Adjuvant and Salvage Radiation Therapy After Prostatectomy: American Society for Radiation Oncology/American Urological Association Guidelines

Purpose: The purpose of this guideline was to provide a clinical framework for the use of radiation therapy after radical prostatectomy as adjuvant or salvage therapy. Methods and Materials: A systematic literature review using PubMed, Embase, and Cochrane database was conducted to identify peer-reviewed publications relevant to the use of radiation therapy after prostatectomy. The review yielded 294 articles; these publications were used to create the evidence-based guideline statements. Additional guidance is provided as Clinical Principles when insufficient evidence existed. Results: Guideline statements are provided for patient counseling, use of radiation therapy in the adjuvant and salvage contexts, defining biochemical recurrence, and conducting a restaging evaluation. Conclusions: Physicians should offer adjuvant radiation therapy to patients with adverse pathologic findings at prostatectomy (ie, seminal vesicle invastion, positive surgical margins, extraprostatic extension) and salvage radiation therapy to patients with prostate-specific antigen (PSA) or local recurrence after prostatectomy in whom there is no evidence of distant metastatic disease. The offer of radiation therapy should be made in the context of a thoughtful discussion of possible short- and long-term side effects of radiation therapy as well as the potential benefits of preventing recurrence. The decision to administer radiation therapy should be made by the patient and the multidisciplinary treatment team with full consideration of the patient's history, values, preferences, quality of life, and functional status. American Society for Radiation Oncology (ASTRO) 2012 Workforce Study: The Radiation Oncologists' and Residents' Perspectives

Purpose: The American Society for Radiation Oncology (ASTRO) conducted the 2012 Radiation Oncology Workforce Survey to obtain an up-to-date picture of the workforce, assess its needs and concerns, and identify quality and safety improvement opportunities. The results pertaining to radiation oncologists (ROs) and residents (RORs) are presented here. Methods: The ASTRO Workforce Subcommittee, in collaboration with allied radiation oncology professional societies, conducted a survey study in early 2012. An online survey questionnaire was sent to all segments of the radiation oncology workforce. Respondents who were actively working were included in the analysis. This manuscript describes the data for ROs and RORs. Results: A total of 3618 ROs and 568 RORs were surveyed. The response rate for both groups was 29%, with 1047 RO and 165 ROR responses. Among ROs, the 2 most common racial groups were white (80%) and Asian (15%), and the male-to-female ratio was 2.85 (74% male). The median age of ROs was 51. ROs averaged 253.4 new patient consults in a year and 22.9 on-treatment patients. More than 86% of ROs reported being satisfied or very satisfied overall with their career. Close to half of ROs reported having burnout feelings. There was a trend toward more frequent burnout feelings with increasing numbers of new patient consults. ROs' top concerns were related to documentation, reimbursement, and patients' health insurance coverage. Ninety-five percent of ROs felt confident when implementing new technology. Fifty-one percent of ROs thought that the supply of ROs was balanced with demand, and 33% perceived an oversupply. Conclusions: This study provides a current snapshot of the 2012 radiation oncology physician workforce. There was a predominance of whites and men. Job satisfaction level was high. However a substantial fraction of ROs reported burnout feelings. Perceptions about supply and demand balance were mixed. ROs top concerns reflect areas of attention for the healthcare sector as a whole.

The short-term focus was on the disposition of excess weapons plutonium, while the longer-range issue concerned the disposition of the plutonium being produced in the civil nuclear fuel cycle. For the short term, the ANS panel strongly endorsed the concept that all plutonium scheduled for release from the US and Russian weapons stocks should be converted to a form that is intensively radioactive in order to protect the plutonium from theft of seizure (the spent fuel standard). However, since the conversion will at best take several years to complete, the panel has concluded that immediate emphasis should be placed on the assurance that all unconverted materials are protected as securely as when they were part of the active weapon stockpiles. More importantly, the panel also recommended prompt implementation of the so-called reactor option for disposing of surplus US and Russian weapons plutonium. The longer-term issues covered by the panel were those posed by the growing stocks of both separated plutonium and spent fuel generated in the world`s civil nuclear power programs. These issues included what fuel cycle policies should be prudently pursued in light of proliferation risks and likely future energy needs, what steps should be taken in regard to the increase in the demand for nuclear power in the future, and how civil plutonium in its various forms should be protected and managed to minimize proliferation. Overall, the panel concluded that plutonium is an energy resource that should be used and not a waste material to be disposed of.

An Assessment of the Current US Radiation Oncology Workforce: Methodology and Global Results of the American Society for Radiation Oncology 2012 Workforce Study

Purpose: To determine the characteristics, needs, and concerns of the current radiation oncology workforce, evaluate best practices and opportunities for improving quality and safety, and assess what we can predict about the future workforce. Methods and Materials: An online survey was distributed to 35,204 respondents from all segments of the radiation oncology workforce, including radiation oncologists, residents, medical dosimetrists, radiation therapists, medical physicists, nurse practitioners, nurses, physician assistants, and practice managers/administrators. The survey was disseminated by the American Society for Radiation Oncology (ASTRO) together with specialty societies representing other workforce segments. An overview of the methods and global results is presented in this paper. Results: A total of 6765 completed surveys were received, a response rate of 19%, and the final analysis included 5257 respondents. Three-quarters of the radiation oncologists, residents, and physicists who responded were male, in contrast to the other segments in which two-thirds or more were female. The majority of respondents (58%) indicated they were hospital-based, whereas 40% practiced in a free-standing/satellite clinic and 2% in another setting. Among the practices represented in the survey, 21.5% were academic, 25.2% were hospital, and 53.3% were private. A perceived oversupply of professionals relative to demand was reported by the physicist, dosimetrist, and radiation therapist segments. An undersupply was perceived by physician's assistants, nurse practitioners, and nurses. The supply of radiation oncologists and residents was considered balanced. Conclusions: This survey was unique as it attempted to comprehensively assess the radiation oncology workforce by directly surveying each segment. The results suggest there is potential to improve the diversity of the workforce and optimize the supply of the workforce segments. The survey also provides a benchmark for future studies, as many changes in the healthcare field exert pressure on the workforce.

Solar energy prediction using linear and non-linear regularization models: A study on AMS (American Meteorological Society) 2013Ė14 Solar Energy Prediction Contest

Abstract In 2013, American Meteorological Society Committees on AI (artificial intelligence) Applications organized a short-term solar energy prediction competition aiming at predicting total daily solar energy received at 98 solar farms based on the outputs of various weather patterns of a numerical weather prediction model. In this paper, a methodology to solve this problem has been explained and the performance of ordinary LSR (least-square regression), regularized LSR and ANN†(artificial neural network) models has been compared. In order to improve the generalization capability of the models, more experiments like variable segmentation, subspace feature sampling and ensembling of models have been conducted. It is observed that model accuracy can be improved by proper selection of input data segments. Further improvements can be obtained by ensemble of forecasts of different models. It is observed that the performance of an ensemble of ANN and LSR models is the best among all the proposed models in this work. As far as the competition is concerned, Gradient Boosting Regression Tree has turned out to be the best algorithm. The proposed ensemble of ANN and LSR model is able to show a relative improvement of 7.63% and 39.99% as compared to benchmark Spline Interpolation and Gaussian Mixture Model respectively.

Because the logic of dtruth cannot be classical (assuming the language to be suitably resourceful), theories of dtruth are accordingly nonclassical. The chapter focuses largely on a few broad approaches to dtruthónamely, ďparacompleteĒ and ďparaconsistent.Ē The chapter discusses in detail dtruth and dtruth-theoretic paradox, in particular, Liars. It covers a few different ďparacompleteĒ approaches to dtruth, beginning with a very informal, leisurely discussion of Kripke's nonclassical (least fixed point, empty ground model) proposal. The discussion continues with explaining ďparaconsistentĒ (and, in particular, ďdialetheicĒ) approaches. A brief discussion of validity and ďdtruth-preservationĒ is presented. A few standard parametric and contextualist approaches are discussed, including the ďindexiealistĒ idea, the ďquantifier-variabilityĒ proposal, and ďsituational truth.Ē The chapter proceeds to sketch the basic ďrevision-theoreticĒ idea, and concludes with discussing ďsetĒ-theoretic paradox, its relation to semantics, and the concept of ďrevenge.Ē

Many people are involved with evaluating engineering developing and opposing wind power projects. While there are many worldviews on global warming renewable energy and green technologies this paper focuses on the philosophical viewpoints surrounding noise from wind power projects. After identifying various stakeholders the paper investigates how some great historical and current minds have contributed likely unknowingly to this field including Gifford Pinchot John Muir Jonathan Edwards Mark Sagoff Bryan Norton and Cass Sunstein. A brief review of the history of noise in the rural environment comparing transportation noise with wind turbinenoise will be provided. The relation between cost?benefit analysis the precautionary principle and wind powernoise ordinances will be discussed. The paper will wrap up with some thoughts of what Sagoff may have to offer regarding NIMBYism and why Nortons call for convergence is also a call for acousticians to practice good science.

In early 2011, a dialogue was initiated within the Board of Directors (BOD) of the American Society for Radiation Oncology (ASTRO) regarding the future of the basic sciences of the specialty, primarily focused on the current state and potential future direction of basic research within radiation oncology. After consideration of the complexity of the issues involved and the precise nature of the undertaking, in August 2011, the BOD empanelled a Cancer Biology/Radiation Biology Task Force (TF). The TF was charged with developing an accurate snapshot of the current state of basic (preclinical) research in radiation oncology from the perspective of relevance to the modern clinical practice of radiation oncology as well as the education of our trainees and attending physicians in the biological sciences. The TF was further charged with making suggestions as to critical areas of biological basic research investigation that might be most likely to maintain and build further the scientific foundation and vitality of radiation oncology as an independent and vibrant medical specialty. It was not within the scope of service of the TF to consider the quality of ongoing research efforts within the broader radiation oncology space, to presume to consider their future potential, or to discourage in any way the investigators committed to areas of interest other than those targeted. The TF charge specifically precluded consideration of research issues related to technology, physics, or clinical investigations. This document represents an Executive Summary of the Task Force report.

Hesitate there a moment -- children always do -- and you'll see several Little Black Ants hurrying in and out, bringing up particles of earth from below as they enlarge their underground home. At another time you may see columns of them in an ant safari with two-way traffic as they cross a lawn or invade a kitchen to forage for food. All ants live in colonies. Ants are called social insects because within each colony there is a division of labor with males, females and one or more castes of workers each performing certain tasks for the benefit of the whole group. A single colony may vary in size from a few dozen individuals up to millions. Some naturalists suppose that ants are more numerous than any other type of land animal others think that plant lice outnumber them.

I captured it and now have it living in a large box. I have been feeding it commericial wild finch seed, niger seed and some sunflower seed. I have also provided a small cup of fine sand and a dish of water. Am I missing anything in it's diet? I had hoped to find someone to take it and care for it until it could fly again but have been unsuccessful so I may end up caring for it. It's wing is not visibly injured, however it can only flutter. I have been caring for it for 6 days now and it appears OK. Have also provided it with a small perch (branch) which it seems to use most of the time. Any other suggestions?

CHAPMAN*, GERALD R. DICKENS**, BRANDON DUGAN**, and GEORGE J. HIRASAKI* † ABSTRACT. Gas hydrates dominated by methane naturally occur in deep marine sediment along continental margins. These compounds form in pore space between the seafloor and a sub-bottom depth where appropriate stability conditions prevail. However, the amount and distribution of gas hydrate within this zone, and free gas below, can vary significantly at different locations. To understand this variability, we develop a one-dimensional numerical model that simulates the accumulation of gas hydrates in marine sediments due to upward and downward fluxes of methane over time. Abstract Traditional public relations histories begin in the late 19th and early 20th centuries. This study expands public relations history to the early 1800s by analyzing the use of public relations methods and tactics in an American anti-slavery movement, The American Colonization Society. In focusing on the American Colonization Society and backlash against it from abolitionist groups, this paper finds the use of newspapers to promote a cause, promotion of high profile endorsements, attempts to persuade a key public, creation of publications, efforts to lobby legislatures, and hired agents to found auxiliaries, all beginning in the early 1800s.

Gout in African Americans

AbstractPurpose African Americans have a substantially higher prevalence of risk factors for gout than Caucasians. The aim of the present study was to compare the risk for incident gout among African Americans and Caucasians. Methods Incidence rates of physician-diagnosed gout among 11,559 Caucasian men and 931 African American men aged 35 to 57 years and at high cardiovascular risk, observed for 7 years as a part of the Multiple Risk Factor Intervention Trial, were analyzed. Cox regression models were used to account for potential confounding by age, body mass index, diuretic use, hypertension and diabetes status, aspirin and alcohol consumption, and kidney disease. Results At baseline, after accounting for risk factors, African Americans had a 14% lower prevalence of hyperuricemia than Caucasians. Incidence of gout increased with increasing prevalence of risk factors in both Caucasians and African Americans. Ethnic disparities in incidence rates were most apparent among those without other risk factors for gout. In separate Cox regression models, after accounting for risk factors, African American ethnicity was associated with a hazard ratio of 0.78 (95% confidence interval [CI], 0.66-0.93) for physician-diagnosed gout and 0.88 (95% CI, 0.85-0.90) for incident hyperuricemia. Significant interactions were observed; the association was the strongest (hazard ratio 0.47; 0.37-0.60). These associations were unaffected by addition of serum urate as a covariate or by using alternate case definitions for gout. Conclusions After accounting for the higher prevalence of risk factors, African American ethnicity is associated with a significantly lower risk for gout and hyperuricemia compared with Caucasian ethnicity.

In ancient times there may have been a bridge of solid ice between the two shores, or there might have been an actual land connection between the two continents. But even if there were a 5-mile gap of water it could have been crossed in primitive canoes by hungry people hunting for game. Hunger has been the mainspring of civilization.

Los Alamos National Laboratory has been working with five Central American countries to assist in the development of their energy and mineral resources. Since 1985, mineral resources in Costa Rica, peat resources in Costa Rica and Panama, geothermal energy resources in Honduras and Guatemala, and geothermal field development in El Salvador and Costa Rica have been topics of study. The Central American Energy Resources Project is designed to help the countries of Honduras, Costa Rica, El Salvador, Guatemala, and Panama develop their geothermal resources. Auxiliary project goals are to increase economic development and employment in Central America; to provide a scientific basis for the private sector to develop natural resources; and to provide scientific training to in-country scientists, engineers, and technicians. The energy resources project began in March 1985. The progress being made in El Salvador (production well logging and geophysical operations), Panama (site selection for the first temperature-gradient well), and Honduras (well drilling of temperature-gradient wells) is summarized.

Geothermal energy for American Samoa

The geothermal commercialization potential in American Samoa was investigated. With geothermal energy harnessed in American Samoa, a myriad of possibilities would arise. Existing residential and business consumers would benefit from reduced electricity costs. The tuna canneries, demanding about 76% of the island's process heat requirements, may be able to use process heat from a geothermal source. Potential new industries include health spas, aquaculture, wood products, large domestic and transhipment refrigerated warehouses, electric cars, ocean nodule processing, and a hydrogen economy. There are no territorial statutory laws of American Samoa claiming or reserving any special rights (including mineral rights) to the territorial government, or other interests adverse to a land owner, for subsurface content of real property. Technically, an investigation has revealed that American Samoa does possess a geological environment conducive to geothermal energy development. Further studies and test holes are warranted.
