Title: Gallium scan for detecting Pneumocystic Carinii pneumonitis in patients with acquired immunodeficiency syndrome (AIDS)

Pneumocystic carinii pneumonitis (PCP) is one of the common causes of death in patients with AIDS. Bronchoscopy with washing and transbronchial biopsy are necessary for diagnosis of PCP, however, they are invasive procedures. Forty Ga-67 scans were performed in 33 male patients (age 18-53) with AIDS to evaluate the possibility of PCP infection. Multiple projections over the thorax were obtained at 24-48 and/or 72 hr. after IV of 3 mCi of Ga-67. The positive Ga scans included 15 with diffuse lung activity (7 homogeneous and 8 inhomogeneous), 7 with focal lung activity and 3 with hilar activity. When using the criteria of Ga activity approx. = 2+ represents PCP, the sensitivity and specificity were 92% and 83% respectively. The authors conclude that the Ga lung scan in PCP may present as both focal or diffuse involvement, is a sensitive non-invasive test for detecting PCP in pts with AIDS and thus it may be useful in AIDS pts when PCP is suspected.
