    Purpose: To review our institutional outcomes of patients treated with radiation therapy (RT) for Graves' orbitopathy (GO), assess the role of orbital reirradiation, and identify prognostic factors of complete response (CR). Methods and Materials: This is a retrospective review of 211 patients who presented with a diagnosis of GO and received RT between January 2000-2010. RT dose was 20 Gy in 10 fractions. Patient median age was 51 years (range, 15-84 years), median follow-up was 11 months (range, 1-88 months). Patient symptoms included any combination of proptosis (90.9%), extraocular muscle dysfunction (78.9%), soft tissue signs (68.4%), and diplopia (58.4%). Corticosteroids were used as first-line therapy in 20.6% of patients. Among those who achieved either CR or partial response (PR), prognostic factors were evaluated. Results: Stabilization of disease without recurrence was clinically achieved overall in 202 patients (96.7%). At the completion of RT, 176 patients (84.2%) reported a symptomatic improvement of pretreatment symptoms. CR of GO symptoms was achieved using multiple treatment modalities, including RT by 93 patients (44.5%), of which 32 patients received RT only. Corticosteroids were discontinued in 97.8% of patients who received them as initial therapy. Surgical intervention following radiotherapy was required for 144 (68.9%) of all patients. Fourteen patients received orbital reirradiation for persistent or recurrent symptoms. Five of these achieved a CR, and the other nine achieved disease stabilization but retained persistent ocular symptoms. Long-term side effects of RT included dry eyes (12%). Of the prognostic factors we investigated, only gender predicted CR, which was less common in men (33.9%) than in women (49.7%) p = 0.0471. Conclusions: Orbital radiation for GO is an established treatment modality for patients. Orbital reirradiation is beneficial for patients who do not respond to initial RT or experience symptom recurrence without an apparent risk of increased morbidity.

    Entry into hazardous areas without the proper protective equipment is extremely dangerous and must be prevented whenever possible. Current postings of radiological hazards at the Rocky Flats Environmental Technology Site (RFETS) do not incorporate recent findings concerning effective warning presentation. Warning information should be highly visible, quickly, and easily understood. While continuing to comply with industry standards (e.g., Department of Energy (DOE) guidelines), these findings can be incorporated into existing radiological sign design, making them more effective in terms of usability and compliance. Suggestions are provided for designing more effective postings within stated guidelines.

    The geothermal commercialization potential in American Samoa was investigated. With geothermal energy harnessed in American Samoa, a myriad of possibilities would arise. Existing residential and business consumers would benefit from reduced electricity costs. The tuna canneries, demanding about 76% of the island's process heat requirements, may be able to use process heat from a geothermal source. Potential new industries include health spas, aquaculture, wood products, large domestic and transhipment refrigerated warehouses, electric cars, ocean nodule processing, and a hydrogen economy. There are no territorial statutory laws of American Samoa claiming or reserving any special rights (including mineral rights) to the territorial government, or other interests adverse to a land owner, for subsurface content of real property. Technically, an investigation has revealed that American Samoa does possess a geological environment conducive to geothermal energy development. Further studies and test holes are warranted.

    US Department of Energy (DOE) Order 5400.1 requires the establishment of a groundwater protection management program to ensure compliance with DOE requirements and applicable Federal, state, and local laws and regulations. The Uranium Mill Tailings Remedial Action (UMTRA) Project Office has prepared a ``Groundwater Protection Management Program Plan`` (groundwater protection plan) of sufficient scope and detail to reflect the program`s significance and address the seven activities required in DOE Order 5400.1, Chapter 3, for special program planning. The groundwater protection plan highlights the methods designed to preserve, protect, and monitor groundwater resources at UMTRA Project processing and disposal sites. The plan includes an overview of the remedial action status at the 24 designated processing sites and identifies project technical guidance documents and site-specific documents for the UMTRA groundwater protection management program. In addition, the groundwater protection plan addresses the general information required to develop a water resources protection strategy at the permanent disposal sites. Finally, the plan describes ongoing activities that are in various stages of development at UMTRA sites (long-term care at disposal sites and groundwater restoration at processing sites). This plan will be reviewed annually and updated every 3 years in accordance with DOE Order 5400.1.

    Generic physical protection logic trees, designed for application to nuclear facilities and materials, are presented together with a method of qualitative evaluation of the trees for design and analysis of physical protection systems. One or more defense zones are defined where adversaries interact with the physical protection system. Logic trees that are needed to describe the possible scenarios within a defense zone are selected. Elements of a postulated or existing physical protection system are tagged to the primary events of the logic tree. The likelihood of adversary success in overcoming these elements is evaluated on a binary, yes/no basis. The effect of these evaluations is propagated through the logic of each tree to determine whether the adversary is likely to accomplish the end event of the tree. The physical protection system must be highly likely to overcome the adversary before he accomplishes his objective. The evaluation must be conducted for all significant states of the site. Deficiencies uncovered become inputs to redesign and further analysis, closing the loop on the design/analysis cycle.

    An intermittent impressed current cathodic protection technique using photovoltaic energy was evaluated to determine it`s ability to protect bridge concrete piles in marine environments against corrosion. The technique uses commercially available anode systems to deliver the cathodic protection current to the concrete and onto the reinforcing steel. Cathodic protection current is only applied during the daytime hours. The magnitude of the applied current was based on sunlight availability. An evaluation was conducted on laboratory specimens as well as in the field. The laboratory work was performed on steel reinforced concrete specimens placed in simulated salt water tanks. For the field evaluation, ten prestressed concrete piles of a bridge structure with an existing rectifier powered cathodic protection system were used. In both cases, intermittent cathodic protection was provided. Polarization and depolarization of the steel reinforcement as well as the protection current delivered were monitored to evaluate the cathodic protection performance as well as the behavior of periodic polarization-depolarization.

    The purpose of the Insider Protection Program is to study and identify protection strategies and mechanisms to defend the Hanford Site against insider adversaries. Levels of protection required by US Department of Energy (DOE) Orders are to be met in a cost effective manner. The Insider Protection works in coordination with the Vulnerability Assessment (VA)/Master Safeguards and Security Agreement (MSSA) process to provide this protection. The VA studies are carried out in detail and provide useful information on the vulnerabilities and defense mechanisms identified at the time the study was made. The VA/MSSA results are an essential component of a general approach to defending against the insider.

    In February 2009, the Office of Inspector General received a letter from Congressman Mark Steven Kirk of Illinois, which included constituent allegations that an exclusive technology licensing agreement by Argonne National Laboratory was tainted by inadequate competition, conflicts of interest, and other improprieties. The technology in question was for the Program for Response Options and Technology Enhancements for Chemical/Biological Terrorism, commonly referred to as PROTECT. Because of the importance of the Department of Energy's technology transfer program, especially as implementation of the American Recovery and Reinvestment Act matures, we reviewed selected aspects of the licensing process for PROTECT to determine whether the allegations had merit. In summary, under the facts developed during our review, it was understandable that interested parties concluded that there was a conflict of interest in this matter and that Argonne may have provided the successful licensee with an unfair advantage. In part, this was consistent with aspects of the complaint from Congressman Kirk's constituent.

This Standard replaces certain mandatory fire protection requirements that were formerly in DOE 5480.7A, ``Fire Protection``, and DOE 6430.1A, ``General Design Criteria``. It also contains the fire protection guidelines from two (now canceled) draft standards: ``Glove Box Fire Protection`` and ``Filter Plenum Fire Protection``.     The U.S. Department of Energy (DOE), Office of River Protection (ORP) manages the River Protection Project (RPP). The RPP mission is to retrieve and treat Hanford's tank waste and close the tank farms to protect the Columbia River. As a result, ORP is responsible for the retrieval, treatment, and disposal of approximately 57 million gallons 1 of radioactive waste contained in the Hanford Site waste tanks and closure2 of all the tanks and associated facilities. The previous revision of the System Plan was issued in May 2008. ORP has made a number of changes to the tank waste treatment strategy and plans since the last revision of this document, and additional changes are under consideration. ORP has contracts in place to implement the strategy for completion of the mission and establish the capability to complete the overall mission. The current strategl involves a number of interrelated activities. ORP will reduce risk to the environment posed by tank wastes by the following: (1) Retrieving the waste from the single-shell tanks (SST) to double-shell tanks (DST) and delivering the waste to the Waste Treatment and Immobilization Plant (WTP). (2) Constructing and operating the WTP, which will safely treat all of the high-level waste (HLW) fraction contained in the tank farms. About one-third of the low-activity waste (LAW) fraction separated from the HLW fraction in the WTP will be immobilized in the WTP LAW Vitrification Facility. (3) Developing and deploying supplemental treatment capability assumed to be a second LAW vitrification facility that can safely treat about two-thirds of the LAW contained in the tank farms. (4) Developing and deploying supplemental pretreatment capability currently assumed to be an Aluminum Removal Facility (ARF) using a lithium hydrotalcite process to mitigate sodium management issues. (5) Developing and deploying treatment and packaging capability for contact-handled transuranic (CH-TRU) tank waste for possible shipment to and disposal at the Waste Isolation Pilot Plant (WIPP) in New Mexico. (6) Deploying interim storage capacity for the immobilized high-level waste (IHLW) pending determination of the final disposal pathway. (7) Closing the SST and DST tank farms, ancillary facilities, and all associated waste management and treatment facilities. (8) Optimizing the overall mission by resolution of technical and programmatic uncertainties, configuring the tank farms to provide a steady, well-balanced feed to the WTP, and performing trade-offs of the required amount and type of supplemental treatment and of the amount of HLW glass versus LAW glass. ORP has made and continues to make modifications to the WTP contract as needed to improve projected plant performance and address known or emerging risks. Key elements needed to implement the strategy described above are included within the scope of the Tank Operations Contract (TOC). Interim stabilization of the SSTs was completed in March 2004. As of April 2009, retrieval of seven SSTs has been completed and retrieval of four additional SSTs has been completed to the limits of technology. Demonstration of supplemental LAW treatment technologies has stopped temporarily pending revision of mission need requirements. Award of a new contract for tank operations (TOC), the ongoing tank waste retrieval experience, HLW disposal issues, and uncertainties in waste feed delivery and waste treatment led to the revision of the Performance Measurement Baseline (PM B), which is currently under review prior to approval. 6 This System Plan is aligned with the current WTP schedule, with hot commissioning beginning in 2018, and full operations beginning in late 2019. Major decisions regarding the use of supplemental treatment and the associated technology, the ultimate needed capacity, and its relationship to the WTP have not yet been finalized. This System Plan assumes that the outcome of these decisions will be to provide a second LAW vitrification facility. The ORP will reduce risk to the environment posed by tank wastes by: (1) Retrieving the waste from the single-shell tanks (SST) to double-shell tanks (DST) for treatment and disposal; (2) Constructing and operating the WTP, which will safely treat all of the high-level waste (HLW) and about half of the low-activity waste (LAW) contained in the tank farms, and maximizing its capability and capacity; (3) Developing and deploying supplemental treatment capability or a second WTP LAW Facility that can safely treat about half of the LAW contained in the tank farms; (4) Developing and deploying treatment and packaging capability for transuranic (TRU) tank waste for shipment to and disposal at the Waste Isolation Pilot Plant (WIPP); (5) Deploying interim storage capacity for the immobilized HLW and shipping that waste to Yucca Mountain for disposal; (6) Operating the Integrated Disposal Facility for the disposal of immobilized LAW, along with the associated secondary waste, (7) Closing the SST and DST tank farms, ancillary facilities, and al1 waste management and treatment facilities, (8) Developing and implementing technical solutions to mitigate the impact from substantial1y increased estimates of Na added during the pretreatment of the tank waste solids, This involves a combination of: (1) refining or modifying the flowsheet to reduce the required amount of additional sodium, (2) increasing the overall LAW vitrification capacity, (3) increasing the incorporation of sodium into the LAW glass, or (4) accepting an increase in mission duration, ORP has made and continues to make modifications to the WTP contract as needed to improve projected plant performance and address known or emerging risks, Key elements of the implementation of this strategy are included within the scope of the Tank Operations Contract, currently in procurement Since 2003, the ORP has conducted over 30 design oversight assessments of the Waste Treatment and Immobilization Plant (WTP). The estimated cost at completion has increased and the schedule for construction and commissioning of the WTP has extended, The DOE, Office of Environmental Management (EM), sanctioned a comprehensive review of the WTP flowsheet, focusing on throughput. In 2005, the TFC completed interim stabilization of the SSTs and as of March 2007, has completed the retrieval of seven selected SSTs. Demonstration of supplemental treatment technologies continues. The ongoing tank waste retrieval experience, progress with supplemental treatment technologies, and changes in WTP schedule led to the FY 2007 TFC baseline submittal in November 2006. The TFC baseline submittal was developed before the WTP schedule was fully understood and approved by ORP, and therefore reflects an earlier start date for the WTP facilities.     U.S. Department of Energy (DOE) Order 5400.1 requires the establishment of a ground water protection management program to ensure compliance with DOE requirements and applicable federal, state, and local laws and regulations. The Uranium Mill Tailings Remedial Action (UMTRA) Project Office was prepared this Ground Water Protection Management Program Plan (ground water protection plan) whose scope and detail reflect the program`s significance and address the seven activities required in DOE Order 5400.1, Chapter III, for special program planning. This ground water protection plan highlights the methods designed to preserve, protect, and monitor ground water resources at UMTRA Project processing and disposal sites. The plan includes an overview of the remedial action status at the 24 designated processing sites and identifies technical guidance documents and site-specific documents for the UMTRA Project ground water protection management program.     This document is an initial energy assessment for American Samoa, the first of many steps in developing a comprehensive energy strategy. On March 1, 2010, Assistant Secretary of the Interior Tony Babauta invited governors and their staff from the Interior Insular Areas to meet with senior principals at the National Renewable Energy Laboratory (NREL). Meeting discussions focused on ways to improve energy efficiency and increase the deployment of renewable energy technologies in the U.S. Pacific Territories. In attendance were Governors Felix Camacho (Guam), Benigno Fitial (Commonwealth of the Northern Mariana Islands), and Togiola Tulafono, (American Samoa). This meeting brought together major stakeholders to learn and understand the importance of developing a comprehensive strategic plan for implementing energy efficiency measures and renewable energy technologies. For several decades, dependence on fossil fuels and the burden of high oil prices have been a major concern but never more at the forefront as today. With unstable oil prices, the volatility of fuel supply and the economic instability in American Samoa, energy issues are a high priority. In short, energy security is critical to American Samoa's future economic development and sustainability. Under an interagency agreement, funded by the Department of Interior's Office of Insular Affairs, NREL was tasked to deliver technical assistance to the islands of American Samoa. Technical assistance included conducting an initial technical assessment to define energy consumption and production data, establish an energy consumption baseline, and assist with the development of a strategic plan. The assessment and strategic plan will be used to assist with the transition to a cleaner energy economy. NREL provided an interdisciplinary team to cover each relevant technical area for the initial energy assessments. Experts in the following disciplines traveled to American Samoa for on-island site assessments: (1) Energy Efficiency and Building Technologies; (2) Integrated Wind-Diesel Generation; (3) Transmission and Distribution; (4) Solar Technologies; and (5) Biomass and Waste-to-Energy. In addition to these core disciplines, team capabilities also included expertise in program analysis, project financing, energy policy and energy planning. The intent of the technical assessment was to provide American Samoa with a baseline energy assessment. From the baseline, various scenarios and approaches for deploying cost effective energy efficiency and renewable energy technologies could be created to meet American Samoa's objectives. The information provided in this energy assessment will be used as input in the development of a draft strategic plan and the development of scenarios and strategies for deploying cost-effective energy efficiency and renewable products.

    The circumstances for the protection of nuclear power plants are special in Sweden. A very important factor is that armed guards at the facilities are alien to the Swedish society. They do not use them. The Swedish concept of physical protection accepts that the aggressor will get into the facility. With this in mind, the Swedish Nuclear Power Inspectorate (SKI) has established the policy that administrative, technical, and organizational measures will be directed toward preventing an aggressor from damaging the reactor, even if he has occupied the facility. In addition, the best conditions possible shall be established for the operator and the police to reoccupy the plant. The author believes this policy is different from that of many other countries. Therefore, he focusses on the Swedish philosophy and techniques for the physical protection of nuclear power plants.

    A system that employs thermophoresis to protect lithographic surfaces from particle deposition and operates in an environment where the pressure is substantially constant and can be sub-atmospheric. The system (thermophoretic pellicle) comprises an enclosure that surrounds a lithographic component whose surface is being protected from particle deposition. The enclosure is provided with means for introducing a flow of gas into the chamber and at least one aperture that provides for access to the lithographic surface for the entry and exit of a beam of radiation, for example, and further controls gas flow into a surrounding low pressure environment such that a higher pressure is maintained within the enclosure and over the surface being protected. The lithographic component can be heated or, alternatively the walls of the enclosure can be cooled to establish a temperature gradient between the surface of the lithographic component and the walls of the enclosure, thereby enabling the thermophoretic force that resists particle deposition.

    DEMO aims to (i) integrate, demonstrate and validate all relevant technology necessary to convert fusion energy to electrical energy and (ii) that the machine and its operations are economically and environmentally acceptable. To maintain the efficiency and availability of the machine, there are several physics and combined physics/technology issues as well as the engineering issues. Machine Protection (also known as Protection of Investment) addresses both the risks to plant (to avoid costly repair or replacement) and the risks to normal operating time (to avoid loss of productivity and the return on investment). The plasma-related Machine Protection issues involve measurement and control of plasma stability, plasma purity, and plasma-wall interactions. Machine Protection aims to avoid hitting catastrophic limits by using early warning alarm systems, and controlled termination or avoidance, involving coordinated actions of the magnets, gas and auxiliary heating or current-drive systems. This article outlines the key processes, some of which are used in present-day tokamaks and some of which are new specifically for DEMO (e.g. First wall and divertor power handling) and reveals the need to research and develop new science and technology for Machine Protections in DEMO's high radiation and thermal fields.
