    A rapid deployable global sensing hazard alert system and associated methods of operation are provided. An exemplary system includes a central command, a wireless backhaul network, and a remote monitoring unit. The remote monitoring unit can include a positioning system configured to determine a position of the remote monitoring unit based on one or more signals received from one or more satellites located in Low Earth Orbit. The wireless backhaul network can provide bidirectional communication capability independent of cellular telecommunication networks and the Internet. An exemplary method includes instructing at least one of a plurality of remote monitoring units to provide an alert based at least in part on a location of a hazard and a plurality of positions respectively associated with the plurality of remote monitoring units.

    Despite the end of the Cold War, the US and Russia continue to maintain their ICBMs and many SLBMs in a highly alerted state--they are technically prepared to launch the missiles within minutes of a command decision to do so. Some analysts argue that, particularly in light of the distressed condition of the Russian military, these high alert conditions are tantamount to standing on the edge of a nuclear cliff from which we should now step back. They have proposed various bilateral ''de-alerting'' measures, to be taken prior to and outside the context of the formal strategic arms reduction treaty (START) process. This paper identifies several criteria for a stable de-alerting regime, but fails to find de-alerting measures that convincingly satisfy the criteria. However, some de-alerting measures have promise as de-activation measures for systems due for elimination under the START II and prospective START III treaties. Moreover, once these systems are deactivated, a considerable part of the perceived need to keep nuclear forces on high alert as a survivability hedge will be reduced. At the same time, the U.S. and Russia could consider building on their earlier cooperative actions to reduce the risk of inadvertent nuclear war by enhancing their communications links and possibly joining in efforts to improve early warning systems.

    In recent years, a new generation of space missions has offered great opportunities for discovery in high-energy astrophysics. In this article we focus on the scientific operations of the Gamma-Ray Imaging Detector (GRID) on board the AGILE space mission. AGILE-GRID, sensitive in the energy range of 30 MeV-30 GeV, has detected many ?-ray transients of both galactic and extragalactic origin. This work presents the AGILE innovative approach to fast ?-ray transient detection, which is a challenging task and a crucial part of the AGILE scientific program. The goals are to describe (1) the AGILE Gamma-Ray Alert System, (2) a new algorithm for blind search identification of transients within a short processing time, (3) the AGILE procedure for ?-ray transient alert management, and (4) the likelihood of ratio tests that are necessary to evaluate the post-trial statistical significance of the results. Special algorithms and an optimized sequence of tasks are necessary to reach our goal. Data are automatically analyzed at every orbital downlink by an alert pipeline operating on different timescales. As proper flux thresholds are exceeded, alerts are automatically generated and sent as SMS messages to cellular telephones, via e-mail, and via push notifications from an application for smartphones and tablets. These alerts are crosschecked with the results of two pipelines, and a manual analysis is performed. Being a small scientific-class mission, AGILE is characterized by optimization of both scientific analysis and ground-segment resources. The system is capable of generating alerts within two to three hours of a data downlink, an unprecedented reaction time in ?-ray astrophysics.

    This paper presents a framework for evaluating the technical merits of strategic ballistic missile de-alerting measures, and it uses the framework to evaluate a variety of possible measures for silo-based, land-mobile, and submarine-based missiles. De-alerting measures are defined for the purpose of this paper as reversible actions taken to increase the time or effort required to launch a strategic ballistic missile. The paper does not assess the desirability of pursuing a de-alerting program. Such an assessment is highly context dependent. The paper postulates that if de-alerting is desirable and is used as an arms control mechanism, de-alerting measures should satisfy specific cirteria relating to force security, practicality, effectiveness, significant delay, and verifiability. Silo-launched missiles lend themselves most readily to de-alerting verification, because communications necessary for monitoring do not increase the vulnerabilty of the weapons by a significant amount. Land-mobile missile de-alerting measures would be more challenging to verify, because monitoring measures that disclose the launcher's location would potentially increase their vulnerability. Submarine-launched missile de-alerting measures would be extremely challlenging if not impossible to monitor without increasing the submarine's vulnerability.

  4. Improving the Quality of Alerts and Predicting Intruder's Next Goal with Hidden Colored Petri-Net

    Intrusion detection systems (IDS) often provide poor quality alerts, which are insufficient to support rapid identification of ongoing attacks or predict an intruder’s next likely goal. In this paper, we propose a novel approach to alert post-processing and correlation, the Hidden Colored Petri-Net (HCPN). Different from most other alert correlation methods, our approach treats the alert correlation problem as an inference problem rather than a filter problem. Our approach assumes that the intruder’s actions are unknown to the IDS and can be inferred only from the alerts generated by the IDS sensors. HCPN can describe the relationship between different steps carried out by intruders, model observations (alerts) and transitions (actions) separately, and associate each token element (system state) with a probability (or confidence). The model is an extension to Colored Petri-Net (CPN) .It is so called “hidden” because the transitions (actions) are not directly observable but can be inferred by looking through the observations (alerts). These features make HCPN especially suitable for discovering intruders’ actions from their partial observations (alerts,) and predicting intruders’ next goal. Our experiments on DARPA evaluation datasets and the attack scenarios from the Grand Challenge Problem (GCP) show that HCPN has promise as a way to reducing false positives and negatives, predicting intruder’s next possible action, uncovering intruders’ intrusion strategies after the attack scenario has happened, and providing confidence scores.

  5. Federal technology alert. Parabolic-trough solar water heating

    Parabolic-trough solar water heating is a well-proven renewable energy technology with considerable potential for application at Federal facilities. For the US, parabolic-trough water-heating systems are most cost effective in the Southwest where direct solar radiation is high. Jails, hospitals, barracks, and other facilities that consistently use large volumes of hot water are particularly good candidates, as are facilities with central plants for district heating. As with any renewable energy or energy efficiency technology requiring significant initial capital investment, the primary condition that will make a parabolic-trough system economically viable is if it is replacing expensive conventional water heating. In combination with absorption cooling systems, parabolic-trough collectors can also be used for air-conditioning. Industrial Solar Technology (IST) of Golden, Colorado, is the sole current manufacturer of parabolic-trough solar water heating systems. IST has an Indefinite Delivery/Indefinite Quantity (IDIQ) contract with the Federal Energy Management Program (FEMP) of the US Department of Energy (DOE) to finance and install parabolic-trough solar water heating on an Energy Savings Performance Contract (ESPC) basis for any Federal facility that requests it and for which it proves viable. For an ESPC project, the facility does not pay for design, capital equipment, or installation. Instead, it pays only for guaranteed energy savings. Preparing and implementing delivery or task orders against the IDIQ is much simpler than the standard procurement process. This Federal Technology Alert (FTA) of the New Technology Demonstration Program is one of a series of guides to renewable energy and new energy-efficient technologies.

    Systems and methods are described for passive pavement-mounted acoustical alert of the occupants of a vehicle. A method of notifying a vehicle occupant includes providing a driving medium upon which a vehicle is to be driven; and texturing a portion of the driving medium such that the textured portion interacts with the vehicle to produce audible signals, the textured portion pattern such that a linguistic message is encoded into the audible signals. The systems and methods provide advantages because information can be conveyed to the occupants of the vehicle based on the location of the vehicle relative to the textured surface.

    Power extracted from fast moving tidal currents has been identified as a potential commercial-scale source of renewable energy. Device developers and utilities are pursuing deployment of prototype tidal turbines to assess technology viability, site feasibility, and environmental interactions. Deployment of prototype turbines requires permits from a range of regulatory authorities. Ensuring the safety of marine animals, particularly those under protection of the Endangered Species Act of 1973 (ESA) and the Marine Mammal Protection Act of 1972 has emerged as a key regulatory challenge for initial MHK deployments. The greatest perceived risk to marine animals is from strike by the rotating blades of tidal turbines. Development of the marine mammal alert system (MAAS) was undertaken to support monitoring and mitigation requirements for tidal turbine deployments. The prototype system development focused on Southern Resident killer whales (SRKW), an endangered population of killer whales that frequents Puget Sound and is intermittently present in the part of the sound where deployment of prototype tidal turbines is being considered. Passive acoustics were selected as the primary means because of the vocal nature of these animals. The MAAS passive acoustic system consists of two-stage process involving the use of an energy detector and a spectrogram-based classifier to distinguish between SKRW’s calls and noise. A prototype consisting of two 2D symmetrical star arrays separated by 20 m center to center was built and evaluated in the waters of Sequim Bay using whale call playback.

  15. Use of Circadian Lighting System to improve night shift alertness and performance of NRC Headquarters Operations Officers

    The Nuclear Regulatory Commission`s (NRC) Headquarters Operations Officers (HOOs) receive and respond to events reported in the nuclear industry on a 24-hour basis. The HOOs have reported reduced alertness on the night shift, leading to a potential deterioration in their on-shift cognitive performance during the early morning hours. For some HOOs, maladaptation to the night shift was also reported to be the principal cause of: (a) reduced alertness during the commute to and from work, (b) poor sleep quality, and (c) personal lifestyle problems. ShiftWork Systems, Inc. (SWS) designed and installed a Circadian Lighting System (CLS) at both the Bethesda and Rockville HOO stations with the goal of facilitating the HOOs physiological adjustment to their night shift schedules. The data indicate the following findings: less subjective fatigue on night shifts; improved night shift alertness and mental performance; higher HOO confidence in their ability to assess event reports; longer, deeper and more restorative day sleep after night duty shifts; swifter adaptation to night work; and a safer commute, particularly for those with extensive drives.

    A growing number of nuclear power plants in the United States have adopted routine 12-hr shift schedules. Because of the potential impact that extended work shifts could have on safe and efficient power plant operation, the U.S. Nuclear Regulatory Commission funded research on 8-hr and 12-hr shifts at the Human Alertness Research Center (HARC) in Boston, Massachusetts. This report describes the research undertaken: a study of simulated 8-hr and 12-hr work shifts that compares alertness, speed, and accuracy at responding to simulator alarms, and relative cognitive performance, self-rated mood and vigor, and sleep-wake patterns of 8-hr versus 12-hr shift workers.

    The Marine Animal Alert System (MAAS) in development by the Pacific Northwest National Laboratory is focused on providing elements of compliance monitoring to support deployment of marine hydrokinetic energy devices. An initial focus is prototype tidal turbines to be deployed in Puget Sound in Washington State. The MAAS will help manage the risk of injury or mortality to marine animals from blade strike or contact with tidal turbines. In particular, development has focused on detection, classification, and localization of listed Southern Resident killer whales within 200 m of prototype turbines using both active and passive acoustic approaches. At the close of FY 2011, a passive acoustic system consisting of a pair of four-element star arrays and parallel processing of eight channels of acoustic receptions has been designed and built. Field tests of the prototype system are scheduled for the fourth quarter of calendar year 2011. Field deployment and testing of the passive acoustic prototype is scheduled for the first quarter of FY 2012. The design of an active acoustic system that could be built using commercially available off-the-shelf components from active acoustic system vendors is also in the final stages of design and specification.

  18. NIOSH alert: Request for assistance in preventing electrocutions of crane operators and crew members working near overhead power lines

    In this alert, NIOSH warned that crane operators and crew members may be electrocuted when working near overhead power lines. Five cases were described which resulted in six electrocutions. Case 1 involved a 29 year old who pushed the crane cable on a 1 yard cement bucket into a 7,200 volt power line. Case 2 involved a 33 year old well driller who was electrocuted when a metal pipe lifted by a truck mounted crane contacted a 12,000 volt overhead power line. The third case involved a 24 year old forman for a telecommunications company who was electrocuted when he grabbed the door handle of a truck mounted crane whose boom was in contact with a 7,200 volt overhead power line. Case 4 involved a 37 year old construction laborer electrocuted while pulling a wire rope attached to a crane cable toward a load. The fifth case involved a 20 year old male truck driver and his 70 year old male employer who were electrocuted when the boom of a truck mounted crane contacted a 7,200 volt conductor of an overhead power line.

    The Coma Cluster of galaxies hosts the brightest radio halo known and has therefore been the target of numerous searches for associated inverse Compton (IC) emission, particularly at hard X-ray energies where the IC signal must eventually dominate over thermal emission. The most recent search with the Suzaku Hard X-ray Detector failed to confirm previous IC detections with RXTE and BeppoSAX, instead setting an upper limit 2.5 times below their non-thermal flux. However, this discrepancy can be resolved if the IC emission is very extended, beyond the scale of the cluster radio halo. Using reconstructed sky images from the 58-month Swift Burst Alert Telescope (BAT) all-sky survey, the feasibility of such a solution is investigated. Building on Renaud et al., we test and implement a method for extracting the fluxes of extended sources, assuming specified spatial distributions. BAT spectra are jointly fit with an XMM-Newton EPIC-pn spectrum derived from mosaic observations. We find no evidence for large-scale IC emission at the level expected from the previously detected non-thermal fluxes. For all non-thermal spatial distributions considered, which span the gamut of physically reasonable IC models, we determine upper limits for which the largest (most conservative) limit is {approx}<4.2 x 10{sup -12} erg s{sup -1} cm{sup -2} (20-80 keV), which corresponds to a lower limit on the magnetic field B > 0.2 {mu} G. A nominal flux upper limit of <2.7 x 10{sup -12} erg s{sup -1} cm{sup -2}, with corresponding B > 0.25 {mu} G, is derived for the most probable IC distribution given the size of the radio halo and likely magnetic field radial profile.

    The gamma-ray burst (GRB) rate is essential for revealing the connection between GRBs, supernovae, and stellar evolution. Additionally, the GRB rate at high redshift provides a strong probe of star formation history in the early universe. While hundreds of GRBs are observed by Swift, it remains difficult to determine the intrinsic GRB rate due to the complex trigger algorithm of Swift. Current studies of the GRB rate usually approximate the Swift trigger algorithm by a single detection threshold. However, unlike the previously flown GRB instruments, Swift has over 500 trigger criteria based on photon count rate and an additional image threshold for localization. To investigate possible systematic biases and explore the intrinsic GRB properties, we develop a program that is capable of simulating all the rate trigger criteria and mimicking the image threshold. Our simulations show that adopting the complex trigger algorithm of Swift increases the detection rate of dim bursts. As a result, our simulations suggest that bursts need to be dimmer than previously expected to avoid overproducing the number of detections and to match with Swift observations. Moreover, our results indicate that these dim bursts are more likely to be high redshift events than low-luminosity GRBs. This would imply an even higher cosmic GRB rate at large redshifts than previous expectations based on star formation rate measurements, unless other factors, such as the luminosity evolution, are taken into account. The GRB rate from our best result gives a total number of 4568{sub ?1429}{sup +825} GRBs per year that are beamed toward us in the whole universe.

    In an image registration method, edges are detected in a first image and a second image. A percentage of edge pixels in a subset of the second image that are also edges in the first image shifted by a translation is calculated. A best registration point is calculated based on a maximum percentage of edges matched. In a predefined search region, all registration points other than the best registration point are identified that are not significantly worse than the best registration point according to a predetermined statistical criterion.

    An apparatus for enabling accurate retaining of a precise position, such as for reacquisition of a microscopic spot or feature having a size of 0.1 mm or less, on broad-area surfaces after non-in situ processing. The apparatus includes a sample and sample holder. The sample holder includes a base and three support posts. Two of the support posts interact with a cylindrical hole and a U-groove in the sample to establish location of one point on the sample and a line through the sample. Simultaneous contact of the third support post with the surface of the sample defines a plane through the sample. All points of the sample are therefore uniquely defined by the sample and sample holder. The position registration system of the current invention provides accuracy, as measured in x, y repeatability, of at least 140 .mu.m.

    AAPM Task Group 132 was charged with a review of the current approaches and solutions for image registration in radiotherapy and to provide recommendations for quality assurance and quality control of these clinical processes. As the results of image registration are always used as the input of another process for planning or delivery, it is important for the user to understand and document the uncertainty associate with the algorithm in general and the Result of a specific registration. The recommendations of this task group, which at the time of abstract submission are currently being reviewed by the AAPM, include the following components. The user should understand the basic image registration techniques and methods of visualizing image fusion. The disclosure of basic components of the image registration by commercial vendors is critical in this respect. The physicists should perform end-to-end tests of imaging, registration, and planning/treatment systems if image registration is performed on a stand-alone system. A comprehensive commissioning process should be performed and documented by the physicist prior to clinical use of the system. As documentation is important to the safe implementation of this process, a request and report system should be integrated into the clinical workflow. Finally, a patient specific QA practice should be established for efficient evaluation of image registration results. The implementation of these recommendations will be described and illustrated during this educational session. Learning Objectives: Highlight the importance of understanding the image registration techniques used in their clinic. Describe the end-to-end tests needed for stand-alone registration systems.     Image registration of low contrast image sequences is provided. In one aspect, a desired region of an image is automatically segmented and only the desired region is registered. Active contours and adaptive thresholding of intensity or edge information may be used to segment the desired regions. A transform function is defined to register the segmented region, and sub-pixel information may be determined using one or more interpolation methods.

  1. Interactive initialization of 2D/3D rigid registration

    Purpose: Registration is one of the key technical components in an image-guided navigation system. A large number of 2D/3D registration algorithms have been previously proposed, but have not been able to transition into clinical practice. The authors identify the primary reason for the lack of adoption with the prerequisite for a sufficiently accurate initial transformation, mean target registration error of about 10 mm or less. In this paper, the authors present two interactive initialization approaches that provide the desired accuracy for x-ray/MR and x-ray/CT registration in the operating room setting. Methods: The authors have developed two interactive registration methods based on visual alignment of a preoperative image, MR, or CT to intraoperative x-rays. In the first approach, the operator uses a gesture based interface to align a volume rendering of the preoperative image to multiple x-rays. The second approach uses a tracked tool available as part of a navigation system. Preoperatively, a virtual replica of the tool is positioned next to the anatomical structures visible in the volumetric data. Intraoperatively, the physical tool is positioned in a similar manner and subsequently used to align a volume rendering to the x-ray images using an augmented reality (AR) approach. Both methods were assessed using three publicly available reference data sets for 2D/3D registration evaluation. Results: In the authors' experiments, the authors show that for x-ray/MR registration, the gesture based method resulted in a mean target registration error (mTRE) of 9.3 5.0 mm with an average interaction time of 146.3 73.0 s, and the AR-based method had mTREs of 7.2 3.2 mm with interaction times of 44 32 s. For x-ray/CT registration, the gesture based method resulted in a mTRE of 7.4 5.0 mm with an average interaction time of 132.1 66.4 s, and the AR-based method had mTREs of 8.3 5.0 mm with interaction times of 58 52 s.     Purpose: A 2D/3D registration algorithm is proposed for registering orthogonal x-ray images with a diagnostic CT volume for high dose rate (HDR) lung brachytherapy. Methods: The algorithm utilizes a rigid registration model based on a pixel/voxel intensity matching approach. To achieve accurate registration, a robust similarity measure combining normalized mutual information, image gradient, and intensity difference was developed. The algorithm was validated using a simple body and anthropomorphic phantoms. Transfer catheters were placed inside the phantoms to simulate the unique image features observed during treatment. The algorithm sensitivity to various degrees of initial misregistration and to the presence of foreign objects, such as ECG leads, was evaluated. Results: The mean registration error was 2.2 and 1.9 mm for the simple body and anthropomorphic phantoms, respectively. The error was comparable to the interoperator catheter digitization error of 1.6 mm. Preliminary analysis of data acquired from four patients indicated a mean registration error of 4.2 mm. Conclusions: Results obtained using the proposed algorithm are clinically acceptable especially considering the complications normally encountered when imaging during lung HDR brachytherapy.

    Purpose: To investigate the feasibility of using deformable registration in clinical practice to fuse MR and CT images of the head and neck for treatment planning. Method and Materials: A state-of-the-art deformable registration algorithm was optimized, evaluated, and compared with rigid registration. The evaluation was based on manually annotated anatomic landmarks and regions of interest in both modalities. We also developed a multiparametric registration approach, which simultaneously aligns T1- and T2-weighted MR sequences to CT. This was evaluated and compared with single-parametric approaches. Results: Our results show that deformable registration yielded a better accuracy than rigid registration, without introducing unrealistic deformations. For deformable registration, an average landmark alignment of approximatively 1.7 mm was obtained. For all the regions of interest excluding the cerebellum and the parotids, deformable registration provided a median modified Hausdorff distance of approximatively 1 mm. Similar accuracies were obtained for the single-parameter and multiparameter approaches. Conclusions: This study demonstrates that deformable registration of head-and-neck CT and MR images is feasible, with overall a significanlty higher accuracy than for rigid registration.

    The amount of information available about urban traffic from aerial video imagery is extremely high. Here we discuss the collection of such video imagery from a helicopter platform with a low-cost sensor, and the post-processing used to correct radial distortion in the data and register it. The radial distortion correction is accomplished using a Harris model. The registration is implemented in a two-step process, using a globally applied polyprojective correction model followed by a fine scale local displacement field adjustment. The resulting cleaned-up data is sufficiently well-registered to allow subsequent straight-forward vehicle tracking.

Existing methods for calculating the center of rotation have been reviewed and an improved algorithm to register the rotation axis in tomographic data is presented. The performance of the proposed method is evaluated using synchrotron-based microtomography data on geological samples with and without artificial reduction of the signal-to-noise ratio. The proposed method improves the reconstruction quality by correcting both the tilting error and the translational offset of the rotation axis. The limitation of this promising method is also discussed.

  2. Do Tumors in the Lung Deform During Normal Respiration? An Image Registration Investigation

    Purpose: The purpose of this study was to investigate whether lung tumors may be described adequately using a rigid body assumption or whether they deform during normal respiration. Methods and Materials: Thirty patients with early stage non-small-cell lung cancer underwent four-dimensional (4D) computed tomography (CT) simulation. The gross tumor volume (GTV) was delineated on the 4D CT images. Image registration was performed in the vicinity of the GTV. The volume of interest for registration was the GTV and minimal volume of surrounding non-GTV tissue. Three types of registration were performed: translation only, translation + rotation, and deformable. The GTV contour from end-inhale was mapped to end-exhale using the registration-derived transformation field. The results were evaluated using three metrics: overlap index (OI), root-mean-squared distance (RMS), and Hausdorff distance (HD). Results: After translation only image registration, on average OI increased by 21.3%, RMS and HD reduced by 1.2 mm and 2.0 mm, respectively. The succeeding increases in OI after translation + rotation and deformable registration were 1.1% and 1.4% respectively. The succeeding reductions in RMS were 0.1 mm and 0.2 mm respectively. No reduction in HD was observed after translation + rotation and deformable image registration compared with translation only registration. The difference in the results from the three registration scenarios was independent of GTV size and motion amplitude. Conclusions: The primary effect of normal respiration on lung tumors was the translation of tumors. Rotation and deformation of lung tumors was determined to be minimal.

    MREG V1.1 is the sixth generation SAR image registration algorithm developed by the Signal Processing&Technology Department for Synthetic Aperture Radar applications. Like its predecessor algorithm REGI, it employs a powerful iterative multi-scale paradigm to achieve the competing goals of sub-pixel registration accuracy and the ability to handle large initial offsets. Since it is not model based, it allows for high fidelity tracking of spatially varying terrain-induced misregistration. Since it does not rely on image domain phase, it is equally adept at coherent and noncoherent image registration. This document provides a brief history of the registration processors developed by Dept. 5962 leading up to MREG V1.1, a full description of the signal processing steps involved in the algorithm, and a user's manual with application specific recommendations for CCD, TwoColor MultiView, and SAR stereoscopy.

    Remote DMA (RDMA) enables high performance networks to reduce data copying between an application and the operating system (OS). However RDMA operations in some high performance networks require communication memory explicitly registered with the network adapter and pinned by the OS. Memory registration and pinning limits the flexibility of the memory system and reduces the amount of memory that user processes can allocate. These issues become more significant on multicore platforms, since registered memory demand grows linearly with the number of processor cores. In this paper we propose a new memory registration/deregistration strategy to reduce registered memory on multicore architectures for HPC applications. We hide the cost of dynamic memory management by offloading all dynamic memory registration and deregistration requests to a dedicated memory management helper thread. We investigate design policies and performance implications of the helper thread approach. We evaluate our framework with the NAS parallel benchmarks, for which our registration scheme significantly reduces the registered memory (23.62% on average and up to 49.39%) and avoids memory registration/deregistration costs for reused communication memory. We show that our system enables the execution of problem sizes that could not complete under existing memory registration strategies.

  13. Multi-System Verification of Registrations for Image-Guided Radiotherapy in Clinical Trials

    Purpose: To provide quantitative information on the image registration differences from multiple systems for image-guided radiotherapy (IGRT) credentialing and margin reduction in clinical trials. Methods and Materials: Images and IGRT shift results from three different treatment systems (Tomotherapy Hi-Art, Elekta Synergy, Varian Trilogy) have been sent from various institutions to the Image-Guided Therapy QA Center (ITC) for evaluation for the Radiation Therapy Oncology Group (RTOG) trials. Nine patient datasets (five head-and-neck and four prostate) were included in the comparison, with each patient having 1-4 daily individual IGRT studies. In all cases, daily shifts were re-calculated by re-registration of the planning CT with the daily IGRT data using three independent software systems (MIMvista, FocalSim, VelocityAI). Automatic fusion was used in all calculations. The results were compared with those submitted from institutions. Similar regions of interest (ROIs) and same initial positions were used in registrations for inter-system comparison. Different slice spacings for CBCT sampling and different ROIs for registration were used in some cases to observe the variation of registration due to these factors. Results: For the 54 comparisons with head-and-neck datasets, the absolute values of differences of the registration results between different systems were 2.6 {+-} 2.1 mm (mean {+-} SD; range 0.1-8.6 mm, left-right [LR]), 1.7 {+-} 1.3 mm (0.0-4.9 mm, superior-inferior [SI]), and 1.8 {+-} 1.1 mm (0.1-4.0 mm, anterior-posterior [AP]). For the 66 comparisons in prostate cases, the differences were 1.1 {+-} 1.0 mm (0.0-4.6 mm, LR), 2.1 {+-} 1.7 mm (0.0-6.6 mm, SI), and 2.0 {+-} 1.8 mm (0.1-6.9 mm, AP). The differences caused by the slice spacing variation were relatively small, and the different ROI selections in FocalSim and MIMvista also had limited impact.     Purpose: Three-dimensional to two-dimensional (3D–2D) image registration is a key to fusion and simultaneous visualization of valuable information contained in 3D pre-interventional and 2D intra-interventional images with the final goal of image guidance of a procedure. In this paper, the authors focus on 3D–2D image registration within the context of intracranial endovascular image-guided interventions (EIGIs), where the 3D and 2D images are generally acquired with the same C-arm system. The accuracy and robustness of any 3D–2D registration method, to be used in a clinical setting, is influenced by (1) the method itself, (2) uncertainty of initial pose of the 3D image from which registration starts, (3) uncertainty of C-arm’s geometry and pose, and (4) the number of 2D intra-interventional images used for registration, which is generally one and at most two. The study of these influences requires rigorous and objective validation of any 3D–2D registration method against a highly accurate reference or “gold standard” registration, performed on clinical image datasets acquired in the context of the intervention. Methods: The registration process is split into two sequential, i.e., initial and final, registration stages. The initial stage is either machine-based or template matching. The latter aims to reduce possibly large in-plane translation errors by matching a projection of the 3D vessel model and 2D image. In the final registration stage, four state-of-the-art intrinsic image-based 3D–2D registration methods, which involve simultaneous refinement of rigid-body and C-arm parameters, are evaluated. For objective validation, the authors acquired an image database of 15 patients undergoing cerebral EIGI, for which accurate gold standard registrations were established by fiducial marker coregistration.   1. Volumetric Image Guidance Using Carina vs Spine as Registration Landmarks for Conventionally Fractionated Lung Radiotherapy

    Purpose: To compare the relative accuracy of 2 image guided radiation therapy methods using carina vs spine as landmarks and then to identify which landmark is superior relative to tumor coverage. Methods and Materials: For 98 lung patients, 2596 daily image-guidance cone-beam computed tomography scans were analyzed. Tattoos were used for initial patient alignment; then, spine and carina registrations were performed independently. A separate analysis assessed the adequacy of gross tumor volume, internal target volume, and planning target volume coverage on cone-beam computed tomography using the initial, middle, and final fractions of radiation therapy. Coverage was recorded for primary tumor (T), nodes (N), and combined target (T+N). Three scenarios were compared: tattoos alignment, spine registration, and carina registration. Results: Spine and carina registrations identified setup errors {>=}5 mm in 35% and 46% of fractions, respectively. The mean vector difference between spine and carina matching had a magnitude of 3.3 mm. Spine and carina improved combined target coverage, compared with tattoos, in 50% and 34% (spine) to 54% and 46% (carina) of the first and final fractions, respectively. Carina matching showed greater combined target coverage in 17% and 23% of fractions for the first and final fractions, respectively; with spine matching, this was only observed in 4% (first) and 6% (final) of fractions. Carina matching provided superior nodes coverage at the end of radiation compared with spine matching (P=.0006), without compromising primary tumor coverage. Conclusion: Frequent patient setup errors occur in locally advanced lung cancer patients. Spine and carina registrations improved combined target coverage throughout the treatment course, but carina matching provided superior combined target coverage.

  2. Registration of clinical volumes to beams-eye-view images for real-time tracking

    Purpose: The authors combine the registration of 2D beam’s eye view (BEV) images and 3D planning computed tomography (CT) images, with relative, markerless tumor tracking to provide automatic absolute tracking of physician defined volumes such as the gross tumor volume (GTV). Methods: During treatment of lung SBRT cases, BEV images were continuously acquired with an electronic portal imaging device (EPID) operating in cine mode. For absolute registration of physician-defined volumes, an intensity based 2D/3D registration to the planning CT was performed using the end-of-exhale (EoE) phase of the four dimensional computed tomography (4DCT). The volume was converted from Hounsfield units into electron density by a calibration curve and digitally reconstructed radiographs (DRRs) were generated for each beam geometry. Using normalized cross correlation between the DRR and an EoE BEV image, the best in-plane rigid transformation was found. The transformation was applied to physician-defined contours in the planning CT, mapping them into the EPID image domain. A robust multiregion method of relative markerless lung tumor tracking quantified deviations from the EoE position. Results: The success of 2D/3D registration was demonstrated at the EoE breathing phase. By registering at this phase and then employing a separate technique for relative tracking, the authors are able to successfully track target volumes in the BEV images throughout the entire treatment delivery. Conclusions: Through the combination of EPID/4DCT registration and relative tracking, a necessary step toward the clinical implementation of BEV tracking has been completed. The knowledge of tumor volumes relative to the treatment field is important for future applications like real-time motion management, adaptive radiotherapy, and delivered dose calculations.
