  1. Is the 10th and 11th Intercostal Space a Safe Approach for Percutaneous Nephrostomy and Nephrolithotomy?

The aim of this study was to determine the rate of complications in percutaneous nephrostomy (PCN) and nephrolithotomy (PCNL) performed through the 11th and 10th intercostal spaces using our monitoring technique and to discuss the safety of the procedure. Out of 398 PCNs and PCNLs carried out during a 3-year period, 56 patients had 57 such procedures performed using an intercostal approach. The 11th intercostal route was used in 42 and the 10th in 15 cases. One patient had two separate nephrostomies performed through the 10th and 11th intercostal spaces. The technique utilizes bi-planar fluoroscopy with a combination of a conventional angiographic machine to provide anterior-posterior fluoroscopy and a C-arm mobile fluoroscopy machine to give a lateral view, displayed on two separate monitors. None of the patients had clinically significant thoracic or abdominal complications. Two patients had minor chest complications. Only one developed changes (plate atelectasis, elevation of the hemi-diaphragm) directly related to the nephrostomy (2%). The second patient had bilateral plate atelectasis and unilateral congestive lung changes after PCNL. These changes were not necessarily related to the procedure but rather to general anesthesia during nephrolithotomy. The authors consider PCN or PCNL through the intercostal approach a safe procedure with a negligible complication rate, provided that it is performed under bi-planar fluoroscopy, which allows determination of the skin entry point just below the level of pleural reflection and provides three-dimensional monitoring of advancement of the puncturing needle toward the target entry point.

  16. Roots of terrorism: a reassessment after September 11th

    SciTech Connect (OSTI)

    Pilat, Joseph F.

    2002-01-01T23:59:59.000Z

The brutal terrorist attacks of September 11th, the anthrax attacks that followed and growing knowledge of al Qaeda's pursuit of nuclear, biological and chemical weapons have not only intensified concerns about terrorism but also created doubts about our understanding of terrorism. These attacks were in many ways unprecedented, and ultimately raise the question of the roots or causes of terrorism. Historically and today, there have been divergent views on this question, which reflect philosophical, religious, political, sociological and other differences. These differences are not merely academic, as they can affect our understanding of both the threat and of responses to terrorism in the aftermath of September 11th, Terrorism is too complex and diverse a phenomenon to speak easily of causes. But we may be able to discern the causes of specific acts. Our response to 9/11 and other acts of terrorism will be affected by our understanding of their causes. If 9/11 was caused by US Middle East policies, the response must involve a review of these policies. If it is a backlash against globalization, the response must address the realities underlying anti-globalization sentiments. Addressing causes will not in any case end terrorism, and addressing the wrong causes will be counterproductive. Actions to reduce those conditions that create support for terrorism and aid its recruitment, which need to be clearly identified, are critical in any counterterrorism strategy. So we must understand the reasons for terrorism and, in particular, for the attacks of September 11th.T his paper will look at the question of the roots of terrorism and then look to the specific case of 911 and its aftermath, with a special view to the impact of globalization.

Carbonaceous particles are a minor constituent of the atmosphere but have a profound effect on air quality, human health, visibility and climate. The importance of carbonaceous particles has been increasingly recognized and become a mainstream topic at numerous conferences. Such was not the case in 1978, when the 1st International Conference on Carbonaceous Particles in the Atmosphere (ICCPA), or ''Carbon Conference'' as it is widely known, was introduced as a new forum to bring together scientists who were just beginning to reveal the importance and complexity of carbonaceous particles in the environment. Table 1 lists the conference dates, venues in the series as well as the proceedings, and special issues resulting form the meetings. Penner and Novakov (Penner and Novakov, 1996) provide an excellent historical perspective to the early ICCPA Conferences. Thirty years later, the ninth in this conference series was held at its inception site, Berkeley, California, attended by 160 scientists from 31 countries, and featuring both new and old themes in 49 oral and 83 poster presentations. Topics covered such areas as historical trends in black carbon aerosol, ambient concentrations, analytic techniques, secondary aerosol formation, biogenic, biomass, and HULIS1 characterization, optical properties, and regional and global climate effects. The conference website, http://iccpa.lbl.gov/, holds the agenda, as well as many presentations, for the 9th ICCPA. The 10th ICCPA is tentatively scheduled for 2011 in Vienna, Austria. The papers in this issue are representative of several of the themes discussed in the conference. Ban-Weiss et al., (Ban-Weiss et al., accepted) measured the abundance of ultrafine particles in a traffic tunnel and found that heavy duty diesel trucks emit at least an order of magnitude more ultrafine particles than light duty gas-powered vehicles per unit of fuel burned. Understanding of this issue is important as ultrafine particles have been shown to adversely affect human health (Lighty et al., 2000; Pope and Dockery, 2006). Gan et al. (Gan et al., accepted) examined the indoor air quality aboard submarines and found that the diesel particulate matter concentrations exceeded the EPA 24 hour standard. Claeys et al. (Claeys et al., accepted) studied the importance and sources of secondary organic aerosol (SOA) in remote marine environment during a period of high biological activity. Methanesulphonate was the major SOA compound detected and there was no evidence for SOA from isoprene. The optical properties of gasoline and diesel vehicle particulate emissions and their relative contribution to radiative forcing was studied by Strawa et al. (Strawa et al., accepted).

Terrorism is too complex and diverse a phenomenon to speak easily of causes. But we may be able to discern the causes of specific acts. Our response to 9/11 and other acts of terrorism will be affected by our understanding of their causes, as well as by possible political requirements to address widespread perceptions of causes. If 9/11 was caused by Islamic radicalism, the near-term response must be to ensure the terrorists are defeated and pose no fiuther danger. In the longer term, education is critical. If the attacks were caused by US Middle East policies, the response should involve a review of those policies. This may or may not result in changes to policy, public diplomacy, etc. If the attacks were a backlash against globalization, the response must address the realities underlying anti-globalization sentiments. Addressing causes (real and perceived) will not in any case end terrorism, and addressing the wrong causes can be counterproductive. Actions to reduce those conditions that create support for terrorism and aid its recruitment effort are critical to any counterterrorism strategy. For this reason alone, we must do everything possible to understand the reasons terrorism may be undertaken, including the attacks of September 1 1 th. This paper will look at the question of the roots of terrorism and then look to the specific case of 9/11 and its aftermath, with a special view to the impact of globalization.

During the period 1980 to 2002, China experienced a 5% average annual reduction in energy consumption per unit of gross domestic product (GDP). The period 2002-2005 saw a dramatic reversal of the historic relationship between energy use and GDP growth: energy use per unit of GDP increased an average of 3.8% per year during this period (NBS, various years). China's 11th Five Year Plan (FYP), which covers the period 2006-2010, required all government divisions at different levels to reduce energy intensity by 20% in five years in order to regain the relationship between energy and GDP growth experienced during the 1980s and 1990s. This report provides an assessment of selected policies and programs that China has instituted in its quest to fulfill the national goal of a 20% reduction in energy intensity by 2010. The report finds that China has made substantial progress toward its goal of achieving 20% energy intensity reduction from 2006 to 2010 and that many of the energy-efficiency programs implemented during the 11th FYP in support of China's 20% energy/GDP reduction goal appear to be on track to meet - or in some cases even exceed - their energy-saving targets. It appears that most of the Ten Key Projects, the Top-1000 Program, and the Small Plant Closure Program are on track to meet or surpass the 11th FYP savings goals. China's appliance standards and labeling program, which was established prior to the 11th FYP, has become very robust during the 11th FYP period. China has greatly enhanced its enforcement of new building energy standards but energy-efficiency programs for buildings retrofits, as well as the goal of adjusting China's economic structure to reduce the share of energy consumed by industry, do not appear to be on track to meet the stated goals. With the implementation of the 11th FYP now bearing fruit, it is important to maintain and strengthen the existing energy-saving policies and programs that are successful while revising programs or adding new policy mechanisms to improve the programs that are not on track to achieve the stated goals.

China's 11th Five-Year Plan (FYP) sets an ambitious target for energy-efficiency improvement: energy intensity of the country's gross domestic product (GDP) should be reduced by 20% from 2005 to 2010 (NDRC, 2006). This is the first time that a quantitative and binding target has been set for energy efficiency, and signals a major shift in China's strategic thinking about its long-term economic and energy development. The 20% energy intensity target also translates into an annual reduction of over 1.5 billion tons of CO2 by 2010, making the Chinese effort one of most significant carbon mitigation effort in the world today. While it is still too early to tell whether China will achieve this target, this paper attempts to understand the trend in energy intensity in China and to explore a variety of options toward meeting the 20% target using a detailed end-use energy model.

This volume contains the proceedings of the 10th International Workshop on Quantum Physics and Logic (QPL X), which was held July 17-19, 2013 at ICFO in Castelldefels (Barcelona), Spain. The goal of this workshop series is to bring together researchers working on mathematical foundations of quantum physics, quantum computing and spatio-temporal causal structures, and in particular those that use logical tools, ordered algebraic and category-theoretic structures, formal languages, semantic methods and other computer science methods for the study of physical behavior in general. Over the past few years, there has been growing activity in these foundational approaches, together with a renewed interest in the foundations of quantum theory, which complement the more mainstream research in quantum computation.

China's 11th Five-Year Plan (FYP) sets an ambitious target to reduce the energy intensity per unit of gross domestic product (GDP) by 20% from 2005 to 2010 (NDRC, 2006). In the building sector, the primary energy-saving target allocated during the 11 FYP period is 100 Mtce. Savings are expected to be achieved through the strengthening of enforcement of building energy efficiency codes, existing building retrofits and heat supply system reform, followed by energy management of government office buildings and large scale public buildings, adoption of renewable energy sources. To date, China has reported that it achieved the half of the 20% intensity reduction target by the end of 2008, however, little has been made clear on the status and the impact of the building programs. There has also been lack of description on methodology for calculating the savings and baseline definition, and no total savings that have been officially reported to date. This paper intends to provide both quantitative and qualitative assessment of the key policies and programs in building sector that China has instituted in its quest to fulfill the national goal. Overall, this paper concludes that the largest improvement for building energy efficiency were achieved in new buildings; the program to improve the energy management in government and large scale public buildings are in line with the target; however the progress in the area of existing building retrofits, particularly heating supply system reform lags behind the stated goal by a large amount.

The PowerTrap{trademark} is a non-exhaust temperature dependent system that cannot become blocked and features a controlled regeneration process independent of the vehicle's drive cycle. The system has a low direct-current power source requirement available in both 12-volt and 24-volt configurations. The system is fully programmable, fully automated and includes Euro IV requirements of operation verification. The system has gained European component-type approval and has been tested with both on- road and off-road diesel fuel up to 2000 parts per million. The device is fail-safe: in the event of a device malfunction, it cannot affect the engine's performance. Accumulated mileage testing is in excess of 640,000 miles to date. Vehicles include London-type taxicabs (Euro 1 and 2), emergency service fire engines (Euro 1, 2, and 3), inner city buses, and light-duty locomotives. Independent test results by Shell Global Solutions have consistently demonstrated 85-99 percent reduction of ultrafines across the 7-35 nanometer size range using a scanning mobility particle sizer with both ultra-low sulfur diesel and off-road high-sulfur fuel.

The DOE funds helped support an International Conference, Calix 2007, whose focus was on Supramolecular Chemistry. The conference was held at the University of Maryland from August 6-9, 2007 (Figure 1). The conference website is at www.chem.umd.edu/Conferences/Calix2007. This biannual conference had previously been held in the Czech Republic (2005), Canada (2003), Netherlands (2001), Australia (1999), Italy (1997), USA (Fort Worth, 1995) Japan (1993) and Germany (1991). Calixarenes are cup-shaped compounds that are a major part of Supramolecular Chemistry, for which Cram, Lehn and Pederson were awarded a Nobel Prize 20 years ago. Calixarene chemistry has expanded greatly in the last 2 decades, as these compounds are used in synthetic and mechanistic chemistry, separations science, materials science, nanoscience and biological chemistry. The organizing committee was quite happy that Calix 2007 encompassed the broad scope and interdisciplinary nature of the field. Our goal was to bring together leading scientists interested in calixarenes, molecular recognition, nanoscience and supramolecular chemistry. We believe that new research directions and collaborations resulted from an exchange of ideas between conferees. This grant from the DOE was crucial toward achieving that goal, as the funds helped cover some of the registration and accommodations costs for the speakers.

While screening commercial cargo containers for the possible presence of WMD is important and necessary smugglers have successfully exploited the many other vehicles transporting cargo into the US including medium and small vessels at sea. These vessels provide a venue that is currently not screened and widely used. Physics limits that make screening of large vessels prohibitive impractical do not prohibit effective screening of the smaller vessels. While passive radiation detection is probably ineffective at sea active interrogation may provide a successful approach. The physics limits of active interrogation of ships at sea from standoff platforms are discussed. Autonomous platforms that could carry interrogation systems at sea, both airborne and submersible, are summarized and their utilization discussed. An R&D program to investigate the limits of this approach to screening ships at sea is indicated and limitations estimated.

The objective of this research is to determine commercially attainable biomass yields given the best site tending possible under the constraints of this extensive network. Biomass yields are reported from the best clones planted in one acre blocks on 8 sites over the four states. Biomass yields are presented of short rotation intensively cultured poplar plantations established in Wisconsin, Minnesota, North and South Dakota during 1987--88. It was reported at that time that the mean annual increment had not peaked in the plantations. Growth measurements were continued through the 1997 growing season when the plantations had completed their 10th and 11th growing season.

    1994-06-01T23:59:59.000Z

A loblolly pine (Pinus taeda) plantation was subjected to two cultural treatments to examine seasonal cumulative pine development in the 9th through 12th growing seasons: (1) pine stocking was either reduced by thinning to 303 trees per acre at a 12- by 12-ft spacing or the plots were left uncut with an original density of 1,210 trees per acre at a 6- by 6-ft spacing, and (2) either no fertilizer was applied or diammonium phosphate was broadcast at 134 lb of phosphorus and 120 lb of nitrogen per acre. Competing vegetation was controlled on all plots. Thinning resulted in less spring height growth in the 9th and 10th growing seasons that not cutting, but thinning increased diameter growth each year. Beginning in the 10th growing season, fertilization increased height, diameter, and basal area per acre growth, with the effect of fertilization on diameter growth being most pronounced on the thinned plots. Therefore, fertilization of thinned plots was more beneficial than thinning alone, and thinning alone resulted in less height and basal area per acre growth than the other treatment combinations for the 4-year period.

Solid particle erosion (SPE) is caused by oxide particles in steam. Hard oxide particles exfoliate from the inside surfaces of boiler tubes and steam lines and are carried by the steam to the turbine where they impact and erode stationary and moving turbine parts (nozzles, moving blades, stationary blades, seal strips, and shrouds around the blades). The first stages of the high pressure (HP) and intermediate pressure (IP) turbines experience the greatest amount of SPE. Though many owners experience erosion in the first stages of both HP (main steam) and IP (reheat steam) turbines, TVA's principal erosion experience is in the IP or reheat steam turbines. This report is limited to the first few stages of IP turbines, referred to as ''first reheat stages.'' Hard chromium carbide plasma coatings have been developed. The coating was installed in Paradise Unit-2 IP turbine, 9th and 10th stages. Pretest inspection report is given.

    2009-04-10T23:59:59.000Z

The importance of Arctic mixed-phase clouds on radiation and the Arctic climate is well known. However, the development of mixed-phase cloud parameterization for use in large scale models is limited by lack of both related observations and numerical studies using multidimensional models with advanced microphysics that provide the basis for understanding the relative importance of different microphysical processes that take place in mixed-phase clouds. To improve the representation of mixed-phase cloud processes in the GISS GCM we use the GISS single-column model coupled to a bin resolved microphysics (BRM) scheme that was specially designed to simulate mixed-phase clouds and aerosol-cloud interactions. Using this model with the microphysical measurements obtained from the DOE ARM Mixed-Phase Arctic Cloud Experiment (MPACE) campaign in October 2004 at the North Slope of Alaska, we investigate the effect of ice initiation processes and Bergeron-Findeisen process (BFP) on glaciation time and longevity of single-layer stratiform mixed-phase clouds. We focus on observations taken during October 9th-10th, which indicated the presence of a single-layer mixed-phase clouds. We performed several sets of 12-hour simulations to examine model sensitivity to different ice initiation mechanisms and evaluate model output (hydrometeors concentrations, contents, effective radii, precipitation fluxes, and radar reflectivity) against measurements from the MPACE Intensive Observing Period. Overall, the model qualitatively simulates ice crystal concentration and hydrometeors content, but it fails to predict quantitatively the effective radii of ice particles and their vertical profiles. In particular, the ice effective radii are overestimated by at least 50%. However, using the same definition as used for observations, the effective radii simulated and that observed were more comparable. We find that for the single-layer stratiform mixed-phase clouds simulated, process of ice phase initiation due to freezing of supercooled water in both saturated and subsaturated (w.r.t. water) environments is as important as primary ice crystal origination from water vapor. We also find that the BFP is a process mainly responsible for the rates of glaciation of simulated clouds. These glaciation rates cannot be adequately represented by a water-ice saturation adjustment scheme that only depends on temperature and liquid and solid hydrometeors contents as is widely used in bulk microphysics schemes and are better represented by processes that also account for supersaturation changes as the hydrometeors grow.
