is the software program used by the University of Virginia Medical Center for reporting and
tracking of Quality and Safety Events. A major goal of the Medical Center is to promote quality health
care through a systematic program of performance improvement and patient safety enhancements.
An Event or "variance" is any occurrence that is not consistent with the ordinary course or expected
outcomes of Medical Center operations and clinical activities. Reporting of a quality or safety
event/variance, and the subsequent review by a manager or designee, is to be completed within 3 days
of discovery of the occurrence. Medical Center Policy No. 0132 outlines the Quality Reporting Process:
