Association of FDG uptake with proliferating cell nuclear antigen, Ki-67 and DNA ploidy pattern in non-small cell lung carcinoma

Immunohistochemical staining for proliferating cell nuclear antigen (PCNA) and Ki-67 provides a reliable method for evaluation of tumor growth fraction, and the measurement of cellular DNA content by Flow cytometry (FCM) provides prognostic information for non-small cell lung carcinoma. The purpose of this study was to determine the association of FDG uptake with PCNA, Ki-67 and DNA ploidy pattern. Twelve patients with untreated non-small cell lung carcinoma (8 adenocarcinomas, 3 squamous cell carcinomas and 1 adenosquamous carcinoma) underwent this study. The tumor sized ranged form 1.0 to 4.0cm. FDG PET was performed 40 mins after i.v. injection of 111 MBq F-18 FDG. For quantitative analysis of the FDG uptake, the standardized uptake value (SUV) was calculated. Tumor FDG activity was corrected for partial volume effect using recovery coefficients determined by spherical phantom measurements. In all patients, FDG PET delineated focal lesions with an increase in tracer accumulation. All twelve patients underwent surgical resection within 2 weeks after the PET study. Immunohistochemical staining for PCNA and Ki-67 in resected cancer tissues was performed, and more than 1000 cells were counted in minimum of six different high power fields. DNA ploidy analysis of fresh frozen tissues was also performed by FCM. Significant positive correlations were found between SUV and the number of PCNA positive nuclei (r=0.818, p<0.01) and between SUV and Ki-67 positive nuclei (r=0.730, p<0.01). The aneuploidy pattern of DNa content was observed in 67% patients. There was no significant difference between SUV of lung cancer with aneuploidy pattern and SUV of lung cancer with diploidy pattern. In conclusion, tumor uptake of FDG may be representative of tumor growth fraction, while does no provide information of DNA ploidy pattern for non-small cell lung carcinoma.
