EyeMasters and Wal-Mart.
Please call Davis Vision at 1-800-501-1459 to obtain a list
of the network providers nearest you, or you may access our
Web site at www.davisvision.com and utilize our "Open
Enrollment" feature. You will be prompted to enter your
Client Control Number, which is 4495.
How do I receive services from a Davis Vision
network provider?
· For providers other than EyeMasters or Wal-Mart,
call the network provider of your choice and schedule
an appointment. Many locations will accept outside
prescriptions; please confirm this service when making
your appointment.
