Dear students and parents,
The University of Minnesota is pleased to offer the University-sponsored Voluntary Student Dental
Plan administered by the Office of Student Health Benefits.
Network and claims administration services for the Voluntary Student Dental Plan are provided by
MetLife, giving you access to thousands of providers in their national network.
All eligible students who wish to take advantage of the 2011-2012 Voluntary Student Dental Plan must
complete the enrollment process online at www.shb.umn.edu by September 19, 2011. Enrollment is on
a yearly basis and must be completed during the open enrollment period April 1-September 19, 2011.
