    Purpose: The purpose of this guideline was to provide a clinical framework for the use of radiation therapy after radical prostatectomy as adjuvant or salvage therapy. Methods and Materials: A systematic literature review using PubMed, Embase, and Cochrane database was conducted to identify peer-reviewed publications relevant to the use of radiation therapy after prostatectomy. The review yielded 294 articles; these publications were used to create the evidence-based guideline statements. Additional guidance is provided as Clinical Principles when insufficient evidence existed. Results: Guideline statements are provided for patient counseling, use of radiation therapy in the adjuvant and salvage contexts, defining biochemical recurrence, and conducting a restaging evaluation. Conclusions: Physicians should offer adjuvant radiation therapy to patients with adverse pathologic findings at prostatectomy (ie, seminal vesicle invastion, positive surgical margins, extraprostatic extension) and salvage radiation therapy to patients with prostate-specific antigen (PSA) or local recurrence after prostatectomy in whom there is no evidence of distant metastatic disease. The offer of radiation therapy should be made in the context of a thoughtful discussion of possible short- and long-term side effects of radiation therapy as well as the potential benefits of preventing recurrence. The decision to administer radiation therapy should be made by the patient and the multidisciplinary treatment team with full consideration of the patient's history, values, preferences, quality of life, and functional status.     This proposal supported Chemistry Symposia at the 2011 American Association for the Advancement of Science (AAAS) Meeting in Washington, DC February 17-21, 2011. The Chemistry Section of AAAS presented an unusually strong set of symposia for the 2011 AAAS meeting to help celebrate the 2011 International Year of Chemistry. The AAAS meeting provided an unusual opportunity to convey the excitement and importance of chemistry to a very broad audience and allowed access to a large contingent of the scientific press. Excellent suggestions for symposia were received from AAAS Chemistry Fellows and from the chairs of the American Chemical Society Technical Divisions. The AAAS Chemistry executive committee selected topics that would have wide appeal to scientists, the public, and the press for formal proposals of symposia. The symposia proposals were peer reviewed by AAAS. The Chemistry Section made a strong case to the program selection committee for approval of the chemistry symposia and 6 were approved for the 2011 annual meeting. The titles of the approved symposia were: (1) Powering the Planet: Generation of Clean Fuels from Sunlight and Water, (2) Biological Role and Consequences of Intrinsic Protein Disorder, (3) Chemically Speaking: How Organisms Talk to Each Other, (4) Molecular Self-Assembly and Artificial Molecular Machines, (5) Frontiers in Organic Materials for Information Processing, Energy and Sensors, and (6) Celebrating Marie Curie's 100th Anniversary of Her Nobel Prize in Chemistry. The Chemistry Section of AAAS is provided with funds to support only 1-2 symposia a year. Because of the much greater number of symposia approved in conjunction with observance of the 2011 International Year of Chemistry, additional support was sought from DOE to help support the 30 invited speakers and 8 symposia moderators/organizers. Support for the symposia provided the opportunity to highlight the excitement of current chemical research, to educate the public about the achievements of chemistry and its contributions to the well-being of humankind. The 2011 AAAS Annual Meeting provided an important opportunity to play a prominent role in the global celebration of the 2011 International Year of Chemistry.

    This is the 17th and final report of the Marshall Islands Medical Program as carried out by the Brookhaven National Laboratory (BNL). The purpose of these publications has been to provide information on the medical status of 253 Marshallese exposed to radiation fallout in 1954. The medical program fulfills a commitment to disclose unique medical information relevant to public health. Details of the Bravo thermonuclear accident that caused the exposure have been published. A 1955 article in the Journal of the American Medical Association, which described the acute medical effects on the population that required special medical care, remains a definitive and relevant description of events. Marshallese participation in this Congressionally mandated program is voluntary. Throughout the 44 years of the program, each participating individual`s relevant medical findings, laboratory data, disease morbidity, and mortality have been published in the BNL reports in a manner preserving patient confidentiality. In each report, there has been an attempt to interpret these findings and to infer the role of radiation exposure in their development. An equally important aspect of the reports has been the presentation of data that allows for analyses of the medical consequences of the Marshallese exposure.

    The purpose of this report is to disseminate information concerning the medical status of 253 Marshallese exposed to fallout radiation in 1954. This report discusses the medical care provided and the medical findings for the years 1988-1991. Details of the BRAVO thermonuclear accident that caused the exposure have been published, and a 1955 article in the Journal of the American Medical Association describing the acute medical effects in the exposed population remains a definitive and relevant description of events. Participation in the Marshall Islands Medical Program by the exposed Marshallese is voluntary. In the spring and fall of each year, medical surveillance is provided to exposed and unexposed cohorts. Examinations performed include: a cancer-related examination as defined by the American Society, an annual thyroid examination and thyroid function testing, serum prolactin testing looking for pituitary tumors, annual blood counts to include platelets, and evaluation for paraneoplastic evidence of neoplasms. This report details the medical program, medical findings, and thyroid surgery findings. Deaths (4 exposed and 10 nonexposed) that occurred during the reporting period are discussed. There is a mild but relatively consistent depression of neutrophil, lymphocyte, and platelet concentrations in the blood of the exposed population. This depression appears to be of no clinical significance. Thyroid hypofunction, either clinical or biochemical, has been documented as a consequence of radiation exposure in 14 exposed individuals. Previously, one other exposed person was diagnosed with basal cell carcinoma. During this reporting period, a thyroid nodule was identified in an individual who was in utero during the exposure. Upon pathologic review, the nodule was diagnosed as occult papillary carcinoma.

    A Draft Technical Information Document (HNF-1855) is being prepared to evaluate proposed interim tritium and medical isotope production at the Fast Flux Test Facility (FFTF). This assessment examines the potential health and safety impacts of transportation operations associated with the production of medical isotopes. Incident-free and accidental impacts are assessed using bounding source terms for the shipment of nonradiological target materials to the Hanford Site, the shipment of irradiated targets from the FFTF to the 325 Building, and the shipment of medical isotope products from the 325 Building to medical distributors. The health and safety consequences to workers and the public from the incident-free transportation of targets and isotope products would be within acceptable levels. For transportation accidents, risks to works and the public also would be within acceptable levels. This assessment is based on best information available at this time. As the medical isotope program matures, this analysis will be revised, if necessary, to support development of a final revision to the Technical Information Document.

    ABSTRACT: Burnout and compassion fatigue (CF) adversely affect medical professionals, including those employed in radiation oncology. Previously conducted research acknowledged the presence of burnout in populations of radiation therapists, radiation oncologists, and oncology nursing staff. The aim of the following research was to measure the incidence of burnout or CF in the specific population of medical dosimetrists surveyed. As professional members of the radiation oncology team, this group had not been included in published research data to date. The hypothesis of the subsequent study stated that a comparable incidence of burnout would be observed among medical dosimetrists as had been reported by earlier researchers for a population of radiation therapists. A survey tool based on the Maslach Burnout Inventory (MBI) and distributed to full members of the American Association of Medical Dosimetrists (AAMD) was utilized as the research measurement method. Results obtained indicated an incidence rates of burnout or CF for medical dosimetrists were less than the rates previously measured for radiation therapists (53% vs 11% for emotional exhaustion [EE] and 45% vs 27% for depersonalization [DP]). The incidence of burnout was based on the Burnout Inventory (BI) developed for the research project. Each of the subscales, EE, DP, and decreased personal accomplishment (PA), was considered and analyzed independently. Although not as prevalent among medical dosimetrists as a variety of additional radiation oncology professionals, a significant portion of the population demonstrated signs of burnout or CF. Future concerns abound for the population of medical dosimetrists as a large number of members scored positive for intermediate risk of burnout and CF. Additionally, a large portion of the population was found to be rapidly approaching retirement.

In one embodiment, the system provides an excitation light source to excite the fluorescent substance and a visible light source for general illumination within the same optical guide that is used to capture images. In another embodiment, the system is configured for use in open surgical procedures by providing an operating area that is closed to ambient light. More broadly, the systems described herein may be used in imaging applications where a visible light image may be usefully supplemented by an image formed from fluorescent emissions from a fluorescent substance that marks areas of functional interest.

Energy conservation and renewable energy production (including solar energy) play a significant role in community sustainability goals. Public officials and engaged citizens have many opportunities throughout the planning process to help support and advance solar energy initiatives in their communities. LONG-RANGE COMMUNITY VISIONING AND GOAL SETTING Community visioning is often the first step in developing any type of community plan, for establishing new standards, policies, and incentives, for doing development work, and for making public investment decisions. Whether part of a planning process or on its own, visioning is an important first chance to identify new opportunities and prioritiesincluding those related to solar energy. Planners often conduct visioning exercises that produce a communitys long-term goals and objectives. By expressing their views of renewable energy generation during these visioning exercises (including responding to surveys and participating in visioning meetings), citizens enable planners to gauge the level of awareness and importance of solar energy in the community. Planners use this information to develop policies and action items for the community. Once a community agrees upon its long-range goals and objectives, municipal officials should look to them when reviewing development proposals, making budget decisions, and performing other related tasks. Both citizens and public officials should use these visioning meetings to discuss how solar energy is connected to other community goals and values and how solar energy could help achieve other community goals.     Healthcare professionals, like all individuals, are human, fallible and subject to medical illnesses, including addiction.     The geothermal commercialization potential in American Samoa was investigated. With geothermal energy harnessed in American Samoa, a myriad of possibilities would arise. Existing residential and business consumers would benefit from reduced electricity costs. The tuna canneries, demanding about 76% of the island's process heat requirements, may be able to use process heat from a geothermal source. Potential new industries include health spas, aquaculture, wood products, large domestic and transhipment refrigerated warehouses, electric cars, ocean nodule processing, and a hydrogen economy. There are no territorial statutory laws of American Samoa claiming or reserving any special rights (including mineral rights) to the territorial government, or other interests adverse to a land owner, for subsurface content of real property. Technically, an investigation has revealed that American Samoa does possess a geological environment conducive to geothermal energy development. Further studies and test holes are warranted.

A bioactive agent is covalently bonded to the silane derivative coating. An implantable stent device including a stent core having an oxidized surface with a layer of silane derivative covalently bonded thereto. A spacer layer comprising polyethylene glycol (PEG) is covalently bonded to the layer of silane derivative and a protein is covalently bonded to the PEG. A method of making a medical implant device including providing a substrate having a surface, oxidizing the surface and reacting with derivitized silane to form a silane coating covalently bonded to the surface. A bioactive agent is then covalently bonded to the silane coating. In particular instances, an additional coating of bio-absorbable polymer and/or pharmaceutical agent is deposited over the bioactive agent.

    In late 1994, Sandia National Laboratories in Albuquerque, New Mexico, (SNL/NM), was instructed by the Department of Energy (DOE) Isotope Production and Distribution Program (IPDP) to examine the feasibility of producing medically useful radioisotopes using the Annular Core Research Reactor (ACRR) and the Hot Cell Facility (HCF). Los Alamos National Laboratory (LANL) would be expected to supply the targets to be irradiated in the ACRR. The intent of DOE would be to provide a capability to satisfy the North American health care system demand for {sup 99}Mo, the parent of {sup 99m}Tc, in the event of an interruption in the current Canadian supply. {sup 99m}Tc is used in 70 to 80% of all nuclear medicine procedures in the US. The goal of the SNL/NM study effort is to determine the physical plant capability, infrastructure, and staffing necessary to meet the North American need for {sup 99}Mo and to identify and examine all issues with potential for environmental impact.

    The Former Hanford Worker Medical Monitoring Program, directed by the Occupational and Environmental Medicine Program at the University of Washington, served former production and other non-construction workers who were potentially exposed to workplace hazards while working for the USDOE or its contractors at Hanford. The USDOE Former Workers Program arose from Congressional action in the Defense Authorization of 1993 (Public Law 102). Section 3162 stated that, The Secretary shall establish and carry out a program for the identification and ongoing medical evaluation of current and former Department of Energy employees who are subject to significant health risks as a result of exposure of such employees to hazardous or radioactive substances during such employment. (This also covers former employees of USDOE contractors and subcontractors.) The key objective has been to provide these former workers with medical evaluations in order to determine whether workers have experienced significant risk due to workplace exposure to hazards. Exposures to asbestos, beryllium, and noise can produce specific medical conditions: asbestosis, berylliosis, and noise-induced hearing loss (NIHL). Each of these conditions can be identified by specific, non-invasive screening tests, which are widely available. Treatments are also available for individuals affected by these conditions. This project involved two phases. Phase I involved a needs and risk assessment, characterizing the nature and extent of workplace health hazards which may have increased the risk for long-term health effects. We categorized jobs and tasks by likelihood of exposures to specific workplace health hazards; and located and established contact with former Hanford workers. Phase II involved implementation of medical monitoring programs for former workers whose individual work history indicated significant risk for adverse health effects. We identified 118,000 former workers, employed from 1943 to 1997. After excluding current workers, construction workers, and deceased workers, the total estimated number of former workers eligible for screening was 72,611. By September, 2006, 53,010 workers had been contacted, 20,298 responded, 2,835 were eligible and authorized, and 2,773 workers were ultimately screened. The cohort was 80% male, 85% white, and had a mean age of 63 years (range 24-96 years) at the time of first exam. Participants completed an occupational health history survey prior to the medical exam. Former Hanford workers were considered eligible for an exam if they reported exposure to asbestos, beryllium, or noise, or if a review of their Hanford work history indicated possible or probable exposure to one of these three hazards. We also invited any former Hanford worker who requested an exam to participate, regardless of documentation of exposure. The screening exam included a problem-focused physical exam, along with screening tests for one or more of three specific medical conditions: asbestosis (chest X-ray and spirometry), berylliosis (chest X-ray, spirometry, and beryllium-induced lymphocyte proliferation test), and NIHL (audiometry). We assisted ill workers in filing appropriate workers compensation claims, and facilitated appropriate follow-up medical care. This program has made an important contribution to the health of former DOE contractor workers at the Hanford defense nuclear site.

    The present invention relates to an apparatus for producing sterile ice slurries for medical cooling applications. The apparatus is capable of producing highly loaded slurries suitable for delivery to targeted internal organs of a patient, such as the brain, heart, lungs, stomach, kidneys, pancreas, and others, through medical size diameter tubing. The ice slurry production apparatus includes a slurry production reservoir adapted to contain a volume of a saline solution. A flexible membrane crystallization surface is provided within the slurry production reservoir. The crystallization surface is chilled to a temperature below a freezing point of the saline solution within the reservoir such that ice particles form on the crystallization surface. A deflector in the form of a reciprocating member is provided for periodically distorting the crystallization surface and dislodging the ice particles which form on the crystallization surface. Using reservoir mixing the slurry is conditioned for easy pumping directly out of the production reservoir via medical tubing or delivery through other means such as squeeze bottles, squeeze bags, hypodermic syringes, manual hand delivery, and the like.

    In late 1990 staff from the US Department of Energy's Office of Technical and Financial Assistance identified the Polish-American Children's Hospital as a potential site for application of DOE expertise to reduce the Hospital's energy requirements. Visits to the hospital in the fall and winter of 1991--1992 provided initial scoping of the energy related activities that would be supported by DOE. In addition to reducing energy requirements, the Hospital staff expressed a need for assistance in the areas of power quality and medical waste incineration. Subsequently, a power quality study supported by the Electric Power and Research Institute has been initiated and medical waste incineration will be provided by a Polish organization. The resulting scope of work for this effort is to survey the Hospital's thermal energy supply and demand systems to identify no-cost and low-cost measures that will reduce the Hospital's energy requirements. Applicable measures would then be demonstrated in the Hospital through a partnership with US and Polish organizations and a sister hospital in the United States.

    Medical robotics includes assistive devices used by the physician in order to make his/her diagnostic or therapeutic practices easier and more efficient. This chapter focuses on such systems. It introduces the general field of Computer-Assisted Medical Interventions, its aims, its different components and describes the place of robots in that context. The evolutions in terms of general design and control paradigms in the development of medical robots are presented and issues specific to that application domain are discussed. A view of existing systems, on-going developments and future trends is given. A case-study is detailed. Other types of robotic help in the medical environment (such as for assisting a handicapped person, for rehabilitation of a patient or for replacement of some damaged/suppressed limbs or organs) are out of the scope of this chapter.

    This article provides a review of hazardous medical waste production and its management in Croatia. Even though Croatian regulations define all steps in the waste management chain, implementation of those steps is one of the country's greatest issues. Improper practice is evident from the point of waste production to final disposal. The biggest producers of hazardous medical waste are hospitals that do not implement existing legislation, due to the lack of education and funds. Information on quantities, type and flow of medical waste are inadequate, as is sanitary control. We propose an integrated approach to medical waste management based on a hierarchical structure from the point of generation to its disposal. Priority is given to the reduction of the amounts and potential for harm. Where this is not possible, management includes reduction by sorting and separating, pretreatment on site, safe transportation, final treatment and sanitary disposal. Preferred methods should be the least harmful for human health and the environment. Integrated medical waste management could greatly reduce quantities and consequently financial strains. Landfilling is the predominant route of disposal in Croatia, although the authors believe that incineration is the most appropriate method. In a country such as Croatia, a number of small incinerators would be the most economical solution.

    This report is a compilation of submitted abstracts of papers presented at the DOE-supported workshop on medical information and the right to privacy held at the National Academy of Sciences in Washington, DC, on June 9 and 10, 1994. The aim of this meeting is to provide a forum to discuss the legal, ethical and practical issues related to the computerization and use of medical data, as well as the potential impact the use of these data may have on an individual`s privacy. Topical areas include an overview of the Federal and legal requirements to collect medical data, historical experiences with worker screening programs, currently available medical surveillance technologies (both biomedical and computer technologies) and their limitations. In addition, an-depth assessment of the needs and interests of a wide spectrum of parties as they relate to the use of medical data from both a legal and privacy perspective is provided. The needs of the individual, the public (e.g., blood and tissue banks), private enterprises (e.g., industry and insurance carriers), and the government (e.g., FBI) are discussed. Finally, the practical and legal issues relating to the use of computers to carry, store and transmit this information are also examined. The abstracts are presented in the intended order of presentation as indicated in the agenda for the meeting.

    Purpose: In 2004, the American Society for Radiation Oncology (ASTRO) published its first physics education curriculum for residents, which was updated in 2007. A committee composed of physicists and physicians from various residency program teaching institutions was reconvened again to update the curriculum in 2009. Methods and Materials: Members of this committee have associations with ASTRO, the American Association of Physicists in Medicine, the Association of Residents in Radiation Oncology, the American Board of Radiology (ABR), and the American College of Radiology. Members reviewed and updated assigned subjects from the last curriculum. The updated curriculum was carefully reviewed by a representative from the ABR and other physics and clinical experts. Results: The new curriculum resulted in a recommended 56-h course, excluding initial orientation. Learning objectives are provided for each subject area, and a detailed outline of material to be covered is given for each lecture hour. Some recent changes in the curriculum include the addition of Radiation Incidents and Bioterrorism Response Training as a subject and updates that reflect new treatment techniques and modalities in a number of core subjects. The new curriculum was approved by the ASTRO board in April 2010. We anticipate that physicists will use this curriculum for structuring their teaching programs, and subsequently the ABR will adopt this educational program for its written examination. Currently, the American College of Radiology uses the ASTRO curriculum for their training examination topics. In addition to the curriculum, the committee updated suggested references and the glossary. Conclusions: The ASTRO physics education curriculum for radiation oncology residents has been updated. To ensure continued commitment to a current and relevant curriculum, the subject matter will be updated again in 2 years.

    In March 1954, radioactive debris from a thermonuclear weapon test at Bikini Atoll deviated from predicted trajectories and contaminated several atolls in the northern Marshall Islands. As a result, 239 native inhabitants of these islands along with 28 American servicemen and 23 Japanese fishermen received variably severe exposures to diverse ionizing radiations. Fallout material consisted largely of mixed fission products with small amounts of neutron-induced radionuclides and minimal amounts of fissionable elements, producing a complex spectrum of electromagnetic and particulate radiation. Individuals were exposed to deeply penetrating, whole-body gamma irradiation, to internal radiation emitters assimilated either by inhalation or by ingestion of contaminated water and food, and to direct radiation from material accumulating on body surfaces. That accident initiated a cascade of events, medical, social and political, which continue in varying forms to this day. Most of these have been discussed in the open medical literature and in periodic reports issued by the medical team headquartered at Brookhaven National Laboratory. This report attempts to summarize some of the principal findings of medical significnce that have been observed during the subsequent 26 years with particular emphasis on the last six years.

    CPWR is carrying out a screening program for former Hanford construction workers. This program includes continuing screening and re-screening services for the former worker population. The Program contains the following general components: Start-up planning/needs assessment: A modified exposure assessment will be conducted to identify high-risk buildings or areas, primary exposures, and worker populations at risk. Outreach: CPWR, as the research arm of the Building and Construction Trades Department, AFL-CIO, has direct access to workers. CPWR will rely on direct mailings to lists of former workers, and work through and rely on existing organizations (unions, union pension funds, employers, DOE site administrators, etc.) to reach former workers and "get the word out." CPWR will establish/maintain an outreach office at each site listed above. This office will serve as the face of the Program to workers and their communities. Communications and intake center: CPWR has two established toll-free phone numbers (1-800-866-9663 and 1-888-464-0009). There is also a dedicated website for the program (btmed.org). Workers can register with the Program by mail, telephone, or on-line. Work history: A standardized, structured work history is administered with modules that accommodate unique exposure scenarios for different occupations and different DOE sites. A work history interview is administered by a trained program interviewer. The work histories are used to determine whether a participant is eligible for the medical examination and to interpret the findings from the medical examination. Medical evaluation: The Program contracts with local medical providers qualified to deliver occupational medical screening services. All providers are credentialed. The Program contracts with a certified national laboratory and with NIOSH certified B-readers to review x-rays. Based on the work history, the participant is referred to a credentialed medical provider who is located close to the participant's home. If it is not convenient to use a credentialed provider, the Program will make arrangements, if necessary, for the participant to receive a physical exam through the National Supplemental Screening Program. Eligible participants will receive the same core medical exam (including a Beryllium Lymphocyte Proliferation Test, BeLPT), and in addition, based on their work history, they may be assigned to exposure specific modules for asbestos, silica, lead, noise, cadmium, and chromium. Lab work will be sent to a national laboratory for processing, except the blood samples for the BeLPT, which will be sent to a DOE-approved laboratory for evaluation. Determination of work-relatedness and follow-up: A letter of findings will be sent to the participant within 60 days of the exam. The letter is written and/or reviewed by occupational medical health personnel with knowledge of the DOE site(s) where the participant has worked and will include specific follow-up recommendations. Urgent findings are followed up by the provider without delay. Evaluation and quality assurance: All data are entered into the Program Data Management System (DMS). The DMS is web-based and relies on electronic submission of results, whenever possible. A de-identified data set on all participants is provided to Duke University Medical Center for evaluation and analysis. Each participant is asked to complete a satisfaction survey. The DMS will be used for quality assurance purposes and to also report summary data to the DOE. The BTMED.ORG website is encrypted using the industry-standard Secure Sockets Layer (SSL) technology (128-bit encryption keys). Each individual that accesses the website is assigned a unique login ID, password, and token. Passwords are required to meet standards for "strength," including minimum length, multi-case, and use of numbers. The website is also protected by additional security layers including additional encryption, hardware and software firewalls, Virtual Private Networks (VPNs), and virus protection.

    Jackson Park Hospital completed the construction of a new Medical Office Building on its campus this spring. The new building construction has adopted the City of Chicago's recent focus on protecting the environment, and conserving energy and resources, with the introduction of green building codes. Located in a poor, inner city neighborhood on the South side of Chicago, Jackson Park Hospital has chosen green building strategies to help make the area a better place to live and work. The new green building houses the hospital's Family Medicine Residency Program and Specialty Medical Offices. The residency program has been vital in attracting new, young physicians to this medically underserved area. The new outpatient center will also help to allure needed medical providers to the community. The facility also has areas designated to women's health and community education. The Community Education Conference Room will provide learning opportunities to area residents. Emphasis will be placed on conserving resources and protecting our environment, as well as providing information on healthcare access and preventive medicine. The new Medical Office Building was constructed with numerous energy saving features. The exterior cladding of the building is an innovative, locally-manufactured precast concrete panel system with integral insulation that achieves an R-value in excess of building code requirements. The roof is a 'green roof' covered by native plantings, lessening the impact solar heat gain on the building, and reducing air conditioning requirements. The windows are low-E, tinted, and insulated to reduce cooling requirements in summer and heating requirements in winter. The main entrance has an air lock to prevent unconditioned air from entering the building and impacting interior air temperatures. Since much of the traffic in and out of the office building comes from the adjacent Jackson Park Hospital, a pedestrian bridge connects the two buildings, further decreasing the amount of unconditioned air that enters the office building. The HVAC system has an Energy Efficiency Rating 29% greater than required. No CFC based refrigerants were used in the HVAC system, thus reducing the emission of compounds that contribute to ozone depletion and global warming. In addition, interior light fixtures employ the latest energy-efficient lamp and ballast technology. Interior lighting throughout the building is operated by sensors that will automatically turn off lights inside a room when the room is unoccupied. The electrical traction elevators use less energy than typical elevators, and they are made of 95% recycled material. Further, locally manufactured products were used throughout, minimizing the amount of energy required to construct this building. The primary objective was to construct a 30,000 square foot medical office building on the Jackson Park Hospital campus that would comply with newly adopted City of Chicago green building codes focusing on protecting the environment and conserving energy and resources. The energy saving systems demonstrate a state of the-art whole-building approach to energy efficient design and construction. The energy efficiency and green aspects of the building contribute to the community by emphasizing the environmental and economic benefits of conserving resources. The building highlights the integration of Chicago's new green building codes into a poor, inner city neighborhood project and it is designed to attract medical providers and physicians to a medically underserved area.

    This document is an initial energy assessment for American Samoa, the first of many steps in developing a comprehensive energy strategy. On March 1, 2010, Assistant Secretary of the Interior Tony Babauta invited governors and their staff from the Interior Insular Areas to meet with senior principals at the National Renewable Energy Laboratory (NREL). Meeting discussions focused on ways to improve energy efficiency and increase the deployment of renewable energy technologies in the U.S. Pacific Territories. In attendance were Governors Felix Camacho (Guam), Benigno Fitial (Commonwealth of the Northern Mariana Islands), and Togiola Tulafono, (American Samoa). This meeting brought together major stakeholders to learn and understand the importance of developing a comprehensive strategic plan for implementing energy efficiency measures and renewable energy technologies. For several decades, dependence on fossil fuels and the burden of high oil prices have been a major concern but never more at the forefront as today. With unstable oil prices, the volatility of fuel supply and the economic instability in American Samoa, energy issues are a high priority. In short, energy security is critical to American Samoa's future economic development and sustainability. Under an interagency agreement, funded by the Department of Interior's Office of Insular Affairs, NREL was tasked to deliver technical assistance to the islands of American Samoa. Technical assistance included conducting an initial technical assessment to define energy consumption and production data, establish an energy consumption baseline, and assist with the development of a strategic plan. The assessment and strategic plan will be used to assist with the transition to a cleaner energy economy. NREL provided an interdisciplinary team to cover each relevant technical area for the initial energy assessments. Experts in the following disciplines traveled to American Samoa for on-island site assessments: (1) Energy Efficiency and Building Technologies; (2) Integrated Wind-Diesel Generation; (3) Transmission and Distribution; (4) Solar Technologies; and (5) Biomass and Waste-to-Energy. In addition to these core disciplines, team capabilities also included expertise in program analysis, project financing, energy policy and energy planning. The intent of the technical assessment was to provide American Samoa with a baseline energy assessment. From the baseline, various scenarios and approaches for deploying cost effective energy efficiency and renewable energy technologies could be created to meet American Samoa's objectives. The information provided in this energy assessment will be used as input in the development of a draft strategic plan and the development of scenarios and strategies for deploying cost-effective energy efficiency and renewable products.

    A workshop was held at the Idaho National Engineering Laboratory, August 16--18, 1994 on the topic of risk assessment on medical devices that use radioactive isotopes. Its purpose was to review past efforts to develop a risk assessment methodology to evaluate these devices, and to develop a program plan and a scoping document for future methodology development. This report contains a summary of that workshop. Participants included experts in the fields of radiation oncology, medical physics, risk assessment, human-error analysis, and human factors. Staff from the US Nuclear Regulatory Commission (NRC) associated with the regulation of medical uses of radioactive materials and with research into risk-assessment methods participated in the workshop. The workshop participants concurred in NRC`s intended use of risk assessment as an important technology in the development of regulations for the medical use of radioactive material and encouraged the NRC to proceed rapidly with a pilot study. Specific recommendations are included in the executive summary and the body of this report. An appendix contains the 8 papers presented at the conference: NRC proposed policy statement on the use of probabilistic risk assessment methods in nuclear regulatory activities; NRC proposed agency-wide implementation plan for probabilistic risk assessment; Risk evaluation of high dose rate remote afterloading brachytherapy at a large research/teaching institution; The pros and cons of using human reliability analysis techniques to analyze misadministration events; Review of medical misadministration event summaries and comparison of human error modeling; Preliminary examples of the development of error influences and effects diagrams to analyze medical misadministration events; Brachytherapy risk assessment program plan; and Principles of brachytherapy quality assurance.

    The ACNS provides a focal point for the national neutron user community to strengthen ties within this diverse group, while at the same time promoting neutron research among colleagues in related disciplines identified as would-be neutron users. The American Conference on Neutron Scattering thus serves a dual role as a national user meeting and a scientific meeting. As a venue for scientific exchange, the ACNS showcases recent results and provides forums for scientific discussion of neutron research in diverse fields such as hard and soft condensed matter, liquids, biology, magnetism, engineering materials, chemical spectroscopy, crystal structure, and elementary excitations, fundamental physics and development of neutron instrumentation through a combination of invited talks, contributed talks and poster sessions. As a super-user meeting, the ACNS fulfills the main objectives of users' meetings previously held periodically at individual national neutron facilities, with the advantage of a larger and more diverse audience. To this end, each of the major national neutron facilities (NIST, LANSCE, HFIR and SNS) have an opportunity to exchange information and update users, and potential users, of their facility. This is also an appropriate forum for users to raise issues that relate to the facilities. For many of the national facilities, this super-user meeting should obviate the need for separate user meetings that tax the time, energy and budgets of facility staff and the users alike, at least in years when the ACNS is held. We rely upon strong participation from the national facilities. The NSSA intends that the American Conference on Neutron Scattering (ACNS) will occur approximately every two years, but not in years that coincide with the International or European Conferences on Neutron Scattering. The ACNS is to be held in association with one of the national neutron centers in a rotating sequence, with the host facility providing local organization and planning assistance. Additional logistical support is being provided this year through a partnership with the conferencing office of the Materials Research Society (MRS). The ACNS, targeting the entire potential neutron North American user community, complements the annual NIST, ANL and LANSCE neutron and scattering schools which give hands-on experience primarily to graduate students who anticipate using neutron scattering in their thesis research. The summer schools are promoted at the ACNS and represent a natural path for students to take after being inspired by the activities of the ACNS.

    A medical imaging system provides simultaneous rendering of visible light and fluorescent images. The system may employ dyes in a small-molecule form that remain in the subject's blood stream for several minutes, allowing real-time imaging of the subject's circulatory system superimposed upon a conventional, visible light image of the subject.     In late 1990 staff from the US Department of Energy`s Office of Technical and Financial Assistance identified the Polish-American Children`s Hospital as a potential site for application of DOE expertise to reduce the Hospital`s energy requirements. Visits to the hospital in the fall and winter of 1991--1992 provided initial scoping of the energy related activities that would be supported by DOE. In addition to reducing energy requirements, the Hospital staff expressed a need for assistance in the areas of power quality and medical waste incineration. Subsequently, a power quality study supported by the Electric Power and Research Institute has been initiated and medical waste incineration will be provided by a Polish organization. The resulting scope of work for this effort is to survey the Hospital`s thermal energy supply and demand systems to identify no-cost and low-cost measures that will reduce the Hospital`s energy requirements. Applicable measures would then be demonstrated in the Hospital through a partnership with US and Polish organizations and a sister hospital in the United States.

    Abstract The International Monitoring System (IMS) of the Comprehensive-Nuclear-Test-Ban-Treaty monitors the atmosphere for radioactive xenon leaking from underground nuclear explosions. Emissions from medical isotope production represent a challenging background signal when determining whether measured radioxenon in the atmosphere is associated with a nuclear explosion prohibited by the treaty. The Australian Nuclear Science and Technology Organisation (ANSTO) operates a reactor and medical isotope production facility in Lucas Heights, Australia. This study uses two years of release data from the ANSTO medical isotope production facility and Xe-133 data from three IMS sampling locations to estimate the annual releases of Xe-133 from medical isotope production facilities in Argentina, South Africa, and Indonesia. Atmospheric dilution factors derived from a global atmospheric transport model were used in an optimization scheme to estimate annual release values by facility. The annual releases of about 6.81014 Bq from the ANSTO medical isotope production facility are in good agreement with the sampled concentrations at these three IMS sampling locations. Annual release estimates for the facility in South Africa vary from 1.21016 to 2.51016 Bq and estimates for the facility in Indonesia vary from 6.11013 to 3.61014 Bq. Although some releases from the facility in Argentina may reach these IMS sampling locations, the solution to the objective function is insensitive to the magnitude of those releases.

    These guidelines describe procedures to comply with all Federal and State laws and regulations and Lawrence Berkeley National Laboratory (LBNL) policy applicable to State-regulated medical and unregulated, but biohazardous, waste (medical/biohazardous waste). These guidelines apply to all LBNL personnel who: (1) generate and/or store medical/biohazardous waste, (2) supervise personnel who generate medical/biohazardous waste, or (3) manage a medical/biohazardous waste pickup location. Personnel generating biohazardous waste at the Joint Genome Institute/Production Genomics Facility (JGI/PGF) are referred to the guidelines contained in Section 9. Section 9 is the only part of these guidelines that apply to JGI/PGF. Medical/biohazardous waste referred to in this Web site includes biohazardous, sharps, pathological and liquid waste. Procedures for proper storage and disposal are summarized in the Solid Medical/Biohazardous Waste Disposal Procedures Chart.     Hydrogeology research has been very active in both Russia and the US because of the concerns for migration of radioactive and chemical contaminants in soils and geologic formations, as well as for water problems related to mining and other industrial operations. Russian hydrogeologists have developed various analysis and field testing techniques, sometimes in parallel with US counterparts. These Proceedings come out of a Seminar held to bring together a small group (about 15) of active Russian researchers in geologic flow and transport associated with the disposal of radioactive and chemical wastes either on the soils or through deep injection wells, with a corresponding group (about 25) of American hydrogeologists. The meeting was intentionally kept small to enable informal, detailed and in-depth discussions on hydrogeological issues of common interest. Out of this interaction, the authors hope that, firstly, they will have learned from each other and secondly, that research collaborations will be established where there is the opportunity. This proceedings presents the summaries and viewgraphs from the presentations. What cannot be conveyed here is the warm and cooperative atmosphere of these interactions, both inside and outside the formal sessions, which may well lead to future collaborations.
