency electromagnetic fields and radon gas levels (expected correlates of certain tectonic strain fields), suggest that increased incidence (odds ratios greater 1:3) of brain tumors and leukemia should be evident within flap areas. In addition the frequency of variants of temporal lobe lability, psychological depression and posttraumatic stress should be significantly elevated. UFO field investigators, because they have repeated, intermittent close proximity to these fields, are considered to be a particularly high risk population for these disorders. 22 references.

    The Radiation Embrittlement Archive Project (REAP), which is being conducted by the Probabilistic Integrity Safety Assessment (PISA) Program at Oak Ridge National Laboratory under funding from the U.S. Nuclear Regulatory Commission s (NRC) Office of Nuclear Regulatory Research, aims to provide an archival source of information about the effect of neutron radiation on the properties of reactor pressure vessel (RPV) steels. Specifically, this project is an effort to create an Internet-accessible RPV steel embrittlement database. The project s website, https://reap.ornl.gov, provides information in two forms: (1) a document archive with surveillance capsule(s) reports and related technical reports, in PDF format, for the 104 commercial nuclear power plants (NPPs) in the United States, with similar reports from other countries; and (2) a relational database archive with detailed information extracted from the reports. The REAP project focuses on data collected from surveillance capsule programs for light-water moderated, nuclear power reactor vessels operated in the United States, including data on Charpy V-notch energy testing results, tensile properties, composition, exposure temperatures, neutron flux (rate of irradiation damage), and fluence, (Fast Neutron Fluence a cumulative measure of irradiation for E>1 MeV). Additionally, REAP contains data from surveillance programs conducted in other countries. REAP is presently being extended to focus on embrittlement data analysis, as well. This paper summarizes the current status of the REAP database and highlights opportunities to access the data and to participate in the project.

    In August 2008, the U.S. Department of Homeland Security (DHS) issued its final Protective Action Guide (PAG) for radiological dispersal device (RDD) and improvised nuclear device (IND) incidents. This document specifies protective actions for public health during the early and intermediate phases and cleanup guidance for the late phase of RDD or IND incidents, and it discusses approaches to implementing the necessary actions. However, while the PAG provides specific guidance for the early and intermediate phases, it prescribes no equivalent guidance for the late-phase cleanup actions. Instead, the PAG offers a general description of a complex process using a site-specific optimization approach. This approach does not predetermine cleanup levels but approaches the problem from the factors that would bear on the final agreed-on cleanup levels. Based on this approach, the decision-making process involves multifaceted considerations including public health, the environment, and the economy, as well as socio-political factors. In an effort to fully define the process and approach to be used in optimizing late-phase recovery and site restoration following an RDD or IND incident, DHS has tasked the NCRP with preparing a comprehensive report addressing all aspects of the optimization process. Preparation of the NCRP report is a three-year (2010-2013) project assigned to a scientific committee, the Scientific Committee (SC) 5-1; the report was initially titled, Approach to Optimizing Decision Making for Late- Phase Recovery from Nuclear or Radiological Terrorism Incidents. Members of SC 5-1 represent a broad range of expertise, including homeland security, health physics, risk and decision analysis, economics, environmental remediation and radioactive waste management, and communication.     The Analysis Function of the US-CERT Control Systems Security Center (CSSC) at the Idaho National Laboratory (INL) has prepared this report to document cyber security incidents for use by the CSSC. The description and analysis of incidents reported herein support three CSSC tasks: establishing a business case; increasing security awareness and private and corporate participation related to enhanced cyber security of control systems; and providing informational material to support model development and prioritize activities for CSSC. The stated mission of CSSC is to reduce vulnerability of critical infrastructure to cyber attack on control systems. As stated in the Incident Management Tool Requirements (August 2005) ''Vulnerability reduction is promoted by risk analysis that tracks actual risk, emphasizes high risk, determines risk reduction as a function of countermeasures, tracks increase of risk due to external influence, and measures success of the vulnerability reduction program''. Process control and Supervisory Control and Data Acquisition (SCADA) systems, with their reliance on proprietary networks and hardware, have long been considered immune to the network attacks that have wreaked so much havoc on corporate information systems. New research indicates this confidence is misplaced--the move to open standards such as Ethernet, Transmission Control Protocol/Internet Protocol, and Web technologies is allowing hackers to take advantage of the control industry's unawareness. Much of the available information about cyber incidents represents a characterization as opposed to an analysis of events. The lack of good analyses reflects an overall weakness in reporting requirements as well as the fact that to date there have been very few serious cyber attacks on control systems. Most companies prefer not to share cyber attack incident data because of potential financial repercussions. Uniform reporting requirements will do much to make this information available to

    SLAC Archives and History Office (AHO) backlog of unprocessed material is over 3,000 cubic feet and growing. Because much of this material was directly transferred to off-site storage, the provenance and contents are a mystery. The costs of off-site storage, lack of knowledge of the contents, and the impending federal regulations in 2009 concerning storage facilities all suggest the need for a long-term plan for the backlog. AHO presents these options to SLAC management: (1) Continue with the status quo, adding new accessions to OffSite Records Management, LLC (hereinafter referred to as OffSite) storage; (2) Pull the backlog back a segment at a time for box-level processing, determine what is in each box, get rid of extraneous material, and return what is left to OffSite storage; (3) Gradually retrieve the backlog for thorough, folder-level processing and then transfer to the Federal Records Center in San Bruno (hereinafter referred to as FRC) or the Archives side of the operations at the National Archives and Records Administration in San Bruno (hereinafter referred to as NARA); and (4) Gradually retrieve the backlog for a combination of box-level processing with return to OffSite storage and thorough folder-level processing with transfer to FRC or NARA.

    Predation by pinnipeds, such as California sea lions Zalophus californianus, Pacific harbor seals Phoca vitulina, and Stellar sea lions Eumetopias jubatus on adult Pacific salmon Oncorhynchus spp in the lower Columbia River has become a serious concern for fishery managers trying to conserve and restore runs of threatened and endangered fish. As a result, Smith-Root, Incorporated (SRI; Vancouver, Washington), manufacturers of electrofishing and closely-related equipment, proposed a project to evaluate the potential of an electrical barrier to deter marine mammals and reduce the amount of predation on adult salmonids (SRI 2007). The objectives of their work were to develop, deploy, and evaluate a passive, integrated sonar and electric barrier that would selectively inhibit the upstream movements of marine mammals and reduce predation, but would not injure pinnipeds or impact anadromous fish migrations. However, before such a device could be deployed in the field, concerns by regional fishery managers about the potential effects of such a device on the migratory behavior of Pacific salmon, steelhead O. mykiss, Pacific lampreys Entoshpenus tridentata, and white sturgeon Acipenser transmontanus, needed to be addressed. In this report, we describe the results of laboratory research designed to evaluate the effects of prototype electric barriers on adult steelhead and Pacific lampreys. The effects of electricity on fish have been widely studied and include injury or death (e.g., Sharber and Carothers 1988; Dwyer et al. 2001; Snyder 2003), physiological dysfunction (e.g., Schreck et al. 1976; Mesa and Schreck 1989), and altered behavior (Mesa and Schreck 1989). Much of this work was done to investigate the effects of electrofishing on fish in the wild. Because electrofishing operations would always use more severe electrical settings than those proposed for the pinniped barrier, results from these studies are probably not relevant to the work proposed by SRI.     The Sandia Data Archive (SDA) format is a specific implementation of the HDF5 (Hierarchal Data Format version 5) standard. The format was developed for storing data in a universally accessible manner. SDA files may contain one or more data records, each associated with a distinct text label. Primitive records provide basic data storage, while compound records support more elaborate grouping. External records allow text/binary files to be carried inside an archive and later recovered. This report documents version 1.0 of the SDA standard. The information provided here is sufficient for reading from and writing to an archive. Although the format was original designed for use in MATLAB, broader use is encouraged.

    The Twitter Archiver system allows a user to enter their Twitter developer account credentials (obtained separately from the Twitter developer website) and read from the freely available Twitter sample stream. The Twitter sample stream provides a random sample of the overall volume of tweets that are contributed by users to the system. The Twitter Archiver system consumes the stream and serializes the information to text files at some predefined interval. A separate utility reads the text files and creates a searchable index using the open source Apache Lucene text indexing system.

    A traditional view of incidents is that they are caused by shortcomings in human competence, attention, or attitude. It may be under the label of “loss of situational awareness,” procedure “violation,” or “poor” management. A different view is that human error is not the cause of failure, but a symptom of failure – trouble deeper inside the system. In this perspective, human error is not the conclusion, but rather the starting point of investigations. During an investigation, three types of information are gathered: physical, documentary, and human (recall/experience). Through the causal analysis process, apparent cause or apparent causes are identifiedmore » as the most probable cause or causes of an incident or condition that management has the control to fix and for which effective recommendations for corrective actions can be generated. A causal analysis identifies relevant human performance factors. In the following presentation, the anatomy of a radiological incident is discussed, and one case study is presented. We analyzed the contributing factors that caused a radiological incident. When underlying conditions, decisions, actions, and inactions that contribute to the incident are identified. This includes weaknesses that may warrant improvements that tolerate error. Measures that reduce consequences or likelihood of recurrence are discussed.« less

In addition, natural disasters and other disasters such as catastrophic fires may also fall into the same category. These incidents, like any other breach of acceptable behavior, may or may not involve actual law breaking. A computer incident response team should be created as a first priority. This report discusses the establishment and operation of a response team.

  3. Long-Term Spatial Data Preservation and Archiving: What Are the Issues?

    The Department of Energy (DOE) is moving towards Long-Term Stewardship (LTS) of many environmental restoration sites that cannot be released for unrestricted use. One aspect of information management for LTS is geospatial data archiving. This report discusses the challenges facing the DOE LTS program concerning the data management and archiving of geospatial data. It discusses challenges in using electronic media for archiving, overcoming technological obsolescence, data refreshing, data migration, and emulation. It gives an overview of existing guidance and policy and discusses what the United States Geological Service (USGS), National Oceanic and Atmospheric Administration (NOAA) and the Federal Emergency Management Agency (FEMA) are doing to archive the geospatial data that their agencies are responsible for. In the conclusion, this report provides issues for further discussion around long-term spatial data archiving.

    This report presents a code assessment study based on a real plant transient that occurred at the DOEL 2 power plant in Belgium on June 25th 1979. DOEL 2 is a two-loop WESTINGHOUSE PWR plant of 392 MWe. A steam generator tube rupture occurred at the end of a heat-up phase which initiated a plant transient which required substantial operator involvement and presented many plant phenomena which are of interest for code assessment. While real plant transients are of special importance for code validation because of the elimination of code scaling uncertainties, they introduce however some uncertainties related to the specifications of the exact initial and boundary conditions which must be reconstructed from available on-line plant recordings and on-line computer diagnostics. Best estimate data have been reconstructed for an assessment study by means of the code RELAP5/MOD2/CYCLE 36.01. Because of inherent uncertainties in the plant data, the assessment work is focussed on phenomena whereby the comparison between plant data and computer data is based more on trends than on absolute values. Such approach is able to uncover basic code weaknesses and strengths which can contribute to a better understanding of the code potential.

  9. Security incidents on the Internet, 1989--1995

    This paper presents an analysis of trends in Internet security based on an investigation of 4,299 Internet security-related incidents reported to the CERT{reg_sign} Coordination Center (CERT{reg_sign}/CC) from 1989 through 1995. Prior to this research, knowledge of actual Internet security incidents was limited and primarily anecdotal. This research: (1) developed a taxonomy to classify Internet attacks and incidents, (2) organized, classified, and analyzed CERT{reg_sign}/CC incident records, (3) summarized the relative frequency of the use of tools and vulnerabilities, success in achieving access, and results of attacks, (4) estimated total Internet incident activity, (5) developed recommendations for Internet users and suppliers, and (6) developed recommendations for future research. With the exception of denial-of-service attacks, security incidents were found to be increasing at a rate less than Internet growth. Estimates showed that most, if not all, severe incidents were reported to the CERT{reg_sign}/CC, and that more than one out of three above average incidents (in terms of duration and number of sites) were reported. Estimates also indicated that a typical Internet site was involved in, at most, around one incident (of any kind) per year, and a typical Internet host in, at most, around one incident in 45 years. The probability of unauthorized privileged access was around an order of magnitude less likely. As a result, simple and reasonable security precautions should be sufficient for most Internet users.
